Objective: To determine the effects of fasting of Ramadan in patients undergoing in-vitro fertilization (IVF)/intracytoplasmic sperm injection (ICSI) cycles. Design: A prospective cohort study conducted during Ramadan. Setting: Ain Shams University IVF centre. Population: Three hundred fasting, infertile women undergoing their first trial of IVF/ICSI and 300 non-fasting matched controls undergoing the same procedure during Ramadan. Methods: Anxiety and depression were measured by the Hospital Anxiety and Depression Scale, at the start of the induction protocol. All the successful pregnant women were followed up until delivery. Main Outcome Measures: The primary outcome measure was the live birth rate. Results: The live birth rate was higher in the fasting group than in the non-fasting group;this was not significant (43% vs. 40%, P = 0.46). The fasting group needed higher doses and durations of induction. Embryo quality did not differ between both groups. The pregnancy complication rate among successful cases was higher in the fasting group (52.9% vs. 40.4%, P = 0.03);however, the frequencies of anxiety and depression were significantly lower than those in the non-fasting group (18% vs. 38%, P P < 0.00001, respectively). Conclusion: Fasting during Ramadan does not seem to significantly affect the IVF/ICSI outcome;however, it significantly decreased the anxiety and depression usually associated with these procedures.

The aim of this study was to investigate whether the sperm chromatin structure assay （SCSA） results after swim-up are related to fertilization rates, embryo quality and pregnancy rates following in vitrofertilization （IVF）. A total of 223 couples undergoing IVF in our hospital from October 2008 to September 2009 were included in this study. Data on the IVF process and sperm chromatin structure assay results were collected. fertilization rate, embryo quality and IVF success rates of different DNA fragmentation index （DFI） subgroups and high DNA stainability （HDS） subgroups were compared. There were no significant differences in fertilization rate, clinical pregnancy or delivery rates between the DFI and HDS subgroups. However, the group with abnormal DFI had a lower good embryo rate. So, we concluded that the SCSA variables, either DFI or HDS after swim-up preparation, were not valuable in predicting fertilization failure or pregnancy rate, but an abnormal DFI meant a lower good embryo rate following IVF.

Objective:To determine the correlation of different serum estradiol levels on the trigger day with the clinical and laboratory outcomes of in-vitro fertilization(IVF)cycles comprising a single fresh top-quality blastocyst ***:This was a retrospective observational study performed in Morula IVF Clinic *** hundred forty-two women were recruited and grouped according to their serum estradiol levels on the trigger day of follicular maturation as follows:0.05).Nonetheless,the study demonstrated a positive correlation of the serum estradiol levels with the overall laboratory outcomes including the number of retrieved,mature,and fertilized oocytes,the total sum of derived embryos,and top-quality embryos at cleavage and blastocyst stage(P<0.001).The subject group with estradiol level of≥4000 pg/mL was superior to the other groups in its respective median number of retrieved,mature,fertilized oocytes,total derived embryos,and top-quality cleavage-and blastocyst-stage ***:Although an apparent positive correlation is observed between estradiol levels and laboratory outcomes,serum estradiol level on hCG trigger day is not associated with the clinical outcomes of IVF.

