Tuberculosis patients with liver dysfunction were detrimental to their treatment and prognosis because of immune dysfunction. Malnutrition played a vital role in liver dysfunction;however, there were only limited data. This study aimed to clarify the association between macronutrients intake and liver dysfunction in tuberculosis patients and to explore a new idea of alleviating liver dysfunction in tuberculosis patients with nutritional improvement. Methods In this cross-sectional study, 3052 active tuberculosis patients aged >18 years were included based on the data from primary tuberculosis clinics in Linyi City, China. Basic information was collected by the baseline questionnaire. macronutrients intake of tuberculosis patients was assessed by the 24-hour dietary recalls. ALT or AST values were measured using an automatic biochemical analyzer. The subjects were divided into normal liver function group and liver dysfunction group based on ALT or AST values greater than 40 U/L. Results Among all the tuberculosis patients, 439(14.4%) had liver dysfunction. The patients with liver dysfunction were more likely to be male, less education and higher alcohol intake among drinkers compared to patients with normal liver function. About the daily intake of macronutrients, compared to normal liver function group, patients with liver dysfunction had significantly less protein(P = 0.008), and fat(P intake of alcohol, higher intakes of protein or fat were both associated with lower incidence of liver dysfunction(all P-trend intake by gender, intake of energy and the ratio of protein: fat intake were further conducted. Among those with a high intake ratio of protein: fat(≥ 1.92), the pattern suggested both inverse associations between high intake of protein or fat and the odds of liver dysfunction whether age, gender, education and
