Comparative study for success rate of vaginal birth after cesarean section following labor induction by two forms of vaginal dinosprostone: A pilot study

Background: Cesarean delivery has become the most common major surgical procedure in many parts of the world. Induction of labor in women with prior cesarean delivery is an alternative to mitigate the rising cesarean rates. Objectives: To compare the VBAC success rate between two vaginal forms of dinoprostone for labor induction in women with prior cesarean section. Material and Methods: A pilot study was conducted at a large Governmental Hospital, Dhahran, Saudi Arabia, including 200 women with prior cesarean section and planned for labor induction. Participants were randomly allocated into two groups. Group A (n = 100) received dinoprostone 1.5 mg vaginal tablet. Group B (n = 100) received 10 mg dinoprostone sustained release vaginal pessary. Primary outcome was vaginal delivery rate. Secondary outcomes included maternal and neonatal outcomes. Results: The dinoprostone vaginal tablet and dinoprostone vaginal pessary had a comparable vaginal delivery rate (67% and 64%, respectively;p = 0.78). The median patient satisfaction with the birth process was superior in the dinoprostone vaginal pessary group (p = 0.04). Maternal and neonatal outcomes were similar in both groups. Conclusion: Both forms of dinoprostone were effective methods for labor induction in women with prior cesarean section. However, the patient satisfaction with the birth process was in favor of the dinoprostone sustained release vaginal pessary.

With the full-speed development of economy,the new urbanization has been accelerated in Guizhou Province in order to make the people reach a well-off standard of living by 2020. From the whole society,the key point is the development of the rural economy,and the development of the rural economy depends on the economic development of rural households. The income from rural labor force transfer has become the main income source and accounts for more than half of rural household income,which will play a major role in survival and development of rural households.

Prolonged Obstructed labor Is an Uncommon Presentation of a Giant Bladder Calculus: A Case Report and Literature Review

Prolonged obstructed labor is a critical intrapartum emergency. This intricacy is, however, unusual as a primary presentation of a giant bladder calculus. We report a case of a 25-year-old Para 6 + 1, who presented with a history of labor pains of 72 hours duration. She had background lower urinary tract symptoms with supra-pubic swelling and hematuria of 2 years duration. She had a single antenatal visit at 32 weeks however;no obstetric ultrasound scan was done before she went into labor. Examination revealed a highly placed fetal head with increase fetal heart rate. There was a hard mass bulging under the anterior vaginal wall. The diagnosis was a prolonged obstructed labor secondary to bladder mass with fetal distress was made. She had a caesarean section (CS) with the delivery of fresh stillborn. The bi-manually palpable bladder mass persisted post-CS. Her abdominopelvic ultrasound scan and abdominal X-ray post-CS revealed giant vesical calculus with obstructive uropathy. Urine microscopy culture and sensitivity yielded E. coli sensitive to Ciprofloxacin;other laboratory investigations were normal. She was treated for UTI and 2 weeks post-CS, she had opened cystolithotomy. The stone weighed 536 g. Her postoperative recovery was uneventful.

Role of Vaginal Progesterone in Prevention of Preterm labor in Women with Previous History of One or More Previous Preterm Births

Objective: To evaluate whether prophylactic administration of 200 mg vaginal progesterone can reduce the incidence of preterm birth in women with documented history of preterm birth Setting: Obstetrics and Gynecology Department, Zagazig University Hospital, Egypt. Methods: Ninety patients with previous history of preterm birth prior to 37 week presenting with singleton pregnancy between 20 - 24 weeks were randomly allocated to receive either the progesterone 200 mg vaginal suppository or no treatment. Results: The incidence of preterm labor before 37 weeks of gestation was significantly lower in the study group than in the control group (22.2% vs. 53.3%) especially in earlier gestational ages. While, the mean birth weight was significantly higher in the study group than in control group (2872.67 ± 565.76 gm vs. 2487.78 ± 742.40 gm). The neonatal morbidities and mortality associated with preterm labor were significantly lower in the study group than in the control group as shown by lower incidence of neonatal RDS (13.3% vs. 31.1%;P = 0.043) and lower incidence of the need for NICU admission (15.6% vs. 35.5%;P = 0.03). Conclusion: Administration of prophylactic vaginal progesterone (200 mg, daily) can significantly reduce the rate of preterm birth before 37, 32 and 28 wks of gestation among women with previous spontaneous preterm birth. In addition, the rates of RDS and admission to NICU were significantly decreased among infants of women assigned to progesterone treatment. Also, there was an additional benefit of vaginal progesterone for prevention of preterm birth in women who had prior spontaneous preterm birth and cervical length 25 mm.

