Role of the proportion of dominant follicles in patients with polycystic ovary syndrome undergoing in vitro fertilization-embryo transfer

Background: There is no absolute consensus for the best time for triggering. The aim of this study was to investigate the effect of different proportion of dominant follicles (PDF) on the human chorionic gonadotropin (HCG) day for the clinical outcomes in patients with polycystic ovary syndrome (PCOS) of different ovarian stimulation protocols. Methods: A total of 371 cycles of the gonadotropin-releasing hormone (GnRH) agonist long protocol and 347 cycles of GnRH antagonist protocol from January 2014 to December 2016 were included in this retrospective study. Based on the PDF on the day of the HCG administration, the included patients were divided into three groups: Group A (low PDF), PDF 40%. The measurements regarding ovarian stimulation characteristics, fertilization rate, top quality embryo rate, clinical pregnancy rate, and ovarian hyperstimualtion syndrome (OHSS) rate were compared in different PDF groups with different protocols. Results: In both the GnRH antagonist protocol and GnRH agonist long protocol, the characteristics such as mean age, anti-Mullerian hormone, antral follicle count (AFC), and body mass index were comparable between groups. The number of oocytes retrieved decreased statistically significantly as the PDF and rate of matured oocytes increased. In the GnRH agonist long protocol, the rate of normally fertilized oocytes was highest in Group A (59.74 ± 31.21 vs. 49.70 ± 37.95, 49.67 ± 36.62;F = 3.743, P = 0.025). There were no significant differences in the rate of top-quality embryos and the clinical pregnancy rate between the groups. The clinical pregnancy rate was similar in the three groups (63.6%, 62.5%, 67.5%, respectively,χ2 = 0.989, P = 0.911). The moderate and severe OHSS rate increased statistically significantly when the PDF increased, which was highest in group C (1.4%, 3.1%, 6.7%, respectively,χ2 = 12.014, P = 0.017). NANOG Alleviates the Damage of Human Hair Follicle Mesenchymal Stem Cells Caused by H_2O_2 through Activation of AKT Pathway

Objective To explore the protective effect of NANOG against hydrogen peroxide(H_2O_2)-induced cell damage in the human hair follicle mesenchymal stem cells(hHF-MSCs). Methods NANOG was expressed from a lentiviral vector, pLVX-IRES-ZsGreen. NANOG hHF-MSCs and vector hHF-MSCs were treated with 400 μmol/L hydrogen peroxide(H_2O_2) for 2 h, the cell survival rate, cell morphology, ROS production, apoptosis and expression of AKT, ERK, and p21 were determined and compared. Results Our results showed that NANOG could activate AKT and upregulate the expression of p-AKT, but not p-ERK. When treated with 400 μmol/L H_2O_2, NANOG hHF-MSCs showed higher cell survival rate, lower ROS production and apoptosis, higher expression of p-AKT, higher ratio of p-AKT/AKT. Conclusion Our results suggest that NANOG could protect hHF-MSCs against cell damage caused by H_2O_2 through activating AKT signaling pathway.

Background:Heat stress is known to affect follicular dynamics,oocyte maturation,and fertilization by impairing steroidogenic ability and viability of bovine granulosa cell(bGCs).The present study explored the physiological and molecular response of bGCs to different heat stress intensities in-vitro.We exposed the primary bGCs to heat stress(HS)at 39℃,40℃ and 41℃ along with control samples(38℃)for 2 h.To evaluate the impact of heat stress on bGCs,several in vitro cellular parameters including cell apoptosis,intracellular reactive oxygen species(ROS)accumulation and HSP70 kinetics were assessed by flow cytometry,florescence microscopy and western blot,respectively.Furthermore,the ELISA was performed to confirm the 17β-estradiol(E2)and progesterone(P4)levels.In addition,the RNA sequencing(RNA-Seq)method was used to get the molecular based response of bGCs to different heat treatments.Results:Our findings revealed that the HS significantly decreased the cell viability,E2 and P4 levels in bGCs,whereas,increased the cellular apoptosis and ROS.Moreover,the RNA-Seq experiments showed that all the treatments(39℃,40℃ and 41℃)significantly regulated many differentially expressed genes(DEGs)i.e.BCL2L1,STAR,CYP11A1,CASP3,SOD2,HSPA13,and MAPK8IP1 and pathways associated with heat stress,apoptosis,steroidogenesis,and oxidative stress.Conclusively,our data demonstrated that the impact of 40°C treatment was comparatively detrimental for cell viability,apoptosis and ROS accumulation.Notably,a similar trend of gene expression was reported by RT-qPCR for RNA-seq data.Conclusions:Our study presented a worthy strategy for the first time to characterize the cellular and transcriptomic adaptation of bGCs to heat stress(39,40 and 41°C)in-vitro.The results infer that these genes and pathways reported in present study could be useful candidates/indicators for heat stress research in dairy cattle.Moreover,the established model of bGCs to heat stress in the current study provides an appropriate platform to understand the mechanism of how hea

