Sex differences in injury rates in team-sport athletes:A systematic review and meta-regression analysis

Background:Team-sport players have a particularly high injury risk.Although female sex is considered a risk factor,it is still unknown whether female and male team-sport players,in fact,differ in their injury rates.We aimed to compare injury rates between female and male players by systematically reviewing and meta-analyzing injury surveillance studies of both sexes in order to evaluate sex-specific differences in team-sport injuries.Methods:Studies that prospectively collected injury data for high-level female and male players(age≥16 years)in basketball,field hockey,football(soccer),handball,rugby(union and sevens),and volleyball were included.Two reviewers(AZ and ALR)independently assessed study quality and extracted data for overall,match,training,and severe injuries(>28 days’time loss)as well as data regarding injury locations and types.Incidence rate ratios(IRRs)were pooled in a meta-analysis,and meta-regression analysis was performed when 10 or more studies were available.Results:Of 20 studies,9 studies reported injury data from football,3 studies from rugby,3 studies from handball,1 study from basketball,1 study from field hockey,2 studies from volleyball,and 1 study from basketball and field hockey.For overall injuries,the pooled IRR=0.86(95%confidence interval(95%CI):0.76-0.98)indicated significantly more injuries in male than in female players.For injury location,the pooled IRR showed higher injury rates in male athletes than in female athletes for upper extremity,hip/groin,thigh,ande foot injuries.Female players had a significantly higher rate of anterior cruciate ligament injuries(IRR=2.15,95%CI:1.27-3.62)than male players.No significant sex-specific differences in IRR were found for match,training,severe injuries,concussions,or ankle sprains.Conclusion:Our meta-analysis provides evidence for sex-specific differences in the injury rates in team sports.Further epidemiological studies including both sexes in sports other than football are needed in order to strengthen the evidence.

How to deal with severe spinal cord injuries(SCI)?This question remains an everlasting challenge for neurologists.Recent innovative therapies such as neuromodulation,which targets the surviving tissue,have shown the potential to restore cognitive functions in patients with severe SCI after rehabilitation[1,2].These therapies might enable SCI patients to sustain active movements during rehabilitation training to enhance the reorganization of the neuronal pathways,however,the complete recovery of body movement functions was still not reached with these therapies.A characteristic feature of SCI is the impairment of neural information communication between the neurons in the spinal cord and the brain,which are responsible for regulating body movement.Brain-computer interfaces(BCIs)have been successfully applied in dealing with movement dysfunction by establishing a direct connection between brain cortical activity and electrical muscle stimulation,thereby restoring limb function in cases of paralysis[2].

Traumatic injuries range from simple to complicated multiple injuries.The identification of patients with critical injuries and disrupted organ perfusion is essential to prevent tissue hypoxia.A study estimated that a fast and accurate response to thoracic trauma should reduce the mortality by 30%.[1]The tissue perfusion value is an essential indicator of mortality and morbidity for patients admitted to the hospital with thoracic trauma.Thus,this study aimed to show the role of the perfusion index(PI)in predicting the prognosis of patients admitted to the emergency department for thoracic trauma.

Understanding pediatric surgical needs in North Korea: a modeling analysis

Background Sixty-five percent of children worldwide lack access to surgical care,the majority of whom live in low-income and middle-income countries(LMiCs).Developing surgical infrastructure requires information on surgical need;however,this information is often limited in LMICs.North Korea(Democratic People's Republic of Korea,DPRK)has a low amount of publicly available data.Here,we analyzed available modeled data to understand the causes of pediatric deaths due to conditions treatable with surgery in DPRK.Methods We used World Bank data and models from the Institute for Health Metrics and Evaluation to identify causes of pediatric deaths affecting pediatric patients(ageinjuries(53.9%),congenital conditions(34.2%),tumors(8.8%),and abdominal conditions(3.2%).DPRK has a lower relative rate of pediatric deaths compared with other LMICs with similar gross domestic product per capita.However,it has a higher relative rate of pediatric deaths due to conditions requiring treatment with surgery.Transport injuries contribute significantly to the high rate of pediatric deaths in DPRK.Conclusions Although DPRK may be allocating overall resources toward pediatric healthcare more efficiently than economic peers,DPRK may benefit from improvement in pediatric surgical capacity.Improved availability of data and close international collaboration could be potential solutions to bridge this gap.

