coronary artery bypass grafting in the octogenarians： should we intervene, or leave them be？

Objective coronary artery bypass grafting （CABG） is gradually increasing in the elderly population. We aimed to investigate the risk factors and the results of CABG along with the long term survival in patients at an age of 80 and older. Methods Between Januaa-y 2002 and December 201 I, a total of i01 consecutive patients at an age of S0 and older who underwent CABG in our hospital were included in the study. The patients were followed and the long-term survival was estimated. Results The mean age of the patients was 82.98 ~ 2.27 years. Sixty-four （63.4%） were males and 37 （36.6%） were females. Emergency surgery, duration of cardiopulmonary bypass, the intensive care unit （ICU） stay, inotropic support, intra aortic balloon pulsation application, amount oferythrocyte transfusion and flesh frozen plasma transfusion and ventilation period were significantly higher in the patients who died in the hospital. The duration of cardiopulmonary bypass （CPB） was found to be an inde- pendent predictor of mortality （OR： 1.18, 95% CI 1.01-1.38, P = 0.034）. The in-hospital mortality was 16.8%. Kaplan-Meier analyses revealed a survival ratio of 91.3% at one year, 82.9% at three years and 69.0% at five years. Conclusions Patients at the age of 80 and older can be candidates for the CABG procedure bearing in mind that they may have a longer ventilation period and intensive care unit stay. The morbidity and mortality of this age group is considered within an acceptable range. Approaches to minimize CPB, or the choice of off-pump surgery, may be a preventive method to lower the incidence of mortality. Hence, CABG may be performed in this age group with a satisfactory survival ratio.

Research to date indicates that the number of coronary artery bypass graft （CABG） surgery patients affected by depression （i.e., major, minor, dysthymia） approximates between 30% and 40% of all cases. A longstanding empirical interest on psychosocial factors in CABG surgery patients highlights an association with increased risk of morbidity in the short and longer term. Recent evidence suggests that both depression and anxiety increase the risk for mortality and morbidity after CABG surgery independent of medical factors, although the behavioral and biological mechanisms are poorly understood. Though neither depression nor anxiety seem to markedly affect neuropsy- chological dysfunction, depression confers a risk for incident delirium. Following a comprehensive overview of recent literature, practical advice is described for clinicians taking into consideration possible screening aids to improve recognition of anxiety and depression among CABG surgery patients. An overview of contemporary interventions and randomized, controlled trials are described, along with suggestions for future CABG surgery research.

Repair of left ventricular aneurysm during off-pump coronary artery bypass surgery

Background Acute myocardial infarction can result in left ventricularaneurysm, which may in turn cause congestive heart failure, ventricular arrhythmia andthromboembolic events. This study evaluates results achieved with a modified linear closure of leftventricular aneurysms during off-pump coronary artery bypass surgery. Methods From January 2001 toMay 2004, 75 patients were operated on for nonruptured, postinfarctional, left ventricular aneurysmduring off-pump coronary artery bypass surgery. Repair was completed on the beating heart tominimize ischaemia and allow assessment of wall function and viability to guide closure. Allpatients presented with symptoms of angina and congestive heart failure or ventricular *** majority (75% ) of the patients were in NYHA functional class Ⅲ or Ⅳ. Preoperative ejectionfraction was 26% ±9%. The mean left ventricular, end diastolic diameter was (57. 5 ±7. 1) mm. Theventricular preoperative and postoperative performances were compared. χ~2 test and Student' s ttest were used to analyse the outcomes. A P value less than 0. 05 was considered *** Hospital mortality was 1. 3% (1/75). coronary artery bypass was performed with an average of(3. 3 ±1.2) grafts per patient. At the time of followup, all the patients had no symptoms. Themean NYHA class and ejection fraction increased significantly (P coronary artery bypass. Theoperation is associated with a low inhospital mortality and morbidity. A postoperative improvementin the early term cardiac functions and symptoms and quality of life was documented, increasing ourexpectations of an increased long-term survival.

Preoperative P-wave duration as a predictor of atrial fibrillation after coronary artery bypass grafting: A prospective cohort study with meta-analysis

Objectives:Reported prediction rules for postoperative atrial fibrillation(AF)have suffered from inconsistent results and controversy surrounding the predictive value of a preoperative P-wave duration(PreOPWD).This study examined PreOPWD as a predictor for AF after coronary artery bypass grafting(CABG).Methods:Two-hundred-and-ninety-nine patients with sinus rhythm before off-pump CABG were recruited into the ***'demographic and clinical data were evaluated *** were continuously monitored for the first seven postoperative *** logistic regressionwas used to determine significant predictors of *** were then combined with similar studies and a metaanalysis was ***:Postoperative AF was observed in 33.1%of 299 *** with AF were older,had a prolonged PreOPWD,higher incidences of hypertension,aortic regurgitation,and mitral regurgitation.A cut-off point of PreOPWD105ms achieved a specificity of 74%,and a sensitivity of 65%for predictive of *** analysis showed that PreOPWD105ms(odds ratio[OR]4.63,95%confidence intervals[CI]2.66 to 8.03,P<0.001),age60 years(OR 2.72,95%CI 1.51 to 4.90,P<0.01)and hypertension(OR 2.10,95%CI 1.08 to 4.07,P<0.05)independently predicted postoperative AF.A meta-analysis of this data combined with those of ten other studies showed that PreOPWD was greater in patients with POAF,with a weighted mean difference of 3.95 ms(95%CI 1.97 to 5.92,P<0.001).Conclusion:This study confirmed,among other predictive characteristics,that PreOPWD is a powerful independent predictor of POAF.

