Induction of labor with PGE2 after One Previous Cesarean Section: 18 Years Experience in a University Hospital

Objective: To review the outcome of prostaglandin induction of labor in term pregnant women with previous one ce-sarean section compared to those without previous Cesarean section. Design: 18 years retrospective review of hospital records and case note review of index cases. Setting: University hospital. Population: Three hundred and twenty two women who had their labor induced with prostaglandin E2. One hundred and sixty one women had one previous Ce-sarean section. Methods: This study was conducted at King Fahad University Hospital, University of Dammam. It is a tertiary referral center with approximately 2300 births per year. We searched the hospital's records of deliveries from January 1992 to December 2009 and reviewed all indications and outcomes of prostaglandin induction of labor in women with one previous Cesarean section. The control group was composed of women who had their labor induced with prostaglandin but without previous Cesarean section. Main outcome measures: labor outcome and uterine rup-ture Results: Three hundred and twenty two women were included. All received prostaglandin E2 for induction of labor. One hundred and sixty one women had one previous Cesarean section (study group) and the rest had no previous Ce-sarean section (control group). There was no difference in the rate of vaginal delivery between study and control group, 68.3% and 79.5% (p value 0.3), respectively. The rate of uterine rupture was 30 times higher in study group (2.5% Vs 0.033%). Conclusion: In women with one previous Cesarean section, induction of labor with prostaglandin leads to comparable rate of vaginal delivery similar to those without prior Cesarean section but with relatively high risk of uter-ine rupture.

Maternal and perinatal outcomes of induction of labor at term in the university clinics of Kinshasa, DR Congo

Objective: This paper aims at assessing outcomes following induction of labor and characteristics likely to predict vaginal delivery. Study design: This is a descriptive retrospective cohort study including all women with singleton pregnancies who delivered at term in the university clinics of Kinshasa, DR Congo, from January 01, 2006 until December 31, 2010. Induction was initiated regardless of cervical status. Methods of induction included: oxytocin perfusion, vaginal Misoprostol, intracervical insertion of the Foley catheter and amniotomy. Results of induction were compared in terms of failure of labor, cesarean section, fetal distress, and neonatal distress. Logistic regression was used to seek for independent contributing factors for adverse outcomes. Results: During the period of the study 115 patients at term (3.2%) were concerned with induction of labor. Means for maternal age, gestational age and weight at confinement were 30.5 ± 5.7 years, 37.95 ± 1.54 weeks and 69.3 ± 15.1 kg, respectively. The mean parity and gravidity were 2.4 ± 1.9 and 2.9 ± 1.9, respectively. The mean Bishop score was 6.2 ± 1.5 at the first induction, with 66 women (57.3%) having less than 7. Indications for induction were: preeclampsia (52 = 54.1%), premature rupture of membranes (34 = 29.5%), post term (17 = 14.6%), gestational diabetes (5 = 4.3%), stillbirth (5 = 4.3%), polyhydramnios (3 = 2.6%) and cardiopathy (1 = 0.8%). Methods of induction at the first attempt included: oxytocin (86 = 74.7%), vaginal misoprostol (20 = 17.3%), transcervical Foley catheter balloon (14 = 12.1%), and amniotomy (1 = 0.8%). Failure to induce uterine contraction at the first attempt was noted in 9/115 (7.8%) women. Vaginal delivery occurred in 78 (66.9%) women, and cesarean section in 34 (29.6%). The majority of cesarean sections were performed at the primary induction, most of them (29/34 = 85.3%) in women with bad Bishop score. Granger Causality Test of Urban-rural Income Gap and Employment of Rural labor Forces——A Case Study of Shouguang City

Taking Shouguang City in Shandong Province as an example, this article researches the mutual relationship between urban-rural income and employment of rural labor forces, and conducting cointegration test and Granger causality test on the variable sequences using statistical data. The results show that the average annual income of rural labor forces influences the income per capita of rural residents, and there is a Granger causality relationship between the income per capita of rural residents and urban employees' average wage. Engaging in production concerning agriculture, forestry, animal husbandry and fishery makes the total income per capita in rural areas of Shouguang City higher than the average wage of urban residents in Shouguang City, even in Weifang City. The comparative advantage in terms of income causes the city to loose attraction to rural residents in Shouguang City. The comparative advantage drives more rural labor forces in Shouguang City to engage in primary industry, thereby greatly reducing the pressure of employment and transfer of rural labor forces. Finally important following measures are put forward to promote employment and on-the-spot transfer of rural labor forces: vigorously propel agricultural industrialization; increase rural residents' income; improve the living conditions for rural residents.