Cumulus Cells and their Associations with Immune Functions

The recent progress in the association of cumulus cells with immune functions is a largely ignored area. With over 350 million new sexually transmitted infections occurring annually in adults of reproductive age, we feel the need to explore more about how the cumulus cells defend themselves and protect the oocytes during the development through the ovulation period. Application of assisted reproductive technologies allows scientists to study and better understand cumulus cells. There are still many immune factors to be taken into consideration to optimize the oocyte quality besides ovarian stimulations. The objective of this review is to summarize the key elements of cumulus cells and their association with the immune function.

Follicular Fluid Cortisol Releasing Hormone (CRH) Levels and Assisted Reproductive Treatment (ART) Outcomes

Purpose: Can Cortisol Releasing Hormone (CRH) levels in follicular fluid predict outcomes following assisted reproductive treatment (ART) cycles? Methods: Prospective cohort study of 50 women undergoing in vitro fertilisation (IVF)/intra-cytoplasmic sperm injection (ICSI) cycles over a two month study period. All patients were treated on the long stimulation protocol;follicular fluid was aspirated and pooled for each patient. The samples were processed appropriately and assayed using CRH radioimmunoassay (RIA). Results: This study confirmed that CRH was present in follicular fluid. The average level detected was 173 ± 9 pg/mL (mean ± standard error of mean [SEM]). The data suggests a positive correlation of CRH follicular fluid levels greater than 145 pg/mL with successful ART outcomes. Conclusion: The data indicates a positive correlation between ART outcomes and the presence of follicular fluid CRH levels greater than 145 pg/mL. The results should be interpreted with caution due to the small sample size and pooling of follicular fluid per patient. Furthermore, the pooling of follicular fluid is not representative of CRH levels in an individual follicle, and thus, mature oocyte. This study serves as a reminder to what has previously been hypothesised.

Efficacy of Qigong for the treatment of alopecia universalis: a clinical case report

Qigong is a part of traditional Chinese medicine. According to the antique treaties, the foundations of traditional Chinese medicine are based in maintaining the harmony between Qi (energy), Jing (essence) and Shen (spirit). Alterations in this harmony can develop and determine the appearance of disease. Therapeutic effects of Qigong are helpful in the prevention and treatment of several diseases. It's main role in restoring body functions is due to the communication between internal organs, tissues and cells. The main goal of this work is to show the efficacy of Qigong in the treatment of a dermatological disease characterized by the appearance of circular or oval patches of missing hair, known as alopecia universalis. For western medicine the exact cause of this illness is not totally revealed, however, it's attributed to psycological, genetics and metabolic alterations. From the traditional Chinese medicine perspective, hair alterations are framed in the area of the water energy, represented by the kidney organ. The results of Qigong treatment suggested that the cranial hair follicles had been activated since the first treatment. As treatment progressed, the cranial hair follicles continued to grow, what's more, the brightness and pigmentation of the hair also increased. Therefore, in the clinical conditions evaluated, Qigong could be an effective alternative treatment in consideration of the visible evidence about rapid and long lasting results. Besides, we did not observe any side effects of Qigong in this case.

