Characterization of the ear Canal Bacterial Flora Present in Hearing Aids (HA) Wearing Subjects

The use of hearing aids (HA) is considered a predisposing factor for ear microbial infections. We undertook this study to compare the presence and nature of the microbial flora inhabiting of ears of HA and non-HA (nHA) users. Swab samples of the ears of HA and nHA users were collected from the Institute of Otolaryngology, “Cattolica del Sacro Cuore” University “Agostino Gemelli”, Rome, Italy. Swab samples were taken from the ear canal of 57 HA and 33 nHA users. The components of the microbial flora present on each swab sample were identified and characterized at the level of species. A total of 41 different bacterial species were identified. A statistically significant prevalence of polymicrobial communities was found in ears presenting signs of inflammation (2.5 ± 1.7 vs 2.1 ± 1.3;P = 0.02) and in HA users (2.3 ± 1.2 vs 1.7 ± 1.0;P = 0.002). Few putative pathogens were detected. Candida albicans spp. was not isolated in our study. A small number of swab samples presented no microbial growth. Bacterial species isolated from HA users with and without inflammation were assayed for the ability to form biofilm. Among gram-positive and gram-negative bacteria, S. aureus, CoNS, P. aeruginosa and K. pneumoniae were found to be strong biofilm producers. S. aureus and P. aeruginosa, isolated only from the ears of HA and nHA users presenting signs of inflammation, were further analyzed for their antibiotic-resistance profile and characterized by the Multilocus Sequence Typing (MLST) assay. The highest rates of antibacterial resistance were in S. aureus to penicillin (75.5%) and in P. aeruginosa, to amoxicillin-clavulanic acid, cefotaxime, ertapenem, tigecycline and trime-thoprim-sulfamethoxazole (100%). Moreover, three S. aureus strains (37.5%) were methicillin-resistant (MRSA). Of the eight S. aureus isolates, we identified six sequence types (ST) indicating that 75% are likely independent clones. Survival in Primary Carcinoma of the External and Middle ear Is Strongly Dependent on Stage at Diagnosi

Conclusion: early-stage detection of temporal bone carcinoma improves the survival rate. When detected at an advanced stage, predicted prognosis is poor even with a combination of surgery, radiotherapy and chemotherapy. Objectives: We evaluated treatment strategies and survival rates in cases of carcinomas of the external and middle ear. Method: Retrospective review of patients treated at Ehime University Hospital during the 32 years from 1977 to 2008. Subjects were 41 patients, consisting of 21 men and 20 women with a mean age of 63.2 years. Modified Pittsburgh staging system: stage I in 11 cases (27%), stage II in 4 (10%), stage III in 6 (15%) and stage IV in 20 (49%). Histopathology: squamous cell carcinoma (SCC) in 35 cases, adenoid cystic carcinoma (ACC) in 3, and adenocarcinoma (AC) in 3. Thirty-eight patients were treated by surgery. Radiation was added postoperatively in 23 patients. Three patients were treated by chemoradiotherapy. Results: The survival rate of carcinomas detected at an advanced stage was poor with a disease-specific 5-year survival rate of 100% in early-stage disease (stage I and II) versus 20.8% in stage III and 27.5% in stage IV disease, irrespective of histopathology of the tumor.

Salivary Gland Choristoma of the Middle ear and Review of the Literature

Conductive hearing loss due to middle ear masses is uncommon and usually diagnosed after biopsy. We present a case of a permanent facial palsy occurred following an uneventful biopsy during an exploratory tympanotomy in a salivary gland choristoma of the middle ear. Most salivary gland choristomas have been found in the head and neck. Its location in the ear is extremely rare, and thus we present the 38th case in English and non-English literature from the first publication by Taylor in 1961. Complete surgical removal of salivary gland choristomas of the middle ear is indicated when may not result in permanent damage to the facial nerve. Only biopsy and observation are recommended when the mass is intimately associated with the facial nerve or there are unsafe facial nerve abnormalities. Although the facial nerve is involved in 40% of cases, transient or even permanent facial palsies are exceptional. The reactivation of latent herpes virus in the facial canal may be involved in facial palsy’ etiology following minimal and uneventful middle ear surgery like a biopsy rather than nerve injury related to facial canal malformations.

