Compensation trends of the angulation of first molars:retrospective study of 1403 malocclusion cases

We investigated the compensatory trends of mesiodistal angulation of first molars in malocclusion cases. We compared differences in the angulation of first molars in different developmental stages, malocclusion classifications and skeletal patterns. The medical records and lateral cephalogrammes of 1 403 malocclusion cases taken before treatment were measured to evaluate compensation of molar angulation in relation to the skeletal jaw. The cases were stratified by age, Angle classification and skeletal patterns. Differences in the mesiodistal angulation of the first molars were compared among the stratifications. We observed three main phenomena. first,angulation of the upper first molar varied significantly with age and tipped most distally in cases aged,12 years and least distally in cases aged.16 years. The lower first molar did not show such differences. Second, in Angle Class II or skeletal Class II cases, the upper first molar was the most distally tipped, the lower first molar was the most mesially tipped, and opposite angulation compensation was observed in Class III cases. Third, in high-angle cases, the upper and lower first molars were the most distally tipped, and opposite angulation compensation was observed in low-angle cases. These data suggest that the angulation of the molars compensated for various growth patterns and malocclusion types. Hence, awareness of molar angulation compensation would help to adjust occlusal relationships, control anchorage and increase the chances of long-term stability.

Objective:(1)To investigate both physiological and mechanical factors influencing the mesiodistal tipping of maxillary first molars in malocclusion patients. (2)To establish multiple regression models of the mesiodistal tipping of maxillary first molars before treatment(preUM/PP),treatment changes(UM/PP),after treatment(postUM/PP).(3)To evaluate how the factors influenced the mesiodistal tipping. Materials and Methods:1403 orthodontic patients who had been treated in the department of orthodontics,Peking university school and hospital of stomatology were enrolled in this retrospective study.100 variables collected from medical records and cephalomatrics data were analyzed as independent variables,while preUM/PP, UM/PP,postUM/PP were analyzed as dependent variables respectively.Significant variables were included in the multiple linear regression models respectively. Results:The regression models of preUM/PP,UM/PP,postUM/PP identified 19,14, 18 significant variables respectively which could explain 56.3%,36.8%,and 52.0%of the variances.During treatment,the upper molars tipped mesially.After treatment,the situation how the tipping were effected were similar to that before treatment. Conclusions:(1)PreUM/PP was most influenced by SNB angle.The more the mandible retruded,or mandibular plane angle increased,the more the upper first molar tipped distally.The postUM/PP condition was similar to preUM/PP. (2)UM/PP was most affected by its tipping condition before treatment.The more the upper molar tipped distally before treatment,the more it changed mesially during treatment.Tipping mesially of molar means anchorage loss.Hence,maintaining the distal-tipping condition of upper molars,Especially for classⅡpatients who have the most distal-tipping upper molars may stabilize its compensatory angulation and reduce the anchorage loss.