The Effect of <i>Momordica charantia</i>on Ovarian Ischemia-Reperfusion

Ovarian torsion is one of the gynecological emergencies that affect ovarian tissue integrity and fertility. In this study, we aim to investigate the effects of Momordica charantia (MC) on ovarian tissue injury induced by ovarian ischemia-reperfusion. Twenty-four rats were assigned to three groups: control group, ischemia-reperfusion (I/R) group and IR + MC group. 600 mg/kg MC extract were given rats via orogastric route. 3-hour ischemia and 3-hour reperfusion were performed for IR protocol. At the end of experiment, ovarian tissues were excised, fixed in zinc-formalin and processed for routine paraffin wax tissue embedding. Sections were stained with hematoxylin and eosin. Ovarian sections of control group showed regular histological appearance. In ovarian sections of I/R group, hemorrhage, fibrin accumulation, mononuclear cell infiltration and degenerated follicles were observed. I/R + MC group showed improved histopathology caused by I/R injury. We think that MC extract may protect ovarian tissue integrity and its histological structures.

Investigation of Ovarian Follicular Waves and Major Hormonal Profile in Red Chittagong Cattle

Red Chittagong Cattle (RCC) is one of the renowned varieties of cattle which is indigenous and most potential type of domestic animal genetic resource in Bangladesh with distinct phenotypic characteristics. Reproductive management might be a process for multiplying this cattle variety. Therefore, the goals of this study were to characterize the follicular waves of RCC and to determine the Progesterone (P4), Follicle Stimulating Hormone (FSH) and Luteinizing Hormone (LH) profile during three consecutive estrus cycle. Follicular wave emergence (Day 0) was identified using trans-vaginal ultrasound technique and follicular growth was then monitored from Day 0 to Day 23 during three consecutive estrus cycle and blood samples were collected. After measuring the growth and diameter of the follicles, hormonal assays of collected blood samples were performed using Enzyme Linked Immunosorbent Assay (ELISA). We found that, both RCC cows and heifers showed two waves inter-ovulatory intervals. The length of inter-ovulatory intervals and the diameter of the ovulatory follicle of RCC cows and heifers were slightly different from each other. The identification of the dominant follicle was confirmed from the start of its deviation from other subordinate follicles till it reached to the maximum diameter and identified as dominant follicle which increases linearly for approximately six days (growing phase). This dominant follicle is remained approximately same size for five days (static phase), and then begins to regress (regressing phase) decreasing the antral diameter. The follicular waves are first detectable as 4 - 5 mm follicles approximately at day 0 and day 10 both for RCC cows and heifers which showed two waves inter-ovulatory intervals. The circulating varied concentrations of LH, FSH and P4 were determined using ELISA in case of both cows and heifers and the concentration curves followed the similar pattern of change during the estrus cycle. Anti-Mullerian Hormone: Establishing the Ovarian Reserve Range with Age in Rivers State Women, Niger Delta Region of Nigeria

Background: The measurement of circulating anti-Mullerian hormone (AMH) in the plasma of adult women has been used as a tool in the assessment of ovarian reserve. This is based on its ability to reflect the number of growing follicles in the ovaries and knowing the level of AMH in a woman’s blood is generally a good indicator of her ovarian reserve and this can be achieved by making reference to a decision values. Objective: The purpose of this study was to measure the level of this hormone in normal, apparently healthy subjects in Rivers State, Nigeria with respect to age. The percentage of women, at their reproductive age with infertility problems in the state is in the increase and the essence was to establish a reference range for the hormone because of its clinical application in women fertility. Materials/method: A total of one thousand and sixty two apparently healthy women divided into four age groups were recruited for this study;this comprised of 300 each in age group of 20 - 30, 31 - 40, 41 - 50 respectively and 162 in 51 - 60 years age group from May 2014 to June 2017. They were randomly selected after filling a questionnaire form that has the information of age, tribe, last date of menses and whether on medication for any infertility problems. Excluded from the study were subjects identified with different endocrine dysfunction and/or on drugs for different types of anovulatory dysfunction. The Enzyme linked Immunoassay method was used in the measurement of the AMH. Results: The result from the measurement of plasma AMH levels showed a Mean ± SD value of 3.193 ± 0.943, 1.644 ± 0.691, 0.516 ± 0.276 and 0.135 ± 0.081 ng/ml respectively for the 1st, 2nd, 3rd and 4th age groups respectively. The mean value for the AMH decreases with increasing age and was statistically significant at the different age group levels (P −2.250 - 4.136;0.953 - 2.350;0.240 - 0.792 and 0.054 - 0.216 respectively for the age groups.
