A surveillance study on penicillin-resistant streptococcus pneumoniae in China

Objectives To find out whether there are also penicillinresistant str eptococcus pneumoniae and the resistant rate in China Methods A surveillance study which is a part of the international surve illance on pneumococci resistance to penicillin and other antimicrobial agents w as conducted in Beijing, China More than 900 pediatric patients with respirato r y tract infections aged from six months to three years selected from two pediatr ic units were enrolled in the study Perthroat swabs were immediately streaked o nto blood agar plates Isolates were identified as pneumococci by their typical appearance, gram stain, confirmation tests Antibiotic susceptibility was asses s ed by the disk diffusion method and minimal inhibition concentration (MIC) deter mination according to Protocol and National Committee for Clinical Laboratory St andards (NCCLS). Results Of the 51 streptococcus pneumoniae isolates, 5 strains had zone s of inhibition indicative of penicillin resistance, of which two had penicillin MICs01?mg/L and were considered to be relatively or fully resistant to pen i cillin The MICs of two penicillin resistant strains were 2?mg/L and 4?mg/L R esistant rates to ceftriaxone and cefotaxime were 0% and 10% respectively For the othe r ten antimicrobial agents, the resistant rates were as follows: ampicillin 12% , piperacillin 26%, furbenicillin 14%, cefuroxime 6%, erythromycin 54%, clarithr omycin 52%, meleumycin 64%, roxithromycin 52%, chloramphenicol 35%, tetracycline 100% Conclusions The study suggested that penicillin resistant streptococcus pneumoniae strains existed in China and were also resistant to ampicillin, pipe racillin, furbenicillin and cefuroxime Moreover, the resistant rates of penici llin resistant streptococcus pneumoniae to macrolides and tetracycline were high

Diffuse panbronchiolitis （DPB） is a relatively recently recognized, idiopathic progressive disease, originally described-in Japan in 1969. It has distinctive clinicopathological features characterized by chronic pulmonary inflammation in the respiratory bronchioles of the lung and paranasal sinuses. It has been reported more extensively among Japanese than other ethnic populations. The first case of DPB in China was reported in 1996. Since then, an increasing number of cases have been described, which suggest that this disease may not be rare in China.

Background Azithromycin can reduce neutrophil accumulation in neutrophilic pulmonary ***,the precise mechanism behind this action remains *** experiment assessed whether azithromycin inhibits neutrophil accumulation in the airways by affecting interleukin-17 （IL-17） downstream *** Mice were pretreated with azithromycin before murine IL-17A （mlL-17） *** the mlL-17 stimulation,the levels of six neutrophil-mobilizing cytokines were determined by enzyme-linked immunosorbent assay （ELISA） tests in bronchoalveolar lavage （BAL） fluid; IL-6,CXC chemokine ligand-1 （CXCL-1）,CXCL-5,macrophage inflammatory protein-2 （MIP-2）,granulocyte colony-stimulating factor （G-CSF）,and granulocyte macrophage colony-stimulating factor （GM-CSF）.The number of neutrophils in BAL fluid were evaluated by cytospin *** （1） Azithromycin pretreatment significantly inhibited both the release of three neutrophil-mobilizing cytokines （MIP-2,CXCL-5 and GM-CSF） and the accumulation of neutrophils in airways caused by mlL-17 stimulation.（2） The levels of three neutrophil-mobilizing cytokines （IL-6,MIP-2 and GM-CSF） were positively correlated with the numbers of neutrophil in BAL *** Azithromycin can inhibit neutrophil accumulation in the airways by affecting IL-17 downstream *** finding suggests that macrolide antibiotic application might be useful in prevention of neutrophilic pulmonary diseases characterized by high levels of IL-17.