Direct differentiation of ears and tassels from cultured shoot apices of maize

In vitro morphogenesis of inflorescences from the cultured corn seedling shoot tips was obtained on modified Murashige and Skoog (MS) medium in complete darkness. Some shoot tip meristems excised from seedlings of inbred line 515, inbred line 8112 and their filial generations would directly give rise to florets on modified MS medium supplemented with 2.0 mg/L N6-bezyladenine (6-BA) in five or six weeks. On the medium with 1.0 mg/L 6-BA and 0. 2 mg/L 2, 4-dichlorophenoxy acetic acid (2, 4-D), the explants swelled first, and produced multiple shoot clumps, then the culture of the shoot tips from all of the six inbred lines in experiment would ultimately initiate to develop ears and tassels accompanied by multiple shoot clumps developing on the medium with 1.0 mg/L 6-BA and 0. 2 mg/Lin-dole-3-butyric acid (IBA). The developmental patterns of the corn inflorescences were similar to the controls of normal plants in the field, but the number of the ears was much more than that of the tassels in vitro. It

Sociodemographic Characteristics and Prevalence of Self ear Cleaning in Sokoto Metropolis

Background: Self ear cleaning with cotton buds and other objects is a common practice. Objective: To determine the sociodemographic characteristics and the prevalence of self ear cleaning habit among patients attending a family medicine clinic in a teaching hospital in Sokoto metropolis. Materials and Methods: This was a 4 weeks prospective study carried out between May and June, 2013 at Usmanu Danfodiyo University Teaching Hospital and Specialist Hospital Sokoto. A structured self response questionnaire containing items on sociodemographic characteristics, frequency of administering objects into the ear and complications of self ear cleaning was administered to the respondents after institutional research ethical clearance. The data were analysed using SPSS for windows version 16.0. P-value was considered significantly if less than 0.05. Results: A total of 200 subjects were included in this study, comprising of 93 (46.5%) males and 107 (53.5%) females. Their ages ranged from 18-57 years with the mean age being 30.29 years (±8.70). Eighty percent (160) of the respondents practise self ear cleaning. Types of objects included writing objects and cotton buds. For many of the subjects, the frequency of insertion was once daily and both ears were frequently cleaned. Complications that have arisen due to the practice included otitis externa. Conclusion: Self ear cleaning was prevalent in the subjects. Considering the complications that may arise, ear cleaning is better done by trained health personnel.

Field Comparison of the Impact of Different Treatment Durations in the Treatment of Acute Otitis Externa

Background: Acute otitis externa is a common multi-factorial disorder in the dog. Several topical preparations are available on the veterinary market, which are licensed for an either specified duration of treatment or for a discretionary period that is determined by the clinician. Objectives: To compare the efficacy of two topical products, both licensed for the treatment of otitis externa in the dog, but with different treatment durations. Animal Population: One hundred and sixty dogs were enrolled in this multicentre field study from which 157 dogs were analysed in the Per Protocol sample (73 Aurizon?treated animals and 84 Easotic?treated animals). Method: Dogs were randomly assigned to Aurizon?or Easotic?treatment groups. Aurizon?(Vétoquinol SA: marbofloxacin, clotrimazole, dexamethasone) was administered daily in the affected ear(s) for 7 or 14 days, and was compared with a daily administration of Easotic?(Virbac SAS: gentamicin, miconazole, hydrocortisone aceponate) for 5 days. General and localised clinical signs were scored on days 0 (D0), 3 (D3), 7 (D7), 14 (D14) and 21 (D21). Results: Clinical cure rates at the end of treatment were 56.3% and 48.8% (p=0.35) in the Aurizon?and Easotic?groups respectively and 81.2% versus 74.7% one week after completing the course of treatment (p = 0.34). Twenty-one days after initially presenting for the study, cure rates were 84.3% in the Aurizon?group and 73.8% in the Easotic?(p=0.12). A relationship between severity of clinical signs and treatment duration was observed. Conclusion and Clinical Significance: At the end of the trial period, cure rates showed a tendency to be higher in the Aurizon?treated animals. The flexible dosage and the veterinary monitoring permitted treatment duration to be adjusted based upon the severity of otitis externa thus increasing the likelihood of clinical cure.

Chromsome and pedigree analysis of congenital malformation of external and middle ear

Objective:Using chromosome analysis of a Chinese pedigree to investigate the genetic relationship between chromosome and congenital malformation of the external and middle ear combined with pre-auricular fistula.Methods: Clinical data of all members from 3 family with congenital malformation of the external and middle ear combined with pre-auricular fistula were collected. A pedigrees chart of each family was draw Venous blood samples of all participants were taken for karyotype analysis.Results: A total of 21 members in three generations from three families were screened. Chromosomal abnormalities were detected in 9 people, including 3 patients and 6 carriers. The carriers were fathers and paternal grandparents. Structural abnormalities in chromosomes 1, 2 and 11 were identified.Conclusion:The genetic patterns of congenital malformation of the external and middle ear combined with the pre- auricular fistula in these three families are consistent with autosomal recessive inheritance. Family survey is helpful for further study of this disease.

