AIM:To analyze whether high-intensity focused ultrasound(HIFU) ablation is an effective bridging therapy for patients with hepatocellular carcinoma(HCC).METHODS:From January 2007 to December 2010,49 consecutive HCC patients were listed for liver transplantation(UCSF criteria).The median waiting time for transplantation was 9.5 ***-nine patients received transarterial chemoembolization(TACE) as a bringing therapy and 16 patients received no treatment before *** patients received HIFU ablation as a bridging *** five patients with the same tumor staging(within the UCSF criteria) who received HIFU ablation but not on the transplant list were included for *** were comparable in terms of Child-Pugh and model for end-stage liver disease scores,tumor size and number,and cause of ***:The HIFU group and TACE group showed no difference in terms of tumor size and tumor *** patient in the HIFU group and no patient in the TACE group had gross *** median hospital stay was 1 d(range,1-21 d) in the TACE group and two days(range,1-9 d) in the HIFU group(P < 0.000).No HIFU-related complication *** the HIFU group,nine patients(90%) had complete response and one patient(10%) had partial response to the *** the TACE group,only one patient(3%) had response to the treatment while 14 patients(48%) had stable disease and 14 patients(48%) had progressive disease(P = 0.00).Seven patients in the TACE group and no patient in the HIFU group dropped out from the transplant waiting list(P = 0.559).CONCLUSION:HIFU ablation is safe and effective in the treatment of HCC for patients with advanced *** may reduce the drop-out rate of liver transplant candidate.

Height of fractured zone inside overlying strata under high-intensity mining in China

The height of fractured zone(HFZ) at the high-intensity longwall mining panel plays a vital role in the safety analysis of coal mining under bodies of water. This paper described definitions of the highintensity mining. The processes of overburden failure transfer(OFT) were analyzed, which were divided into the development stage and the termination stage. Through theoretical analysis, the limited suspension-distance and the limited overhanging distance were proposed to judge the damage of each stratum. Mechanical models of strata suspended integrity and overhanging stability were established.A theoretical method to predict the HFZ at the high-intensity longwall mining panel was put forward based on the processes of OFT. Taking a high-intensity longwall mining panel(No. 11915 panel) as an example, the theoretical method proposed, the engineering analogy and the empirical formulas in the Regulation were used to predict the HFZ. The results show that the theoretical result is consistent with the engineering analogies' result and empirical formulas' result. The rationality and reliability of the theoretical method proposed is verified.

Large deformation and failure mechanism analyses of Tangba high slope with a high-intensity and complex excavation process

The Tangba high slope is mainly composed of coarse soils and supplies core wall materials for the construction of the Changheba dam. Since the filling intensity of the Changheba dam is high, the Tangba high slope suffers from a high-intensity excavation process, and reinforcement measures are usually not implemented immediately. Moreover, the distribution of useful materials is uneven and insufficient, and the mixing of different soil materials is necessary; thus, multiple simultaneous excavations and secondary excavation are inevitable. In the construction period from 2012 to 2016, large deformations occurred in this area, and one of the largest monitored horizontal deformations whose direction points to the opposite side of the valley even reached more than 8000 mm. According to field investigation, site monitoring and theoretical analysis, the large deformation in the Tangba high slope can be divided into two phases. In the first phase, the excavation construction breaks the original stress equilibrium state; in the second phase, the precipitation infiltration accelerates the deformation. Thus, the excavation construction and precipitation infiltration are the two major factors promoting the deformation, and the high-intensity and complex excavation process is the fundamental cause. Notably, rate of slope deformation significantly accelerated in rainy seasons due to precipitation infiltration; the rate also accelerated in early 2016 due to the high-intensity, complex excavation process. Comprehensively considering the above factors, timely and effective reinforcement measures are essential.

AIM To determine the impact ofm low volume high-intensity interval training(LVHIIT) and continuous low to moderate-intensity exercise training(CLMIT) on cardiovascular disease(CVD) risk and health outcomes in cancer *** Sedentary cancer survivors(n=75,aged 51±12year)within 24 months of diagnosis,were randomised into three groups for 12 wk of LVHIIT(n=25),CLMIT(n=25)or control group(n=25).The exercise intervention involved 36 sessions(three sessions per week).The LVHIIT group performed 7 x 30 s intervals(≥85%predicted maximal heart rate)with a 60 s rest between intervals,and the CLMIT group performed continuous aerobic training for 20 min(≤55%predicted maximal heart rate)on a stationary *** variables were measured at baseline and at12 weeks and analysed using a 3 x 2(group x time)repeated measures ANCOVA to evaluate main and interaction *** Significant improvements(time)were observed for seven of the 22 variables(ES 0.35-0.97,P≤0.05).There was an interaction effect(Phigh-intensity training shows promise as an effective exercise prescription within the cancer population,showing greater improvements in cardiorespiratory fitness,lower body strength and waist circumference compared with traditional CLMIT and control *** LVHIIT and CLMIT improved quality of life.A proposed benefit of LVHIIT is the short duration(3 min)of exercise required,which may entice more cancer survivors to participate in exercise,improving health outcomes and lowing the risk of CVD.

