BACKGROUND:New-onset diabetes after transplantation(NODAT) has become one of the major factors that affect the overall survival and long-term life quality in liver transplantation(LT) recipients. Previous studies found that the serum adiponectin concentration of diabetic patients is significantly lower than that of healthy subjects. Adiponectin regulates the blood glucose level by increasing body sensitivity to insulin through various mechanisms. In this study, we aimed to investigate the impact of diabetes related gene polymorphisms on the development of NODAT in liver recipients.METHODS:A total of 256 LT patients in a single-center were selected retrospectively for the study. Genomic DNA was extracted from explanted liver tissues, and tested for twelve diabetes mellitus associated single nucleotide polymorphisms by Sequenom Mass ARRAY. Modified clinical models in predicting NODAT were established and evaluated.RESULTS:The GG genotype of ADIPOQ rs1501299 gene polymorphism was significantly more frequent in NODAT than non-NODAT LT patients(56% vs 39%, P=0.014). Dominant model(GG vs GT+TT, P=0.030) and recessive model(GT+GG vs TT, P=0.005) also confirmed the genotype distribution difference between NODAT and non-NODAT groups. Age(OR=1.048, P=0.004), BMI(OR=1.107, P=0.041), and blood tacrolimus level at 1-month LT(OR=1.170, P=0.003) were clinical independent risk factors of NODAT. Furthermore, rs1501299 could improve the ability of clinical model in predicting NODAT(AUROC=0.743, P<0.001).CONCLUSION:ADIPOQ rs1501299 gene polymorphism is associated with an increased risk of NODAT, which should be added to the clinical models in predicting the occurrence of NODAT in LT recipients.

Background： Patients with diabetes mellitus （DM） have a higher risk of thromboembolic events： however, the optimal duration of dual antiplatelet therapy （DAPT） remains unclear. The goal of this study was to assess the efficacy and safety of various DAPT durations in patients with DM undergoing drug-eluting stent implantation. Methods： We conducted a literature search for randomized controlled trials （RCTs）. We searched databases including EMBASE, PubMed, Cochrane Library, and Scopus up to June 2016. Investigators extracted data independently, including outcomes, characteristics, and study quality. A random-effect model was used to pool odds ratios （ORs） with 95% confidence intervals （C/s） of the clinical outcomes. Results： Six RCTs totaling 6040 patients with DM were included in the study. Shorter-duration DAPT resulted in an increased rate of stent thrombosis （ST）（OR, 1.83, 95% CI： 1.03-3.26, P = 0.04）, but did not increase the risk of myocardial inihrction （OR. 1.33, 95% CI： 0.71 2.47, P=0.37）, stroke （OR, 0.96, 95% CI： 0.52-1.77, P 0.90）, target vessel revascularization （OR, 1.19, 95% CI： 0.46-3.07, P = 0.71 ）, all-cause death （OR： 0.72, 95% CI： 0.48-1.09, P = 0.12）, or cardiac death （OR, 0.82, 95% CI： 0.49-1.36, P= 0.44） significantly. Shorter-duration DAPT was associated with a decreased risk of major bleeding （OR. 0.60, 95% CI： 0.38-0.94, P = 0.02）. Conclusion： In patients with DM, longer-duration DAPT had a lower risk of ST, but was associated with an increased bleeding risk.

Background:diabetes mellitus(DM)is an important risk factor in the long-term outcomes of surgical revascularization.However,few studies have focused on patients with ischemic heart failure(IHF)and DM,and the results are controversial.This study aimed to evaluate the effect of DM on the long-term outcomes of IHF patients undergoing coronary artery bypass grafting(CABG).Methods:In this propensity-matched study,data of IHF patients who underwent CABG in our hospital from January 2007 to December 2017 were analyzed.With a mean 73-month follow-up time,the patients were divided into two groups according to whether they had DM.The primary endpoint was all-cause death,and the secondary endpoint was a composite of all-cause death,stroke,recurrent myocardial infarction,and revascularization.Results:There was no significant difference in all-cause mortality between the two groups(5.8%vs.4.1%,P=0.216).The incidence of main adverse cardiovascular and cerebrovascular events(MACCE)in the secondary endpoint was significantly higher in the DM group than that in the non-DM group(10.4%vs.8.1%,P=0.023).Conclusions:DM can negatively affect the long-term outcomes of IHF patients undergoing CABG by significantly increasing the overall incidence of MACCE,though the long-term survival does not show a significant difference between the DM and non-DM patients.

