Mesenteric vein thrombosis following impregnation via in vitro fertilization-embryo transfer

Pregnancy is an acquired hypercoagulable state. Most patients with thrombosis that develops during pregnancy present with deep vein leg thrombosis and/or pulmonary embolism, whereas the development of mesenteric vein thrombosis(MVT) in pregnant patients is rare. We report a case of MVT in a 34-year-old woman who had achieved pregnancy via in vitro fertilization-embryo transfer(IVFET). At 7 wk of gestation, the patient was referred to us due to abdominal pain accompanied by vomiting and hematochezia, and she was diagnosed with superior MVT. Following resection of the gangrenous portion of the small intestine, anticoagulation therapy with unfractionated heparin and thrombolysis therapy via a catheter placed in the superior mesenteric artery were performed, and the patient underwent an artificial abortion. Oral estrogen had been administered for hormone replacement as part of the IVF-ET procedure, and additional precipitating factors related to thrombosis were not found. Pregnancy itself, in addition to the administered estrogen, may have caused MVT in this case. We believe that MVT should be included in the differential diagnosis of a pregnant patient who presents with an acute abdomen.

Tuberculosis (TB) is a prominent infectious disease globally that imposes a substantial health burden. Genital TB (GTB), an extrapulmonary manifestation, leads to complications such as tubal adhesions, blockage, and diminished ovarian function, culminating in infertility, and is recognized as a prevalent cause of infertility in nations with high-burden TB. In regions with low TB rates, infertility and active TB during pregnancy have been reported to be most common among female immigrants from countries with high-burden TB. In the context of TB, pregnant women often exhibit exacerbated symptoms afterin vitro fertilization-embryo transfer (IVF-ET), heightening the risk of dissemination. Miliary pulmonary TB and tuberculous meningitis pose a serious threat to maternal and fetal health. This article integrates recent epidemiological data and clinical research findings, delineating the impact of TB on infertility and assisted reproduction and particularly focusing on the diagnosis and treatment of GTB, underscored by the imperative of TB screening before IVF-ET. Our objective is to increase awareness among respiratory and reproductive health professionals, promoting multidisciplinary management to enhance clinical vigilance. This approach seeks to provide patients with judicious reproductive plans and scientifically rigorous pregnancy management, thereby mitigating adverse pregnancy outcomes related to TB activity.

Prospective,randomized,controlled study of in vitro fertilization-embryo transfer with single-dose and multiple-dose GnRH antagonist (cetrorelix) or a short protocol of the GnRH agonist (triptorelin) in Chinese women

Objective:To confirm the value of GnRH antagonist(GnRHant) cetrorelix in preventing the occurrence of premature LH surges and compare a multiple dose and a single dose GnRHant protocols for the COS of IVF/ICSI patients with a short GnRHa protocol. Methods:Single-center randomized,prospective,controlled study was conducted in our *** infertile women who underwent ovarian stimulation for IVF-ET were *** dose of 0.25 mg(Group I) and single dose of 3 mg(GroupⅡ)of cetrorelix(60 patients) was administered in the late follicular phase.A depot preparation of triptorelin(Decapeptyl) was chosen as a control agent(30 patients)(GroupⅢ).Ovarian stimulation was conducted with FSH. Results:No premature LH surge occurred after cetrorelix *** mean numbers of mature oocytes obtained from patients were(13.47 7.60),(18.66 8.37) and(15.77 10.78) in GroupsⅠ,ⅡandⅢ,respectively, showing no statistical *** fertilization rates were(78.97 18.11)%in GroupⅠ,(72.35 25.44)%in GroupⅡand(76.80 23.10)%in GroupⅢ.The clinical pregnancy rates of patients were statistically comparable in the GnRHa group(30.8%) and in the GnRHant group(37.9%in GroupⅠ,28.6%in GroupⅡ),as ongoing pregnancy rates of embryo transfer were 31.0%,25.9%,30.8%for the GroupⅠ,ⅡandⅢ,*** miscarriage rate was not different in the three *** were four patients cancelled embryo transfer for preventing severe ovarian hyperstimulation syndrome(OHSS) in triptorelin group,and 1 patient in each cetrorelix *** severe or moderate OHSS of cetrorelix groups occurred. Efficacy of Qizi Yusi Pill on Pregnancy Outcomes in Women of Advanced Reproductive Age:A Multicenter,Randomized,Double-Blind,Placebo-Controlled Trial
