epidemiologic, Diagnostic, and Therapeutic Features of Urinary Stone Disease in Northern Benin from 2018 to 2023

Objective: To determine the epidemiologic, diagnostic, and therapeutic features of urinary in northern Benin from 2018 to 2023. Patients and Method: The study was an observational and descriptive one. We exhaustively and retrospectively collected data from the medical records of the patients that were managed from February 1, 2018 to February 1, 2023 at the Department of Surgery of “Centre Hospitalier Universitaire Départmental Borgou-Alibori or CHUD-BA”. Results: Among the 4522 patients managed, 75% or 1.7% had urinary stone disease. Their mean age was 41 years. The sex ratio was 2.75, the males being predominantly affected. Of the 75 cases of urinary stone disease, 30.7% (n = 23) were renal, 46.7% (n = 35) were ureteral, and 22.7% (n = 17) were vesical. The patients came from variable geographical areas, ethnic groups, and professional sectors. Their main complaints were pain (57.3%), hematuria (14.7%), dysuria (12%), and fever (5.3%). 15 (20%) patients, i.e., 7 renal stone and 8 ureteral stone patients, had an obstructive renal failure. The imaging tools available were ultrasonography, tomography and computed tomographic scan. 11 of the bladder stone cases (64.7%) were associated with a bladder outlet obstruction. Overall, 38.7% of the patients had either diabetes mellitus (6.7%), hypertension or high blood pressure (6.7%), benign prostate hyperplasia (8%), urinary schistosomiasis (6.7%), ureteral pelvic junction obstruction (2.7%), and prostate cancer (1.3%). Of the 19 patients whose dietary behavior was investigated 15 (79%), 11 (57.9%) et 11 (57.9%) demonstrated a diet which was respectively rich in milk, fromage and meat. 100% of bladder stones were removed through cystolithotomy. Renal stones were removed by pyelolithtomy (52.2%) and nephrolithotomy (21.7%). Ureteral stones were removed through ureterolithotomy (40%), expelled by means of alpha-blockers (40%) or managed with ureteral pigtail stent (20%). The objective of this study is to report clinical, electrocardiographic, and echocardiographic data on heart failure for the evaluation of its cardiovascular risk factors and causes at the Institute of Cardiology of Abidjan and the National Police Hospital. Patients and Methods: The 12-month prospective study included 989 patients who were diagnosed with heart failure at the Institute of Cardiology of Abidjan and the Heart Department of the National Police Hospital in Cote d’Ivoire. Results: The mean age of the patients was 55 ± 12 years. Patients ≤ 50 years, aged 50 and 70 years, and ≥ 70 years were 42%, 60%, and 18%, respectively. The major cardiovascular risk factors were high blood pression (60%), smoking (19%), type 2 diabetes (11%), and hypercholesterolemia (8%). The status of 11% of HIV positive patients were ignored upon admission to the Heart Hospitals. Severe kidney failure (25%) was found in all hypertensive patients. Atrial fibrillation patients and sinus rhythm subjects were 20% and 80%, respectively. The average duration of QRS was 102 ± 24 ms. 20% of patients had a complete left bundle branch block. The mean of the left ventricular ejection fraction (LVEF) was 35.8% ± 13%. The LVEF was? ≤ 45% in 57% of cases and ≤ 30% in 30% of cases. The identified causes of heart failure were ischemic heart disease (60%), hypertensive heart disease (20%), and rheumatic valvulopathy (12%). Among the patients with ischemic heart disease, 80% were infarction. The coronarography and the myocardial revascularization were performed in 50% of cases. The primitive cardiomyopathy was diagnosed in 15% of cases. The average follow-up was 12.6 ± 8 months. 18% of patients died, 14% of whom deceased within 1 year after the initial diagnosis and 5% of death occurred in 19 months following the first symptoms of heart failure. Background: Hepatocellular carcinoma (HCC) is one of the most common malignant tumors worldwide as well as in Egypt where it constitutes a significant public health problem. HCC is the third most common cause of death from cancer and chronic infection with HBV and HCV are the most important precursors for HCC development. Objective: To study the epidemiologic, clinical and pathological features of advanced HCC in clinical oncology and Nuclear medicine department, Suez Canal University hospital. Method: This is a retrospective study done to analyse the clinical, demographic and pathological data of advanced HCC patients that attended clinical oncology and Nuclear Medicine Department, Suez Canal University Hospital in the period between January 2013 and December 2016. Results: Results revealed that mean age of the patients was 60.2 ± 7.69, 71.74% male and 28.26% Female, 91.3% were HCV positive while 6.5% were HBV positive. Liver cirrhosis was in 89.13% of patients. Majority of the patients (78.2%) were child Pugh Score B and 21.7% were child Pugh Score C. 67.39% of patients had diabetes. Portal vein thrombosis was present in 50% of cases. Alpha fetoprotein was elevated >400 in 56.52% of cases. Thirty four patients received treatment in the form of chemotherapy while 12 patients did not receive treatment. There is a highly significant increase in survival time P 0.001 in treated group compared to untreated group. Median survival time (in months) was five months in the treated group versus one month in the untreated group. Conclusion: HCC is one of the increasing major health problems in both developing and developed countries. The most important risk factor is cirrhosis which is mainly due to HCV infection. Thus, much effort should be put into the field of prevention and treatment of viral hepatitis infections and chronic liver disease. Need for surveillance of concomitant peripheral artery disease in patients with coronary disease: results of the AGATHA survey in Malaysia