The knowledge, attitude, practice and predictors of complementary and alternative medicine use among type 2 diabetes mellitus patients in Indonesia

Objective: The prevalence of complementary and alternative medicine(CAM) usage among patients with type 2 diabetes mellitus(T2DM) in Indonesia is high. However, to date, little is known about why Indonesian T2DM patients choose CAM therapies, how their knowledge, attitude and practice(KAP) of CAM affects their choices, or how demographics correlate with patient choices. Therefore, this study aimed to investigate the KAP and predictors of CAM usage in T2DM patients in Indonesia.Methods: This was an observational, cross-sectional study. Patients were interviewed using a questionnaire. Chi-square tests or Fisher’s exact tests were used to compare demographic and clinical data, as well as KAP assessments, between T2DM patients who use and do not use CAM. Multivariate logistic regression analyses were used to investigate predictors of CAM usage.Results: A total of 628 T2DM patients were enrolled in the study. CAM therapies were used by 341 patients(54.3%). The most common therapies were herbs and spiritual healing, used by 100.0% and 68.3% of CAM-using patients, respectively. CAM therapies were frequently recommended by family members(91.5%), and CAM users had significantly more knowledge and more positive attitudes toward CAM therapies than nonusers. Among users, 66% said they would not follow their healthcare providers’ instructions to not use CAM therapies, and 69.5% said they would not disclose their plan to use CAM therapies with their healthcare provider. Neither demographic nor clinical characteristics were associated with CAM use. The factors that best predicted the use of CAM therapies were their availability and low cost(odds ratio [OR] = 4.59;95% confidence interval [CI]: 3.01–7.01), the belief that CAM therapies were safe(OR = 2.04;95% CI: 1.40–2.95), the belief that CAM therapies could help with diabetes control(OR = 1.75;95% CI: 1.15–2.66), and the belief that CAM therapies could help maintain physical health(OR = 1.68;95% CI: 1.13–2.49).Conclusion: CAM therapy users were more knowledgeable and had more

OBJECTIVE: To conduct a review of the systematic reviews and meta-analyses on the Traditional Chinese Medicine(TCM) treatment of type 2 diabetes mellitus(T2DM).METHODS: We searched Pub Med, Cochrane, Web of Science, Chinese Biomedical Literature Database(CBM), China National Knowledge Infrastructure(CNKI), China Science and Technology Journal Database(VIP), Wanfang, and other databases from database inception to May 2014 for systematic reviews and meta-analyses on TCM treatment of T2 DM. Manuscripts were read by two investigators if they met the inclusion criteria, and data were extracted. A Measurement Tool to Assess Systematic Reviews(AMSTAR) was used to classify research quality, and the evidence quality was graded by the Grade of Recommendation, Assessment, Development, and Evaluation(GRADE) system.RESULTS: Eighteen systematic reviews and meta-analyses were considered. Fifteen analyzed the efficacy of Chinese herbal medicines, and three investigated the efficacy of acupuncture. AMSTAR evaluation ranged from 3-10, and re-evaluation of the main results implied that treatment of T2 DM with TCM has certain advantages when compared with conventional Western medicine. However, the evidence quality was generally low.CONCLUSION: This work shows favorable evidence for the clinical treatment of TCM on T2 DM. However, it is recommended that evidence-based decisions are made based on clinical trials because of the GRADE scores of the studies. To achieve higher quality of clinical research, clinical research on TCM requires specific and suitable research methods. Further trials may increase the quality of evidence to evaluate the clinical efficacy of TCM for T2 DM.

Objective\ To study the influence of soybean fibers on blood sugar, blood_lipid metabolism, and histomorphology in mice models with diabetes mellitus (DM). Method\ After normal mice and mice with Streptozocin (STZ)_induced DM were fed with forage containing a certain amount of soybean fiber (SF) for 5 weeks, blood sugar and blood lipid were determined, and morphological changes of hepatic and nephritic tissues were observated. Result\ The results of 5_week experiment had shown: 1. In normal mice fed with SF, the blood sugar level was decreased (P<0.05). There were no obvious changes in serum total cholesterol (TC) and the triglyceride (TG). The high_density_lipoprotein cholesterol (HDL_C) level was increased significantly (P<0.05); 2. Among DM mice, blood sugar was decreased considerably (P<0.001), TC and TG also decreased noticeably (P<0.05), HDL_C increased clearly (P<0.05); 3. SF has a protective role to the liver and kidney of DM mice. Conclusion\ It is suggested that SF has the function of decreasing blood sugar, blood_lipid and improving their metabolism, and of protecting liver and kidney of DM mice.

