Tuina for infant congenital muscular torticollis:A randomized controlled trial

Objective:To compare the differences in the clinical effect of congenital muscular torticollis between the modified tuina(Chinese massage)manipulation and the textbook-version tuina manipulation in infant.Methods:From January 2017 to December 2018,68 infants with congenital muscular torticollis were collected from the Department of Acupuncture-Moxibustion of the Affiliated Hospital of Yanbian University.They were randomized into a modified tuina group and a textbook-version tuina group,34 cases in each one.In the modified tuina group,the infants were treated with the stroking and circular kneading manipulation,200 times,the plucking manipulation,500 times and the modified rolling manipulation,500 times.In the textbook-version tuina group,the infants were treated with the tuina for muscular torticollis described in Tuina,in the sequence of the circular kneading for 5-6 min,the pinching and grasping for3-5 min,neck traction for several times,the pushing and circular kneading for 3-5 min and gently holding Jianjing(肩井GB21)for 3-5 times.In each group,the treatment was given once every day and there was 1-day rest after every 6-day treatment.Totally,the duration of treatment was 60 days.Separately,before treatment,in 30 days and 60 days of treatment,the lump diameter and the infants'adaptability were observed and compared between the two groups.Results:The effective rates were 94.1%and 70.6%in the modified tuina group and the textbook-version tuina group respectively,presenting a significant difference(P<0.05).In 30 days of treatment,12 cases were cured in the modified tuina group and 7 cases in the textbook-version tuina group.In 60 days of treatment,the color Doppler ultrasound examination found that the lump diameter in the modified tuina group was obviously smaller than that in the textbook-version tuina group,indicating a significant difference(P<0.05).During the treatment,there was no case dropped out in the modified tuina group,but in the textbook-version tuina group,because of crying and low adaptability durin

The controlled release of two kinds of drugs,5-fluorouracil (5-FU) and hydrocortisonum (Hydro.) loaded in poly(ethylene-vinylalcohol) (EVAL) was dealt with,of which 5-FU/EVAL and Hydro /EVAL matrix systems are composed.The results were analyzed using the pseudo-steady-diffusion models coupled with the fundamental concepts of percolation theory.The percolation thresholds for the two systems were calculated,which could indicate the contributions of pore diffusion and matrix diffusion.

Pestle needling at Yāoyángguān-Bāzhèn points for intractable lumbodynia after lumbar disc herniation surgery: A randomized controlled trial

Objective:To observe the differences of clinical efficacy of intractable lumbodynia after lumbar disc herniation surgery treated by pestle needling at Yāoyángguān Bāzhèn points,electroacupuncture and western medication.Methods:A total of 210 patients with intractable lumbodynia after lumbar disc herniation were randomly divided into pestle needle group,electroacupuncture group and western medication group,with 70 cases in each group.The pestle needle group was treated with pestle needle poking Yāoyángguān-Bāzhèn(GV3-Bāzhèn)points.The electroacupuncture group was treated with conventional electroacupuncture,and the western medication group was treated with oral diclofenac sodium dual release enteric-coated capsules(Difene).The pestle needle group and the electroacupuncture group were treated once a day,and there was 1 day of rest after 6 days of treatment,and a course of treatment included 7 days,and a total of 3 courses of treatment were performed.The western medication group took 1 Difene capsule(75 mg)at a time,with once a day continuing for 21 days.Visual Analog Scale(VAS),Pain Rating Index(PRI)and Present Pain Intensity(PPI)were used to observe the improvement of pain before and after treatment,and the waist muscle tone was examined and the clinical efficacy was observed.Results:The post-treatment VAS,PRI,and PPI scores of the three groups were all lower than pretreatment,and the differences were statistically significant(all P0.05).Meanwhile,they were all lower than the electroacupuncture group,and the differences were statistically significant(both P<0.05).After treatment,the waist muscle tone of the three groups was higher than that pre-treatment,and the differences were statistically significant(all P<0.05).After treatment,the waist muscle tone of the pestle needle group was higher than the electroacupuncture group,and the electroacupuncture group was higher t

Verapamil is a calcium channel blocker drug which reduces heart rhythm and blood pressure in high blood pressure patient.The steady state of this drug has to be continually controlled to obtain optimal efficacy.In addition,patient compliance needs to be considered.Therefore,controlled release tablet of verapamil hydrochloride was essentially developed using hydrophilic matrix polymer,namely hydroxypropylmethyl cellulose(HPMC)and sodium alginate,and compared to marketed product[1].HPMC,a pH-independent polymer,hydrates to form a gel layer at the surface of the tablet to sustain the drug release.Sodium alginate,a pH-dependent gelling polymer,converts to insoluble alginic acid after exposure to acidic condition in stomach.

