Changing indications for lamellar keratoplasty in Shandong, 1993-2008

Background With the advancement of microsurgical techniques, lamellar keratoplasty （LK） has been more valued and performed to treat corneal blindness. This study aimed to evaluate the indications and changing trends for LK during the past 16 years in Shandong Eye Institute, an eye center in China. Methods A total of 1529 eyes, predominantly from male （70.7%） patients undergoing LK between January 1993 and December 2008, were enrolled in this study. Data were collected by reviewing patient medical records for demographic characteristics, clinical diagnosis, laboratory findings and risk factors. Surgical times, initial diagnoses and causes of regrafting were recorded. All LKs were sequentially divided into period 1 （from 1993 to 2000） and period 2 （from 2001 to 2008）. Results Rural-dwelling patients in this study numbered 1089 （71.2%）; in all cases of infectious keratitis, rural patients accounted for 90.5%. The leading indications for LK were infectious keratitis （31.0%）, corneal trauma （21.1%）, keratoconus （18.7%）, corneal dystrophy and degeneration （7.3%）, regrafting （7.1%）, immunologic disorders （6.7%）, congenital abnormalities and corneal tumor （4.1%） and corneal scarring （3.1%）. The most common subcategory of infectious keratitis was fungal keratitis （67.5%）. Fusarium solani was the most morbigenous fungi. Among the patients, 73.1% of thermal burns were caused by hot molten metal, and 47.8% of alkali burns by lime. Dermoid was the most common indication for congenital abnormalities. The two main initial diagnoses from 109 regrafting cases were corneal trauma （45.9%） and Mooren＇s ulcer （27.5%）. The most common cause for regrafting was immune rejection （35.8%）. Infectious keratitis was the most common indication for LK from 16.0% in period 1 to 41.1% in period 2, followed by keratoconus （16.8%） and corneal trauma （14.5%）. · AIM: To compare the corneal endothelial cell density (ECD) of clear grafts after penetrating keratoplasty (PK) and deep anterior lamellar keratoplasty (DALK). · METHODS: The study included 44 and 54 patients treated with PK and DALK, respectively, between March 2006 and April 2010. Corneal ECD was examined using specular microscopy at postoperative 1, 3, 6, 12, and 18 months, and the values were compared. · RESULTS: Corneal ECD reduction in the PK group was 7.4%, 15.2%, 23.5%, and 28.9% at 3, 6, 12 and 18 months respectively after surgery, compared with 4.2 % in the first month (P 0.05). · CONCLUSION: Compared with DALK,PK significantly reduced ECD of the clear grafts. These results suggest that survival of endothelial cells in grafts is better after DALK than after PK. ·

AIM: To study the tear film stability after lamellar keratoplasty METHODS: Five female and eight male patients with lamellar keratoconus, aged from 18 to 32, were involved. After lamellar keratoplasty, Schirmer I test (S I t), tear break-up time (BUT) test, fluorescein staining test were used to judge the effect of the surgery at different time point. RESULTS: The S 1 t were greatly increased in 7 clays post operation (11.86+/-2.28 -25.14+/-1.97, 19.86+/-1.61) (P0.05). The mean break-up time decreased in 7 days post operation (5.00+/-1.31 -2.71+/- 0.18, 2.57+/- 0.20, 2.71+/- 0.36, 2.43+/- 0.20) (P lamellar keratoplaty influence the tear film stability, artificial tears and improving corneal epithelium cured medicine should be used after surgery.

AIM: To evaluate the biological functions of tissue-engineered human corneal epithelium (TE-HCEP) by corneal transplantation in limbal stem cell deficiency (LSCD) rabbit models. METHODS: TE-HCEPs were reconstructed with DiI-labeled untransfected HCEP cells and denuded amniotic membrane (dAM) in air-liquid interface culture, and their morphology and structure were characterized by hematoxylin-eosin (HE) staining of paraffin-sections, immunohistochemistry and electron microscopy. LSCD models were established by mechanical and alcohol treatment of the left eyes of New Zealand white rabbits, and their eyes were transplanted with TE-HCEPs with dAM surface outside by lamellar keratoplasty (LKP). Corneal transparency, neovascularization, thickness, and epithelial integrality of both traumatic and post transplantation eyes were checked once a week by slit-lamp corneal microscopy, a corneal pachymeter, and periodic acid-Schiff (PAS) staining. At day 120 post surgery, the rabbits in each group were sacrificed and their corneas were examined by DiI label observation, HE staining, immunohistochemistry and electron microscopy. RESULTS: After cultured for 5 days on dAM, HCEP cells, maintaining keratin 3 expression, reconstructed a 6-7 layer TE-HCEP with normal morphology and structure. The traumatic rabbit corneas, entirely opaque, conjunctivalized and with invaded blood vessels, were used as LSCD models for TE-HCEP transplantation. After transplantation, obvious edema was not found in TE-HCEP-transplanted corneas which became more and more transparent, the invaded blood vessels reduced gradually throughout the monitoring period. The corneas decreased to normal thickness on day 25, while those of dAM eyes were over 575 mu m in thickness during the monitoring period. A 45 layer of epithelium consisting of TE-HCEP originated cells attached tightly to the anterior surface of stroma was reconstructed 120 days after TE-HCEP transplantation, which was similar to the normal control eye

AIM: To evaluate the outcomes and safety of lamellar keratoplasty(LK) assisted by fibrin glue in corneal perforations. ·METHODS: Six eyes of 6 patients affected by different corneal pathologies(2 posttraumatic corneal scar and 3 bacterial keratitis) underwent LK procedures by using fibrin glue. The mean corneal perforation diameter was 1.35 ±0.64mm(range,0.7-2.5mm),and the greatest diameter of the ulcerative stromal defect was 2.47 ± 0.77mm in average(range,1.5-3.5mm). The donor corneal lamella diameters were 0.20-mm larger and thicker than the recipient to restore a physiologic corneal thickness and shape: mean donor diameter was 8.34 ± 0.28mm(range,8.2-8.7mm) and mean thickness was 352±40.27mm(range,220-400mm). Mean follow-up was 7.33±1.97 months(range,6-11 months). Postoperatively,the graft status,graft clarity,anterior chamber response,the visual prognosis,intraocular pressures,and postoperative complications were recorded. ·RESULTS:Allthecornealperforationsweresuccessfully healed after the procedure. The best-corrected visual acuity(BCVA) ranged from 20/1 000 to 20/50 in their initial presentation,and from 20/100 to 20/20 in their last visit,showed increase in all the patients. No major complications such as graft dislocation and graft failure were noted. Neovascularization developed in the superficial stroma of donor graft in 1 case. High intraocular pressure developed on day 2 after surgery,while was remained in normal range after application of anti-glaucomatous eyedrops for 1 week in 1 case. ·CONCLUSION: Fibrin glue-assisted sutureless LK is valuable for maintaining the ocular integrity in the treatment of corneal perforations.
