Characteristic endoscopic findings and risk factors for cytomegalovirus-associated colitis in patients with active ulcerative colitis

AIM: To identify characteristic endoscopic findings and risk factors for cytomegalovirus(CMV)-associated colitis in patients with active ulcerative colitis(UC).METHODS: A total of 149 UC patients admitted to the Department of Gastroenterology, Nagoya University Hospital, from January 2004 to December 2013 with exacerbation of UC symptoms were enrolled in this retrospective study. All medical records, including colonoscopy results, were reviewed. CMV infection was determined by the presence of CMV antigen, CMV inclusion bodies in biopsy specimens, or positive specific immunohistochemical staining for CMV. Multivariate analysis was used to identify independent risk factors for CMV colitis.RESULTS: Multivariate analysis indicated independent associations with the extent of disease(pancolitis) anduse of > 400 mg corticosteroids for the previous 4 wk. In contrast, no association was seen with sex, age at UC diagnosis, immunomodulator use, or infliximab use. Punched-out ulceration was also significantly associated with CMV infection in patients with active UC(odds ratio = 12.672, 95%CI: 4.210-38.143).CONCLUSION: Identification of a total corticosteroid dose > 400 mg for 4 wk, extensive colitis and a specific endoscopic finding of punched-out ulcer might facilitate the more rapid diagnosis and timely initiation of antiviral therapy for CMV-associated colitis in patients with active UC.

Objective To investigate the relationship between cytomegalovirus (CMV) infection and the production of anticardiolipin antibody (ACA) in renal transplant recipients.Methods Polymerase chain reaction (PCR) was used qualitatively for detection of CMV DNA in 146 renal transplant recipients.Meanwhile,enzyme linked immunosorbent assay (ELISA) was used for detection of ACA IgG in blood serum samples from these recipients and 32 healthy individuals. Results The ACA positive rate was 17.1% among the 146 renal transplant recipients,and that of the control group was 6.3%.There was no significant difference.However,the ACA positive rate of the renal transplant recipients infected with CMV was 31.2%.It was clearly higher than that of those with no infection of CMV and that of the control group ( P <0.005). Conclusion The production of ACA was closely related to CMV infection.It might be one of the factors of chronic angiopathy of the transplanted kidney due to CMV infection.

AIM: To study the impact of association between cytomegalovirus(CMV) pathogenesis with dendritic cell(DC) maturation and function was evaluated in CMV reactivated liver transplanted patients in comparing with non-reactivated ones, and healthy controls. METHODS: Monocyte derived dendritic cells(Mo DCs) was generated from collected ethylenediaminetetraacetic acid-treated blood samples from patient groups and controls. In these groups, expression rates and mean fluorescent intensity of DC markers were evaluated using flowcytometry technique. Secretion of cytokines including: interleukin(IL)-6, IL-12 and IL-23 were determined using enzyme-linked immunosorbent assay methods. The gene expression of toll-like receptor 2(TLR2), TLR4 and IL-23 were analyzed using in-house real-time polymerase chain reaction protocols.RESULTS: Results have been shown significant decreases in: Expression rates of Mo DC markers including CD83, CD1 a and human leukocyte antigen DR(HLA-DR), the mean fluorescence intensitys for CD1 a and HLA-DR, and secretion of IL-12 in CMV reactivated comparedwith non-reactivated liver transplanted patients. On the other hand, significant increases have been shown in the secretions of IL-6 and IL-23 and gene expression levels of TLR2, TLR4 and IL-23 from Mo DCs in CMV reactivated compared with non-reactivated liver transplanted recipients. CONCLUSION: DC functional defects in CMV reactivated recipients, such as decrease in expression of DC maturation markers, increase in secretion of proinflammatory cytokines, and TLRs can emphasize on the importance of CMV infectivity in development of liver rejection in transplanted patients.

Background Leukocyte-associated immunoglobulin-like receptor (LAIR) includes LAIR-1(CD305) and LAIR-2(CD306). LAIR-1 is a transmembrane protein and exist s with both membrane-bound and soluble forms. LAIR-2 is just a secretory protein. Several studies have focused on LAIR-1. There were few literatures about LAIR-2. By now, subtotal studies about LAIR were basal researches. There was almost no investigation about the function of LAIR-2. We explored the correlation of the expression of soluble LAIR and cytomegalovirus pneumonitis after kidney transplantation for the first time. Methods A total of 45 serum specimens from living donor kidney transplantation recipients were divided into 2 groups: cytomegalovirus pneumonitis group (n=23) and control group (n=22). The concentration of soluble LAIR was detected with sandwich ELISA. The data was analyzed by t test. Results sLAIR-1 in both 2 groups did not accord with normal distribution. The level of sLAIR-1 was 0.000～3.039 ng/ml in pneumonitis group and 0.000～8.375 ng/ml in control group. LAIR-2 in pneumonitis group did not accord with Gaussian distribution. It was in 0.000～0.017ng/ml. LAIR-2 of control group was in consistent with the normal distribution. The concentration was 0.046∮0.035 ng/ml. The data was analyzed by t test after they were transformed and confirmed to be in consistent with normal distribution. sLAIR-1 was not significantly different between 2 groups (P≈0.05). But LAIR-2 was markedly lower in pneumonitis group than in control group (P≌0.01). Conclusion LAIR participates in immune regulation indeed. LAIR-2 can regulate sLAIR-1ˇs function through competitive binding with their identical ligand. The detection of LAIR-2 will be a new way of auxiliary diagnosis about cytomegalovirus pneumonitis following kidney transplantation.