Knowledge of Fathers’ Support during labor: From Arabic Fathers’ Perspective

Aim: The aim of this study was to assess the kind of labor support Arab fathers could offer their wives. Design and Method: An interpretative phenomenological approach was used to reveal the meaning attributed by Arabic fathers regarding their support during labor. Individual, semi-structured interviews were conducted with 19 Arabic fathers. The analysis was done by using Smith’s model of interpretive phenomenological analysis (IPA) together with software package NVivo7. Findings: The analysis of the interviews illustrated the types of labor support offered by Arabic fathers to their wives. Three main themes emerged from the data: physical support, emotional support, and spiritual support. Conclusion: The support offered by Arabic fathers was in the minimal degree and missing practical support. Socio cultural issues and health policies prevented fathers from playing an active role during labor. There is a need to develop policies and regulations that enhance more inclusion of fathers in maternity services including the labor room.

The Dialectical Basis and Ecological Implication of Marx’s labor Value Theory

Left-wing ecologists generally think that Marx’s theory of labor value is completely opposite to ecology because it ascribes the substance of value to labor.The contemporary Marxist theorists regard this as the lack of true knowledge of the dialectical basis of labor theory of value.Dialectics is a method of studying things in a holistic and relational way,while the method of value analysis is the result of Marx’s study of capitalist mode of production in a holistic and relational way.According to the method of value analysis,the unity of opposites among value,exchange value,and use value constitutes the fundamental contradiction of capitalist mode of production,which inevitably leads to the adversarial relationship of capitalist value form.Marx constantly ascended from abstract to concrete,from part to whole,and then,the meaning of value analysis expanded to the whole material world,thus,the ecological dimension of labor value theory was completely expanded.

The “Glocal” Dynamics of Construction labor and Digital Architecture: Preston Scott Cohen’s Addition to the Tel Aviv Museum as Case Study

Globalization and technological change are transforming the ways in which buildings are being designed and built. An overlooked aspect of this development is its impact on construction labor, and the significance of labor for assessing the work of architecture. The paper draws upon the sociological concept of the “glocal” to analyse the construction site as the product of the tension between global and local conditions of architectural production and consumption. The construction of Preston Scott Cohen’s 2010 Tel Aviv Museum Addition serves as its case study for theorizing the “glocal” dynamics of digital architecture, building technology and construction labor. This methodological approach highlights the role of migrant guest workers and technological transfer in contemporary construction culture. To realize with precision the complex design under local constraints, the contractor developed a hybrid work process that interspersed labor saving automated manufacturing techniques with artisanal, skilled construction work. In addition, workers and contractors exercised a high level of control over the pace and method of construction, and devised building solutions which improved upon the architect’s design. In conclusion, the paper argues that construction activity differs from broader trends in manufacturing due to the self-reflexivity of architectural design to its condition of production, but that at the same time, this critical capacity is enabled by the globalization of construction labor.

Differing Effects of Intra-Product International Division of labor on Technological Progress:Comparing Proactive Participation with Passive Inclusion

This paper has identified two models of intra-product international division of labor:the model of passive inclusion and the model of proactive participation.Their difference is essentially whether participation in international division of labor is dominated by multinational firms or by local firms.At the level of representative sectors,some characteristic variables of participation in international division of labor demonstrate significant industry differences.Models of division of labor affect technology progress.According to the empirical analysis result at the micro-level of four sectors,no matter for export or the contribution of learning by doing to corporate technology progress,the model of proactive participation is superior to the model of passive inclusion;under the model of passive inclusion,the technology spillover effect of import is smaller than "learning by doing " effect;under the model of proactive participation,technology spillover effect is over two times the "learning by doing" effect.

China's Uneven labor Flow and Its Effect on Regional Disparities—Empirical analysis based on non-parameter additive model

This paper is the first to use a non-parameter additive model in the empirical analysis of the impact of cross-regional laborflow on industrialization and economic growth in China’s eastern,central and western regions.Results indicate that laborflow has significantly different effects on the direction and intensity of impact on economic development across regions:massive labor inflow from the central and western regions to the eastern region significantly promoted the industrializationprocess and economic growth in the East,while the impact of labor outflow from the central and western regions on theregional economies is in a significantly inverted-U shaped non-linear relationship.Uneven labor flow across regions haswidened China’s regional economic disparities.

The Law of the People's Republic of China on Employment Contracts has stoked wide concern across China.One of the focuses of debate is on termination costs.Utilizing an enterprise attitude survey,this article discusses the impact of the law on termination costs and employment behavior of China's enterprises.The research shows that the law has different impacts on enterprises of varied ownerships and scales.The law,while giving rise to higher termination costs for enterprises,urges them to take diversified measures to adjust their human resources.
