Aims: Establish profiles of young “at risk” of injuries, first, on an overall point of view and, secondly, for some types of injuries (sport, home, road traffic, school and work injuries). Methods: We have taken nearly 50 variables into consideration: 17 variables for construction of the socioeconomic status, 9 variables for the investigation of symptoms, 11 concerning drugs, 5 for healthy habits, 3 for investigating the violence behavior, 4 concerning the school, 3 for subjective health and finally 3 for social network. We have used the principal component analysis, the multiple correspondence analysis and the weighted-frequency score for reducing the number of them. After these reductions, 15 variables were available for analyses. The relationship between injuries and investigated factors was assessed using the Pearson’s chi-square test. We also calculated odds ratio (OR) with their 95% confidence intervals (95%CI) to estimate the strengths of the associations. To further assess these relationships but also for taking into account the potentials confounding effects of some variables, logistic regression model and multinomial logistic regression model were applied. Results: The whole injury prevalence was equal to 45.6% and among the injured, the proportion of the several types was equal to 33.8% for sport injuries, 32.2% for home injuries, 16.6% for traffic injuries, 11.6% for school injuries and 5.7% for work injuries. We can say that, in light of the variables studied, the “at risk” profile for having reported an injury is being a boy, being younger, having drug experiences, with the violent profile, and declaring several symptoms. There are no consistent and marked deviations in this study from the findings obtained in previous studies. Conclusion: Analyzing injuries in general is interesting but for preventing them it is important to know in which activities children and students are engaged when they are injured.

Professional’s skills in assessment of perineal tears after childbirth—A systematic review

Perineal tears are one of the most common complications of vaginal births and may cause discomfort and pain long time after childbirth. Visual and digital examination of perineal tears is the most common way to assess and classify a perineal tear. Recent research indicates that many tears diagnosed are misclassified. The aim of this systematic literature review was to outline research that investigates healthcare professionals’ clinical knowledge in assessment and classification of perineal tears in connection with childbirth. Searches were performed in PubMed and CINAHL. Six studies on the topic were identified and ?used to collect data for questionnaires. An integrative review was used in the analysis. Poor knowledge in perineal anatomy and lack of training in clinical assessment and classification of perineal trauma was evident among both physicians and midwives. These findings indicate that healthcare providers lack adequate knowledge and that they make incorrect assessments and errors in classification of perineal tears. The training in assessment and classification is crucial. Midwives are in a unique position to improve the standard of care in this field since they are often the first and many times the only to assess the injury.

Magnet Attract Shell during Head and Neck Surgery

Objective: New method to use magnet for localization and extraction of shell during head and neck surgery. Method: Series of 3 cases study all are boy’s 17_15_5 years old respectively. All are with penetrating shell injuries to head and neck in Salahaden General Hospital-Tikrit city-Iraq from 25-Oct.-2016 to 30 March 2017. This hospital is a central general governmental hospital that receives referral cases from region distal to the center of Salahaden province at north of Baghdad city (capital of Iraq) and plays a major rule in management of injured patient during war. The cases are selected after explanation of this new method to the patients, it is expected result, risk, possible complication, and unexpected sequelae that may happen after surgery. Prior consent from patients and their families about this sort of surgery has been got. These patients are from families of low socio economic group except the last one he is medium class. All are undergoing surgical management with the aid of magnet. Result: we have 3 cases of shell injuries to head and neck magnet is used successfully for identification of shells and helps in their extraction. Our trial is limited for metallic objects and limited depth. Conclusion: These results suggest that magnet aid removal of shell during head and neck surgery, is less invasive technique, minimal trauma to the surrounding tissues and reducing time of surgery.

Agreement with Microcurrent Conductance, Serum Myoglobin, and Diagnostic Ultrasound When Evaluating Delayed Onset Muscle Soreness

Purpose/Hypothesis: To determine if agreement exists between microcurrent conductance through the skin over an injury, myoglobin levels in the serum, and diagnostic ultrasound measurements of swelling. Number of Subjects: 140. Materials/Methods: Subject underwent microcurrent conductance, serum myoglobin, diagnostic ultrasound, and strength testing before and after delayed onset muscle soreness (DOMS) was induced. Non-control subjects were also given a therapy wrap, dry heat, moist heat, or cold, either applied immediately or delayed 24 hours. Results: After induction of DOMS, there was agreement between significant 37% decreases in microcurrent conductance, significant 37% serum myoglobin increases, and significant 20% increases in quadriceps size, measured by diagnostic ultrasound. When either dry heat or cold was applied immediately, changes in these measurements were insignificant and less than 10%. Agreement was weaker when treatment was delayed 24 hours. Also, cold packs showed less than a 5% decrease in microcurrent conductance for covered areas compared to a 22% decrease for uncovered areas around the knee with 24 hour delay. Moist heat packs showed an insignificant change overall, but a significant 11% decrease for the covered center of the quadriceps with immediate treatment. Strength decreased 25% after DOMS, and all immediate treatments, along with 24 hour delayed cold and moist heat showed significantly smaller decreases. Conclusions: Changes in microcurrent conductance through the skin over injured tissue appear to be indicative of initial injury and recovery, and also for detecting the specific area of the injury. Clinical Relevance: Microcurrent through the skin over injured tissue appears to be a promising objective measurement of tissue injury as well as recovery from injury.