Sequential Vein bypass Grafting is Not Associated with an Increase of Either In-hospital or Mid-term Adverse Events in Off-pump coronary artery bypass Grafting

Background：The impact of sequential vein bypass grafting on clinical outcomes is less known in off-pump coronary artery bypass grafting （CABG）.We aimed to evaluate the effects of sequential vein bypass grafting on clinical outcomes in off-pump CABG.Methods：From October 2009 to September 2013 at the Fuwai Hospital,127 patients with at least one sequential venous graft were matched with 127 patients of individual venous grafts only,using propensity score matching method to obtain risk-adjusted outcome ***-hospital measurement was composite outcome of in-hospital death,myocardial infarction （MI）,stroke,requirement for intra-aortic ballon pump （IABP） assistance and prolonged *** adverse cardiac events （MACEs：Death,MI or repeat revascularization） and angina recurrence were considered as mid-term endpoints.Results：No significant difference was observed among the groups in baseline *** mean blood flow per vein graft was 40.4 ml in individual venous grafts groups versus 59.5 ml in sequential venous grafts groups （P 〈 0.001）.There were no differences between individual and sequential venous grafts groups with regard to composite outcome of in-hospital mortality,MI,stroke,IABP assistance and prolonged ventilation （11.0% vs.14.2%,P =0.45）.Individual in-hospital measurement also did not differ significantly between the two *** about four years follow-up,the survival estimates free from MACEs （92.5% vs.97.3%,P =0.36） and survival rates free of angina recurrence （80.9% vs.85.5%,P =0.48） were similar among individual and sequential venous grafts groups with a mean follow-up of 22.5 *** the Cox regression analysis,sequential vein bypass grafting was not identified as an independent predictor of both MACEs and angina recurrence.Conclusions：Compared to individual vein bypass grafting,sequential vein bypass grafting was not associated with an increase of either in-hospital or mid-term adverse events in patients under

Risk factors for mortality after coronary artery bypass grafting in patients with low left ventricular ejection fraction

Background We managed to assess and confirm the risk factors for mortality after coronary artery bypass grafting （CABG） operations so as to map out the proper guidance of surgical strategy especially in patients with low left ventncular ejection fraction （LVEF） in domestic polyclinic like ours. Methods Five hundred and forty-eight consecutive patients underwent CABG from December 1999 through August 2005 were analyzed retrospectively. Eighty-nine cases had an LVEF of 40% or less. All together twenty-two candidate factors were evaluated for their association with perioperative death using univariate and multivariate stepwise Logistic analysis. Results When data from all the patients who had undergone CABG were taken into account, LVEF, left ventricular end diastolic diameter （LVEDD）, mitral regurgitation, aneurysm of the heart wall, mitral repair/replacement, resection of aneurysm, concomitant aortic valve replacement, and perioperative intra-aortic balloon counter-pulsation （IABP）, left ventricular assist device （LVAD） and cardiopulmonary bypass （CPB） all showed an association with perioperative death in univariate analysis, while an LVEF of 〉40%, on the other hand, appeared to be a protective factor. In multivariate analysis, moderate to severe mitral regurgitation, aneurysm of the heart wall, repair of septal perforation and aortic regurgitation were proved to be risk factors. When the analysis was restricted to patients with an LVEF of 40% or less, such variables as age, LVEDD, mitral regurgitation, mitral repair/replacement, IABP, and CPB were qualified as risk factors in a univariate analysis. Age, moderate mitral regurgitation, aneurysm of the heart wall, CPB, left main coronary artery disease and female were associated with pefioperative death in a multivariate logistic regression analysis. The left internal mammary artery （LIMA） had become the conduit of choice for myocardial revascularization, since it has been rates of LIMA grafts are proved that 10 ears patency more than 90%. The arterial graft trunk dissection is a rare event, affecting the procedure effects and related to reoperation.3 According to Kim and coworker＇s study,4 the arterial graft trunk delayed dissection manifested by early post-operative angiography only occurred in 6 of 1111 of the off-pump coronary artery bypass grafts. So up to now, the consequence of dissections without severe hemodynamic abnormality is beyond our knowledge.

Predictors of atrial fibrillation after coronary artery bypass graft surgery

OBJECTIVE: To identify the clinical predictors of atrial fibrillation (AF) after coronary artery bypass grafting (CABG). METHODS: 322 consecutive patients who had undergone isolated CABG were reviewed. Preoperative, intraoperative and postoperative data were collected. Patients were grouped according to whether AF appeared postoperatively. RESULTS: AF occurred in 75 patients (23.3%). Most cases of AF (85.6%) appeared on or before the third postoperative day. The mean age for patients with AF was 62.5 years compared with 56.7 years for patients without AF (P or = 65 years (OR 2.7; 95% CI 1.5 to 5.1), lesions in the right coronary artery (OR 2.5; 95% CI 1.4 to 4.5), and early postoperative withdrawal of beta blocker (OR 3.9; 95% CI 2.1 to 7.7). CONCLUSIONS: AF remains the most common complication after CABG. Age and lesions in the right coronary artery can influence the incidence of AF, and beta blocker and magnesium may be the most economical and effective prevention for AF early after CABG.

Retrospective analysis of exercise capacity in patients with coronary artery disease after percutaneous coronary intervention or coronary artery bypass graft