SARS-CoV-2 is a novel RNA coronavirus responsible of a deadly pandemic: the clinical illness COVID-19. With only one authorized drug for emergency use in critically ill patients: Remdesivir, there is not any other approved drug or vaccine yet with proven potential to overcome this infection. We exposed here many scientific evidences to support our novel idea that a macrolide, basically Clarithromycin, could be effective as a single agent for treatment and prophylaxis of COVID-19. Clarithromycin could change the history of this pandemic. It could reduce the costs of treatment and the potential adverse effects when combining more than one drug such as with Hydroxychloroquine. Clarithromycin treatment and prophylaxis as a single agent could be much more simple, safe and cheaper as giving Chloroquine or Hydroxychloroquine alone or in combination with Azithromycin as well as other therapeutic options.

Since the discovery of penicillin by Fleming in 1928, the knowledge of the antibiotics’ spectrum and mechanism of action has been steadily increasing. Antibiotics became an effective tool in the fight against many pathogens, changing the prognosis of outcome for many serious diseases. It is already known that antibiotics not only have the antibacterial activity, but many of them—e.g. macrolides, sulphonamides and tetracyclines—have immunomodulating effect, affecting functions of lymphocytes, macrophages and costimulatory molecules, macrophage migration and phagocytosis, as well as proinflammatory cytokine secretion. The expanding knowledge of the effects of antibiotics on the immune system has brought with it new applications of antibiotics in organ transplantation, invasive cardiology and treatment of autoimmune diseases such as rheumatoid arthritis or asthma.

A study of invasive strains of Streptococcus agalactiae (GBS) from Cordoba and Misiones, Argentina;was conducted to determine serotypes, the susceptibility to macrolides and molecular profiles of virulence and resistance. We studied 17 strains, recovered from cerebrospinal fluid, blood and cellulite and, a strain of trophoblastic remnants from Misiones. The serotypes were determined by agglutination with sera and phenotypes of resistance to macrolide-lincosamide-streptogramin B (MLSB), were determined with the double-disk test (D-test). The confirmation was performed by E-test by ERI and CLI respectively that determined the minimum inhibitory concentration (MIC). Results were interpreted as recommended by the Clinical and Laboratory Standards Institute (CLSI) 2013. Resistance genes: ermB, ermTR and mefA and the virulence genes: bac, bca, rib, lmb, hylB, scpB, fbsA, fbsB and cylB were investigated by conventional PCR. Serotype III (50%) and Ia (50%) were detected in Cordoba. One strain showed cMLSB phenotype, confirmed by MIC. The same strains showed a resistance gene ermB. All studied virulence genes were detected in 100% of these strains. In Misiones, serotypes were III (72.7%), Ia (18.2%) and Ib (9.1%). All strains were susceptible to CLI and ERI by D-test, confirmed by MIC. None of the strains showed resistance genes. Virulence genes bca, rib, hylB, lmb, fbsA, fbsB and cylB were detected in 100% of the strains, bac in 81.8% and scpB in 90.9%. Our results are in accordance with international data, associating higher frequency of serotype III of invasive neonatal disease followed by Ia. The presence of serotype Ib could indicate a regional difference for Misiones. We highlight the macrolides susceptibility in strains of Misiones and consistency in the results for D-test, MIC and PCR for the single strain resistant phenotype cMLSB from Cordoba. The virulence genes studied were presented with high frequency as expected for invasive strains.

As one group of the most widely used oral antibacterial drugs, macrolides have been clinically applied for more than 50 years because of their safety and efficacy. Recently, the non-antibacterial applications of macrolides are frequently reported, such as the therapy of malignant tumor, exudative pleurisy, pulmonary fibrosis, rheumatoid arthritis, bronchiectasis, asthma, gastrointestinal peristalsis disorders, and so on with new pharmacological effects, including reverse of tumor resistance, anti-chemical adherence, anti-fibrosis, and modulations of gastrointestinal motility as well as immune activity. The in-depth research of marcolides’ non-antibiotic actions can not only extend the application fields of macrolides, but provide a new way to treat some refractory diseases.
