An enrichment model using regular health examination data for early detection of colorectal cancer

Objective: Challenges remain in current practices of colorectal cancer(CRC) screening, such as low compliance,low specificities and expensive cost. This study aimed to identify high-risk groups for CRC from the general population using regular health examination data.Methods: The study population consist of more than 7,000 CRC cases and more than 140,000 controls. Using regular health examination data, a model detecting CRC cases was derived by the classification and regression trees(CART) algorithm. Receiver operating characteristic(ROC) curve was applied to evaluate the performance of models. The robustness and generalization of the CART model were validated by independent datasets. In addition, the effectiveness of CART-based screening was compared with stool-based screening.Results: After data quality control, 4,647 CRC cases and 133,898 controls free of colorectal neoplasms were used for downstream analysis. The final CART model based on four biomarkers(age, albumin, hematocrit and percent lymphocytes) was constructed. In the test set, the area under ROC curve(AUC) of the CART model was 0.88 [95%confidence interval(95% CI), 0.87-0.90] for detecting CRC. At the cutoff yielding 99.0% specificity, this model’s sensitivity was 62.2%(95% CI, 58.1%-66.2%), thereby achieving a 63-fold enrichment of CRC cases. We validated the robustness of the method across subsets of test set with diverse CRC incidences, aging rates, genders ratio, distributions of tumor stages and locations, and data sources. Importantly, CART-based screening had the higher positive predictive value(1.6%) than fecal immunochemical test(0.3%).Conclusions: As an alternative approach for the early detection of CRC, this study provides a low-cost method using regular health examination data to identify high-risk individuals for CRC for further examinations. The approach can promote early detection of CRC especially in developing countries such as China, where annual health examination is popular but regular CRC-specific screening is rare.

Automatic Unusual Activities Recognition Using Deep Learning in Academia

In the current era,automatic surveillance has become an active research problem due to its vast real-world applications,particularly for maintaining law and order.A continuous manual monitoring of human activities is a tedious task.The use of cameras and automatic detection of unusual surveillance activity has been growing exponentially over the last few years.Various computer vision techniques have been applied for observation and surveillance of real-world activities.This research study focuses on detecting and recognizing unusual activities in an academic situation such as examinationhalls,which may help the invigilators observe and restrict the students from cheating or using unfairmeans.To the best of our knowledge,this is the first research work in this area that develops a dataset for unusual activities in the examination and proposes a deep learning model to detect those unusual activities.The proposed model has been named Automatic Unusual Activity Recognition(AUAR),which employsmotion-based frame extraction approaches to extract key-frames and then applies advanced deep learning Convolutional Neural Network algorithm with diverse configurations.The evaluation using standard performancemeasures confirmthat theAUARmodel outperforms the already proposed approaches for unusual activity recognition.Apart from evaluating the proposed model on the examination dataset,we also apply AUAR on Violent and Movies datasets,widely used in the relevant literature to detect suspicious activities.The results reveal that AUAR performs well on various data sets compared to existing state-of-the-art models.

After the establishment of the seismic observation network of southern Okinawa in August,1988 by the JMA(Japan Meteorological Agency),many hypocenters of earthquakes have been located.However,due to the small number of observation stations and narrow configuration of the array,the hypocenters lo-cated contain some systematic errors.Numerical experiments on the extent of the errors and the reliabilityof the JMA seismic observation network showed that the obtained hypocenters deeper than 100 km werereliable,but that for the obtained hypocenters shallower than 100 km,only the epicenters were reliable.

We are very glad to see that the Asian Journal of Andrology published a Special Issue on Semen Analysis in 21st Century Medicine, which well revealed some behind-the-scene controversies of the 5th edition of the WHO laboratory manual for the examination and processing of human semen [1]. Three articles from the special issue, two on the reference values of semen parameters [2, 3] and another presenting the investigation results of 118 laboratories performing semen analysis in China's Mainland [4], are very thoughtprovoking and we would like to share some of our views on these topics.

Rehabilitation of the imperial examination system

Since the abolishment of the imperial examination system(the IES)in 1905,the system has been the target of severe criticism by most Chinese people.This paper is intended to clear up the misunderstanding of the IES in seven respects based on some ideas of rehabilitation of the IES over the past 100 years.In the author's opinion,the IES is actually not so contemptible as most people think,at least it is not a bad system.The examination system was an attempt to recruit men on the basis of merit rather than because of social position or political connections,which is a great invention of China.Although it was abolished,the method to select talents by means of examination has not yet been and cannot be exterminated,and it is still proper to be used in the present society.The IES with a 1,300-year history has rich experiences and valuable lessons,which have implications to various examination reforms.

Highlights of the physical examination in heart failure

Because decompensated heart failure (HF) patients present primarily with symptoms of congestion, the assessment of volume status is of paramount importance. Despite the addition of new technologies that can predict intracardiac filling pressures, the physical exam (PE) remains the most accessible and cost-effective tool available to clinicians. An elevated jugular venous pressure (JVP)useful cutoff is that if the JVP is greater than 3 cm in vertical distance above the sternal angle, the central venous pressure is elevated. In addition to assessment of volume status, the PE in HF can reveal adverse prognostic signs, namely: elevated JVP, presence of third heart sound, elevated heart rate, low systolic BP, and low proportional pulse pressure (＜ 25%). This article will review the evidence for the diagnostic and prognostic utility of common PE findings in HF.

Precision surgery for primary liver cancer

Liver resection remains the best curative option for primary liver cancer, such as hepatocellular carcinoma(HCC) and intrahepatic cholangiocarcinoma.In particular, in liver resection for HCC, anatomical resection of the tumor-bearing segments is highly recommended to eradicate the intrahepatic metastases spreading through portal venous branches.Anatomical liver resection,including anatomical segmentectomy and subsegmentectomy using the dye-injection method, is technically demanding and requires experience for completion of a precise procedure.The recent development of imaging studies and new computer technologies has allowed for the preoperative design of the operative procedure, intraoperative navigation, and postoperative quality evaluation of the anatomical liver resection.Although these new technologies are related to the progress of artificial intelligence, the actual operative procedure is still performed as human-hand work.A precise anatomical liver resection still requires meticulous exposure of the boundary of hepatic venous tributaries with deep knowledge of liver anatomy and utilization of intraoperative ultrasonography.
