While injuries of the upper extremity are widely discussed in rock climbers, reports about the lower extremity are rare. Nevertheless almost 50 percent of acute injuries involve the leg and feet. Acute injuries are either caused by ground falls or rock hit trauma during a fall. Most frequently strains, contusions and fractures of the calcaneus and talus. More rare injuries, as e.g., osteochondral lesions of the talus demand a highly specialized care and case presentations with combined iliac crest graft and matrix associated autologous chondrocyte transplantation are given in this review. The chronic use of tight climbing shoes leads to overstrain injuries also. As the tight fit of the shoes changes the biomechanics of the foot an increased stress load is applied to the fore-foot. Thus chronic conditions as subungual hematoma, callosity and pain resolve. Also a high incidence of hallux valgus and hallux rigidus is described.

The landmark Convention on the Rights of the Child states that children around the World have right to safe environment. In Belgium, as in many countries, children spend, on average, 4.5 weekdays at school, during nearly 8 hours per day. Studies have shown that, the risk of school-related injury exceeds the risk during leisure time. Literature reports that school accidents account for 10 to 30% of all accidents among pupils. Despite that, few papers treat of the school-related injuries. Consequently, based on 1540 accidents forms from an insurance company, injury places, mechanisms involved, body parts injured and nature of injuries were described. Head injuries, upper and lower limbs injuries were investigated in more details. Pearson’s chi-square test was used to assess the relationships between the variables and multivariate logistic regression models were used to study the three specific types of injuries cited above. Gender ratio (M/F) was equal to 1.6 with 25.9% of children under 6 years, 29.4% of 6 - 9 years, 26.9% of 10 - 12 years and 17.8% from 13 years or more. The major places of injuries were the playground (56.9%) and the physical education (19.7%). Falls were observed in 52.1% of cases and contacts were reported in 24.3%. The head injuries account for 40.6%, the upper limbs for 32.0% and the lower limbs injuries for 20.2%. The bruises and the scratches were observed in 23.8% and the wounds in 21.5%. Fractures were reported in 16.1%. In conclusion, having a routine access to the data from the insurance companies could be an important source of information for an injury surveillance system in which the school injuries will be included. Taking into account this data will require an awareness of all the concerned persons about the relevance of such a system and a harmonization of the accidental forms.

Penetrating Abdominal injuries: Pattern and Outcome of Management in Khartoum

Background: The pattern and presentation of penetrating abdominal trauma vary according to places and the structure of the present health system. As well controversies in management exist ranging from mandatory exploration to selective non-operative management. Objectives: To determine the pattern of penetrating abdominal trauma, current management practiced and outcome in Khartoum. Patients and Methods: The study was prospective, descriptive and hospital-based. It was carried out at the main three hospitals in Khartoum State. It was conducted over a period of one year from 2012 Mar to 2013 Mar. All patients who presented penetrating injury to their abdomen were included. Results: The study included 85 patients with a mean age of 28 years (SD ± 10). The male to female ratio was 11:1. Most of the patients (89.4%) were in the first four decades of their life. Twenty-three patients (27.1%) presented shock. Stab wound is the commonest mode of trauma seen in 83.5% of our patients. The majority of our patients were managed by exploratory laparotomy (81.2%), however 16 (18.8%) underwent conservative measures. Of the operated group, solid organ injuries were found in 22.9%, yet hollow viscous injuries were reported in 86.9% of the patients. Registrars operated on 78.26% of the patients. The rate of negative laparotomy of this study was 8.7%. Complications and mortality were encountered in 25.9% and 4.7% respectively. The mean hospital stay was 8.47 days (SD ± 10.6). Conclusion: This study demonstrates no difference in the pattern of intra-abdominal injuries. The rate of operative treatment is acceptable, but more laparotomies can be avoided if the haemodynamic stable patients without features of peritonitis were given a period of observation. The overall outcome was satisfactory.

Sir,Fingertip amputation injuries compromise the sensation,function,and appearance of the hand.They are often exquisitely painful and take a substantial amount of time to heal,causing significant functional disability.Conservative management of these injuries may result in decreased range of motion,hypo-or hyperesthesia,loss of pulp contour,scarring,and other long-term sequelae.[1,2]In addition,they may prolong patient discomfort with dressing changes,hindering adherence with wound therapy regimens,especially in the presence of multiple finger injuries.

Nerve injuries caused by medical interventions(iatrogenic lesions)can complicate procedures and affect any part of the peripheral nervous system.Available data are fragmentary and little information is accessible on the overall incidence of iatrogenic nerve lesions that ranges from 1.5%to 15%.[1,2]Major drawbacks are the limited number of patients studied and the incomplete and subjective assessment of nerve function.The potential for iatrogenic injuries in the course of any surgical procedure should be thoroughly appreciated by all surgeons and they should be familiar with early diagnostic steps for detecting these lesions.The importance of prompt diagnosis and adequate treatment of iatrogenic nerve injuries for optimal functional recovery should be stressed.Excellent results can be obtained if certain diagnostic and surgical principles are followed.

The objective of this study was to test the hypothesis that a relation exists between slight atmospheric pressure fluctuations (APF) in the far infrasound frequency range and daily number of emergency transport events due to suicidal injuries (EESU). The regression models to compare groups were used to assess the relation of EESU to the mean daily amplitude of APF (APF-A) and to the ratio of APF amplitude averaged over the daytime to the APF amplitude averaged over the nighttime (Rdn). To eliminate the confounding effects of basic meteorological parameters and annual trend in EESU, the non- parametric smoothing method was used in a stepwise manner. The low APF-A (95% CI = 1.06- 1.16 Pa) compared to their common middle levels and the high (95% CI = 3.18-3.64 Pa), as well the low Rdn (СI = 0.83-0.92) and very high Rdn (CI = 3.05-3.77) compared to their more closed to common regular values (СI = 1.69-1.90) turned out to be more beneficial factors promoting the decrease in the incidence of EESU. We suppose that more attention needs to be paid to the meteorotropic effects of APF on certain kinds of psychopathology resulting in suicidal behaviour, and further investigations in different geograph- ical and climatic conditions, especially in those with more intense atmospheric perturbations, are necessary.

Dear editor Health care workers are generally predisposed to injuries from sharps as a health hazard.This is more pronounced among waste handlers.Exposure to medical waste in hospitals and health facilities results in injuries and contagious diseases.Therefore,health workers,including nurses,doctors,technicians,and hospital cleaning workers,are always alerted to exercise the utmost caution and not to be exposed to the waste produced by the patient,especially through cuts or scrapes from the patient’s sharp injuries,touching or inhalation or exposure to spills or splashes of liquids from medical waste or any waste resulting from patient care or contact with his body fluids[1].
