Usefulness of the Guglielmi detachable coil for embolization of a systemic venous collateral after Fontan operation:A case report

embolization of collateral veins is often treated with rigid coils(Gianturco and interlocking detachable coils type).However,when dealing with tortuous and dilated collateral veins,there is a high risk for technical failure and coil migration due to inflexibility of the coils.To safely and successfully solve this problem,Guglielmi detachable coils(GDC) can be used for embolization.Their flexibility allows for easy navigation in tortuous veins,low risk of unintended coil release or coil migration,and safe deployment.A 12-year-old girl with a single ventricle had severe cyanosis and a low exercise tolerance 5 years after Fontan procedure.The symp-toms were caused by a tortuous and dilated collateral from the left phrenic vein into the left pulmonary vein,forming a right-to-left shunt.The collateral,which had a large diameter and high flow,and therefore a high risk of coil migration,was successfully embolized with 8 GDC.There were no complications such as coil migration or cerebral infarction.Transcatheter embolization increased her systemic oxygen saturation from 81%-84% to 94%-95%,and increased her ability to exercise.The embolization procedure using flexible GDC was low risk compared with other rigid coil embolization techniques when performing embolization of tortuous and dilated collateral veins.

Transcatheter embolization as the new reference standard for endoscopically unmanageable upper gastrointestinal bleeding

Acute nonvariceal upper gastrointestinal bleeding(UGIB) is a major medical emergency problem associated with significant morbidity and mortality.Endoscopy is considered the first method of choice to detect and treat UGIB.Endoscopic therapy usually achieves primary hemostasis,but 10%-30% of these patients have repeat bleeding.In patients in whom hemostasis is not achieved with endoscopic techniques,treatment with transcatheter angiographic embolization(TAE) or surgery is needed.Surgical intervention is usually an expeditious and gratifying endeavor,but it can be associated with high operative mortality rates.A large number of studies support the use of TAE as salvage therapy as an alternative to surgery.However,few studies have compared the results of TAE with that of emergency surgery in terms of efficiency,the frequency of repeat bleeding,and complications.Recently,Ang et al retrospectively compared the outcome of TAE and surgery as salvage therapy of UGIB after failed endoscopic treatment.There were no significant differences in 30 d mortality,complication rates and length of stay although higher rebleeding rates were observed after TAE compared with surgery.In this commentary,we discuss the advantages and drawbacks of these two therapeutic strategies for UGIB.We also attempt to define the exact role of TAE for acute nonvariceal UGIB.

A 59-year-old man underwent liver radiofrequency ablation under laparotomy for recurrent hepatic carcinoma located in the caudate lobe,however,near-fatal bleeding occurred 1 wk after the operation.The intraoperative ultrasound study during laparotomy revealed left hepatic artery pseudoaneurysm.Suture and packing with ribbon gauze was used to obtain hemostasis.A secondary hemorrhage followed 11 h later and hepatic angiography was performed immediately.Bleeding from the pseudoaneurysm in a branch of the left hepatic artery was found and the artery branch was embolized with coils.Other than slight bile leakage,post-embolization continued satisfactorily.Bleeding did not reoccur.The follow up visit 1 mo later found the pseudoaneurysm disappearing and no tumor recurrence.

Safety of bronchial arterial embolization with n-butyl cyanoacrylate in a swine model

AIM: To compare the efficacy and safety of bronchial artery embolization (BAE) with n-butyl cyanoacrylate (NBCA) and gelatin sponge particles (GSPs). METHODS: Six healthy female swine were divided into two groups to be treated with BAE using NBCA-lipiodol (NBCA-Lp) and using GSPs. The occlusive durability, the presence of embolic materials, the response of the vessel wall, and damage to the bronchial wall and pulmonary parenchyma were compared. RESULTS: No animals experienced any major complication. Two days later, no recanalization of the bronchial artery was observed in the NBCA-Lp group, while partial recanalization was seen in the GSP group. Embolic materials were not found in the pulmonary artery or pulmonary vein. NBCA-Lp was present as a bubble-like space in bronchial branch arteries of 127-1240 μm, and GSPs as reticular amorphous substance of 107-853 μm. These arteries were in the adventitia outside the bronchial cartilage but not in the fine vessels inside the bronchial cartilage. No damage to the bronchial wall and pulmonary parenchyma was found in either group. Red cell thrombus, stripping of endothelial cells, and infiltration of inflammatory cells was observed in vessels embolized with NBCA-Lp or GSP. CONCLUSION: NBCA embolization is more potent than GSP with regard to bronchial artery occlusion, and both materials were present in bronchial branch arteries≥100 μm diameter.

