Development of fetal growth charts in twins stratified by chorionicity and mode of conception:a retrospective cohort study in China

Background:Twin pregnancies continue to increase worldwide;however,the current clinical prenatal evaluation for the intrauterine growth of twins still relies on the growth standards of *** attempted to establish a set of fetal biometric references for Chinese twin pregnancies,stratified by chorionicity and conception mode as spontaneously conceived monochorionic diamniotic(SC-MCDA),spontaneously conceived dichorionic diamniotic(SC-DCDA),and assisted reproductive technology dichorionic diamniotic(ART-DCDA)***:From 2016 to 2019,the ultrasonographic fetal biometric measurements were longitudinally collected in pregnant women,including fetal weight,biparietal diameter,head circumference,abdominal circumference,femur length,and humerus *** linear mixed models were used to test the difference of growth patterns between groups,and the growth curve of each biometric parameter was modeled by a generalized additive model for location scale and ***:A total of 929 twin pregnant women and 2019 singleton pregnant women,met the inclusion *** twin pregnancies,148 were SC-MCDA,215 were SC-DCDA,and 566 were ART-DCDA ***,SC-DCDA twins grew faster than SC-MCDA twins,while slower than ART-DCDA twins(all Pfetal growth charts of each fetal biometric parameter were,respectively,constructed for SC-MCDA,SC-DCDA,and ART-DCDA ***:The fetal biometric trajectories demonstrated characteristic patterns according to chorionicity and conception *** fill the gap,we modeled fetal biometric parameters for Chinese SC-MCDA,SC-DCDA,and ART-DCDA twin pregnancies,hoping to provide a reference for the further establishment of fetal growth reference values for Chinese twin fetuses.

Automatic fetal Segmentation Designed on Computer-Aided Detection with Ultrasound Images

In the present research,we describe a computer-aided detection(CAD)method aimed at automatic fetal head circumference(HC)measurement in 2D ultrasonography pictures during all trimesters of *** HC might be utilized toward determining gestational age and tracking fetal *** automated approach is particularly valuable in low-resource settings where access to trained sonographers is *** CAD system is divided into two steps:to begin,Haar-like characteristics were extracted from ultrasound pictures in order to train a classifier using random forests to find the fetal *** identified the HC using dynamic programming,an elliptical fit,and a Hough *** computer-aided detection(CAD)program was well-trained on 999 pictures(HC18 challenge data source),and then verified on 335 photos from all trimesters in an independent test set.A skilled sonographer and an expert in medicine personally marked the test *** used the crown-rump length(CRL)measurement to calculate the reference gestational age(GA).In the first,second,and third trimesters,the median difference between the standard GA and the GA calculated by the skilled sonographer stayed at 0.7±2.7,0.0±4.5,and 2.0±12.0 days,*** regular duration variance between the baseline GA and the health investigator’s GA remained 1.5±3.0,1.9±5.0,and 4.0±14 a couple of *** mean variance between the standard GA and the CAD system’s GA remained between 0.5 and 5.0,with an additional variation of 2.9 to 12.5 *** outcomes reveal that the computer-aided detection(CAD)program outperforms an expert *** paired with the classifications reported in the literature,the provided system achieves results that are comparable or even *** have assessed and scheduled this computerized approach for HC evaluation,which includes information from all trimesters of gestation.

Decidual natural killer cells and the immune microenvironment at the maternal-fetal interface

During early pregnancy, an orchestrated evolutionary maternal adaption toward tolerance of the semiallogeneic fetus is required to ensure decidualization and early embryo development. Remodeling of the immune system involves natural killer cells （NKs）, macrophages, T cells and dendritic cells （DCs） altering the microenvironment in the deciduas. In particular, a unique population of NK cells with a CD56brightCD16 phenotype in the decidua has been proposed to play a key role in the maternal adaptation to pregnancy. However, there is a tendency for pregnancy immunology to reflect transplantation immunology regarding the assumption that the matemal immune system should be suppressed. This tendency is misleading. We discuss how the immune system is formed in early deciduas and the interactions between maternal NK cells and fetal growth. We propose that the maternal immune response must not be fully suppressed and is even necessary for the local response of uterine NK cells.

The number of growth factors involved in female fertility has been extensively studied, but reluctance to add essential growth factors in culture media has limited progress in optimizing embryonic growth and implantation outcomes, a situation that has ultimately led to reduced pregnancy outcomes. Insulin-like growth factor Ⅱ(IGF-Ⅱ) is the most intricately regulated of all known reproduction-related growth factors characterized to date, and is perhaps the predominant growth factor in human ovarian follicles. This review aims to concisely summarize what is known about the role of IGF-Ⅱ in follicular development, oocyte maturation, embryonic development, implantation success, placentation, fetal growth, and in reducing placental cell apoptosis, as well as present strategies that use growth factors in culture systems to improve the developmental potential of oocytes and embryos in different species. Synthesizing the present knowledge about the physiological roles of IGF-Ⅱ in follicular development, oocyte maturation, and early embryonic development should, on the one hand, deepen our overall understanding of the potential beneficial effects of growth factors in female reproduction and on the other hand support development(optimization) of improved outcomes for assisted reproductive technologies.