Background For patients with cardiovascular disease (CVD), co-existence of peripheral artery disease (PAD) predicts increased mortality, and such patients are also more likely to benefit from aggressive therapy. Surveillance of PAD is often neglected at health clinics. Our aim is to highlight the importance and ease of surveillance of PAD in patients with CVD. Objective To determine the prevalence of symptomatic and asymptomatic PAD in a Malaysian patient population with documented CVD. Methods and Results A total of 393 subjects with established CVD were recruited from three centres (85 women and 308 men), as part of a larger international (AGATHA) survey. PAD, determined by presence of claudicant symptoms on interview and/or abnormal ankle-brachial index (ABI) score of less than 0.9, was present in 21.4% of patients-of whom 64% were asymptomatic. Abnormal ABI is associated with higher systolic blood pressure and number of arterial beds affected. Conclusions Concomitant PAD is prevalent among CVD patients in Malaysia. ABI screening is simple and yields a high proportion of patients with extensive atherosclerosis who may require more aggressive atherosclerotic risk management.(J Geriatr Cardiol 2007;4:195-199.)

Functional Ankle Instability Prevalence and Associated Risk Factors in Male Football Players

Background: The injuries sustained in football most frequently occur to the lower extremity, with ankle and knee being the most affected joint segments. Many studies have defined ankle sprain as one of the most common sports injuries with an annual incidence rate of 15% to 45%, however, there’s a lack of studies in football regional leagues. Risk factors like anthropometric measures, field position, dominant limb, previous injury have been described as being associated with ankle injuries in the elite sports context. The primary aim of this study was to determine prevalence rates of self-reported ankle instability in regional leagues. A secondary aim was to explore the presence of extrinsic factors affecting ankle instability in these football players. Methods: The heads of all clubs (n = 66) from Coimbra and Aveiro (Portugal) regional male senior football divisions during season 2016/2017 were contacted by e-mail or telephone to enter the study, and 58 gave their permission to include their athletes. Then, all players from each of these 58 teams were invited to enter the study (n = 1044) and a total of 589 athletes participated. Demographic and anthropometric data were recorded concerning age, height, weight, years of training, mean number of training hours, position in camp along with specific questions related to an ankle injury. All participants were asked about their previous history of injury, reinjury, feeling of giving-away and where also asked to fill the Cumberland Ankle Instability Tool. Results: A total of 290 (49.2%) athletes reported a history of at least one previous sprain and, from these, 170 (58.6%) repeated the injury and 112 (19%) reported sensation of “giving-away”. A significant association was found between injury, recurrence and subjective feeling of giving away (p < 0.05). Introduction: Experimental Clinical studies have shown that loss of up to 77.0% of red cell mass can be tolerated as long as it kept the blood volume. However, the blood volume loss of about 34.0% is fatal. The initial attention in patients with bleeding should be given to maintaining blood volume and oxygen transport. Thus, transfusions of allogeneic blood products are associated with an increased risk of morbidity and mortality in cardiac surgery. Some transfusion risk scores have been proposed for cardiac surgery patients. Objective: To review and discuss literature blood transfusion (blood components gradient) in cardiovascular surgery versus polytrauma, gastrointestinal bleeding, and neoplastic diseases. Methods: The model followed for the review was PRISMA. We used the databases as Scielo, Lilacs, Google Scholar, PubMed articles and works of scientific and doctoral theses master. Results: According to the discussion in the literature, the acute bleeding of large volumes is challenging to emergency services. Accordingly, the need to support hemotherapeutics led to the development of protocols for more rational utilization of the monitored blood components. Conclusion: Hemotherapy services have adopted a very dynamic approach to the clinical condition presented by patients, the need for four or more red blood cell units.
