Clinical profile and outcomes of pediatric endogenous endophthalmitis: A report of 11 cases from South India

AIM To study the clinical profile and outcomes of pediatric endogenous endophthalmitis from a tertiary eye hospital in South India.METHODS A total of 13 eyes of 11 children presented to us with varied symptoms and presentations of endogenous endophthalmitis, over a five-year period from January 2010 to December 2015 were studied. Except for two eyes of a patient, vitreous aspirates were cultured from all 11 eyes to isolate the causative organism. These eleven eyes also received intravitreal injections. All patients were treated with systemic antibiotics. RESULTS Two cases had bilateral endophthalmitis. Ages ranged from 4 d to 11 years. Five cases were undiagnosed and treated, before being referred to our center. Ten of the 13 eyes underwent a core vitrectomy. The vitrectomy was done at an average on the second day after presenting(range 0-20 d). Five of the 11 vitreous aspirates showed isolates. The incriminating organisms were bacteria in three and fungus in two. An underlying predisposing factor was found in seven patients. At a mean follow-up 21.5 mo, outcome was good in 7 eyes of 6 cases(54%), five eyes of four cases(38%) ended up with phthisis bulbi while one child died of systemic complications. CONCLUSION endogenous endophthalmitis is a challenge for ophtha-lmologists. Early diagnosis and intervention is the key for a better outcome.

CRISPR/Cas9-mediated multiple gene editing in Brassica oleracea var.capitata using the endogenous tRNA-processing system

Cabbage(Brassica oleracea var.capitata)is a biennial plant with strong self-incompatibility and an obligate requirement for prolonged vernalization by exposure to low temperatures to induce flowering.These characteristics significantly increase the difficulty of exploiting novel germplasm induced by physical or chemical mutagens.In this study,we report a CRISPR/Cas9 gene-editing system based on endogenous tRNA processing to induce high efficiency and inheritable mutagenesis in cabbage.Using the phytoene desaturase gene BoPDS,the S-receptor kinase gene BoSRK,and the male-sterility-associated gene BoMS1 as the target genes,multisite and multiple gene mutations were achieved using a construct with tandemly arrayed tRNA-sgRNA architecture to express multiple sgRNAs.The BoSRK3 gene mutation suppressed self-incompatibility completely,converting the self-incompatible line into a self-compatible line.In addition,the BoMS1 gene mutation produced a completely male-sterile mutant,which was highly cross compatible with its nonmutant isoline at the flowering stage as a result of a simultaneous BoSRK3 gene mutation,enabling the economic propagation of the male-sterile line through bee-mediated cross-pollination.Interestingly,higher site mutation efficiency was detected when a guide sequence was inserted into a location in the tandemly arrayed tRNA-sgRNA architecture that was distal from the upstream Pol III promoter.In addition,mutation sites were also detected in the paralogous genes of the BoPDS and BoSRK genes that had fully consistent sequences or base mismatches but beyond the“seed”region in the spacer sequence compared with the target sgRNAs.Collectively,our results demonstrate that the CRISPR/Cas9 system,coupled with an endogenous tRNA-processing system,is an efficient tool to improve cabbage traits.

Our previous studies indicated that tomato miR482b could negatively regulate the resistance of tomato to Phytophthora infestans and the expression of miR482b was decreased after inoculation with P.infestans.However,the mechanism by which the accumulation of miR482b is suppressed remains unclear.In this study,we wrote a program to identify 89 long noncoding RNA(lncRNA)-originated endogenous target mimics(eTMs)for 46 miRNAs from our RNA-Seq data.Three tomato lncRNAs,lncRNA23468,lncRNA01308 and lncRNA13262,contained conserved eTM sites for miR482b.When lncRNA23468 was overexpressed in tomato,miR482b expression was significantly decreased,and the expression of the target genes,NBS-LRRs,was significantly increased,resulting in enhanced resistance to P.infestans.Silencing lncRNA23468 in tomato led to the increased accumulation of miR482b and decreased accumulation of NBS-LRRs,as well as reduced resistance to P.infestans.In addition,the accumulation of both miR482b and NBS-LRRs was not significantly changed in tomato plants that overexpressed lncRNA23468 with a mutated eTM site.Based on the VIGS system,a target gene of miR482b,Solyc02g036270.2,was silenced.The disease symptoms of the VIGS-Solyc02g036270.2 tomato plants were in accordance with those of tomato plants in which lncRNA23468 was silenced after inoculation with P.infestans.More severe disease symptoms were found in the modified plants than in the control plants.Our results demonstrate that lncRNAs functioning as eTMs may modulate the effects of miRNAs in tomato and provide insight into how the lncRNA23468-miR482b-NBS-LRR module regulates tomato resistance to P.infestans.

