目的:研究消肿生肌汤辅助经括约肌间瘘管结扎术(Ligation of intersphincteric fistula tract,lift)对高位单纯性肛瘘术后疼痛(Visual analogue scale,VAS)评分、康复进程的影响.方法:选取2020年8月至2022年8月于我院确诊的94例肛瘘患者作为研究对象,依随机对照法分为研究组和对照组,各47例.对照组采用lift术治疗,研究组在对照组基础上联合消肿生肌汤治疗.比较两组临床疗效、治疗后恢复情况及住院时间、治疗前后视觉模拟评分(VAS)、大便失禁严重性指数(Fecal incontinence severity index,FISI)、Wexner肛门失禁评分(Wexner)、肛肠动力学、相关血清指标水平.结果:研究组总有效率高于对照组(P0.05);研究组恢复情况及住院时间短于对照组(Plift对高位单纯性肛瘘患者术后效果显著,可有效促进患者创口愈合,减少炎症反应,降低肛肠疼痛度,缩短临床康复进程.

The transit time difference of fluid particles moving along the upper and lower surfaces of a lift-producing airfoil is studied here both theoretically and numerically.We show that,under thin airfoil assumption and for potential flow,the transit time difference is equal to the circulation divided by the square of the inflow velocity and the lift coefficient is equal to half of the number of chords travelled by the airfoil during the transit time difference.An analysis of transit time difference for very thick airfoil(c.***.very large angle of attack)suggests the transit time may change sign beyond thin airfoil assumption,a conclusion supported by an example of flow with an attached vortex.Thus,fluid particles may transit the upper surface with less,equal and more time than those transiting the lower surface for lift producing airfoils,depending on the configuration of flow structure and geometry.

在风洞中采用烟线流动显示和热线测量方法对弦厚比B/H=3.0的二维旋转振荡矩形板尾流进行了实验研究,研究的雷诺数Re=1500,振幅范围和折算频率范围分别为α=2.5°~50°和f_(e)H/V_(∞)=0.02~0.25,其中f_(e)为振频,V_(∞)为来流速度.发现了6种同步旋涡脱落模式,其中包括3种新的模式:3P模式,一个振荡周期内有三对转向相反的旋涡脱落;2S_(2)模式,一个周期内每侧各有一个同向涡对脱落;T模式,一个周期内有三个涡脱落.找出了每个旋涡模式的存在区域,并在振频-振幅平面上画出了模式区域图.进行了数值模拟,一些典型状态下所模拟的涡量场、速度剖面和尾流宽度,均与实验结果对比而得到了验证.当振频较高时,惯性力中线性的当地非定常项起决定作用,模拟的扭转力矩是板旋转位移和旋转角速度的线性函数.然而当振频较低时,在不同振幅下分别存在2T、3P和2S2模式旋涡脱落,惯性力中的非线性对流项起主要作用,模拟的脉动力矩偏离Scanlan公式.数值模拟数据的统计结果显示,脉动力矩均方根值是单参数η=αf_(e)H/V_(∞)的指数函数.

This paper generalizes the factorization theorem of Gouveia,Parrilo and Thomas to a broader class of convex sets.Given a general convex set,the authors define a slack operator associated to the set and its polar according to whether the convex set is full dimensional,whether it is a translated cone and whether it contains lines.The authors strengthen the condition of a cone lift by requiring not only the convex set is the image of an affine slice of a given closed convex cone,but also its recession cone is the image of the linear slice of the closed convex cone.The authors show that the generalized lift of a convex set can also be characterized by the cone factorization of a properly defined slack operator.

A flow past a circular-section cylinder with a perforated conic shroud, in which the perforation is located at the peak of the conic disturbance as the shroud installed on the cylinder and uniformly distributed with several circular holes, is numerically simulated at a Reynolds number of 100. Two factors in the perforation are taken into account, i.e. the attack angle relative to the direction of incoming flow and diameter of holes. The effect of such perforation on the drag, lift and vortex-shedding frequency is mainly investigated. Results have shown that variation of the attack angle has a little effect, especially on the drag and vortex-shedding frequency, except in certain cases due to the varied vortex-shedding patterns in the near wake. The increasing hole diameter still exhibits a little effect on the drag and frequency of vortex shedding, but really reduces the lift, in particular at larger wavelength, such as the lift reduction reaching almost 66% 68% after introducing the perforation.