Clinical Comparison and Analysis of Decoction of Traditional Chinese Medicine Combined with ear Acupoint Application and Simple Artificial Tears in the Treatment of Dry Eye Syndrome

OBJECTIVE: To study the clinical comparison and analysis of decoction of Traditional Chinese Medicine(TCM) combined with ear acupoint application and simple artificial tears in the treatment of dry eye syndrome. METHODS:A total of 62 patients(124 eyes) with dry eye syndrome treated in the Department of Ophthalmology of The 4th People's Hospital of Shenyang from July 2017 to March 2018 were selected and equally divided into 2 groups according to the admission order. The observation group received the combination therapy of TCM decoction and ear acupoint application, while the control group was given the simple artificial tears. The symptom score after 2 weeks, 4 weeks, and 8 weeks as well as the indicators including break-up time(BUT), Schirmer I test(SIT) and fluorescein(FL) were recorded and compared between groups. RESULTS: The symptom scores had no difference before treatment(P > 0.05), and were better in the observation group than those in the control group after treatment(P ears treatment method, the TCM decoction combined with ear acupoint application can significantly ameliorate the tear film stability, tear secretion, and reduce ocular surface damage for dry eye patients.

Aural Toileting and Dressing—Important Intervention Measures in the Management of Acute Suppurative Otitis Media

Background: Otitis media is one of the most common childhood infections, the leading cause of doctor’s visit by children in ENT clinic. The acute form if not recognized early is commonly characterized by suppuration from the middle ear following perforation of the tympanic membrane otherwise referred to as acute suppurative otitis media (ASOM). If not adequately treated, it progresses to chronic suppurative otitis media with attendant challenges in management. Ceasation of ear discharge is an indication of resolution of disease. The objective of this study is to appraise the value of local ear suction toileting and dressing in the ceasation of ear discharge in acute suppurative otitis media. Materials and Method: Consecutive new cases of acute suppurative otitis media seen at the ear, Nose and Throat clinic of a tertiary health centre in north western Nigeria were assessed for efficacy of aural toileting and dressing with antibioctic impregnated guaze in the treatment of acute suppurative otitis media and compared with patients with similar disease without aural toileting and dressing or dressing with normal saline soaked guaze in addition to systemic antibiotic and antihistamine medications. ear toileting was done by suctioning the discharge with suction machine, and cleaning the external auditory canal with hydrogen peroxide and methylated spirit. They were then dressed with gauze impregnated with antibiotic ear drops daily until there was no more ear discharge. The period of ceasation of ear discharge in each group was assessed. Results: There were 39 patients with acute suppurative otitis media (ASOM). Eighteen were females (46.2%) while 21 were males (53.8%), giving M:F ratio 1:2.1. Their ages ranged between 1 month and 45 years. About 36 (92.3%) were children and out of this, 28 (77.8%) were less than 5 years old. The average period of ceasation of discharge was 24 - 48 hours with those undergoing ear toileting and dressing but varied between 7 and 10 days with those without ear dressing or dressing with no

Amino Acid Composition Reduces Frequency of Common Colds. Results of a Controlled Randomised Completely Masked Trial

Purpose: Special nutrients have an impact on common cold infections. In this trial we have investigated the effectiveness of the specific amino acid composition “aminoplus? immun” on the occurrence, duration and severity of common cold. Material and Methods: Our study is a controlled, randomised completely masked trial comparing genuine trial remedy versus placebo in parallel groups. Eligible subjects were people with recurrent and chronic infections of the ear, nose and pharyngeal area. The treatment period was 90 days. The primary outcome was the mean number of days that the patients had symptoms of infections. Results: From Oct. 2009 to Feb. 2010 a total of 145 patients were recruited, of whom 119 had no major protocol violation and were valid for assessment. Two thirds of the patients were female. The mean age of the patients was 45 years. 65% of participants had come into contact with many people and thereby an increased risk of infection. On average the patients had 11.1 infections during the 12 months before the study. Compliance for the intake of the study preparation was 98.9%. Due to randomisation, the groups were very similar and balanced.
