To the Editor:Endoscopic sleeve gastroplasty(ESG)is a recently developed incisionless transoral gastroscopic bariatric procedure that can shrink the gastric cavity into a tubular structure similar to the capacity of surgical sleeve gastrectomy by means of endoscopic full-thickness sutures.Clinical practice has proven its safety,durability,minimal invasiveness,and long-term weight loss effects,which are close to those of bariatric surgery.

To the Editor:Peripheral nerve injury(PNI)refers to the injury of the peripheral nerve plexus,nerve trunk,or its branches undertheaction of externalforces.Skeletal muscle-derived cells servea strong secretory,function while they can differentiate into Schwann cell-like cells or other ceil types,so that it can,effectively promote regeneration'of peripheral nerves.llUsing some specific acupuncture points with mild mechanical stimulation on the surface can have a positive effect on nerve repair.2i Tuina helps relieve symptoms such as muscle spasms and pain caused by nerve damage.There are three points belonging to the Bladder meridian of foot Taiyang(BL)and the Gallbladder meridian of foot Shaoyang(GB),which usually are selected to carry out Tuina.According to the anatomical nerve position,Yinmen(BL37)is on the sciatic nerve trunk,Chengshan(BL57)is on the tibial nerve,and Yanglingquan(GB34)is on the common peroneal nerve;meanwhile,the three points are on the sciatic nerve or its branches.The three points are on the biceps femoris,gastrocnemius,and tibial anterior muscles.

Objective To compare the efficiency of the different varieties of Bupleurum and Citrus for chinical application and to investigate their mechanisms for clinical application.Methods With oral Blue Dextran 2000 as a marker, mice gastro-intestinal motility was studied with two varieties of Bupleurm and Citrus separately, viz: BcDC, BsW, Ca, and Cs. Gastric emptying time(GET) was evaluated and plasma motilin level was detected in 15 healthy volunteers who had taken daily the mixture of BcDC and Ca for 7 days.Results BcDC and Ca had obvious enhancing effects on gastric emptying and small intestinal propulsive function, while the effect of BsW and Cs showed no difference in negative control group (P>0.05). The GET of 15 healthy persons was shortened and their plasma motilin level was increased after they had taken the mixture for 7 days successively.Conclusion Decoctum of BcDC, Ca, and BcDC with Ca together proved to be strong promoters on gastrointestinal motility and their mechanism was probably by increasing the endogenous plasma motilin level.

The biomedical model,which limits itself to finding the attributions of organic disease,is challenged by gastrointestinal(GI)symptoms.Simultaneously,physicians'attribution of GI symptoms to underlying psychological issues is not readily accepted by patients and can negatively affect the clinical rapport between doctor and patient.In reality,psychosocial aspects are involved in many functional disorders and organic diseases,not just in mental disorders.Time is overdue for gastroenterologists to recognise the inadequacy and limitations of conventional gastroenterology and consider the role of psychological,social and biological variables throughout the entire clinical course of the illness,as is shown in George Engel’s model.This review discusses the following:(1)the current challenges of using the conventional clinical model for both functional and organic GI illness,(2)the inadequacy and limitations of explaining GI symptoms simply as psychological disorders,(3)the exploration of the symptom-centred,stepped reattribution clinical model,(4)the clarification of psychosomatic medical concepts for use in gastroenterology,and(5)the significance of a systematic and interdisciplinary framework for a comprehensive psychosomatic model in gastroenterology.

AIM: To investigate the role of endothelial nitric oxide synthase-786 T > C promoter polymorphism in the etiology of gastric cancer(GC). METHODS: A total of 150 GC patients and 150 control subjects were included in the study. The information on demographic features was elicited with an informed consent from all the patients and control subjects using a structured questionnaire. Helicobacter pylori(H. pylori) infectivity status was tested in antral biopsies from all the subjects by rapid urease test following the method of Vaira et al. Genomic DNA was isolated from whole blood samples following the salting out method of Lahiri et al. Genotype analysis of the rs2070744 polymorphism was carried out by allele-specific polymerase chain reaction method. The genotypes were determined based on the appearance of bands on an agarose gel stained with ethidium bromide under ultraviolet gel documentation with the help of 100 bp ladder. Odds ratios and corresponding 95%CIs were determined using java stat online software. RESULTS: There was a significant difference in the distribution of C allele(C vs T; P = 0.000, OR = 5.038) in patient group compared to the control subjects exhibiting a fivefold increased risk for GC. When the T/T and C/C genotypes were compared, there was an enhanced GC risk for individuals with C/C genotype(T/T vs C/C; P = 0.000). Among the demographic factors, smoking and alcoholism were the common risk factors in patients compared to the control subjects(P < 0.05). Patients with smoking and alcoholism developed cancer even in heterozygous T/C condition(smoking: P = 0.020 and alcoholism: P = 0.005). Individuals with H. pylori infection showed seven fold increased risk for cancer. All the patients with C/C genotype revealed a significant association between H. pylori infection and GC. Among the patients 2.4% of them revealed familial incidence of GC. No significant difference was noticed between cases and controls with regard to consanguinity(P = 0.473).CONCLUSION: The Present data suggest that eN OS-786 C/

Objective A recent publication has suggested that expedited time to theater in gastroschisis results in higher rates of primary closure and decreases the length of stay(LOS).This study primarily aims to assess the impact of time to first management of neonates with gastroschisis on the LOS.Methods Neonates admitted between August 2013 and August 2020 with gastroschisis were included.Data were collected retrospectively,and neonates with complex gastroschisis were excluded.Variables including gestation,birth weight,time of first management,primary/delayed closure and use of patch were evaluated as possible confounding variables.The outcome measures were time to full feeds,time on parenteral nutrition(PN)and LOS.Univariate and multivariate linear regression analyses were performed.Pgastroschisis,eight patients with time to first management not documented).The median LOS for those who underwent primary closure was 21 days(interquartile range(IQR)=16-29)and for those who underwent silo placement and delayed closure was 59 days(IQR=44-130).The mean time to first management was 473 min(standard deviation(SD)=146 min),with only 20% of these infants being operated on at less than 6 hours of age.Univariate and multivariate analyses demonstrated no relationship between time to first management and LOS(r^(2)=0.00,p=0.82)but did demonstrate a consistent positive association between time to first feed and LOS and delayed closure,resulting in a longer time to full feeds and a longer time on PN.Conclusions The time to first management was not associated with a change in LOS in these data.Further prospective evaluation of the impact of reducing the time to first feed on the LOS is recommended.

Dear editor,The incidence of emergency department(ED)visits from gastroparesis and cannabinoid hyperemesis syndrome has increased by two folds over the past decade.[1-3]Roldan et al[4]reported that patients with these conditions often have prolonged ED stays and are likely to be hospitalized because of the danger of dehydration and challenges inherent in relieving symptoms.Opioid analgesia is a particularly troublesome treatment modality;these conditions are often chronic with frequent recurrences,and repeated opiate therapy can cause opioid dependency and narcotic bowel syndrome.[5]Haloperidol(HP)and droperidol,members of the butyrophenone class,have potent antidopaminergic activity in the chemoreceptor trigger zone.[6,7]Intramuscular administration of HP is being used increasingly in the ED to treat nausea,vomiting secondary to gastroparesis,[6]intractable vomiting,[8]and cyclical vomiting.
