Correlation of huamn cytomegalovirus infection with inflammatory factor secretion and myocardial cell injury in patients with acute myocardial infarction

Objective:To study the correlation of human cytomegalovirus infection (HCMV) with inflammatory factor secretion and myocardial cell injury in patients with acute myocardial infarction.Methods: A total of 80 patients with acute myocardial infarction who were treated in our hospital between September 2015 and September 2017 were selected as the acute myocardial infarction (AMI) group of the research, and 60 healthy volunteers who received physical examination in our hospital during the same period were selected as the control group of the research. The serum was collected to qualitatively determine HCMV, and quantitatively determine the inflammatory factors and myocardial injury indicators.Results: Serum IL-10 and TGF-β1 levels of AMI group were significantly lower than those of control group whereas IL-1β, IL-6, CD62P, CCL21, CK-MB, cMyBP-C, sLOX-1, Caspase-3 and MDA levels were significantly higher than those of control group;serum IL-10 and TGF-β1 levels of AMI group of patients with HCMV infection were lower than those of patients with non-HCMV infection whereas IL-1β, IL-6, CD62P, CCL21, CK-MB, cMyBP-C, sLOX-1, Caspase-3 and MDA levels were higher than those of patients with non-HCMV infection. Conclusion: cytomegalovirus infection can promote and aggravate the inflammatory factor secretion and myocardial cell injury in patients with acute myocardial infarction.

Objective:To determine the prevalence of rubella and cytomegalovirus(CMV)among women in Tabriz City,Iran in 2013.Methods:This study was carried out in numerous laboratories in city of Tabriz.In 2013,26618 women aged 18-35 years were enrolled in this study.Serum samples were analyzed with chemiluminescence apparatus and SPSS software was used to conduct the statistical analysis.Results:A total of 26618 individuals were examined(10598 cases for anti-CMV and 16020 cases for anti-rubella).About 98.9%and 1.1%samples were reported positive and negative for CMV immunoglobulin G(IgG),respectively.The positivity for anti-CMV immunoglobulin M(IgM)antibody was found in 0.75%samples,while 99.25%were negative for the anti-CMV IgM antibody.Anti-IgG against rubella seropositivity was found in 95.8%and rubella IgM seropositivity in 0.93%.Anti-IgG against rubella seronegativity was found in 4.2%and rubella IgM seronegativity in 99.07%.Conclusions:According to the results,primary and secondary infections are usually symptomless or inconspicuous,but they can cause serious fetal damage.Routine antenatal care should therefore include serological testing to check the immune status of the woman and to diagnose any fresh virus infections that may arise.

cytomegalovirus viremia and tissue-invasive disease are common after kidney transplantation. Chemoprophylaxis has made substantial improvement in this clinical problem. Here we report a 29-year-old woman who had kidney allograft rejection and received lymphocyte-depleting therapy. She presented with a new oligo-arthritis that led to 2 successive arthrocenteses. The etiology of the inflammation could not be determined initially. On the second arthrocentesis, a synovial fluid cytomegalovirus polymerase chain reaction test was positive. The patient responded to treatment with valganciclovir, had negative follow-up serum cytomegalovirus polymerase chain reaction tests, and experienced resolution of her joint inflammation. We review briefly the data for cytomegalovirus chemoprophylaxis, preemptive screening, and treatment recommendations.

Effects of allitridin on the expression of transcription factor T-bet mRNA of cytomegalovirus-induced myocarditis in mice