high-intensity interval training for health benefits and care of cardiac diseases——The key to an efficient exercise protocol

Aerobic capacity, which is expressed as peak oxygen consumption (VO2peak), is well-known to be an independent predictor of all-cause mortality and cardiovascular prognosis. This is true even for people with various coronary risk factors and cardiovascular diseases. Although exercise training is the best method to improve VO2peak, the guidelines of most academic societies recommend 150 or 75 min of moderate- or vigorous- intensity physical activities, respectively, every week to gain health benefits. For general health and primary and secondary cardiovascular prevention, high-intensity interval training (HIIT) has been recognized as an efficient exercise protocol with short exercise sessions. Given the availability of the numerous HIIT protocols, which can be classified into aerobic HIIT and anaerobic HIIT [usually called sprint interval training (SIT)], professionals in health-related fields, including primary physicians and cardiologists, may find it confusing when trying to select an appropriate protocol for their patients. This review describes the classifications of aerobic HIIT and SIT, and their differences in terms of effects, target subjects, adaptability, working mechanisms, and safety. Understanding the HIIT protocols and adopting the correct type for each subject would lead to better improvements in VO2peak with higher adherence and less risk.

Laser pulses of 200 ps with extremely high intensities and high energies are sufficient to satisfy the demand of shock ignition,which is an alternative path to ignition in inertial confinement fusion(ICF).This paper reports a type of Brillouin scheme to obtain high-intensity 200-ps laser pulses,where the pulse durations are a challenge for conventional pulsed laser amplification *** the amplification process,excited Brillouin acoustic waves fulfill the nonlinear optical effect through which the high energy of a long pump pulse is entirely transferred to a 200-ps laser *** method was introduced and achieved within the SG-Ⅲprototype system in *** favorably with the intensity of 2 GW/cm^2 in existing ICF laser drivers,a 6.96-GW/cm^2 pulse with a width of 170 ps was obtained in our *** practical scalability of the results to larger ICF laser drivers is discussed.

Long-Term Outcomes and Prognosis of Transrectal high-intensity Focused Ultrasound Therapy for Patients with Localized Prostate Cancer—Therapy after Recurrence and Predictive Factors

Objectives: To evaluate the outcomes and prognosis of high-intensity focused ultrasound (HIFU) therapy for patients with localized prostate cancer, and identify suitable candidates for this therapy by investigating the predictive factors. Methods: The 224 patients (low 54, intermediate 111 and high-risk patients 59) with T1-2 stage were treated using the Sonablate device and followed for over 12 months after treatment. Recurrence was determined based on histological findings, prostate-specific antigen (PSA) failure and local or distant metastasis. The factors which are predicting variables with potential effects were investigated by Kaplan-Meier and multivariate analysis. Results: A total of 255 treatment sessions (193 with one, 31 with two) were performed. No patients died of prostate cancer, but 15 died of other causes and 14 patients were lost during follow-up. The 7-year recurrence-free survival (RFS) rates in all patients were 75%, and 5-year RFS rates were 98%, 84% and 59% in the low, intermediate and high-risk patients respectively. In the 216 patients who underwent histological examination at 6 months or later after HIFU, 25 (12%) were positive. In 77 patients with recurrence after first-HIFU, the second treatments were hormonal therapy and HIFU. Of the 31 patients who underwent a second HIFU, the 5-year RFS rates were 64%, and 5-year RFS rates were 100%, 74% and 33% in the low, intermediate and high-risk patients. The significant predictor for recurrence was risk-group, T-stage (T1 vs T2), Gleason score (≤3 + 4 and ≥4 + 3), pretreatment PSA (Conclusions: Prognosis of HIFU for Patients with localized prostate cancer was good, and the low and intermediate-risk patients with T1-staging are suitable indications for HIFU. Effective predictors for outcomes were risk-group, T-stage, Gleason score, pretreatment PSA and nadir PSA.

