Titanium allergy or not? “Impurity” of titanium implant materials

For patients suffering from allergies to nickel, chrome and cobalt, titanium implants are the implants of choice. Nevertheless, titanium implant sensitivity has been reported in the form of “allergies” and an increasing number of patients are confused. This paper aims to use spectral analysis as a diagnostic tool for analyzing different titanium implant alloys in order to determine the percentage of the alloy components and additions that are known to cause allergies. Different materials, such as sponge titanium, TiAl6Nb7, Ti21SRx, TiAl6V4 [forged alloy], TiAl6V4 [cast alloy], TMZF, pure titanium [c. p. 1] and iodide titanium were analyzed for the presence of the elements that have been associated with allergic reactions using spectral analysis. All the implant material samples contained traceable amounts of Be, Cd, Co, up to a maximum of 0.001 percent by weight [wt.%], Cr up to 0.033 wt.%, Cu up to 0.007 wt.%, Hf up to 0.035 wt.%, Mn up to 0.007 wt.%, Ni up to 0.031 wt.%, and Pd up to 0.001 wt.%. This paper demonstrates that all the investigated implant material samples contained a low but consistent percentage of components that have been associated with allergies. For example, low nickel contents are related to the manufacturing pro- cess and are completely dissolved in the titanium grid. Therefore, they can virtually be classified as “impurities”. Under certain circumstances, these small amounts may be sufficient to trigger allergic reactions in patients suffering from the corresponding allergies, such as a nickel, palladium or chrome allergy.

Background: Despite scientific and technological developments on dentistry, man is still losing their teeth due to several reasons. Tooth loss may result in esthetical and functional discrepancies. Although beginning of dental transplantation is quite old, it started to take attraction on scientific basis recently. In recent years, oral implants are being used nearly in all fields of dentistry. Purpose: The purpose of this study was to evaluate 5 year follow-up of a patient with an impacted maxillary canine transplantation and a single-tooth implant. Materials and Methods: In our study an impacted canine was removed and transplanted immediately. Thereafter the missing premolar was replaced by an implant retained cemented crown. The patient was evaluated clinically and radiographically. Clinical periodontal parameters and implant mobility was evaluated, marginal bone level was measured on radiographs. Results: No significant changes have been observed in these parameters and implant mobility between year one and year five of the study. Conclusion: This study has demonstrated that stable long-term results can be achieved with replacement of single teeth with MIS implant, and cemented crown on abutment with adjacent transplanted canine, with cumulative success rate for five years.

The artificial urinary sphincter(AUS)remains the standard of care in men with severe stress urinary incontinence(SUI)following prostate surgery and radiation.While the current AUS provides an effective,safe,and durable treatment option,it is not without its limitations and complications,especially with regard to its utility in some“high-risk”populations.This article provides a critical review of relevant publications pertaining to AUS surgery in specific high-risk groups such as men with spinal cord injury,revision cases,concurrent penile prosthesis implant,and female SUI.The discussion of each category includes a brief review of surgical challenge and a practical action-based set of recommendations.Our increased understandings of the pathophysiology of various SUI cases coupled with effective therapeutic strategies to enhance AUS surgery continue to improve clinical outcomes of many patients with SUI.

Iron-matrix composites with calcium silicate(CS)bioceramic as the reinforcing phase were fabricated through powder metallurgy processes.The microstructures,mechanical properties,apatite deposition and biodegradation behavior of the Fe-CS composites,as well as cell attachment and proliferation on their surfaces,were characterized.In the range of CS weight percentages selected in this study,the composites possessed compact structures and showed differently decreased bending strengths as compared with pure iron.Immersion tests in simulated body fluid(SBF)revealed substantially enhanced deposition of CaP on the surfaces of the composites as well as enhanced degradation rates as compared with pure iron.In addition,the composite containing 20%CS showed a superior ability to stimulate hBMSCs proliferation when compared to pure iron.Our results suggest that incorporating calcium silicate particles into iron could be an effective approach to developing iron-based biodegradable bone implants with improved biomedical performance.

To the Editor:Total cervical disc replacement(TDR)has been widely used to treat cervical degenerative disc diseases(CDDD).However,implant-related complications,including implant migration,implant subsidence,heterotopic ossification(HO),and anterior bone loss(ABL),have been reported by many studies.ABL has received more attention in recent years;however,this phenomenon has been recognized only in cervical artificial discs without keels,including Bryan,Baguera-C,Discocerv,and Mobi-C.[1,2,3,4,5,6]Therefore,the first aim of this study was to evaluate the incidence of ABL in a keeled artificial disc,the Prestige-LP cervical disc system(Medtronic Sofamor Danek Inc.,Memphis,TN,USA).

