Large conventional Osteosarcoma of the Proximal Humerus in a 13-Year-Old Child: Case Report

Introduction: Osteosarcoma is the most common primary malignant bone tumor in children. It is highly aggressive and has a poor prognosis. A late presentation modifies and makes difficult the management affecting the survival of children. We report the case of a large conventional osteosarcoma in a 13-year-old girl. Case Presentation: Adolescent girl admitted for painful swelling of the left shoulder with absolute functional impotence of the thoracic limb and severe anemia. The painful swelling was thought to have been caused by a minor trauma that had occurred six months previously. The patient’s general condition was poor, and she presented with a large, shiny, painful mass over the shoulder and upper 2/3 of the left arm, measuring 28 cm long by 28 cm wide and 57 cm in circumference, and a large fistulous axillary adenopathy. CT scan showed a tumour lesion of the left humerus with liver and lung metastases, raising suspicion of osteogenic osteosarcoma. The tumor was classified according to TNM staging: T2N1M1(a + b). Management was modified when uncontrolled bleeding developed. It consisted of an extended amputation of the left thoracic limb. Pathological analysis showed a high-grade conventional osteosarcoma. Quality improvement was obtained for thirty days, followed by the onset of dyspnea. The evolution was towards death at forty days post-operatively. Conclusion: Osteosarcoma is a highly aggressive cancer. Delayed treatment leads to a fatal outcome. Early diagnosis is one of the challenges to be met in order to improve survival.

Robotic total knee replacement(TKR)surgery has evolved over the years with the aim of improving the overall 80% satisfaction rate associated with TKR surgery.Proponents claim higher precision in executing the pre-operative plan which results in improved alignment and possibly better clinical outcomes.Opponents suggest longer operative times with potentially higher complications and no superiority in clinical outcomes alongside increased costs.This editorial will summarize where we currently stand and the future implications of using robotics in knee replacement surgery.

Interactive antimicrobial and toxicity profiles of Pittosporum angustifolium Lodd. extracts with conventional antimicrobials

Objective: Pittosporum angustifolium Lodd. is used to treat a variety of pathogenic diseases and inflamma-tion by Australian aborigines. Practitioners of complementary medicine frequently use herbal medicines concurrently with conventional antibiotics. There is a need to evaluate their effects in combination. Methods: The bacterial growth inhibitory activity of P. angustifolium leaf extracts was assessed against a panel of pathogenic triggers of some autoimmune diseases by standard disc diffusion and liquid dilution minimum inhibitory concentration (MIC) methods. Combinational effects between the extracts and con-ventional antimicrobials were classified using the sum of the fractional inhibitory concentration. Synergistic interactions were further assessed across a range of ratios by isobologram analysis. The tox-icity of the individual samples and combinations was evaluated by Anemia lethality and 3-(4,5-dimethyl thiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazolium) human dermal fibroblast cell viability assays. Results: P. angustifolium leaf extracts strongly inhibited the growth of several bacterial triggers of autoim-mune diseases. The methanolic, aqueous and ethyl acetate extracts were pwrticularly good inhibitors of Proteus mirabilis and Klebsiella pneumonia growth (MIC=26 and 57 ng/mL respectively). Some combinations of the extracts and conventional antibiotics signiflcantlv potentiated the combined inhibitory activity compared to the individual components. Of the 250 combinations studied, approximately 0.02% showed synergistic interactions, 49.6% were additive, 46.8% showed indifferent interactions and antago-nism occurred in only 0.02% of the combinations. Interestingly, all of the synergistic and antagonistic combinations contained tetracycline as their antibiotic component. Conclusion: P. angustifolium leaf extracts inhibit the growth of pathogenic triggers of some autoimmune diseases. Among the various types of metal matrix composites,SiC particle-reinforced aluminum matrix composites(SiCp/Al)are finding increasing applications in many industrial fields such as aerospace,automotive,and electronics.However,SiCp/Al composites are considered as difficult-to-cut materials due to the hard ceramic reinforcement,which causes severe machinability degradation by increasing cutting tool wear,cutting force,etc.To improve the machinability of SiCp/Al composites,many techniques including conventional and nonconventional machining processes have been employed.The purpose of this study is to evaluate the machining performance of SiCp/Al composites using conventional machining,i.e.,turning,milling,drilling,and grinding,and using nonconventional machining,namely electrical discharge machining(EDM),powder mixed EDM,wire EDM,electrochemical machining,and newly developed high-efficiency machining technologies,e.g.,blasting erosion arc machining.This research not only presents an overview of the machining aspects of SiCp/Al composites using various processing technologies but also establishes optimization parameters as reference of industry applications.

