left ventriculography: A Valuable Diagnostic Option in Apical Hypertrophic Cardiomyopathy

Apical hypertrophic cardiomyopathy（AHC）was defined by three electrocardiographic and angio－graphic criteria: the presence of giant(over 10 mm)inverted T waves in leads V4 and V5 of the resting ECG；an”ace of spades” appearance of left ventricular angiogaphy in end di－ astole in the righ anterior oblique（RAO projection；the electrocardiographic sum RV5+SV1 greater than 35mm Thereafter, authentic cases of apical hypertrophy have been demonstrated by imaging techniques or observed anatomically without the presence of these criteria, the authors reviewed 5 AHC cases pooven by left ventriculography. Their resting ECG presented symmetric giant inverted T waves in leads V2,V3 and *** of them underwent angigraphybecause of chest pain or comfortless. As a result, the "ace of spades" figure were observed during the left ventricular an giography and the difference of pressure was detected in the chamber of left ventricle. We can conclude here that some atypical cases of apical hypertrophic cardiomyopathy could not be easily recognized by non-invasive examinations, for example, echocardiography and magnetic resonance imaging the left ventriculography maybe an useful method with high diagnostic significance.

Tako-tsubo cardiomyopathy

Tako-tsubo cardiomyopathy（TC） is a recently described acute cardiac syndrome, which the latest cardiomyopathy classification of the European Society of Cardiology describes as an unclassified cardiomyopathy. TC mimics acute myocardial infarction（AMI） and is characterised by ischaemic chest symptoms, an elevated electrocardiogram ST-segment, and moderately increased levels of cardiac disease markers. However, patients with TC have no coronary angiogram-detectable or non-obstructive coronary arterial disease（CAD）, and left ventriculography documents transient left apical and middle ventricular wall dysfunction. In this review, we describe TC and evaluate epidemiological, clinical and instrumental features, pathophysiological mechanisms, therapy and prognosis of this syndrome,with a view to raising awareness of the disease.

Takotsubo cardiomyopathy in a 90-year-old Chinese man

A 90-year-old Chinese man was transferred to the Emergency Department of the Affiliated Shantou Hospital of Sun Yat-sen University for treatment of an acute myocardial infarction. He suffered chest pain with three days of cough, dyspnea and fever. A diagnosis of Takotsubo cardiomyopathy was made in normal coronary arteries from urgent coronary angiography and characteristic apical dyskinesis and basal hyper contractility in left ventriculography. The patient died from severe multi-organ failure on the second day of hospitalization.