BACKGROUND: This study aimed to compare the rates of open and laparoscopic cholecystectomies and outcomes in patients with or without type 2 diabetes mellitus (T2DM) in Spain from 2003 to 2013. METHODS: We collected all cases of open and laparoscopic cholecystectomies using national hospital discharge data and evaluated the annual cholecystectomy rates stratiifed by T2DM status. We analyzed tendency for in-hospital mortality (IHM). We also analyzed the impact of T2DM on IHM in patients who underwent cholecystectomies. RESULTS: We identiifed 611 533 cholecystectomies (71.3%laparoscopic) in the patients, in whom 78 227 (12.8%) patients had T2DM. The rates of open cholecystectomies were 3-fold higher (130.0/105 vs 41.1/105) in patients with T2DM than in those without T2DM, and the rate of laparoscopic cholecys-tectomies was almost 2-fold higher (195.2/105 vs 111.8/105) in patients with T2DM. The annual rate of laparoscopic pro-cedures showed an 11-year relative increase of 88.3% (from 117.0/105 to 220.3/105) in T2DM and 49.2% (from 79.2/105 to 118.2/105) in patients without T2DM (P CONCLUSION: The rate of cholecystectomy was higher in patients with T2DM, and laparoscopic cholecystectomy was popularized in the past 11 years both in selectiv

Background：Debate on treatment for young patients with coronary artery disease still exists.This study aimed to investigate the intermediate-and long-term outcomes between coronary artery bypass grafting （CABG） and percutaneous coronary intervention （PCI） in patients aged 18-45 years with diabetes mellitus （DM）.Methods：Between January 2006 and March 2016,a total of 2018 DM patients aged 18-45 years including 517 cases of CABG and 1501 cases of PCI were enrolled in the study.Using propensity score matching （PSM）,406 patients were matched from each group.The intermediate-and long-term data were collected.The primary end point of this study was long-term death.The secondary end points included long-term major adverse cardiovascular and cerebrovascular events （MACCEs）,stroke,angina,myocardial infarction （MI）,and repeat revascularization.Results：Before PSM,the in-hospital mortality was 1.2% in the CABG group and 0.1% in the PCI group,with statistically significant difference （P 〈 0.0001）.The 10-year follow-up outcomes including long-term survival rate and freedom from MACCEs were better in the CABG group than those in the PCI group （97.3% vs.94.5%,P =0.0072;93.2% vs.86.3%,P 〈 0.0001）,but CABG group was associated with lower freedom from stoke compared to PCI group （94.2% vs.97.5%,P =0.0059）.After propensity score-matched analysis,these findings at 10-year follow-up were also confirmed.Freedom from MACCEs was higher in CABG group compared to PCI group,but no significant difference was observed （93.1% vs.89.2%,P =0.0720）.The freedom from recurrent MI was significantly higher in CABG patients compared with PCI patients （95.6% vs.92.5%,P =0.0260）.Furthermore,CABG was associated with a higher rate of long-term survival rate than PCI （97.5% vs.94.6%,P =0.0403）.There was no significant difference in the freedom from stroke between CABG and PCI groups （95.3% vs.97.3%,P =0.9385）.The hospital cost was greater for CABG （13,936 ± 4480 US dollars vs.10,926 ± 7376 US dollars,P 〈 0.0001）.Conclusions：In DM patients aged 18-45 years,t

Voxel-based morphometry has been used in the study of alterations in brain structure in type 1 diabetes mellitus patients. These changes are associated with clinical indices. The age at onset, pathogenesis, and treatment of type 1 diabetes mellitus are different from those for type 2 diabetes mellitus. Thus, type 1 and type 2 diabetes mellitus may have different impacts on brain structure. Only a few studies of the alterations in brain structure in type 2 diabetes mellitus patients using voxel-based morphometry have been conducted, with inconsistent results. We detected subtle changes in the brain structure of 23 cases of type 2 diabetes mellitus, and demonstrated that there was no significant difference between the total volume of gray and white matter of the brain of type 2 diabetes mellitus patients and that in controls. Regional atrophy of gray matter mainly occurred in the right temporal and left occipital cortex, while regional atrophy of white matter involved the right temporal lobe and the right cerebellar hemisphere. The ankle-brachial index in patients with type 2 diabetes mellitus strongly correlated with the volume of brain regions in the default mode network. The ankle-brachial index, followed by the level of glycosylated hemoglobin, most strongly correlated with the volume of gray matter in the right temporal lobe. These data suggest that voxel-based morphometry could detect small structural changes in patients with type 2 diabetes mellitus. Early macrovascular atherosclerosis may play a crucial role in subtle brain atro- phy in type 2 diabetes mellitus patients, with chronic hyperglycemia playing a lesser role.
