Non-invasive diagnosis of H pylori infection: Evaluation of serological tests with and without current infection marker CIM

AIM:To evaluate the performance of commercially available immunochromatographic (ICT) and immunoblot tests covering the current infection marker CIM and conventional ELISA for the diagnosis of H pylori infection in adult dyspeptic patients. METHODS:Consecutive non-treated dyspeptic patients undergoing diagnostic endoscopy were tested for H pylori infection by culture, rapid urease test, and histology of gastric biopsy specimens. Serum from 61 H pylori infected and 21 non-infected patients were tested for anti-H pylori IgG antibodies by commercial ELISA (AccuBindTM ELISA, Monobind, USA), ICT (Assure H pylori Rapid Test, Genelabs Diagnostics, Singapore), and immunoblot (Helico Blot 2.1, Genelabs Diagnostics, Singapore) assays. ICT and immunoblot kits cover CIM among other parameters and their performance with and without CIM was evaluated separately. RESULTS:Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of ELISA were 96.7%, 42.8%, 83.1%, 81.8%, and 82.9%, of ICT were 90.1%, 80.9%, 93.2%, 73.9%, and 87.8%, of ICT with CIM were 88.5%, 90.4%, 96.4%, 73.0%, and 89.0%, of immunoblot were 98.3%, 80.9%, 93.7%, 94.4%, and 93.9%, and of immunoblot with CIM were 98.3%, 90.4%, 96.7%, 95.0%, and 96.3%, respectively. CONCLUSION:immunoblot with CIM had the best performance. ICT with CIM was found to be more specific and accurate than the conventional ELISA and may be useful for non-invasive diagnosis of H pylori infection.

Objective:To determine the profdes of anti-Entamoeba histolytica(***) ***. and IgM in sera of diarrheic and non-diarrheic individuals and partially characterize target ***:Serum samples from thirty diarrheic and thirty non-diarrheic individuals were subjected to IgA,IgG,and IgM profiling through enzyme-linked immunosorbent assay (***),flow cytometry,and ***:ELBA titer results showed that both diarrheic and non-diarrheic individuals possess high levels of ***-specific IgG compared to IgA and *** cytometry data showed that diarrheic serum samples had higher mean reaction percentages against *** cells compared to non-diarrheic *** *** proteins with molecular weights ranging belween 7 kDa and 292 kDa were recognized by diarrheic serum IgG,and 170 kDa and 250 kDa by non-diarrheic serum IgG. Conclusions:Our findings suggest that serum *** IgG,compared with serum *** IgA and IgM responses,was generally high in both diarrheic and nondiarrheic sera,indicating a past exposure to the organism both in symptomatic patients as well as in asymptomatic carriers,*** addition,serum IgG from diarrheic and non-diarrheic patients were able to detect immunogenic *** proteins.

Objectives: The autoantibody profile, which varies between races, has rarely been reported in Chinese systemic sclerosis(SSc) *** present study aims to investigate the autoantibody profile of Chinese SSc patients using a commercial line ***: The prevalence of autoantibodies to Ro-52, PDGFR, Ku, PM75, PM100, Th/To, NOR90, Fib, RP155, RP11, CENP-B, CENP-A and Scl-70 were analyzed in serum samples obtained from healthy controls(n=30), SSc patients(n=320) and non-SSc connective-tissue disease patients(n=100) using an Euroline Systemic Sclerosis Profile ***: SSc patients had increased prevalence of anti-RP11(P=0.032), anti-CENPB(P<0.001), anti-CENPA(P=0.001) and anti-Scl-70(P<0.001), but had decreased prevalence of anti-Ro-52(P=0.004), when compared to non-SSc ***, SSc patients had increased prevalence of anti-Ro-52(P=0.003), anti-CENPB(P=0.034), anti-CENPA(P=0.034) and anti-Scl-70(P<0.001) when compared to *** addition, SSc patients with interstitial lung disease(ILD) had increased prevalence of anti-Ro-52(P=0.046) and anti-Scl-70(P=0.035), but had decreased prevalence of anti-CENPB(P<0.001) and anti-CENPA(P<0.001).Moreover, SSc patients with pulmonary arterial hypertension(PAH) had increased prevalence of anti-Ro-52(P=0.013) and decreased prevalence of anti-Scl-70(P=0.001).Anti-Ro-52 and anti-Scl-70 had increased values for predicting ILD in SSc patients, while anti-Ro-52 had increased values for predicting PAH in SSc ***: Anti-Ro-52, anti-RP11, anti-CENPB, anti-CENPA and anti-Scl-70 are useful for diagnosing SSc in Chinese ***-Ro-52, anti-CENPB, anti-CENPA and anti-Scl-70 have potential value for diagnosing ILD in SSc ***, anti-Ro-52 and anti-Scl-70 may be used to diagnose PAH in SSc patients.

采用改进的融合技术得到318个杂交瘤细胞系。经ELISA筛选,这些细胞系所寸泌的抗体均和病毒抗原发生反应。克隆出26个杂交瘤细胞系,制出特异的单克隆抗体,并采用ELISA、放射免疫沉淀试验（RIPA）、十二烷基磺酸钠-聚丙烯酰胺凝胶电泳（SDS-PAGE）、Immunoblet、病毒中和试验等对单克隆抗体的特性作了鉴定。利用单克隆抗体对牛疱疹病毒I型[Bovine Herpcs Virus I（BHV—1）]主要结构糖蛋白作了分析。结果表明,这些杂交瘤细胞系分泌IgG和IgM两类抗体,其中IgG有192个（60.4％,其余126个39.6％）为IgM。免疫球蛋白IgG具有强的病毒中和作用。利用[35S]—标记BHV-1病毒感染细胞溶解物,经SDS-PAGE分析、病毒至少有25个结构多肽成分,其中15个是主要的,被抗BHV-1免疫血清和单克隆抗体所识别的结构多肽分子量有180Kd、150Kd、130Kd、97Kd、71Kd、5bKd、37/33Kd和25Kd,除低分子量37/33Kd和25Kd外、其余均为糖蛋白。71Kd和97Kd与病毒的致病性有关,其相对应的单克隆抗体＃132、＃186、＃191和＃181、＃128、＃101、＃213等有强的或较强的病毒中和力;在还原条件下,单克隆抗体＃33、＃74、＃76可识别3个多肽,其分子量分别为130Kd、71Kd、56Kd,而在非还原条件下仅能识别130Kd,并证明130Kd/71Kd/56Kd为一三聚体。
