Value and influencing factors of valid Traditional Chinese Medicine compound prescription patents for mammary gland hyperplasia

OBJECTIVE: To explore the value and factors influencing the maintenance of valid Traditional Chinese Medicine compound prescription patents for the management of mammary gland hyperplasia. METHODS: The State Intellectual Property Office Patents Database, China Patent Information Center Patents Database, Soopat Patent Search Platform, and CNIPR Search Platform of Chinese and Foreign Patent Database were searched to identify Traditional Chinese Medicine compound prescription patents for mammary gland hyperplasia that had been maintained for at least 5 years. Logistic regression analysis was used to detect the factors influencing the maintenance of valid patents. RESULTS: From 1 January 1993 to 31 December 2019, a total of 1228, Traditional Chinese Medicine compound prescription patents for mammary gland hyperplasia were authorized. Of these applications, 381 patents were granted and only 176 patents were maintained for more than 5 years. The five independent variables assessed in this study all had significant explanatory functions. CONCLUSIONS: The maintenance of valid patents for Traditional Chinese Medicine compound prescriptions for treating mammary gland hyperplasia is of great importance. The maintenance of such patents depends on many factors, such as the concentration of effective herbs, number of dependent claims, provision of new dosage forms and detailed experimental data, and transfer of patent rights.

hyperplasia of the mammary glands(HMG)is a noninflammatory and non-tumorous lesion.[1]It is a structural disorder of the mammary glands due to different degrees of hyperplasia and subinvolution of the mammary parenchyma and stroma.In the literature,HMG is also called mastopathy,fibroadenosis,fibrocystic breast disease,mastalgia,fibrocystic change,benign mammary dysplasia,or sclerosing adenosis.

Does prostate-specific antigen (PSA)mass or free PSA mass improve the accuracy of predicting total prostate volume in relation to obesity in men with biopsy-proven benign prostatic hyperplasia?

We evaluated whether the prostate-specific antigen (PSA)mass or free PSA (fPSA)mass (i.e.,absolute amount of total circulating PSA or fPSA protein,respectively),versus serum PSA or fPSA concentration,improves the accuracy of predicting the total prostate volume (TPV)in relation to obesity.Among men whose multicore (≥12)transrectal prostate biopsy was negative,586 who had a PSA of <10 ng ml^-1 and underwent the fPSA test prior to biopsy were enrolled.The PSA mass or fPSA mass (pg)was calculated by multiplying the serum level by plasma volume.At each TPV cut-off point (30 ml,40 ml,and 50 ml),the areas under the receiver operating characteristics curve (AUCs)of each variable were compared in obesity-based subgroups.AUCs of fPSA and fPSA mass for predicting TPV were significantly larger than those for PSA and PSA mass by 8.7%-12.1%at all cut-off points. Subgroup analyses based on obesity showed that,although PSA mass and fPSA mass enhanced accuracy by 4%(P =0.031)and 1.8%(P =0.003),respectively,for determining TPVs of ≥30 ml and ≥50 ml in obese and overweight men,they did not improve the accuracy in most other combinations of the degrees of obesity with TPV cut-off points.Thus,compared with serum PSA or fPSA,the absolute amount of PSA or fPSA protein mass improved the accuracy of predicting TPV in obese men very minimally and only for certain TPV cut-off points.Hence,these indicators may not provide clinically meaningful improvement in predicting TPV in obese men.

Multi-parameter ultrasound based on the logistic regression model in the differential diagnosis of hepatocellular adenoma and focal nodular hyperplasia

BACKGROUND Focal nodular hyperplasia(FNH)has very low potential risk,and a tendency to spontaneously resolve.Hepatocellular adenoma(HCA)has a certain malignant tendency,and its prognosis is significantly different from FNH.Accurate identification of HCA and FNH is critical for clinical treatment.AIM To analyze the value of multi-parameter ultrasound index based on logistic regression for the differential diagnosis of HCA and FNH.METHODS Thirty-one patients with HCA were included in the HCA group.Fifty patients with FNH were included in the FNH group.The clinical data were collected and recorded in the two groups.Conventional ultrasound,shear wave elastography,and contrast-enhanced ultrasound were performed,and the lesion location,lesion echo,Young’s modulus(YM)value,YM ratio,and changes of time intense curve(TIC)were recorded.Multivariate logistic regression analysis was used to screen the indicators that can be used for the differential diagnosis of HCA and FNH.A ROC curve was established for the potential indicators to analyze the accuracy of the differential diagnosis of HCA and FNH.The value of the combined indicators for distinguishing HCA and FNH were explored.RESULTS Multivariate logistic regression analysis showed that lesion echo(P=0.000),YM value(P=0.000)and TIC decreasing slope(P=0.000)were the potential indicators identifying HCA and FNH.In the ROC curve analysis,the accuracy of the YM value distinguishing HCA and FNH was the highest(AUC=0.891),which was significantly higher than the AUC of the lesion echo and the TIC decreasing slope(P<0.05).The accuracy of the combined diagnosis was the highest(AUC=0.938),which was significantly higher than the AUC of the indicators diagnosing HCA individually(P<0.05).This sensitivity was 91.23%,and the specificity was 83.33%.CONCLUSION The combination of lesion echo,YM value and TIC decreasing slope can accurately differentiate between HCA and FNH.

