Pelvic floor disorders(PFDs)represent a group of common and frequentlyoccurring diseases that seriously affect the life quality of women,generally including stress urinary incontinence and pelvic organ prolapse.Surgery has been used as a treatment for PFD,but almost 30%of patients require subsequent surgery due to a high incidence of postoperative complications and high recurrence rates.Therefore,investigations of new therapeutic strategies are urgently needed.Stem cells possess strong multi-differentiation,self-renewal,immunomodulation,and angiogenesis abilities and they are able to differentiate into various cell types of pelvic floor tissues and thus provide a potential therapeutic approach for PFD.Recently,various studies using different autologous stem cells have achieved promising results by improving the pelvic ligament and muscle regeneration and conferring the tissue elasticity and strength to the damaged tissue in PFD,as well as reduced inflammatory reactions,collagen deposition,and foreign body reaction.However,with relatively high rates of complications such as bladder stone formation and wound infections,further studies are necessary to investigate the role of stem cells as maintainers of tissue homeostasis and modulators in early interventions including therapies using new stem cell sources,exosomes,and tissueengineering combined with stem cell-based implants,among others.This review describes the types of stem cells and the possible interaction mechanisms in PFD treatment,with the hope of providing more promising stem cell treatment strategies for PFD in the future.

Purpose: Primary reconstruction via transconjunctival approach is a standardized treatment option for orbital floor fractures. The aim of this study was to compare the findings of specific ophthalmologic assessment with the patient's complaints after fracture reduction. Methods: A retrospective medical chart analysis was performed on patients who had undergone transconjunctival orbital floor fracture reduction for fracture therapy with resorbable foil (ethisorb sheet or polydioxanone foil). A follow-up assessment including ophthalmological evaluation regarding visual acuity (eye chart projector), binocular visual field screening (Bagolini striated glasses test) and diplopia (cover test, Hess screen test) was conducted. Additionally, a questionnaire was performed to assess patients' satisfaction. Results: A total of 53 patients with a mean follow-up of 23 months (ranging from 11 to 72) after surgical therapy were included. Diplopia was present preoperatively in 23 (43.4%) and reduced in follow-up examination (n = 12, 22.6%). Limitations in ocular motility reduced from 37.7% to 7.5%. The questionnaire about the patient's satisfaction revealed excellent outcomes in relation to the functional and esthetical parameters. Conclusion: Transconjunctival approach is a safe approach for orbital fracture therapy. Postoperative diplopia is nearly never perceptible for the individual and differs to pathologic findings in the ophthalmic assessment.

Pelvic floor muscle exercise(PFME)is the most common con servative manageme nt for urinary incon tine nee(Ul)after radical prostatectomy(RP).However,whether the PFME guided by a therapist(G-PFME)can contribute to the recovery of urinary continenee for patients after RP is still controversial.We performed this meta-analysis to investigate the effectiveness of G-PFME on Ul after RP and to explore whether the additional preoperative G-PFME is superior to postoperative G-PFME alone.Literature search was con ducted on Cochra ne Library,Embase,Web of Scie nee,and PubMed,to obtai n all re leva nt randomized controlled trials published before March 1,2018.Outcome data were pooled and an a lyzed with Review Man ager 5.3 to compare the conti nence rates of G-PFME with control and to compare additional preoperative G-PFME with postoperative G-PFME.Twenty-two articles with 2647 patients were included.The conti nence rates of G?PFME were all superior to con trol at d iff ere nt follow-up time points,with the odds ratio(OR)(95%confidence interval[Cl])of 2.79(1.53-5.07),2.80(1.87-4.19),2.93(1.19-7.22),4.11(2.24-7.55),and 2.41(1.33-4.36)at 1 mon th,3 months,4 months,6 months,and 12 months after surgery,respectively.However,there was no difference between additional preoperative G-PFME and postoperative G-PFME,with the OR(95%Cl)of 1.70(0.56-5.11)and 1.35(0.41-4.40)at 1 month and 3 months after RP,respectively.G-PFME could improve the recovery of urinary continence at both early and Iong-term stages.Starting the PFME preoperatively might not produce extra benefits for patients at early stage,compared with postoperative PFME.

AIM To introduce a novel,modified primary closure technique of laparoscopic extralevator abdominal perineal excision(LELAPE) for low rectal cancer.METHODS We retrospectively analyzed data from 76 patients with rectal cancer who underwent LELAPE from March 2013 to May 2016.Patients were classified into the modified primary closure group(32 patients) and the biological mesh closure group(44 patients).The total operating time,reconstruction time,postoperative stay duration,total cost,postoperative complications and tumor recur-rence were compared.RESULTS All surgery was successfully performed.The pelvic reconstruction time was 14.6 ± 3.7 min for the modified primary closure group,which was significantly longer than that of the biological mesh closure group(7.2 ± 1.9 min,P floor in LELAPE for low rectal cancer is technically feasible,safe and cost-effective.