Outcomes of fetal Macrosomia and Associated Factors: A Case-Control Facility Based Study

Objective: To identify risk factors of perinatal complications among macrosomic babies in a third level health care facility. Method: We conducted a case-control institutional based study. Cases (macrosomic babies and mothers with perinatal complications) and controls (pairs free of perinatal complication) of singleton live births were extracted from the maternity registry from January 2017 to December 2019. Matching was done for sex and gestational age after exclusion of genetic cause of macrosomia. The main primary outcome was the risk factors for complications. Logistic regression was used to estimate the odds ratio and the magnitude of association between the primary endpoint and the different covariates of the study. Results: Out of 362 couples included, we had 186 cases and 176 controls. The main perinatal complications were the delivery by caesarean section (26.5%) and lesions of the genital canal, 20.2%. There were no maternal deaths. Among newborns, metabolic complications (19.6%) were a leading cause of harmful outcomes before respiratory complications (12.4%), dystocic presentations (6.3%) or traumatic injuries (1.7%). The neonatal case fatality rate was 2.8%. Maternal age ≥30 years (p = 0.024);non-screening for gestational diabetes (p = 0.027);history of caesarean section (p = 0.041);weight gain ≥16 kg (p 0.001);maternal HIV (p = 0.047);birth weight ≥4500 g (p = 0.015) and birth height ≥52.7 ± 1.7 cm (p = 0.026) were risk factors for perinatal adverse outcomes. Conclusion: The delivery of a macrosomic baby remains problematic in this setting, and emphasizes the need to improve routine screening of gestational diabetes within a quality of prenatal follow-up through a multidisciplinary perinatal team involving obstetricians, endocrinologists and neonatal pediatricians.

Background:Maintenance of a pool of NK cells with mature function is essential for the host immune *** latest articles reported that some intracellular growth-promoting factors(GPFs) as a vital new molecular component for NK cell ***,it’s more interesting that our lab recent work show NK cells are potent secretors of an array of GPFs and promote fetal growth at maternal-fetal interface during early ***,GPFs are necessary for NK cells maturation and function of promoting fetal growth by NK cells,but transcriptional regulation mechanism of GPFs genes is ***:To better understand what’s the mechanism for regulating NK cells to express GPFs as a secreted or intracellular isoform,we investigated the potential role of TF1 within the NK cell compartment by using Tf1 gene-targeted ***:We found that TF1 was the key transcriptional factor in regulating NK cells expressing ***1 expressed in GPFs-rich NK cells from decidua was significantly higher than conventional ***1 was knocked down,NK cells were expected to show a serious decline in the ability of expressing ***1 regulated the transcription of GPFs genes via direct binding by using *** the NK cell homeostasis,Tf1-deficient mice harbor reduction of NK cells and defectiveness of mature NK cell in BM niche due to the impairment of intracellular ***,NK cell-conditional deficient Tf1 mice decreases the GPFs-secreting in NK cells impairs fetal development and leads to fetal growth restriction during early ***:Our study indicates that TF1 represent a novel regulatory element for supporting NK cells homeostasis and function of promoting fetal growth by NK cells via promoting GPFs expression in NK cells,and provides the basis for improved approaches to immunotherapy for virus clearance and tumor surveillance even alleviating spontaneous abortion based on TF1 activity.

Natural killer(NK) cells arepresent in large populations at the maternal-fetal interface during earlypregnancy. However, the role of NK cells in fetal growth is unclear. Here, wehave identified a CD49aEomes subset of NK cells that secretedgrowth-promoting factors(GPFs), including pleiotrophin and osteoglycin, inboth humans and mice. The crosstalk between HLA-G and ILT2 served as a stimulusfor GPF-secreting function of this NK cell subset. Decreases in thisGPF-secreting NK cell subset impaired fetal development, resulting in fetalgrowth restriction. Adoptive transfer of induced CD49aEomesNK cells reversed impaired fetal growth and rebuilt an appropriate localmicroenvironment. These findings reveal properties of NK cells in promotingfetal growth. In addition, we have further found that transcriptional factor PBX1 drives pleiotrophin and osteoglycin transcription in dNK cells andfurther promoting fetal development. Decreased PBX1 expression or the PBX1mutant was correlated with fetal growth restriction and pregnancy failure inpatients with unexplained recurrent spontaneous abortion(URSA). Inactivation of Pbx1 in mousedNK cells impaired fetal development through decreasing GPFs from CD49aPBX1dNK cells. Thus, these researches propose themechanism of decidual NK cells promoting fetal growth and suggest possible approachesfor therapeutic administration of NK cells in order to reverse restrictednourishments within the uterine microenvironment during early pregnancy.
