Correlation between the Changes in Lung Function and Lung Density Changes in Patients Following Radio- (Chemo-) Therapy for Thoracic Carcinomas

Purpose: In this analysis we focused on the correlation of patients’ lung function (PFT) data and lung density changes (ΔHU) detected in follow-up CTs. Material and Methods: PFT and lung function data were available for 58 patients 12 weeks and 47 patients 6 months after radio- (chemo-) therapy for thoracic carcinomas (NSCLC, SCLC and esophageal carcinoma). The follow-up CT scans were matched with the planning CT scans of each patient and then subtracted to calculate ΔHU for each voxel using customized research software. PFT data regarding e.g. vital capacity (VC), total lung capacity (TLC) and diffusion capacity for carbon monoxide (DLCO) were collected before and at several follow-up appointments after treatment. Results: 12 weeks after therapy there was a statistically significant correlation between difference in DLCO and the maximum ΔHU as well as the difference in TLC and the minimum ΔHU. 6 months after treatment there was a significant correlation between the difference in VC and DLCO with numerous lung density parameters, e.g. the mean and median lung density changes and the 75th percentile of ΔHU. There was no significant correlation between the PFT parameters FEV1, pCO2 and pO2 and any lung density parameter at any follow-up appointment. Conclusion: There is a significant correlation between DLCO and ΔHU 6 months after treatment that most likely reflects the underlying pathological mechanisms in terms of the development of fibrotic lung tissue after RT. The relevance of the significant correlations 12 weeks after RT is questionable.

Effect of cyproheptadine on changes of 5-HT and 5-HIAA levels in brain cortex and serum following rat brain injury

Changes of 5-hydroxytryptamine(5-HT)and 5-hydroxyindoleacetic acid(5-HIAA)levels in brain cortex and serum were studied using spectrofluorometer follow-ing brain injury in rats.Cyproheptadine,a receptor antagonist of 5-HT,was employedand its effect on alterations of 5-HT and 5-HIAA contents in brain cortex and serum af-ter brain injury was investigated.The results showed that after brain injury levels of5-HT and 5-HIAA in the brain cortex increased markedly,The increase of 5-HT in thebrain cortex reached its peak at 48h after brain injury,and its value was 4.29±0.44μg/g,which was 4.8 times of control value.The increase of 5-HIAA in the brain cortexreached its peak at 24h after brain injury,and its value was 5.48±0.41μg/g,which was5.8times of control value.The change of 5-HT in the serum was not significant,but thelevel of 5-HIAA in the serum was increased significantly.Cyproheptadine could reducelevels of 5-HT and 5-HIAA in this experiment.The value of 5-HT in the brain cortexdecreased to 1.32±0.09μg/g,and the value of 5-HIAA decreased to 1.36±0.10μg/g.Thelevel of 5-HIAA in the serum was also reduced significantly.It was 0.27±0.03μg/ml.

A Short Term Review of Operated Cases in the Plastic Surgery Unit at the Komfo Anokye Teaching Hospital, Kumasi, Ghana

Introduction: Surgical problems are of much disturbance to the world and should therefore be given serious attention. The prevalence of these surgical problems, has made plastic surgery become a broadly relevant and acceptable way for addressing problems like injuries, congenital anomalies, surgical infections and malignancies among others. Aim: This study is to quantify and characterize surgical procedures done in the plastic surgery theatre located in the new Accident and Emergency (A & E) Building of KATH. Materials and Methods: Data were obtained from the Operation Register/Theatre Books in the plastic surgery theatre at the A & E Centre on cases operated on from October 1, 2009 to September 30, 2012. Data entry, presentation and analysis were done using Statistical Package for the Social Sciences (SPSS) 20.0 version. Results: Adults formed the majority of patients who sought for plastic surgery with a percentage of 70.3%. The male patients also outnumbered the females recording (61.5%) out of the total number of patients. Most of the cases recorded were acquired cases (93.2%). Reconstructive surgery was the commonest operation performed (30%);in 53.8% cases general anaesthesia was used. Conclusion: Among all the procedures used reconstructive surgery was the commonest surgery performed in the unit and general anaesthesia was the most type of anaesthesia used for the operations.

Musculoskeletal wounds characteristic of the Second Lebanon War

The Second Lebanon War between Hezbollah and Israel broke out on July 12, 2006, and lasted until August 14, 2006. Out of the wounded Israeli soldiers, 128 were treated at the orthopedic departments of the Rambam Medical Center, Haifa, Israel. Aretrospective study was carried out on these 128 combatants. The objective of the study was to characterize musculoskeletal combat wounds with regard to age, type, location, mechanism, side and intensity. The average age of an injured soldier was 25.7 years. Most injuries (63.9%) were caused by shrapnel. The prevalence of injury to the lower limb was much higher compared with upper limb (43.4% vs. 28.3%). There was no side preference. A considerable number of soldiers had more than one injury (60%). The majority of wounds that occurred in the Second Lebanon War were orthopedic-related, particularly involving the upper and lower extremities. Fragmenting weapons were responsible for most injuries.

Satellites Provided Services for Earthquakes in Nepal and Tibet

Radiographic measurement of morphological abnormalities in thoracolumbar burst fractures: relationship with spinal cord lesion

Objective:To investigate the relationship between morphological abnormalities and spinal cord deficit in thoracolumbar burst fractures. Methods: Seventy-eight patients with thoracolumbar burst fractures were retrospectively reviewed to calculate the stenotic ratio of spinal canal based on the midsagittal diameters and the hyphosis angle according to Cobb. The ASIA scoring of motor function of lower extremities was recorded . Results: The differences (P > 0.05) of the stenotic ratio of spinal canal and the kyphosis angle were not significant between patients without neurological deficit, with incomplete and complete lesions. No significant correlation(P > 0.05) between the stenotic ratio of spinal canal and the kyphosis angle, and ASIA scoring was noted. Conclusion:The severity of spinal cord injuries in thoracolumbar burst fractures is not predicted according to the percentage of canal stenosis or the degree of kyphesis induced by thoracolumbar burst fractures.

Role of Multidetector CT in Evaluation of Polytrauma Patients

Trauma is the leading cause of death in age group less than 45 years. In trauma, time is one of the most crucial factors in predicting prognosis. Outcomes are greatly improved when critical interventions are provided within the golden hour following injury. Whole body multidetector computerised tomography (WBMDCT) can decrease this critical time and increase survival. Emergency CT plays a major role in diagnostic workflow in the evaluation of patients with polytrauma. MDCT scanners are widely used because they rapidly produce high-resolution scans of large areas, offering short examination times for multiple body regions under emergency conditions. Such examinations most often include the head, cervical spine, and thorax to pelvis. Role of MDCT in head trauma remains unparalleled for reasons like its widespread availability and capability to reveal skull fractures, primarily because it is a fast and efficient method to triage the patients with treatable conditions mandating urgent surgical intervention such as extra axial hematomas, mass effect, herniations of brain, hydrocephalus, and midline shift and hence preventing secondary brain injury. MDCT angiography is an important tool to screen patients with suspected vascular injury.