Primitive to conventional geometry projection for efficient phonon transport calculations

The primitive Wigner-Seitz cell and corresponding first Brillouin zone(FBZ)are typically used in calculations of lattice vibrational and transport properties as they contain the smallest number of degrees of freedom and thus have the cheapest computational cost.However,in complex materials,the FBZ can take on irregular shapes where lattice symmetries are not apparent.Thus,conventional cells(with more atoms and regular shapes)are often used to describe materials,though dynamical and transport calculations are more expensive.Here we discuss an efficient anharmonic lattice dynamic method that maps conventional cell dynamics to primitive cell dynamics based on translational symmetries.Such symmetries have not been utilized in typical lattice dynamical calculations.This leads to phase interference conditions that act like conserved quantum numbers and a conservation rule for phonon scattering that is hidden in conventional dynamics which significantly reduces the computational cost.We demonstrate this method for phonon transport in a variety of materials with inputs from first-principles calculations and attribute its efficiency to reduced scattering phase space and fewer summations in scattering matrix element calculations.

Fat clearance and conventional fixation identified ypN0 rectal cancers following intermediate neoadjuvant radiotherapy have similar long-term outcomes

BACKGROUND As a prognostic factor for colorectal cancer,lymph node(LN)status,particularly the number of LN harvested,has been demonstrated to be essential in the evaluation of quality control in terms of surgical specimen.Neoadjuvant chemoradiation,however,decreases the LN harvest.Therefore,certain approaches(such as fat clearance or methylene blue)has drawn significant attention in order to raise LN yield.AIM To compare the long-term oncologic outcome of ypN0 rectal cancer identified using fat clearance(FC)or conventional fixation(CF)following 30 Gy in 10 fractions(30 Gy/10f)of neoadjuvant radiotherapy(nRT).METHODS Three hundred and eighty-two patients with resectable and locally advanced rectal cancer were treated by 30 Gy/10f intermediate nRT(biologically equivalent dose of 36 Gy)plus total mesorectal excision.Two specimen fixation methods(FC or CF)were non-randomly used.The ypN0 status was identified in 124 and 101 patients in the FL and CF groups,respectively.Primary endpoints were local recurrence-free survival(LRFS)and cancer-specific survival(CSS).RESULTS The median follow-up of patients was 5.1 years.The median numbers of retrieved LNs in the FC and CF groups were 19.5(range,4-47)and 12(range,0-44),respectively,with a significant difference(P=0.000).The percentages of patients with 12 or more retrieved nodes were 82.3%and 50.5%(101/159)in the FC and CF groups,respectively,with a significant difference(P=0.000).The LRFS at 5 years were 95.7%and 94.6%in the FC and CF groups,respectively,without statistical difference(P=0.819).The CSS at 5 years were 92.0%and 87.2%in the FC and CF groups,respectively,without statistical difference(P=0.482).CONCLUSION For patients with ypN0 rectal cancer who underwent 30 Gy/10f preoperative radiotherapy,the increased retrieval of LNs using fat clearance is not associated with survival benefit.This time-consuming fixation method has a low efficacy as a routine practice.

Stochastic price based coordinated operation planning of energy storage system and conventional power plant

A generation company(GENCO)which has a conventional power plant(CPP)intends to add an energy storage system(ESS)beside the CPP to increase its flexibility and profitability.For this purpose,a new model is proposed for coordinated operation planning of the CPP and ESS in energy and spinning reserve markets in the presence of a bilateral contract.The proposed model is based on the stochastic price based unit commitment(PBUC)and price based storage commitment(PBSC).The uncertainties of the energy and spinning reserve prices and delivery requests in the spinning reserve market are modeled via scenarios based upon historical data.The proposed model maximizes the profitability of the ESS beside the CPP and encourages the GENCO to invest the ESS.ESS technology options to use beside the CPP are determined by economic assessments.Numerical results show that utilization of ESSs improves the technical operation of CPPs,as well as GENCOs’profitability.

