The Relevance between Symptoms and Magnetic Resonance Imaging Analysis of the Hippocampus of Depressed Patients Given electro-acupuncture Combined with Fluoxetine Intervention - A Randomized,Controlled Trial

Objective： To probe the relevance between depressive symptoms and hippocampal volume and its metabolites detected by magnetic resonance imaging （MRI） in depressed patients who were given electro- acupuncture （EA） combined with Fluoxetine before and after treatment. Methods： A randomized, controlled trial was conducted. A total of 75 cases of mild or moderate depression were randomly assigned to two groups： the EA group which received EA combined with Fluoxetine; the Fluoxetine group which received Fluoxetine only as the control. The 17-item Hamilton Scale for Depression （HAMD） was used to assess the depression level. The relevance between the changes of the hippocampal volume and its metabolites, including N-acetyl aspartate （NAA）/creatine （Cr） and choline containing compounds （Cho）/Cr, and the reduction rate of the HAMD score before and after treatment of the two groups were analyzed. Results： At the end of the treatment, the therapeutic response rates were not statistically different between the two groups （73.53% for the Fluoxetine group and 83.33% for the EA group, respectively）. Compared to that of the Fluoxetine group, a significant difference was shown in the EA group in the reduction rate of the HAMD scores （P〈0.05）. There was a negative correlation between the therapeutic effect and the HAMD scores before treatment in both groups of patients. There was no significant difference in the hippocampal volume before and after treatment. The NAA/Cr ratio of both groups increased after treatment, with the EA group increasing more. There was a negative correlation between the rate of change of the NAA/Cr after treatment and the HAMD scores before treatment in the two groups. In the Fluoxetine group, the Cho/Cr ratio showed no significant difference before and after treatment, which had no relevance with the HAMD scores before treatment either. Pre-ischemia electro-acupuncture potentiates the expression of Bcl-2 and transforming growth factor-beta 1 in rat brains

The expression of the anti-apoptotic molecules Bcl-2 and transforming growth factor-beta 1 is known to confer protective effects on the cerebral ischemia-reperfusion *** current study investigated the expression levels of Bcl-2 and transforming growth factor-beta 1 in response to multiple pre-ischemia electro-acupuncture at acupoints Zusanli（ST36）and Fengchi（GB20） *** were divided into five groups：uninjured,control,non-acupoint,GB20 and ST36. Rats in the non-acupoint,GB20 and ST36 groups received 30 minutes（3 times or 18 times）of electro-acupuncture stimulation before experimental cerebral ischemia was ***-2 and transforming growth factor-beta 1 were found to be significantly increased in the ST36 groups with either 3 or 18 electro-acupuncture treatments（P〈0.05）.The production was higher with 18 electro-acupuncture treatments in the ST36 groups（P〈0.05）.In the GB20 groups,significant increase was only observed in transforming growth factor-beta 1 with 18 electro-acupuncture treatments（P〈0.05）.No significant elevation of the level of transforming growth factor-beta 1 was observed in the non-acupoint ***,the production of Bcl-2 increased with 18 treatments in the non-acupoint groups（P〈0.05）.The data suggest that multiple pre-ischemia electro-acupuncture at ST36 was effective in conferring neuroprotective effect on the brain by means of upregulation of Bcl-2 and transforming growth factor-beta 1 and the effect was increase with the number of treatment.

The selection of electro-acupuncture parameters remains poorly unified between clinical studies. The present study observed the effects of electro-acupuncturing Renzhong （DU 26） with different stimulation parameters on motor function recovery following middle artery occlusion injury in rats. Results showed an optimal stimulation parameter for Renzhong electro-acupuncture that was low frequency and mild current （2 Hz, 1 mA） significantly improved cortical excitability and conductive function, and promoted recovery in a rat model of motor function in middle artery occlusion. Frequency had a greater impact than current or interaction, and played a critical role in electro-acupuncture therapy.

Effect of electro-acupuncture on Tumor Necrosis Factor-α and Vascular Endothelial Growth Factor in Peripheral Blood and Joint Synovia of Patients with Rheumatoid Arthritis

Objective：To observe the effect of electro-acupuncture（EA）on tumor necrosis factor-α（TNF-α） and vascular endothelial growth factor（VEGF）in peripheral blood and joint synovia in patients with rheumatoid arthritis（RA）to verify the clinical efficacy of ***：Adopting randomized controlled principle,the 63 RA patients enrolled were assigned to two groups,the 32 patients in the EA group were treated with EA,and the 31 patients in the simple needling（SN）group treated with simple *** to the integral-local combining method,the acupoints were selected mainly from yang-meridian and local Ashi points（pain-point）.EA or SN was applied every other day,10 times as a course,and each patient received a total of 3 courses of treatment. Results：Blood and synovial levels of TNF-αand VEGF were reduced significantly after treatment in both *** lowering（absolute value and difference value）of TNF-αas well as the absolute value of VEGF, either in blood or in synovia,were similar in the two groups（P0.05）;but the lowering of VEGF after treatment was more significant in the EA group than that in the SN group（P0.05）.Conclusion：EA could effectively lower the contents of TNF-αand VEGF in peripheral blood and joint synovia to improve the internal environment for genesis and development of RA,so as to enhance the clinical therapeutic effectiveness.