Clinical and radiographic evaluation of surgical treatment of zygomatic fractures using 1.5 mm miniplates system

Fractures of the zygomaticomaxillary complex are among the most common face traumas. Based upon the complexity and great variety of reported diagnoses and treatments, the proposal of this study was to evaluate, clinically and radiographically, unilateral zygomatic fractures treated through internal rigid fixation with miniplates and screws of 1.5 mm. Material and Method: 15 patients with unilateral fractures of the zygomaticomaxillary complex were analyzed, and compared with 15 patients without fractures so that a comparative analysis of the area and the perimeter of the orbital cavities could be made, as well as the distance from the nasal point to the zygomatic prominence between both groups. Results: In the radiographic analysis, the both groups presented similarity in the perimeter and in the area of the orbital cavities. Concerning the distance from the nasal point to the zygomatic prominence, only the operated group showed a significant difference between the sides, even though clinically the observation of the asymmetry had been absent or discreet. Conclusion: The treatment of unilateral fractures of the zygomaticomaxillary complex with the use of plates and screws of the 1.5 mm system proved to be effective, showing good esthetic results and low complication rates.

Multipotency and secretome: themechanisms behind the regenerativepotential of adipose-derived stem cells

The use of stem cells for regenerating damaged organs and tissues which are unable to heal on their own is a widely studied field in plastic and reconstructive surgery as well as medicine in general. Among various stem cell types, adipose-derived stem cells (ASCs) are especially considered to be an ideal stem cell population for several clinical situations. These cells could be harvested from fat with relatively less invasive methods with high yield rates. ASCs have proved to be worthy of more research to understand the mechanisms behind their regenerative abilities. However, it remains uncertain if ASCs show their main effects by their multipotency, or by secreting abundant amounts of cytokines and growth factors. The authors have performed a review of the current publications and literature on the ASCs'' immunophenotypical properties and isolation methods as well as basic and clinical science research about the mechanisms behind their regenerative effects. The purpose of this article is to synthesize information regarding ASCs'' paracrine effects and their ability to differentiate into other cell lines, comparing these aspects in order to lead future research for a more effective cell therapy utilizing these cells.

Pattern of mental health service use and risk of injury: A longitudinal study

Objective: The aim of this study is to investigate the association between mental health treatment patterns and risk of injuries among a Western Australian male birth cohort. Method: A population-based birth-cohort of males born between 1980 and 1984 in Western Australia was followed up using linked health data. Results: Participants with mental health conditions were at an increased risk of injury. Those with a continuous mental health treatment pattern without interruption or window periods had lower risk of injury compared to those with treatment interruption or window periods. The adjusted incidence rate ratios (95% confidence interval) for injury among participants: 1) without a mental condition, 2) with a previous mental condition, 3) with a mental condition in the last four years and without interruption in their mental health treatment, and 4) with a mental condition in the last four years with interruptions in mental health service, were 0.38 (0.35 - 0.40), 0.77 (0.71 - 083), 1.0 (reference group) and 2.06 (1.72 - 2.47) respectively. Conclusion: Increasing resources for mental health services and enabling sufficient continuous mental health services and follow-up may reduce the risk of injury among populations with mental health conditions.

On the Principle of Treatment of Burn Wound

The article expounds the therapeutic features of the Moist Exposed Burn Therapy(MEBT)in the treatment of-burn injuries by means of experimental research data and clinical application experiences about MEBT and confirmsthat the principle and method of conventional dry therapy for burn injuries is contrary to burn pathogenesis,whileMEBT is an effective medical technique which is in conformity with burn pathogenesis at present.Histogical studies confirmed:microscopical examination of wound tissue treated with dry exposure therapyshowed that the granulation tissue was atrophic,in which the lobocytes exudated,collagenous fibers wereeosinophilous.An inanimate sign was emerged in the granulation tissue.The wound tissue treated with MEBT wasfull of vitality,fibroblasts proliferated actively and there were aboundant blood vessels.The comparison study be-tween amnion overlying therapy and MEBT in the treatment of superfcialⅢ°burnt wound confrmed that the latercould make the wound heal through epidermization.