Optimal management for alcoholic liver disease: conventional medications, natural therapy or combination?

Alcohol consumption is the principal factor in thepathogenesis of chronic liver diseases.Alcoholic liver disease(ALD)is defined by histological lesions on the liver that can range from simple hepatic steatosis to more advanced stages such as alcoholic steatohepatitis,cirrhosis,hepatocellular carcinoma and liver failure.As one of the oldest forms of liver injury known to humans,ALD is still a leading cause of liver-related morbidity and mortality and the burden is exerting on medical systems with hospitalization and management costs rising constantly worldwide.Although the biological mechanisms,including increasing of acetaldehyde,oxidative stress with induction of cytochrome p4502E1,inflammatory cytokine release,abnormal lipid metabolism and induction of hepatocyte apoptosis,by which chronic alcohol consumption triggers serious complex progression of ALD is well established,there is no universally accepted therapy to prevent or reverse.In this article,we have briefly reviewed the pathogenesis of ALD and the molecular targets for development of novel therapies.This review is focused on current therapeutic strategies for ALD,including lifestyle modification with nutrition supplements,available pharmacological drugs and new agents that are under development,liver transplantation,application of complementary medicines,and their combination.The relevant molecular mechanisms of each conventional medication and natural agent have been reviewed according to current available knowledge in the literature.We also summarized efficacy vs safety on conventional and herbal medicines which are specifically used for the prevention and treatment of ALD.Through a system review,this article highlighted that the combination of pharmaceutical drugs with naturally occurring agents may offer an optimal management for ALD and its complications.It is worthwhile to conduct large-scale,multiple centre clinical trials to further prove the safety and benefits for the integrative therapy on ALD.

Study on the Constitutive Model of Marble Based on the conventional Triaxial Compression Test

The RMB-150B rock mechanics test system was employed to obtain the complete stress-strain test curves under confining pressures of 0-30MPa for marble samples from Ya'an ,Sichuan province. On the basis of former study and the convention triaxial pressure test results ,the complete procedures curves which described the relationships between yielding strength、 peak strength、 residual strength and confining pressure was obtained. Taking the strain softening of rock into account, the bilinear elastic-linear softening-residual perfect plasticity four-linear model was put forward in this paper on the basis of the test results and theory of plasticity. This model was adopted to describe the behaviors of marble in different phases under triaxial compression with the constitutive equation of strain softening phase as focus. The results indicated that the theoretic model fitted in well with the test results.

Updating conventional soil maps by mining soil–environment relationships from individual soil polygons

conventional soil maps contain valuable knowledge on soil–environment relationships.Such knowledge can be extracted for use when updating conventional soil maps with improved environmental data.Existing methods take all polygons of the same map unit on a map as a whole to extract the soil–environment relationship.Such approach ignores the difference in the environmental conditions represented by individual soil polygons of the same map unit.This paper proposes a method of mining soil–environment relationships from individual soil polygons to update conventional soil maps.The proposed method consists of three major steps.Firstly,the soil–environment relationships represented by each individual polygon on a conventional soil map are extracted in the form of frequency distribution curves for the involved environmental covariates.Secondly,for each environmental covariate,these frequency distribution curves from individual polygons of the same soil map unit are synthesized to form the overall soil–environment relationship for that soil map unit across the mapped area.And lastly,the extracted soil–environment relationships are applied to updating the conventional soil map with new,improved environmental data by adopting a soil land inference model(SoLIM)framework.This study applied the proposed method to updating a conventional soil map of the Raffelson watershed in La Crosse County,Wisconsin,United States.The result from the proposed method was compared with that from the previous method of taking all polygons within the same soil map unit on a map as a whole.Evaluation results with independent soil samples showed that the proposed method exhibited better performance and produced higher accuracy.