Effect of electro-acupuncture at Different Acupoints on Neuropeptide and Somatostatin in Rat Brain with Irritable Bowel Syndrome

Objective： To compare the regulatory effects of electro-acupuncture （EA） at acupoints Zusanli （ST36） and Hegu （LI4） on the visceral hyper-sensitivity in the rat model of irritable bowel syndrome （IBS）, and to explore the acting targets and specialty of acupoints. Methods： Except 8 rats of the normal control group, the rest 32 rats were prepared to set up the IBS models. IBS animal model was prepared by enema with acetic acid. Model rats were divided into three groups. Except for rats in the model group for control, those in the other two groups were treated 20 min by EA on ST36 （EA-ST36） and LI4 （EA-LI4） respectively for 2 weeks to observe the effect on behavior response of viscera sensitivity. The changes of neuropepUde （NPY）, the somatostatin （SS） levels in blood and tissues of brain and intestine were monitored as well. Results： The volume thresholds for abdomen uplifting and beck hunching were obviously increased after EA-ST36 （P〈0.05）, but showed insignificant change after EA-LI4. NPY contents lowered and SS contents increased in model rats; both EA-ST36 and EA-LI4 could raise the level of thalamic NPY （P〈0.01 and P〈0.05, respectively）, but showed insignificant effects on NPY in colonic tissue. As for SS content, its colonic level could be reduced by EA-S36 and EA-LI4 （P〈0.01 and P〈0.05, respectively）, however, its blood level was affected only by EA-ST36 （P〈0.05）. Conclusions： EA-ST36 or EA-LI4 could regulate the NPY in thalamus and SS in colonic tissue, the former could affect blood level of SS as well. It is deemed that NPY and SS may be the key substances for regulating the action of acupuncture in the braln-intastinal axis; their different levels could be regarded as an indicator for the functional differenca between the acupoints.

Clinical Study of electro-acupuncture Treatment with Different Intensities for Functional Constipation Patients

Functional constipation （FC） is a common functional bowel disorder disease that affects life quality of a large number of people. This study aimed to explore the impact of different intensities of electro-acupuncture （EA） treatment for FC patients. Totally, 111 patients with FC meeting the Rome III criteria were randomly assigned to different intensities of EA groups （low and high intensity of EA groups） and medicine-controlled （MC） group. In EA groups, patients were treated with EA at quchi （Llll） and shangjuxu （ST37） bilaterally for 4 weeks, 5 times/week in the first 2 weeks, and 3 times/week in the last 2 weeks. In MC group, 5 mg mosapride citrate was administered orally 3 times/day for 4 weeks. Spontaneous bowel movement frequency each day was recorded using a consti- pation diary. Self-rating anxiety scale （SAS） and self-rating depression scale （SDS） were used to assess the patients＇ psychological state. Cortisol （CORT）, substance P （SP）, and vasoactive intestinal polypep- tide （VIP） were evaluated at baseline and at the end of 4 weeks after treatment. As compared with the baseline, there was statistically significant increase in stool frequency every week （P〈0.01）, but there was no statistically significant difference among the three groups. As compared with the baseline, after 4 weeks of EA therapy, the scores of SDS and serum levels of CORT were decreased significantly in low intensity of EA group （P〈0.01）, and the serum levels of SP and VIP were increased significantly （P〈0.05）; the scores of SAS and SDS and serum levels of CORT were decreased significantly in high intensity of EA group （P〈0.05）, and the serum levels of SP and VIP were increased significantly （P〈0.05）; the serum levels of CORT and VIP were increased significantly in MC group （P〈0.05）. Effects of electro-acupuncture on Personality Traits in Depression:A Randomized Controlled Study

Objective： To explore the personality-adjusting effect of electro-acupuncture treatment for depression and compared this treatment with paroxetine treatment. Methods： A non-blinded, randomized controlled trial was adopted. Sixty depressed patients, who met trial criteria, were randomly assigned to the treatment and the control groups. In the treatment group, electro-acupuncture treatment was used, and paroxetine treatment was used in the control group. During the 24-week study period, 12 patients dropped out and 48 patients completed the study. The Minnesota Multiple Personality Inventory （MMPI） was adopted as the evaluation tool. At the same time, the Self-rating Depression Scale （SDS）, Self-rating Anxiety Scale （SAS） and Montgomery-Asberg Depression Rating Scale （MADRS） were used to evaluate the psychological state. Evaluations were done before and after treatment. Results： After treatment, patients＇ psychological state improved significantly in both groups （P〈0.01）. For the treatment group, within-group comparison between baseline and after 24 weeks of treatment showed that severity of depression had significantly decreased （P〈0.01）. MADRS and SDS scores decreased significantly （P〈0.05） and MMPI subscale scores for hypochondriasis, depression, psychopathic deviate, psychasthenia, social introversion and fake decreased significantly （P〈0.05）. For the control group, severity of depression also decreased significantly. MADRS and SDS scores decreased significantly （P〈0.05）; and MMPI subscale scores for hypochondriasis, depression, hysteria, paranoia, and psychasthenia decreased significantly （P〈0.05）. Between-group comparison demonstrated that for the MMPI subscales paranoia and social introversion, the decrease of score was greater in the treatment group than in the control group （P〈0.05）. Effect of electro-acupuncture Stimulation of Ximen(PC4) and Neiguan(PC6) on Remifentanil-induced Breakthrough Pain Following Thoracal Esophagectomy

