Introduction: head and neck cancers represent a significant burden of cancer, collectively ranking fourth for cancer incidence and second for cancer mortality worldwide. Objective: To study the epidemiological and histopathological aspects of head and neck cancers in the ENT-CCF department of Ignace Deen National Hospital. Materials and Methods: This was a descriptive retrospective study covering a period of 5 years from September 1, 2016, to September 30, 2021. Results: We collected 43 cases of head and neck cancers, representing a hospital frequency of 1.85%. The mean age was 45.21 ± 13.23 years. Males predominated, accounting for 65.12%, compared to 34.88% for females, with a male-to-female ratio of 1.87. Housewives accounted for 27.90%, followed by farmers at 23.26% and traders/merchants at 18.60%, with 41.86% being uneducated. The main risk factors were tobacco (37.21%) and alcohol (34.88%). Pain (53.49%), dysphonia (51.16%), and dyspnea (39.53%) were the main presenting symptoms. Squamous cell carcinoma (74.42%) and large cell lymphoma (18.60%) were the most commonly cited histological types. Conclusion: head and neck cancers remain relatively common in our country. Improving prognosis depends on early diagnosis of the condition, resuscitation resources, and prompt management.

Clinical applications of free medial tibial flap with posterior tibial artery for head and neck reconstruction after tumor resection

Tumor resection causes damage in the head and neck which creates problems in swallowing, chewing, articulation, and vision, all of which seriously affect patients' quality of life. In this work, we evaluated the application of a free medial tibial flap in reconstruction of head and neck defects after tumor resection. We discussed the anatomy, surgical technique, and the advantages and disadvantages of the flap. We found several benefits for the flap, such as, it is especially effective for the defects that require thin-layer epithelium to cover or the separated soft tissue defect; a two-team approach can be used because the donor site is far away from the head and neck; and the flap is easy to integrate because of the subcutaneous fat layer of the free medial tibial flap is thin and the flap is soft. Thus, the medial tibial flap could replace the forearm flap for certain applications.

Acquisition repeatability of MRI radiomics features in the head and neck:a dual-3D-sequence multi-scan study

Radiomics has increasingly been investigated as a potential biomarker in quantitative imaging to facilitate personalized diagnosis and treatment of head and neck cancer(HNC),a group of malignancies associated with high ***,the feature reliability of radiomics is a major obstacle to its broad validity and generality in application to the highly heterogeneous head and neck(HN)*** particular,feature repeatability of radiomics in magnetic resonance imaging(MRI)acquisition,which is considered a crucial confounding factor of radiomics feature reliability,is still sparsely *** study prospectively investigated the acquisition repeatability of 93 MRI radiomics features in ten HN tissues of 15 healthy volunteers,aiming for potential magnetic resonance-guided radiotherapy(MRgRT)treatment of *** subject underwent four MRI acquisitions with MRgRT treatment position and immobilization using two pulse sequences of 3D T1-weighed turbo spin-echo and 3D T2-weighed turbo spin-echo on a 1.5T MRI *** repeatability of radiomics feature acquisition was evaluated in terms of the intraclass correlation coefficient(ICC),whereas within-subject acquisition variability was evaluated in terms of the coefficient of variation(CV).The results showed that MRI radiomics features exhibited heterogeneous acquisition variability and uncertainty dependent on feature types,tissues,and pulse *** a small fraction of features showed excellent acquisition repeatability(ICC>0.9)and low within-subject *** MRI scans improved the accuracy and confidence of the identification of reliable features concerning MRI acquisition compared to simple test-retest repeated *** study contributes to the literature on the reliability of radiomics features with respect to MRI acquisition and the selection of reliable radiomics features for use in modeling in future HNC MRgRT applications.

HIGH DOSE FRACTION RADIOTHERAPY FOR MUCOSAL MALIGNANT MELANOMA OF THE head AND neck

Objective: To evatuate the results of high dose fraction radiotherapy for mucosal malignant melanoma of the head and neck (HNMM) Methods: From 1984—1994, 35 patients with HNMM were enrolled in this study Among them, 27 cases localized to the nasal cavity or para nasal sinus, 8 to the oral cavity All patients received high dose fraction radiotherapy (6—8 Gy/fraction) with the total dose ranged from 40 to 60 Gy Results: The minimum follow up was 2 years (ranged 2—7 years) The overall 3 and 5 year survival rate was 45 7% and 24%, respectively Conclusion: High dose fraction radiotherapy is effective for local control of HNMM

Neoadjuvant chemotherapy of cisplatin and fluorouracil regimen in head and neck squamous cell carcinoma： a meta-analysis

Background The benefit of neoadjuvant chemotherapy in the management of head and neck squamous cell carcinomas （HNSCC） still remains controversial. The aim of this meta-analysis is to evaluate the role of the neoadjuvant chemotherapy with the cisplatin and fluororacil （PF） regimen in enhancing the overall survival of and decreasing Iocoregional relapse and distant metastasis in HNSCC patients. Methods Medline and manual searches were performed to identify all published randomized controlled trials （RCTs） investigating the efficacy of the neoadjuvant chemotherapy with the PF regimen. Outcomes assessed by meta-analysis included Iocoregional relapse, distant metastasis, and overall survival. The odds ratio was the principle measurement of effect, which was calculated as the treatment group （chemotherapy plus Iocoregional treatment） versus the control group （Iocoregional treatment alone） and was presented as a point estimate with 95% confidence intervals （CI）. Results Eight RCTs were adopted for analysis. The meta-analysis showed that the odds ratio for the Jocoregional relapse was 0.92 （0.70-1.22, 95%CI）, which was not statistically significant. The odds ratios for distant metastasis and overall survival were 0.47 （0.33-0.68, 95% CI） and 1.28 （1.01-1.62, 95% CI） respectively, which were both statistically significant. Conclusions Neoadjuvant chemotherapy with the PF regimen in HNSCC patients has no effect on Iocoregional relapse. However, it shows a small but significant benefit in reducing distant metastasis and improving the overall survival.