Effect of cOVID-19 pneumonia infection control protocols on nosocomial infection incidence in trauma patients

Purpose:Nosocomial infection is a major threat to the health care system and patient welfare.After the pandemic,new protocols were established in hospitals and communities to protect against the transmission of cOvID-19,which may have changed the incidence of nosocomial transmission.This study was conducted to compare the incidence of nosocomial infection before and after the covID-19 pandemic.Methods:This was a retrospective cohort study performed on trauma patients who were admitted,from May 22,2018 to November 22,2021,to the largest level-1 trauma center in Shiraz,Iran(Shahid Rajaei Trauma Hospital).All the trauma patients over 15 years old admitted during the study time were included in this study.Individuals who were declared dead upon arrival were excluded.Patients were evaluated in 2 periods:before the pandemic(May 22,2018-February 19,2020)and after the pandemic(February 19,2020-November 22,2021).Patients were assessed based on demographic information(age,gender,length of hospital stay,and patient outcome),the occurrence of hospital infection,and the type of infection.The analysis was done using SPSS version 25.Results:Overall,60,561 patients were admitted,with a mean age of 40 years.Nosocomial infection was diagnosed in 4.00%(n=2423)of all admitted patients.The incidence rate of post-COVID-19 hospitalacquired infections decreased by 16.28%(p<0.001)when compared to before the pandemic;in contrast,surgical site infection(p<0.001)and urinary tract infection(p=0.043)were responsible for this change,while hospital-acquired pneumonia(p=0.568)and bloodstream infection(p=0.156)were not significantly different.Overall mortality was 1.79%,while 28.52%of all patients with nosocomial infections died.During the pandemic,there was a 25.78%increase(p<0.001)in the overall incidence rate of mortality,which was also observed among patients with nosocomial infections(17.84%).Conclusion:The incidence of nosocomial infection has decreased during the pandemic,possibly due to the use of more personal protective equipment and mo

The importance of performing an early primary survey in‘silver trauma’patients in the detection of injuries has been well documented in reducing the associated morbidity and mortality.[1]In the past,when whole body computed tomography(WBCT)was not commonly available,following initial examination patients would undergo chest radiograph and pelvic X-ray and then proceed to selective computed tomography(CT).With the relatively widespread availability of CT,the use of these X-rays has diminished.The reliance on clinical examination alone for the detection of underlying injuries is another matter of debate.

Dear Editor,Stem cells,including pluripotent stem cells and adult stem cells,possess the remarkable capability of being able to selfrenew while at the same time having potential to differentiate into different cell lineages and functionally distinct cell types.Human embryonic stem cells(hESCs)can differentiate into all adult stem cell types,including human mesenchymal stem cells(hMSCs)and human neural stem cells(hNSCs),but can also give rise to all terminally differentiated cell types(Wang et al.,2021a).Through the continuous replenishment of differentiated cells,stem cells support tissue homeostasis and respond to tissue injuries.Given the promising applications of stem cells in cell therapy and regenerative medicine,insights into molecular events underlying stem cell maintenance,self-renewal ability and pluripotency,continue to garner strong interest(Shan et al.,2021).Although metabolic pathways have been implicated in the reciprocal regulations of stem cell self-renewal and differentiation as well as organ homeostatic maintenance(Garcia-Prat et al.,2017),central aspects of how metabolic requirements differ and are regulated across the various types of human stem cells in our body remain enigmatic.

Despite the spontaneous regenerative capacity of the peripheral nervous system,clinical nerve repair often results in poor functional recovery with the high socio-economic burden.In presence of peripheral nerve injuries and when distal nerve transfers are not possible,nerve autografts,are the solution of choice to cross a critical nerve gap.However,these are associated with donor site morbidity and offer only suboptimal functional recovery.Thus,the need to improve the rate of effective regeneration has directed research attention towards stem cell therapy and nerve tissue engineering.