Background The Dynamic Interspinous Assisted Motion （DIAM） system was designed to stabilize degenerative spinal segments without fusion surgery, maintain segment motion and prevent adjacent segment degeneration. The aim of thisstudy was to investigate clinical efficacy of the DIAM system in treatment of degenerative lumbar disease in China.Methods Eight cases of lumbar vertebral instability were treated with the DIAM system at Peking Union MedicalCollege Hospital from June 2006 to January 2008. There were 6 female and 2 male subjects with a mean age of 46.9years and a range of 40-52 years. Radiographs and scores on outcome measures included the visual analogue scale （VAS） for pain and the Oswestry disability index （ODI）. These scores were recorded before surgery and after surgery at intervals of 3-month, 6-month, 1-year and the final follow-up visit.Results The follow-up time ranged from 12-31 months, with an average of 20.6 months. There were significant differences between preoperative and postoperative scores at each follow-up evaluation （P 〈0.05）. However, there was no significant difference between each postoperative follow-up score （P 〉0.05）. There were significant differences between preoperative and postoperative L4-5 segment activity at each time interval （P 〈0.05）, but no obvious difference was found within each postoperative follow-up evaluation （P 〉0.05）. The ODI and VAS score improvements were directly correlated with segment activity （r 〉0.7, P 〈0.05）.Conclusions The DIAM system appears to be a useful and effective treatment in the surgical management of degenerative lumbar disease in certain patients. However, long-term follow-up is needed to evaluate the clinical outcomes of the device.

AIM: To determine whether intravitreal dexamethasone(DEX) implant induces posterior vitreous detachment or not. METHODS: We retrospectively reviewed 810 eyes of 405 patients who underwent intravitreal DEX implantation due to macular edema caused by diabetic and retinal venous occlusion in our clinic. The eyes having no injection were determined as the control group. The examination findings of the patients before the injection and 3 mo after the injection and optical coherence tomography(OCT) images were scanned. The pre-injection OCT findings and OCT findings of the patients having no posterior vitreous detachment(PVD) and determined to have partial PVD were compared. RESULTS: The separation in vitreoretinal adhesion and total PVD development of DEX-injected 56/208(26.9%) eyes were statistically greater in comparison with the 12/129(9.3%) eyes that had not been injected(P=0.001). PVD development was observed more in the patients that were younger, had larger macula thickness and lower visual acuity. CONCLUSION: It can be stated that intravitreal DEX implant induces PVD development. Prospective, controlled studies are required in order to determine prognosis of vitreoretinal disease in PVD-developed patients and in non-PVD-developed patients.

These devices improve the quality of life by reliving symptoms of bradycardia including dizziness, fatigue and exercise intolerance. The decision to implant a PPM is often made at the time of the initial encounter with the patient, such as when symptomatic bradyarrhythmia is first diagnosed.

Permanent implants are going to be replaced by the implementation of biodegradable magnesium-based implants in fixation of internal bone fractures because of many concerns associated with conventional implants.However,biodegradable magnesium-based biomaterials exhibit higher biodegradation rate and low mechanical properties which are the main challenges.This work aims to almost overcome both disadvantageous by grain refining of a WE43 magnesium alloy containing 93.04 wt% Mg,4.12 wt% Y,2.15 wt% Nd,0.43 wt% Zr,and 0.26 wt%La.In this study,the consequences of combined severe plastic deformation(SPD) on the mechanical properties,microstructure,and in vivo degradation behavior of WE43 magnesium alloy were investigated.To do so,WE43 magnesium alloy was chosen and processed through multipass equal channel angular pressing(ECAP) at 330℃ for up to four passes followed by an extrusion process.The results showed that higher strength and hardness with minimum ductility less was obtained in the sample processed via two-pass ECAP followed by extrusion.In vivo biodegradation experiments showed higher degradation rate for the unprocessed coarse-grained(CG) WE43 sample.The two-pass ECAP and extruded sample with ultrafine-grained(UFG) structure exhibited the lowest in vivo biodegradation rate besides appropriate mechanical properties.It may be concluded that the WE43 magnesium alloy processed via two-pass ECAP and extrusion could be a very promising candidate for biodegradable implants from both mechanical and biocorrosion viewpoints.