Pipeline for previously stented basilar trunk aneurysm:a case focusing on how the pipeline should be deployed

Background:Recurrent,previously stented basilar trunk aneurysms pose significant challenges to either microsurgical clipping or traditional endovascular treatment.We here presented an intriguing case that was successfully treated by the pipeline embolization device(PED;ev3/Covidien Neurovascular,Irvine,California,USA).Case presentation:A 62-year-old male found a recurrent aneurysm,which had been treated with coiling and stentassisted coiling before.The patient came to our center seeking for a third treatment.In our center,a single PED and additional coils were used to treat this refractory aneurysm.The whole length of the PED was delicately implanted in the previous Enterprise stent(Cordis Corporation,Miami FL,USA),which resembled the double-layer flow diverter FRED(Microvention,Tustin,California,USA).The inflow zone of the aneurysmal sac was further coiled considering that this aneurysm had ruptured.No procedure-related complications occurred.Follow-up angiogram at 6 months indicated that the intractable aneurysm was completely occluded and the patient was free from any neurologic deficit.Conclusions:This is a case that adds knowledge to improve the poor performance of flow diverters in previously stented aneurysms.However,future studies with larger group of patients are needed to further test the safety and efficacy of this technique.

Hepatectomy remains the only curative treatment for many primary and secondary liver cancers.Portal vein embolization(PVE) has been used to increase the volume of the future liver remnant and thus lower the risk of small-for-size syndrome and postoperative liver failure.This technique has proven its safety,with a low post-procedure morbidity rate.Here,we describe a very rare case in which a young patient suffered a glue embolism to the right atrial cavity following PVE in preparation for a major hepatectomy for colorectal metastasis.The foreign body was withdrawn from the heart with a femoral,percutaneous device and trapped against the wall of the femoral vein with a self-expanding metal stent.Our report shows that this previously unknown complication of PVE can be resolved without recourse to sternotomy and open heart surgery.

A Rare Case of Carotid Body Tumor Presenting with Internal Carotid Artery Blood Supply and Carotid Sinus Syndrome

To the Editor： Carotid body tumor （CBT） is a highly vascular neoplasm of neural crest origin arising in paraganglial cells of the carotid bifurcation. Branches from external carotid artery or glomic artery arising at the bifurcation of common carotid artery dominate in the blood supply mode. Only very few cases with supplementary blood supply from an internal carotid artery （ICA） have been reported in literature, but were still short of imaging proof Here, we report an extremely rare case of CBT with blood supply from 1CA, which had been performed 4 times of unaccomplished surgical explorations before the tmnor was excised during the 5th time of surgery. She also presented carotid sinus syndrome that was also extremely rare among CBT patients.

Management of Cesarean Scar Pregnancy: A Case Series

Objective To survey effective treatment strategies for cesarean scar pregnancy(CSP). Methods The clinical data of 78 patients diagnosed with CSP from January 2010 to December 2013 were reviewed. Results Among these patients, 17 patients were first treated at our hospital; of them, 2 were misdiagnosed. The other 61 patients were referred from other hospitals; of them, 21 were initially misdiagnosed. There were 9 patients who were treated with laparotomy, 50 patients with curettage after uterine artery embolization(UAE) with or without local methotrexate(MTX) infusion, 10 patients with dilatation and curettage, 6 patients with transvaginal sonographic guided local intragestational MTX injection, and 3 patients with systemic MTX injection. All patients finally recovered. Patients with excessive vaginal hemorrhage underwent either emergency UAE treatment or laparotomy. These two treatments had similar success rates(81.82% vs. 100%, χ2 =0.289, P>0.05). Conclusions The accurate diagnosis of CSP is important. Curettage after UAE with or without local MTX infusion is a safe and effective method.

Deep lingual arterial chemoembolization of tongue carcinoma with microcapsuled anticancer drug

Objective: Microcapsule chemoembolism is a promising treatment of tumors. We describe a deep lingual arterial embolization of tongue carcinoma with microcapsuled carboplatinum. Methods: Lingual artery cast specimens from cadavers were microscopically examined, and 78 patients with tongue cancer were recruited and treated with the deep lingual arterial embolization therapy. Results: Microcapsule embolism occurred approximately at the fifth or sixth level of the deep lingual artery branches. The five-year survival rate was 88.5% (69 out of 78), and the ten-year survival rate 52.6% (41 out of 78). Conclusion: The deep lingual arterial embolization of tongue carcinoma with microcapsuled carboplatinum is an effective therapy to treat carcinoma in mid-margin or mid-body of the tongue.