A full-scale composite floor plate was tested to investigate the flexural behavior and in-plane effects of the floor slab in a grillage of composite beams that reduces the tendency for longitudinal splitting of the concrete slab along the line of the primary beams. This is important in cases where the steel decking is discontinuous when it is orientated parallel to the beams. In this case, it is important to demonstrate that the amount of transverse reinforcement required to transfer local forces from the shear connectors can be reduced relative to the requirements of Eurocode 4. The mechanism under study involved in-plane compression forces being developed in the slab due to the restraining action of the floor plate, which was held in position by the peripheral composite beams;while the secondary beams acted as transverse ties to resist the forces in the floor plate that would otherwise lead to splitting of the slab along the line of the primary beams. The tendency for cracking along the center line of the primary beam and at the peripheral beams was closely monitored. This is the first large floor plate test that has been carried out under laboratory conditions since the Cardington tests in the early 1990s, although those tests were not carried out to failure. This floor plate test was designed so that the longitudinal force transferred by the primary beams was relatively high (i.e., it was designed for full shear connection), but the transverse reinforcement was taken as the minimum of 0.2% of the concrete area. The test confirmed that the primary beams reached their plastic bending resistance despite the discontinuous decking and transverse reinforcement at the minimum percentage given in Eurocode 4. Based on this test, a reduction factor due to shear connectors at edge beams without U-bars is proposed.

This study aimed to introduce a minimally invasive technique for maxillary sinus floor elevation using the lateral approach(lSFE)and to determine the factors that influence the stability of the grafted area in the sinus cavity.Thirty patients(30 implants)treated with lSFE using minimally invasive techniques from 2015 to 2019 were included in the study.Five aspects of the implant(central,mesial,distal,buccal,and palatal bone heights[BHs])were measured using cone-beam computed tomography(CBCT)before implant surgery,immediately after surgery(T0),6 months after surgery(T1),and at the last follow-up visit(T2).Patients’characteristics were collected.A small bone window(height,(4.40±0.74)mm;length,(6.26±1.03)mm)was prepared.No implant failed during the follow-up period(3.67±1.75)years.Three of the 30 implants exhibited perforations.Changes in BH of the five aspects of implants showed strong correlations with each other and BH decreased dramatically before second-stage surgery.Residual bone height(RBH)did not significantly influence BH changes,whereas smoking status and type of bone graft materials were the potentially influential factors.During the approximate three-year observation period,lSFE with a minimally invasive technique demonstrated high implant survival rate and limited bone reduction in grafted area.In conclusion,lSFE using minimally invasive techniques was a viable treatment option.Patients who were nonsmokers and whose sinus cavity was filled with deproteinized bovine bone mineral(DBBM)had significantly limited bone resorption in grafted area.

Maxillary sinus floor augmentation using lateral window and crestal technique is considered as predictable methods to increase the residual bone height;however,this surgery is commonly complicated by Schneiderian membrane perforation,which is closely related to anatomical factors.This article aimed to assess anatomical factors on successful augmentation procedures.After review of the current evidence on sinus augmentation techniques,anatomical factors related to the stretching potential of Schneiderian membrane were assessed and a decision tree for the rational choice of surgical approaches was proposed.Schneiderian membrane perforation might occur when local tension exceeds its stretching potential,which is closely related to anatomical variations of the maxillary sinus.Choice of a surgical approach and clinical outcomes are influenced by the stretching potential of Schneiderian membrane.In addition to the residual bone height,clinicians should also consider the stretching potential affected by the membrane health condition,the contours of the maxillary sinus,and the presence of antral septa when evaluating the choice of surgical approaches and clinical outcomes.

Indoor floor plans are of vital importance for a wide range of indoor social applications, however, they are often unavailable due to various reasons. This paper proposes a method to automatically construct the indoor floor plan with rooms and corridors as well as landmarks, using inertial traces collected with smartphones. Landmarks,such as turnings, doors, and stairs, are identified according to inertial sensors and WiFi signals. The inertial traces are then partitioned into segments according to the turnings and classified as room type or corridor type according to the doors. Clustering is applied on room type and corridor type trace segments separately to produce room type and corridor type clusters. The construction of room applies the α-shape algorithm on room type clusters and the construction of corridor employs the principal component analysis(PCA) algorithm or the α-shape algorithm on corridor type clusters. Evaluations in two representative scenarios show that the method can construct the floor plan of acceptable accuracy with a relatively small set of inertial traces.

According to the distribution of abutment stress in a stope,this research established the mechanical model of mining abutment pressure transmission in floor base on the theory of semi-infnite plate body in elasticity.This study takes the 762 working face of Haizi Coal Mine as a case in point,and analyzed the dynamic evolution law of seam floor stress during the mining process.With an organic combination of the mining floor stress and surrounding rock stress,the study obtained the change laws of the maximum principle stress and the minimum one for the floor roadway surrounding rock when mining the upper working face.Considering the non-constant pressure force state and the cracks revolution mechanisms of floor roadway surrounding rock,the research built the mechanical model of roadway stress.Simulation results verify the reliability of the above conclusions.Moreover,this model could provide the theoretical basis and technical support for controlling floor roadway surrounding rock.
