BACKGROUND The prognostic value of coronary collateral circulation(CC)in patients undergoing chronic total occlusion(CTO)percutaneous coronary intervention(PCI)is *** purpose of the study was to assess the prognostic value of current two CC grading systems and their association with long-term outcomes in patients with CTO underwent *** We consecutively enrolled patients with single-vessel CTO underwent PCI between January 2010 and *** patients were categorized into well-developed or poor-developed collaterals group according to angiographic Werner's CC(grade 2 *** 0–1)or Rentrop(grade 3 *** 0–2)grading *** primary endpoint was 5-year cardiac *** Of 2452 enrolled patients,the overall technical success rate was 74.1%.Well-developed collaterals were present in686 patients(28.0%)defined by Werner's CC grade 2,and in 1145 patients(46.7%)by Rentrop grade *** to Werner's CC grading system,patients with well-developed collaterals had a lower rate of 5-year cardiac death compared with those with poordeveloped collaterals(1.6%vs.3.3%,P=0.02),those with suboptimal recanalization was associated with higher rate of 5-year cardiac death compared with optimal recanalization(4.7%vs.0.8%,P=0.01)and failure patients(4.7%vs.1.6%,P=0.12).However,the similar effect was not shown in Rentrop grading *** In patients with the single-vessel CTO underwent PCI,well-developed collaterals by Werner's CC definition were associated with lower rate of 5-year cardiac ***'s CC grading system had a greater prognostic value than Rentrop grading system in patients with CTO underwent PCI.

Consensus on glycemic management for patients with coronary heart disease and type 2 diabetes

The prevalence of patients with coronary heart disease(CHD)and diabetes mellitus is notably high,posing sig-nificant residual cardiovascular risks even after routine interventions such as antihypertensive,lipid-lowering,and antithrombot-ic *** studies have demonstrated that certain glucose-lowering medications confer cardiovascular benefits for pa-tients with type 2 ***,a survey indicates that cardiologists may not be fully acquainted with the optimal screen-ing timing,indicators,and diagnostic criteria for type 2 diabetes,and there is insufficient awareness and a low rate of prescrip-tion of novel glucose-lowering medications with proven cardiovascular efficacy,such as glucagon-like peptide-1 receptor agon-ists(GLP-1 RAs)and sodium-glucose co-transporter-2 inhibitors(SGLT-2i).In this context,based on domestic and international guidelines or consensus and the latest evidence-based evidence,this consensus aims to standardize the glycemic management for patients with acute coronary syndrome,chronic coronary syndrome,and perioperative management for percutaneous coronary *** highlights the key points of screening and diagnosis of type 2 diabetes,and the comprehensive management of cardiovascular risk in patients with *** consensus elaborates on the principles and algorithms of glycemic management for CHD patients,without involving acute complications of diabetes,clarifies the clinical practice of glucose-lowering medications with cardiovascular benefits,and promotes the standardized use of these medications in cardiovascular and other related spe-cialty ***,it addresses the glucose-lowering treatment to comprehensively reduce cardiovascular risks.

Application of seven-step finger method combined with external counterpulsation in cardiac rehabilitation after coronary intervention

Background Interventional surgery for coronary heart disease(CHD)is one of the important surgical schemes for the treatment of CHD,which can help patients recover heart function and improve life *** purpose of this study was to observe the application of seven-step finger method combined with extracorporeal counterpulsation in cardiac rehabilitation after CHD intervention and its effect on readmission *** A total of 60 patients who underwent coronary intervention in our hospital from February 2022 to October 2023 were selected as the study objects,and were divided into the conventional group and the study group by random number table method,with 30 cases in each *** conventional group received routine postoperative care,and the study group carried out seven-step finger method combined with external counterpulsation intervention on the basis of the conventional postoperative *** postoperative pain and numbness of the two groups evaluated by visual analogue scale(VAS)at 1h,2h,4h after surgery and 1h after removal of the hemostatic apparatus,cardiac function indicators including left ventricular ejection fraction(LVEF)and left ventricular end-diastolic diameter(LVEDD),and perihospital indicators including 6-minute walking distance,barthel index(BI),length of stay and readmission rate were *** The VAS scores of pain and numbness in the study group were significantly lower than those in the conventional group at 2h,4h and 1h after the hemostat was removed(P<0.05).After intervention,LVEF in the study group was significantly higher than that in the conventional group,and LVEDD was significantly lower than that in the conventional group(P<0.05).At discharge,the 6-minute walking distance and BI value of the study group were significantly higher than that of the conventional group,and the length of hospital stay was significantly lower than that of the conventional group(P<0.05).The readmission rate of the study group(3.33%)was signifi

The application of spectral CT multiparametric myocardial imaging in preoperative non-invasive assessment for percutaneous coronary intervention

Correlation of coronary angiography and blood biochemical indexes among different Chinese medicine syndromes of coronary atherosclerotic heart disease

Long-term outcomes ofintravascular ultrasound-guided drug-eluting stents implantation in patients with acute coronary syndrome:ULTIMATE ACS subgroup

Objective To explore the long-term effects of intravascular ultrasound(IVUS)guidance on patients with acute coronary syndrome(ACS)undergoing drugeluting stents(DES)*** Data used in this study derived from ULTIMATE trial,which was a prospective,multicenter,randomized study.A total of 1448 all-comer patients were enrolled between 2014 August and 2017 *** endpoint of this study was target vessel failure(TVF)at 3 years,including cardiac death,target-vessel-related myocardial infarction,and clinically-driven target vessel *** ACS was present in 1136(78.5%)patients,and 3-year clinical follow-up was available in 1423 patients(98.3%).TVF in the ACS group was 9.6%(109/1136),which was significantly higher than 4.5%(14/312)in the non-ACS group(log-rank P=0.005).There were 109 TVFs in the ACS patients,with 7.6%(43/569)TVFs in the IVUS group and 11.6%(66/567)TVFs in the angiography group(log-rank P=0.019).