AIM To evaluate the safety and feasibility of a new technology combining low-pressure pneumoperitoneum(LPP) and abdominal wall lift(AWL) in laparoscopic total mesorectal excision(TME) for rectal cancer.METHODS From November 2015 to July 2017,26 patients underwent laparoscopic TME for rectal cancer using LPP(6-8 mm Hg) with subcutaneous AWL in Qilu Hospital of Shandong University,Jinan,China.Clinical data regarding patients' demographics,intraoperative monitoring indices,operation-related indices andpathological outcomes were prospectively collected.RESULTS Laparoscopic TME was performed in 26 cases(14 anterior resection and 12 abdominoperineal resection) successfully,without conversion to open or laparoscopic surgery with standard-pressure pneumoperitoneum.Intraoperative monitoring showed stable heart rate,blood pressure and paw airway pressure.The mean operative time was 194.29 ± 41.27 min(range:125-270 min) and 200.41 ± 20.56 min(range:170-230 min) for anterior resection and abdominoperineal resection,respectively.The mean number of lymph nodes harvested was 16.71 ± 5.06(range:7-27).There was no positive circumferential or distal resection margin.No local recurrence was observed during a median follow-up period of 11.96 ± 5.55 mo(range:5-23 mo).CONCLUSION LPP combined with AWL is safe and feasible for laparoscopic TME.The technique can provide satisfactory exposure of the operative field and stable operative monitoring indices.

This review attempts to elucidate the physical origin of aerodynamic lift of an airfoil using simple formulations and notations,particularly focusing on the critical effect of the fluid viscosity.The evolutionary development of the lift problem of a flat-plate airfoil is reviewed as a canonical case from the classical inviscid circulation theory to the viscous-flow model.In particular,the physical aspects of the analytical expressions for the lift coefficient of the plate-plate airfoil are discussed,including Newton’s sine-squared law,Rayleigh’s lift formula,thin-airfoil theory and viscous-flow lift formula.The vortex-force theory is described to provide a solid foundation for consistent treatment of lift,form drag,Kutta condition,and downwash.The formation of the circulation and generation of lift are discussed based on numerical simulations of a viscous starting flow over an airfoil,and the evolution of the flow topology near the trailing edge is well correlated with the realization of the Kutta condition.The presented contents are valuable for the pedagogical purposes in aerodynamics and fluid mechanics.

Transonic wing stall of a blended flying wing common research model based on DDES method

Numerical simulation of wing stall of a blended flying wing configuration at transonic speed was conducted using both delayed detached eddy simulation(DDES) and unsteady Reynolds-averaged Navier-Stokes(URANS) equations methods based on the shear stress transport(SST) turbulence model for a free-stream Mach number 0.9 and a Reynolds number 9.6 × 106. A joint time step/grid density study is performed based on power spectrum density(PSD) analysis of the frequency content of forces or moments, and medium mesh and the normalized time scale0.010 were suggested for this simulation. The simulation results show that the DDES methods perform more precisely than the URANS method and the aerodynamic coefficient results from DDES method compare very well with the experiment data. The angle of attack of nonlinear vortex lift and abrupt wing stall of DDES results compare well with the experimental data. The flow structure of the DDES computation shows that the wing stall is caused mainly by the leeward vortex breakdown which occurred at x/xcr= 0.6 at angle of attack of 14°. The DDES methods show advantage in the simulation problem with separation flow. The computed result shows that a shock/vortex interaction is responsible for the wing stall caused by the vortex breakdown. The balance of the vortex strength and axial flow, and the shock strength, is examined to provide an explanation of the sensitivity of the breakdown location. Wing body thickness has a great influence on shock and shock/vortex interactions, which can make a significant difference to the vortex breakdown behavior and stall characteristic of the blended flying wing configuration.