Objective:To investigate the effects of allitridin on the expression of transcription factor T-bet of cytomegalovirus-induced myocarditis in mice and to analyze its role in anti-cytomegalovirus mechanisms. Methods: Sixty mice were randomly divided into allitridin therapy group, infected controls group and bland controls group. Allitridin therapy group were given to mice via the intraperitoneal (i.p.) route once a day with general dosege (25 mg·kg-1·d-1) at 24 h after infected MCMV; infected controls group were given with the same volume of 0.89% sodium chloride and bland controls group were only given with the same volume of 0.89% sodium chloride, without infected MCMV. All experimental mice were sacrificed on day 3, 5, 7 and 14 i.p. (n= 5 per time point). The expression levels of transcription factor T-bet were measured by RT-PCR. The expression levels of Th1 cytokine IFN-γ were measured by ELISA. Results: MCMV infection could markedly down-modulate the expression of IFN-γ and T-bet mRNA (P<0.01). Allitridin significantly up-regulated the expression of Th1 cytokines IFN-γ and Th1-specific transcription factor T-bet mRNA (P<0.01). Conclusion: Allitridin up-regulates the expression of transcription factor T-bet mRNA which is an important, but not the only factor to mediate the constitutive expression of Th1 cytokines IFN-γ.

cytomegalovirus(CMV)has been increasingly recognized as an important common pathogen in an immunocompromised state.The colon and stomach are the most common sites of its gastrointestinal infection. Symptoms of CMV gastritis are usually nonspecific and include epigastric pain,fever,nausea and bleeding. Endoscopic features are quite variable and include macroscopically normal mucosa,diffuse erythema, nodules,pseudotumors,erosions and ulcers.The bioptic detection of intranuclear inclusions is the hallmark of CMV infection.Most gastrointestinal CMV infection responds well to ganciclovir.We present endoscopic and histopathological features of CMV gastritis in a 71 year old woman receiving long-term prednisolone for pemphigus vulgaris.

Objective To investigate the clinical features and Tlymphocytes subsets in patients with acquired immunedeficiency syndrome ( AIDS ) and cytomegalovirus(CMV) infection. Methods A total of 48 hospitalizedpatients with human immunodeficiency virus (HIV)-1 /AIDS and CMV infections were recruited at Peking UnionMedical College Hospital from Jan 2010 to Aug 2017.Their clinical features and immune function were retrospectivelyanalyzed. Patients with only HIV/AIDS inprevious study were recruited as controls. Results All 48patients were at C3 stage, including 36 men and12 women. Five of them were younger than 30 years old,33 cases within 31 - 50 years old,and 10 cases olderthan 50 years old.

AIM: To determine the incidence of and the risk factors for cytomegalovirus(CMV) symptomatic infection and end-organ disease after autologous stem cell transplantation(ASCT).METHODS: A total of 327 consecutive non CD34+ selected autografts performed from the Hematology and Stem Cell Transplantation Unit of Regina Elena National Cancer Institute of Rome(Italy) in the period comprised between January 2003 to January 2015, were reviewed. Over the 327 autografts, 201 were performed in patients with multiple myeloma, whereas the remaining 126 in patients affected by non-Hodgkin's lymphoma and Hodgkin's lymphoma. The patients who underwent an ASCT for an acute leukemia(n = 20) in the sameperiod were excluded from this analysis. CMV DNA load in the blood has been determined by polymerasechain reaction in the case of a clinical suspicion of reactivation, therefore, no routine monitoring strategy was adopted. In the presence of signs and symptoms of CMV reactivation an antiviral treatment was performed.RESULTS: Overall, 36 patients(11%) required a specific antiviral treatment for a symptomatic CMV reactivation(n = 32) or an end-organ disease(n = 4). We observed 20 and 16 cases of CMV reactivation among lymphoma(16%) and myeloma patients(8%), respectively. Among cases of end-organ disease, 3 were diagnosed as interstitial pneumonia and one remaining case as hemorrhagic enteritis. All cases of CMV reactivation were observed in Ig G seropositive patients, with no documented cases of primary CMV infection. All patients were treated with a specific antiviral therapy, with a global rate of hospitalization of 55%; four patients received intravenous immunoglobulins. Transplantrelated mortality was significantly higher in patients who experienced a CMV reactivation(8.4% ± 4.7% vs 1.7% ± 0.8%; P = 0.047). In univariate analysis, a pretransplant HBc Ig G seropositivity, a diagnosis of T-cell non-Hodgkin's lymphoma and higher median age at transplant were significantly associated with the risk of developing a clinically relevant CMV