Efficacy of Lubeikangru formulation in mice with hyperplasia of the mammary glands induced by estrogen and progesterone

OBJECTIVE: To evaluate the protective effects of Lubeikangru formula(LF) on hyperplasia of the mammary glands(HMG) induced by estrogen and progesterone in mice.METHODS: Female mice were divided randomly into five groups: normal, model, tamoxifen(3 mg/kg),Rupixiao(900 mg/kg) and LF(900 mg/kg). All mice except those in the normal group were treated sequentially with estradiol and progesterone to induce HMG. From the tenth day of induction, mice in normal and model groups received distilled water and mice in the other groups were given the corresponding drugs by gavage, once a day, for 30 d.At the end of treatment, the mammary glands, ovaries, hypothalamus, and serum was collected for whole-mount and hematoxylin and eosin(HE)staining, enzyme-linked immunosorbent assays(ELISAs), or western blotting.RESULTS: Whole-mount and HE staining of mammary glands showed that LF rescued(at least in part) the hyperplasic morphology of the mammary glands, and the number of branch points decreased after LF treatment(P hyperplasia by adjusting hormone levels and regulating the mitogen-activated protein kinase pathway.

Shensong Yangxin (SSYX) Ameliorates Disordered Excitation Transmission by Suppressing Cardiac Collagen hyperplasia in Rabbits with Chronic Myocardial Infarction

The traditional Chinese medicine Shensong Yangxin（SSYX） can improve the clinical symptoms of arrhythmia in an integrated manner. This study aimed to investigate the electrophysiological effect of SSYX on the hearts of myocardial-infarcted rabbits and further explore the mechanism by which SSYX alleviates myocardial fibrosis. Myocardial infarction（MI） was established in rabbits by ligation of the left circumflex coronary. The rabbits were treated with SSYX（0.5 g/kg/d） or saline for 8 weeks by oral administration. Microelectrode array（MEA） technology was used in vivo for extracellular electrophysiological recordings of the infarct border zone. Masson＇s trichrome staining was used to observe myocardial fibrosis. Western blotting was performed to evaluate the protein expression levels of collagen Ⅰ（COL Ⅰ） and collagen Ⅲ（COL Ⅲ）. Quantitative real-time polymerase chain reaction（real-time PCR） was performed to evaluate the TGF-β1 and MMP-2 m RNA expression levels. The results showed that the total activation time（TAT） and the dispersion of TAT were significantly increased and the excitation propagation markedly disordered after MI. SSYX could significantly decrease TAT and the dispersion of TAT, and significantly ameliorate the chaotic spread pattern of excitation. Furthermore, SSYX treatment could significantly decrease COL Ⅰ and COL Ⅲ protein levels and down-regulate TGF-β1 and MMP-2 m RNA expression levels in MI rabbits. It was concluded that SSYX may ameliorate cardiac electrophysiological abnormalities in infarcted hearts by decreasing the protein levels of COL Ⅰ and COL Ⅲ, down-regulating the m RNA expression levels of TGF-β1 and MMP2, and thereby reducing adverse cardiac remodeling.

We report the case of a 69-year-old woman with reactive lymphoid hyperplasia(RLH) of the liver.She underwent partial hepatectomy under a preoperative diagnosis of hepatocellular carcinoma; however,histopathological analysis revealed RLH.The liver nodule showed the imaging feature of perinodular enhancement in the arterial dominant phase on contrast-enhanced computed tomography and magnetic resonance imaging,which could be a useful clue for identifying RLH in the liver.Histologically,the perinodular enhancement was compatible with prominent sinusoidal dilatation surrounding the liver nodule.

Dear Editor, We would like to present a rare case of giant prostatic hyperplasia （GPH）, which is defined as a prostate weight over 200g for Easterners or 500 g for Westerns.： Only 14 cases of GPH exceeding 500 g had been reported in the English literature by 2015 （Supplementary Table 1）. To the best of our knowledge, this is the first report of removing a GPH exceeding 500 g by using minimally invasive laparoscopic technique.

BACKGROUND Nodular lymphoid hyperplasia(NLH)in the small intestine is a rare benign lesion characterized by multiple small nodules on the intestinal surface.Patients with terminal ileal NLH may experience long-term abdominal pain,diarrhea,and abdominal distension,among other symptoms.Supplementation with probiotics could mitigate these symptoms.NLH is linked to the immune system,and it may result from accumulation of plasma-cell precursors due to a maturational defect during the development of B lymphocytes.The intestinal microbiome plays an essential role in the immune system.Thus,we speculate that the gut flora plays a key role in terminal ileal NLH.AIM To explore the correlation between intestinal flora and terminal ileal NLH.METHODS We collected mucosal biopsy samples that were obtained via colonoscopy from 15 patients with terminal ileal NLH(the test group)and 15 normal subjects(the control group).We subsequently performed 16 S-r RNA gene amplicon sequencing of these samples,and the results were evaluated using alpha diversity,beta diversity and microbial composition analyses.The Phylogenetic Investigation of Communities by Reconstruction of Unobserved States was used to predict the metabolic pathways and orthologous groups according to the Kyoto Encyclopedia of Genes and Genomes database.RESULTS Compared with the control group,the terminal ileal NLH group showed an increased alpha diversity(P<0.05).The overall intestinal microbiota in the NLH group was significantly different from that of the control group(P<0.05),implying that there was the dysbiosis in the terminal ileal NLH patients.The relative abundance of phylum Bacteroidetes was significantly lower in the NLH group,while that of Patescibacteria and Campilobacterota was significantly higher.The genus Bacteroides was the dominant gut microbiota in both groups,but its abundance was significantly lower in the test group than it was in the control group.Conversely,the relative abundances of Haemophilus,Streptococcus,Pseudomonas,Actinomyces,TM7 X,Fusobacte