The New Explanation of Market Equilibrium Based on Theory of labor Value

There is market equilibrium when the demand willingness is consistent with the supply willingness. From the individual preferences and budget constraints,Western Economics deduced demand curve,causing the traditional demand willingness to be built on the basis of subjective utility. From the relative labor time of economic entity,this paper discusses the relationship between labor time and price readiness,demand curve or supply curve,and establishes the market equilibrium model based on labor value. In a perfectly competitive environment,the equilibrium exchange price of the model is the ratio of labor value between two commodities,and combines demand and supply sides' recognition of commodity quantity of labor,reflecting the socially necessary labor time contained in commodities.

Effects of Ropivacaine-sufentanil Epidural Analgesia on labor and Maternal and Neonatal Outcomes

[Objectives]This study aimed to investigate the effects of ropivacaine-sufentanil epidural analgesia on labor and maternal and neonatal outcomes.[Methods]A total of 180 primiparas in full-term pregnancy were selected.They were randomly divided into treatment group(n=90)and control group(n=90).The primiparas in the treatment group were injected epidurally with ropivacaine and sufentanil for analgesia,and the primiparas in the control group were subjected to vaginal delivery.The VAS scores at 5,10,30 and 60 min of analgesia were observed.The vaginal bleeding amount,total labor duration,neonatal Apgar score and vaginal delivery rate of the two groups were compared.[Results]Compared with the control group,the VAS score in the treatment group differed insignificantly after 5 min of analgesia(P>0.05),and decreased significantly after 10,30 and 60 min of analgesia(P0.05).In the treatment group,the vaginal delivery rate increased(Plabor was prolonged(Plabor did not change significantly(P>0.05).[Conclusions]Epidural analgesia with ropivacaine and sufentanil has a good analgesic effect and good safety,and is worthy of clinical promotion.

The Impact of Quality of labor on Farmers' Agricultural Production Benefits:A Case Study of Anyang City

In recent years,the Central Document No.1 has been all about the issues concerning agriculture,countryside and farmers,and especially the Central Document No.1 in 2013 highlighted the improvement of quality of the rural labor force to develop modern agriculture.The quality of labor is related to the improvement of production efficiency,and farmers' agricultural production benefits are related to the issues concerning agriculture,countryside and farmers,ultimately affecting the development process of modern agriculture.On the theoretical basis of human capital,this article carries out empirical analysis,calculates the farmers' agricultural production benefits,and analyzes the specific impact of quality of labor on farmers' agricultural production benefits.Finally,some recommendations are put forth to increase farmers' income.

Historical Reflection on Deviation in Transfer Policy of Agricultural Surplus labor at Early Stage of China

This paper firstly made an analysis on transfer of agricultural surplus labor in the end of the 20 th century. On this basis,it made reflection on policy of agricultural surplus labor. Then,it stated that there is a great deviation of transfer of agricultural surplus labor from practical requirement and pointed out basic internationally recognized ideas of agricultural surplus labor transfer. Finally,it came up with recommendations for formulating agricultural surplus labor transfer.

Uterine Rupture in an Unscarred Uterus Following Induced labor with Titrated Oral Misoprostol

Misoprostol (Cytotec, Pfizer) is a prostaglandin E1 analogue widely used in labor induction. Understanding of potential risks of misoprostol in labor induction is lacking as the drug is used off-label in most countries. A 35-year-old G3P2 woman with post-date pregnancy at 40 + 3 weeks was induced using titrated oral misoprostol. Following delivery, post-partum hemorrhage was noted and, upon discovery of a uterine wall rupture due to congenital weakness of the myometrium, emergency abdominal total hysterectomy was conducted. Following surgery, the patient made a full recovery and was discharged. The main contraindications for misoprostol in labor induction are a scarred uterus or multipara greater than 4. Uterine wall weakness should be considered as a potential contraindication. Extreme vigilance is required in use of misoprostol for labor induction.