目的：观察更昔洛韦治疗小儿巨细胞病毒肝炎(CMV肝炎)的疗效。方法：将巨细胞病毒肝炎患儿34例分为更昔洛韦治疗组19例及病毒唑对照组15例进行比较。结果：治疗组和对照组血CMV—IgM或尿CMV—DNA转阴率分别为89．47％和40％，P<0．005；总有效率分别为84．21％和26．67％，P<0．005；且治疗组血清胆红素(SB)及谷丙转氨酶(ALT)恢复正常时间均较对照组缩短。两组治疗过程中均未见明显毒副作用。结论：更昔洛韦治疗小儿CMV肝炎疗效肯定，值得临床推广。

cytomegalovirus(CMV) is a common virus in patients with ulcerative colitis receiving immunosuppressive drugs. Many studies suggested that CMV infection is an exacerbating factor in patients with ulcerative colitis. The role of CMV in exacerbations of ulcerative colitis has been discussed. One of studies starting this discussion is an article entitled "CMV positive ulcerative colitis: A single center experience and literature review" by Kopylov et al. However, we think that there are some points that should be emphasized about the study. Especially, the small number of patients in the study has led to meaningless results. Large controlled prospective trials are needed to clarify the benefit of antiviral therapy for active ulcerative colitis patients.

Effects of allitridin on the expression of transcription factor T-bet mRNA of cytomegalovirus-induced myocarditis in mice

Objective:To investigate the effects of allitridin on the expression of transcription factor T-bet of cytomegalovirus-induced myocarditis in mice and to analyze its role in anti-cytomegalovirus mechanisms. Methods: Sixty mice were randomly divided into allitridin therapy group, infected controls group and bland controls group. Allitridin therapy group were given to mice via the intraperitoneal (i.p.) route once a day with general dosege (25 mg·kg-1·d-1) at 24 h after infected MCMV; infected controls group were given with the same volume of 0.89% sodium chloride and bland controls group were only given with the same volume of 0.89% sodium chloride, without infected MCMV. All experimental mice were sacrificed on day 3, 5, 7 and 14 i.p. (n= 5 per time point). The expression levels of transcription factor T-bet were measured by RT-PCR. The expression levels of Th1 cytokine IFN-γ were measured by ELISA. Results: MCMV infection could markedly down-modulate the expression of IFN-γ and T-bet mRNA (P<0.01). Allitridin significantly up-regulated the expression of Th1 cytokines IFN-γ and Th1-specific transcription factor T-bet mRNA (P<0.01). Conclusion: Allitridin up-regulates the expression of transcription factor T-bet mRNA which is an important, but not the only factor to mediate the constitutive expression of Th1 cytokines IFN-γ.

Human cytomegalovirus (HCMV) is a ubiquitous DNA-containing herpesvirus causes severe and fatal diseases in immunocompromised patients and a prevalent cause of virus-associated birth defects. Blood transfusion donated for neonates, pregnant women, and immunocom-promised patients should be adequately screened for evidences of CMV infection prior to use in clinical management. The effective national programmes for quality-assured screening of donated blood have not yet been fully established, hence this study was undertaken to assess whether any bloodborne-CMV infections pose a significant threat to the safety of the blood supplies. A total of 200 voluntary blood donor subjects admitted to the Blood Bank of Benghazi/Libya were screened for transfusion-transmissible CMV (TT-CMV) using a highly sensitive CMV total IgG and IgM antibody enzyme immunoassay as well as CMV pp65 anti-genemia assays. We determined that the overall seropositivity for IgG antibodies (80.50%) was higher than that of IgM antibodies (39.00%), but only 2 (1.00%) individuals out of these donors were seropositive for the CMV-antigenic protein pp65. The frequency of CMV infection based on gender was incomparable due to the small population number of blood-donated females. According to age, there was not influence of various age groups on prevalence of anti-CMV IgG antibodies, while a progressive increase in seropositivity of CMV-IgM antibodies with age was detected. The age groups were not significantly associated with CMV prevalence. In contrast, only 2 (1.00%) patients were shown to be positive for all three performed assays indicating a recurrent infection. Our findings prove a risk of primary transfusion-associated transmission of CMV and may provide a policy guidance on ensuring safe blood supplies accessible to all patients who require transfusion.

cytomegalovirus(CMV)is frequently detected in inflammatory bowel tissue,especially in corticosteroid-refractory patients,and it has been blamed for adverse outcomes.However,the first acquisition of CMV does not involve the colon.In particular in the colonic mucosa,which evolved due to the gut microbial relationship,CMV promotes inflammation via recruited monocytes and not through replication in resident macrophages.Whether CMV is the last straw in the process of mucosal inflammation,a doomed agent,or an innocent bystander is a difficult question that remains elusive.With this work,we will try to review the relationship between intestinal mucosa and CMV in the framework of basic virological principles.
