Emergency treatment and anesthesia management of internal carotid artery injury during neurosurgery:Four case reports

BACKGROUND During skull base surgery,intraoperative internal carotid artery(ICA) injury is a catastrophic complication that can lead to fatal blood loss or secondary cerebral *** management of ICA injury plays a crucial role in the prognosis of *** have reported multiple techniques and management strategies;however,the literature on managing this complication from the anesthesiologist’s perspective is limited,especially in the aspect of circulation management and airway management when patients need transit for further endovascular *** SUMMARY We describe 4 cases of ICA injury during neurosurgery;there were 3 cases of pathologically proven pituitary adenoma and 1 case of cavernous sinus endothelial *** the onset of ICA injury,all four patients were immediately transferred for endovascular therapy under general anesthesia with vital signs monitored and mechanical *** patients were transferred to the hybrid operating room,and one patient was transferred to the catheter operating *** patients underwent covered stent implantation,and one patient underwent *** four patients experienced hypovolemic shock and received blood products infusion and vasoactive drugs to maintain stable *** the neurosurgery,one patient was extubated and returned to the ward,and the other three were delayed tracheal extubation and returned to the intensive care *** patient died from serious neurological complications after 62 d in the hospital,but the other three showed good clinical *** ICA injury imposes a high risk of massive hemorrhage and subsequent *** treatment is critical and requires interdisciplinary collaboration among neurosurgeons,anesthesiologists,and interventional *** hemostatic methods,stable hemodynamics sufficient to ensure perfusion of vital organs,airway safety during transit,rapid localization and implementa

Deep learning-based fault diagnostic network of high-speed train secondary suspension systems for immunity to track irregularities and wheel wear

Fault detection and isolation of high-speed train suspension systems is of critical importance to guarantee train running safety. Firstly, the existing methods concerning fault detection or isolation of train suspension systems are briefly reviewed and divided into two categories, i.e., model-based and data-driven approaches. The advantages and disadvantages of these two categories of approaches are briefly summarized. Secondly, a 1D convolution network-based fault diagnostic method for highspeed train suspension systems is designed. To improve the robustness of the method, a Gaussian white noise strategy(GWN-strategy) for immunity to track irregularities and an edge sample training strategy(EST-strategy) for immunity to wheel wear are proposed. The whole network is called GWN-EST-1 DCNN method. Thirdly, to show the performance of this method, a multibody dynamics simulation model of a high-speed train is built to generate the lateral acceleration of a bogie frame corresponding to different track irregularities, wheel profiles, and secondary suspension faults. The simulated signals are then inputted into the diagnostic network, and the results show the correctness and superiority of the GWN-EST-1DCNN method. Finally,the 1DCNN method is further validated using tracking data of a CRH3 train running on a high-speed railway line.

Hypophosphatemia as a prognostic tool for post-hepatectomy liver failure:A systematic review

BACKGROUND Post-hepatectomy liver failure(PHLF)is one of the main causes of postoperative mortality and is challenging to predict early in patients after liver *** studies suggest that the postoperative serum phosphorus might predict outcomes in these *** To perform a systematic literature review on hypophosphatemia and evaluate it as a prognostic factor for PHLF and overall *** This systematic review was performed according to preferred reporting items for systematic reviews and meta-analyses statement.A study protocol for the review was registered in the International Prospective Register of Systematic reviews ***,Cochrane and Lippincott Williams&Wilkins databases were systematically searched up to March 31,2022 for studies analyzing postoperative hypophosphatemia as a prognostic factor for PHLF,overall postoperative morbidity and liver *** quality assessment of the included cohort studies was performed according to the Newcastle-Ottawa *** After final assessment,nine studies(eight retrospective and one prospective cohort study)with 1677 patients were included in the systematic *** selected studies scored≥6 points according to the Newcastle-Ottawa *** values of hypophosphatemia varied from<1 mg/dL to≤2.5 mg/dL in selected studies with≤2.5 mg/dL being the most used defining *** studies analyzed PHLF,while the remaining four analyzed overall complications as a main outcome associated with *** two of the selected studies analyzed postoperative liver regeneration,with reported better postoperative liver regeneration in cases of postoperative *** three studies hypophosphatemia was associated with better postoperative outcomes,while six studies revealed hypophosphatemia as a predictive factor for worse patient *** Changes of the postoperative serum phosphorus level might be useful for predicting outcomes after liver ***

BACKGROUND Clear cell renal cell carcinoma(ccRCC)is a common type of tumor that can metastasize to any organs and ***,it is extremely rare for ccRCC to metastasize to the ***,we describe a rare case of iris metastasis from ccRCC with a history of left nephrectomy in *** SUMMARY A 62-year-old male was admitted to the hospital due to blurred vision and red eyes,and a mass was found on the iris in the right eye.B-scan ultrasonography revealed a well-bounded high-density lesion at the corner of the anterior chamber at the 3-4 o’clock *** with simultaneous intraocular lens implantation and iridocyclectomy was performed in the right *** lesion was confirmed to be metastatic ccRCC by histological and immunohistochemical *** patient was still alive at 9 mo after surgical *** metastasis can be an initial sign with a poor *** detection and treatment may improve *** should pay attention to similar metastatic diseases to prevent misdiagnosis leading to missed treatment *** This report of the characteristics and successful management of a rare case of iris metastasis from ccRCC highlights the importance of a comprehensive medical history,histopathology,immunohistochemistry,and clinical manifestation for successful disease diagnosis.

Analysis of the management and therapeutic performance of diabetes mellitus employing special target

Diabetes mellitus (DM) is a chronic metabolic condition characterized predominantlyby hyperglycemia. The most common causes contributing to the pathophysiologyof diabetes are insufficient insulin secretion, resistance to insulin’stissue-acting effects, or a combination of both. Over the last 30 years, the globalprevalence of diabetes increased from 4% to 6.4%. If no better treatment or cure isfound, this amount might climb to 430 million in the coming years. The major fact-ors of the disease’s deterioration include age, obesity, and a sedentary *** new therapies to manage diabetes safely and effectively without jeopardizingpatient compliance has always been essential. Among the medicationsavailable to manage DM on this journey are glucagon-like peptide-1 agonists,thiazolidinediones, sulphonyl urease, glinides, biguanides, and insulin-targetingreceptors discovered more than 10 years ago. Despite the extensive preliminarystudies, a few clinical observations suggest this process is still in its early *** present review focuses on targets that contribute to insulin regulation andmay be employed as targets in treating diabetes since they may be more efficientand secure than current and traditional treatments.