Background：Generally,intracytoplasmic sperm injection （ICSI） may be the preferable method to treat partial globozoospermia,but whether there exist some correlations between ICSI fertilization rate and the proportion of round-headed sperm or morphologically normal sperm remains *** study was to explore the correlation between ICSI fertilization rate and the sperm morphology in patients with partial ***：Thirty-four patients diagnosed with partial globozoospermia accepted the following assisted fertilization treatments-2 cases accepted in-vitro fertilization （IvF） alone,26 cases accepted ICSI alone,and 6 accepted split IVF/*** morphological characteristics were described using Diff-Quik rapid *** cases accepting IVF or ICSI treatment in our reproductive center were considered as the control group after being matched by relevant *** rate,embryo quality,embryo implantation rate and clinical pregnancy rate were ***：Besides very high proportion of round-headed sperm,partial globozoospermia also showed very high proportion of small-acrosomal sperm and very low proportion of morphologically normal *** rate of IVF （IVF alone plus split IVF） was very low in partial globozoospermia （25.4% ± 17.4%）,but ICSI （ICSI alone plus split ICSI） achieved satisfying fertilization rate compared with the control group （66.2% ± 22.5% vs.68.8% ± 29.4%,P 〉 0.05）.In patients with partial globozoospermia,there were no correlations between ICSI fertilization rate and the proportion of round-headed sperm,small-acrosomal sperm,or morphologically normal ***：There was high proportion of small-acrosomal sperm in partial *** patients with partial globozoospermia,ICSI is more preferable than *** fertilization rate does not depend on the proportion of round-headed sperm,small-acrosomal sperm,or morphologically normal sperm.

Competitive Pressures and Access to Infertility Treatments by Single Women

With the increase in fertility problems and delayed childbearing, demand for infertility treatments has been rising. Today, in-vitro fertilization (IVF) is the most successful infertility treatment but access to IVF is uneven due to its high costs and personal and religious attitudes. To meet rising demand for infertility treatments many markets saw an increased entry of infertility clinics. This study examines the relationship between competitive pressures and access to IVF in the United States by single women. We use US data from 1995 and 2014 to show that competitive pressures improve access to infertility treatments for single women across markets and over time. Overall, this study finds that competition among IVF clinics has a beneficial effect on access to IVF.

The present study was conducted to explore ways of JIVET research on Texel sheep,and to provide technical support for the improvement of the JIVET technology system for research on Texel *** was conducted on 4-8-week old Texel sheep,and in-vivo ovum collection,in-vitro oocyte maturation and fertilization,and fertilized-embryo culture and transplantation were adopted to perform JIVET research on Texel *** results showed that 296 available oocytes were obtained at an average of 74 oocytes per sheep;205 fertilized eggs were obtained at the 2-4 cell stage at an average of 51.25 eggs per sheep and a cleavage rate of 69.26%;and 66 of the fertilized eggs were transplanted to 11 receptor *** sheep were conceived at a conception rate of 54.55%,and 11 lambs were *** results indicated that the present test could be used as a method for the study of JIVET technique on Texel sheep.

Effects of Bovine Follicular Microenvironment on Granulosa Cells,Oocytes,and Early Embryonic Development

[ Objective] The aim of this study was to investigate the effects of bovine follicular microenvironment on the quality of granulosa cells and oocytes as well as Subsequent embryonic development after in vitro fertilization. [ Method] The oocytes and granulosa cells were collected from folli- cles of different diameters. Hoechst33342 staining was used to observe nuclear morphology of oocytes. Annexin-V-FITC/PI staining and flow cytometry were used to detect apoptosis of granulosa cells. After the oosperm were cultured in vitro, fertilization rate and blastocyst rate of oosperm were determined. [Result] The granulosa cells in the large follicles with diameter 〉 10 mm or small follicles with diameter of 0 -3 mm had significantly higher apoptosis rate than those in the follicles with diameter of 3 -7 or 7 -10 mm （P 〈 0.05）. Many oocytes in the follicles with diameter 〉 10 mm or with diameter of 0 -3 mm showed chromatin condensation, nuclear fragmentation and pyknosis. The maturation rate of oocytes with diameter of 3 -7 or 7 -10 mm was significantly higher than that in the large or smell follicles （P 〈 0.05）. The cleavage rate and blastocyst rate of oocytes were not significantly different between the follicles with different diameters. [Condusion~ The follicular microenvironment is a crucial aspect affect- ing in-vitro embryo production. In the follicles with diameter of 3 -10 mm, the quality of granulosa cells as well as the in-vitro maturation rate of oocytes was higher. And the diameter of follicles does not affect oocytes fertilization and early embryonic development of oosperm.