The clinical analgesic effect of electro-acupuncture（EA） stimulation（EAS） on breakthrough pain induced by remifentanil in patients undergoing radical thoracic esophagectomy, and the mechanisms were assessed. Sixty patients（ASAⅢ） scheduled for elective radical esophagectomy were randomized into three groups： group A（control） receiving a general anesthesia only; group B（sham） given EA needles at PC4（Ximen） and PC6（Neiguan） but no stimulation; and group C（EAS） electrically given EAS of the ipsilateral PC4 and PC6 throughout the surgery. The EAS consisting of a disperse-dense wave with a low frequency of 2 Hz and a high frequency of 20 Hz, was performed 30 min prior to induction of general anesthesia and continued through the surgery. At the emergence, sufentanil infusion was given for postoperative analgesia with loading dose of 7.5 μg, followed by a continuous infusion of 2.25 μg/h. The patient self-administration of sufentanil was 0.75 μg with a lockout of 15 min as needed. Additional breakthrough pain was treated with dezocine（5 mg） intravenously at the patient＇s request. Blood samples were collected before（T1）, 2 h（T2）, 24 h（T3）, and 48 h（T4） after operation to measure the plasma β-EP, PGE2, and 5-HT. The operative time, the total dose of sufentanil and the dose of self-administration, and the rescue doses of dezocine were recorded. Visual Analogue Scale（VAS） scores at 2, 12, 24 and 48 h postoperatively and the incidence of apnea and severe hypotension were recorded. The results showed that the gender, age, weight, operative time and remifentanil consumption were comparable among 3 groups. Patients in EAS group had the lowest VAS scores postoperatively among the three groups（P〈0.05）. The total dose of sufentanil was 115±6.0 μg in EAS group, significantly lower than that in control（134.3±5.9 μg） and sham（133.5±7.0 μg） groups. Effect of electro-acupuncture Intervention on Cognition Attention Bias in Heroin Addiction Abstinence—A Dot-probe-based Event-related Potential Study

Objective：To study the changes of cognitive attention-related brain function in the heroin addicts before and after electro-acupuncture（EA） intervention for exploring the concerned neuro-mechanism of addictive relapse and the central action role of EA ***：Adopting event-related potential（ERP） technique,the ERP at 64 electrode spots in 10 heroin addicts（test group） were recorded before and after EA intervention with dot-probe experimental form during implementing cognitive task on positive emotional clue （PEC）,negative emotional clues（NEC）,and heroin-related clue（HRC）.The P200 amplitude components on the selected observation points（Fz,Cz,and Pz） were analyzed and compared with those obtained from 10 healthy subjects as the ***：Before EA,the ERP of attention on HRC in the test group was higher than that on PEC and NEC（P0.05） and significantly higher than that in the control group（P0.05）;after EA,the P200 amplitude of attention on HRC at Cz and Pz was significantly lowered（P0.05） and that on PEC at Fz was significantly elevated（P0.05）.After EA,the P200 amplitude at Pz was ranked as NECPECHRC,but in the control group,it showed PECHRC at all three observation points and PECNEC at ***：Heroin addicts show attention bias to HRC,which could be significantly reduced by EA intervention,illustrating that EA could effectively inhibit the attention bias to heroin and so might have potential for lowering the relapse rate.

Clinical Observation on Treatment of Depression by electro-acupuncture Combined with Paroxetine

Objective： To observe the clinical efficacy and adverse reactions of Paroxetine combined with electro-acupuncture （EA） in treating depression. Methods： Forty-two patients with depression were randomly assigned to the observation group （22 patients） treated with EA combined with Paroxetine, and the control group （20 patients） treated with Paroxetine alone, and the therapeutic course for both groups was 6 weeks. The therapeutic efficacy and adverse reactions were evaluated with scores by Hamilton depression scale （HAMD） and treatment emergent symptoms scale （TESS）, respectively. Results： HAMD scores determined at the end of the 1st, 2nd, 4th, and 6th week of the treatment course were significantly lower in the observation group than those in the control group （P〈0.05）. The significant improvement rate evaluated at the end of the 6-week treatment was remarkably higher in the observation group than that in the control group （72.7% vs 40.0%）. No significant difference of TESS scores was found between the two groups. Conclusion： EA combined with Paroxetine has better clinical efficacy than that of Paroxetine alone, with milder adverse reaction and quicker initiation of effect.