Extremely low-volume, high-intensity interval training improves exercise capacity and increases mitochondrial protein content in human skeletal muscle

Purpose: The purpose of the present study was to evaluate whole-muscle content of several proteins involved in the regulation of skeletal muscle mitochondrial protein content and anaerobic capacity following 4 weeks of extremely low-volume high-intensity interval training (HIT). Methods: Young, healthy, recreationally active adult males (n = 8) trained 4 times a week for 4 weeks on a cycle ergometer. Each session involved 4 min of total exercise comprised of eight 20 s intervals at ~170% of peak aerobic power separated by 10 s rest. Muscle biopsies were taken prior to (pre) and ~72 hrs post-training (post). Par- ticipants completed an incremental peak oxygen up- take (VO2peak) test and a Wingate test pre-, mid-, and post-training. Results: VO2peak was elevated (p p < 0.05) and post-training (pre: 40.5 ± 3.8 ml·kg-1·min-1, mid: 43.4 ± 2.5 ml·kg-1·min-1, post-: 47.2 ± 2.9 ml·kg-1·min-1). Wingate mean power also increased with training (pre-: 701.0 ± 73.0 W, mid-: 745.5 ± 73.3 W, post-: 786.8 ± 80.0 W). While maximal citrate synthase activity was unchanged, protein expression of the mitochondrial protein cytochrome c oxidase (COX) subunit I (+27%;p p p = 0.08) increased. Increases (p α (+19%), and nuclear PGC-1α (+46%) were also observed after 4 weeks of HIT. No changes were observed in the whole-muscle contents of PDHe1a, PDK4, SIRT1, mTOR, S6K1, MCT1, or PFK protein. Conclusions: These results demonstrate that several mitochondrial protein (but not citrate synthase activity), PGC-1α protein content, and exercise capacity can be improved in only 4 min of total training time per day, 4 days per wk using HIT cycle training.

Salvage Transrectal high-intensity Focused Ultrasound Therapy for Patients with Recurrence of Vesico-Urethral Anastomosis after Radical Prostatectomy

Background: Salvage radiotherapy has been used as the treatment for patients with local recurrence after radical prostatectomy. However, the therapy is time-consuming and it experiences adverse effects of some kind. Simple and less invasive treatment is highly anticipated. Objectives: To evaluate the outcomes of salvage transrectal high-intensity focused ultrasound (HIFU) therapy for patients with localized recurrence of a vesicourethral anastomosis (VUA) after radical prostatectomy. Material and methods: Sixteen patients with suspected local recurrence of a VUA after prostatectomy were treated with HIFU. All patients had prostate-specific antigen (PSA) failure (>0.2 ng/ml), positive findings of a VUA with biopsy and/or MRI, TRUS and CT, and no distant metastasis by CT, MRI and bone scintigraphy before HIFU. Recurrence after HIFU was determined by PSA failure (>0.2 ng/ml), histological findings, metastasis and start of systemic therapies. Results: HIFU treatments were performed in 16 patients, and followed-up for 7 - 159 months (median 46.5). The pre-HIFU PSA levels ranged from 0.318 to 3.1 ng/ml. Sonication time ranged from 9 - 42 min. All patients had a decline of PSA after HIFU, and 88% of the PSA nadir was Conclusion: HIFU therapy for local recurrence after prostatectomy may become a feasible salvage therapeutic option because of its ease and simple procedure. For salvage HIFU therapy, further research and additional follow-up are required to evaluate and correct the diagnosis of recurrence areas and to provide the sufficient sonication.

Health Promotion in Cardiac Rehabilitation Patients through the Use of a high-intensity Interval Training Protocol

According to the American Heart Association’s (AHA) recent statistical update, over 2150 Americans die each day from cardiovascular disease (CVD), which equals approximately 1 death every 40 seconds;many of which were under the age of 65 years old [1]. In 2009, 386,324 people, 1 in 6 Americans, died as a result of coronary artery disease (CAD) alone [1]. They also estimate 150,000 people have “silent” heart attacks each year [1]. Even though the number of cardiovascular disease deaths has declined in the last 10 years, they still accounted for 32.3% of American deaths [1]. As a result, the AHA updated their 2020 goals to improve the nation’s cardiovascular health by 20% [1]. One of these methods is through the use of cardiac rehabilitation. Cardiac rehabilitation (CR) is a health promotion strategy to help return cardiac patients to their previous level of functioning, increase health, decrease comorbidities and promote education and lifestyle change. For select patients, another alternative exercise plan may exist to gain even better results. high intensity interval training (HIIT) has shown positive training results for athletes and many studies show that it may also be an effective exercise modality for many cardiac patients instead of the traditional circuit training method. This article will review current literature on the effects of HIIT on CR patients as well as a sample HIIT protocol for instituting this treatment with appropriate patients.