Recent studies have showed that RNAs regulate each other with micro RNA(mi RNA) response elements(MREs) and this mechanism is known as "competing endogenous RNA(ce RNA)" hypothesis. Long noncoding RNAs(lnc RNAs) are supposed to play important roles in cancer. Compelling evidence suggests that lnc RNAs can interact with mi RNAs and regulate the expression of mi RNAs as ce RNAs. Several lnc RNAs such as H19, HOTAIR and MEG3 have been found to be associated with mi RNAs in gastric cancer(GC), generating regulatory crosstalk across the transcriptome. These MRE sharing elements implicated in the ce RNA networks(ce RNETs) are able to regulate m RNA expression. The ce RNA regulatory networks including m RNAs, mi RNAs, lnc RNAs and circular RNAs may play critical roles in tumorigenesis, and the perturbations of ce RNETs may contribute to the pathogenesis of GC.

endogenous endophthalmitis is an intraocular infection that results from hematogenous spread of organisms from a distant source of infection. Facial cellulitis is rarely reported as a focus of infection. We report a case of 51-year-old unconscious woman presenting with fever, facial swelling, and decreased visual acuity secondary to facial cellulitis, endogenous endophthalmitis and end-stage renal disease(ESRD). Generally systemic antibiotics in endophthalmitis have low efficacy because of the blood-ocular barrier. The management of endophthalmitis begins with intravitreal antibiotic injections and if the response is not favorable to do a vitrectomy. Twelve hours after the intravitreal antibiotic injection, vitrectomy was considered because of worsening of the vitreal cloudiness. However, the patient's general medical condition precluded vitrectomy. We experienced successful treatment with intravitreal antibiotic injection and continuous intravenous antibiotic administration because of the breakdown of the blood-ocular barrier due to ocular inflammation, especially in the setting of systemic vascular and fluid homeostatic changes.

Introduction Nitrous oxide(N2O/laughing gas)has been used in medical practice as an inhalational anaesthetic and analgesic for more than 150 years.1 In the past decades,N2O exhibited increasing popularity among recreational drug users for its euphoric effects,potentially through its interaction with the endogenous opioid system.2 Recreational use of N2O emerged as the seventh most used drug globally in the past decade.3 The use of N2O can cause myelopathy,myocardial injury,anaemia,severe mood disorders,sensory and motor neuropathy,and psychotic symptoms.45 Prolonged N2O intake results in vitamin B12 deficiency and inhibits methionine synthetase,folate and DNA production,leading to plasma homocysteine(HCY)level elevation and bone marrow haematopoietic dysfunction.6 This study aimed to investigate the neurological symptoms of nitrous oxide use and then explore the relapse trajectory and risk factors for relapse.

CONSPECTUS:Insulin replacement therapy is an essential and effective treatment strategy for controlling the blood glucose level(BGL)of people without sufficient endogenous insulin production.However,insulin therapy faces challenges such as daily multiple injections and hypoglycemia risk arising from its narrow therapeutic index,which together leads to poor BGL control.Thus,glucose-responsive insulin delivery systems that can mimicβ-cell function are developed to improve BGL control and reduce injection frequency.Glucose-responsive materials-based insulin delivery systems are competent in tightly controlling the BGL with an additional benefit of reducing the hypoglycemia risk.

Background: Era of contraception, abortions, [20th, 21st centuries] implemented as family welfare schemes, witnessed increased global incidence of Type 2 diabetes mellitus, systemic hypertension. Objectives: Altruistic association of contraception [if any], with increasing diabetes mellitus, systemic hypertension, was sought after. Methods: In 2012, retrospective analysis of, prevalence of diabetes mellitus, systemic hypertension, in 350 patients of 20 - 35 years, 35 - 50 years, >50 years, age groups, data collected by convenient, stratified random sampling, from different geographical locations, between 2003-2012 and its association with presence, absence of contraception, abortion was undertaken;simultaneously, serum estrogen levels obtained from 105 patients, were also analysed. Results: 10 - 45 fold increase in Type 2 diabetes mellitus was seen in contraceptive users of >20 years to >50 years;15 - 50 fold increase in incidence of systemic hypertension was seen in contraceptive users of >20 years to >50 years. endogenous estrogen was reduced below normal in 75% of contraceptive users, and 25% of contraceptive users had low normal serum estrogen. Cholesterol deprived diet, due to decreased synthesis of endogenous estrogen, androgen also was associated with 50% increase in diabetes mellitus, systemic hypertension. Conclusion: Concept is acquired contraception, abortion status, with smashed fragmentation of germ cells, reduced endogenous estrogen, androgen, results in defaulted genomic repertoire, deranged cell metabolism, increased incidence of diabetes mellitus, systemic hypertension as part of metabolic syndrome. Contraception reversal declined the diseases as a cause and effective phenomenon.

Study on the endogenous phosphorus load of sediments in Miyun reservoir

Based on the research into the physical-chemical properties and distribution of sediments and the characteristics of pore water in sediments of Miyun reservoir, the release flux of the total phosphorus (TP) from sediments is estimated by simulating deposit environment of reservoir bottom in laboratory. They are 0.018 mg·cm-2·d-1 and 0.821 mg·cm-2·d-1 at 2℃ and 8℃ respectively. The gross TP released in a year is 11.34t. As a contrast, the pore water diffusion simulation method is used to measure gross TP released and gains 11.56t. The two results are relatively close and prove that experiment simulation has some reliability. On the basis of experiments, some conclusions can be drawn: (1) endogenous phosphorus from sediments accounts for 27.9 percent of the TP entering the reservoir, and it cannot be ignored; and (2) increasing temperature is helpful to TP releasing from sediments.