AIM:To evaluate the performance of a novel non-invasive controlled attenuation parameter(CAP)to assess liver steatosis.METHODS:This was a multi-center prospective cohort study.Consecutive patients(aged≥18 years)who had undergone percutaneous liver biopsy and CAP measurement were recruited from three Chinese liver centers.Steatosis was categorized as S0:<5%;S1:5%-33%;S2:34%-66%;or S3:≥67%,according to the nonalcoholic fatty liver disease(NAFLD)activity score.The FibroScan?502 equipped with the M probe(Echosens,Paris,France)was used to capture both CAP and liver stiffness measurement values simultaneously.Receiver operating characteristic curves were plotted,and the areas under the curves were calculated to determine the diagnostic efficacy.The accuracy of the CAP values at the optimal thresholds was defined by maximizing the sum of sensitivity and specificity(maximum Youden index).RESULTS:A total of 152 patients were recruited,including 52(34.2%)patients with NAFLD and 100(65.8%)with chronic hepatitis B(CHB)virus infection.After adjustment,the steatosis grade(OR=37.12;95%CI:21.63-52.60,P<0.001)and body mass index(BMI,OR=6.20;95%CI:2.92-9.48,P<0.001)were found independently associated with CAP by multivariate linear regression analysis.CAP was not influenced by inflammation,fibrosis or aetiology.The median CAP values and interquartile ranges among patients with S0,S1,S2 and S3 steatosis were 211(181-240)dB/m,270(253-305)dB/m,330(302-360)dB/m,and 346(313-363)dB/m,respectively.The cut-offs for the CAP values in all patients with steatosis≥5%,≥34%and≥67%were 253 dB/m,285 dB/m and 310 dB/m,respectively.The areas under the curves were 0.92,0.92and 0.88 for steatosis≥5%,≥34%and≥67%,respectively.No significant differences were found in the CAP values between the NAFLD group and the CHB group in each steatosis grade.CONCLUSION:CAP appears to be a promising tool for the non-invasive detection and quantification of hepatic steatosis,but is limited by BMI.

Some new experimental results of optically controlled dielectric resonator oscillators (DROs) are presented. A very stable X-band DRO was found to be optically tunable up to 17.5 MHz with modulation rate of 1.17 MHz/mW with red light illumination. And an even higher modulation rate of 2.24 MHz/mW with illumination of violet light was obtained. Instead of a drop in optically controlled DRO output power, a little rise of output power was achieved.

Shenzhu Guanxin Recipe Granules (参术冠心方颗粒) for Improving Exercise Tolerance in Patients with Stable Angina (SERIES Trial):A Protocol of Multicenter,Randomized,Double-Blind,Placebo Parallel controlled Clinical Trial

Background: Many patients with chronic angina experience anginal episodes despite successful recanalization, antianginal and antiischemic medications. Empirical observations suggested that Shenzhu Guanxin Recipe Granules(参术冠心方颗粒, SGR), a Chinese herbal compound, exerted potential impacts on increased treadmill exercise performance and angina relieve. However, there has been no systematic study to clarify the impact of SGR on exercise tolerance in patients with stable angina. The SERIES(ShEnzhu guanxin Recipe for Improving Exercise tolerance in patients with Stable angina) trial is designed to determine the effects of SGR on exercise duration, electrocardiographic(ECG) evidence of myocardial ischemia, and incidence of major adverse cardiac events(MACE) in stable anginal patients. Methods: A total of 184 eligible patients with stable angina will be randomly assigned to receive placebo or SGR(10 g/day for 12 weeks) in a 1:1 ratio. The primary outcome will be the change from baseline in total exercise tolerance duration, time to onset of angina and ECG ischemia during exercise treadmill testing performed over a 12-week study period. The secondary outcome will include ECG measures, the occurrence and composite of MACE and the Seattle Angina Questionnaire score. Moreover, the coronary microcirculation will be evaluated to explore the possible effects in response to treatment of SGR. After the procedure, all participants will be followed up by interview at 3 and 6 months, enquiring about any cardiac events, hospitalizations, cardiac functional level and medication usage. Additionally, the occurrence of adverse events will be evaluated at each follow-up. Discussion: This study may provide novel evidence on the efficacy of SGR in improving exercise tolerance and potentially reducing clinical adverse events.

Comparison between Xingnaojing Injection(醒脑静注射液)and Naloxone in Treatment of Acute Alcohol Intoxication:An Updated Systematic Review and Meta-Analysis of Randomized controlled Trials

Objectives:To investigate the effectiveness and safety of Xingnaojing Injection(XNJ,醒脑静注射液)compared with naloxone for the treatment of acute alcohol intoxication(AAI),and provide the latest evidence through evidence-based approach.Methods:Seven electro-databases including Pub Med,EMBASE,Cochrane Central Register of controlled Trials,Chinese National Knowledge Infrastructure Databases,Chinese Biomedical Literature Database,Chinese Science and Technology Periodical Database(VIP)and Wanfang Database were searched from the inception to January 2018.Randomized controlled trials(RCTs)comparing XNJ with naloxone for patients with AAI and reporting at least one of the below outcomes were included:patients’conscious recovery time,stay length in emergency department,disappearance time of the ataxia symptom,the severity of the symptoms,the blood alcohol content as wel as the adverse events.Methodological quality of included trials was assessed using the risk of bias tool which recommended by the Cochrane Col aboration.Meta-analysis was conducted by Review Manager 5.3 software.Results:Total y 141 trials with 13,901 patients were included in this review,al of them were assessed as unclear or high risk of bias.Results showed that on the basis of routine therapy,standard dose XNJ(10–20 m L)may have similar results with naloxone on the recovery time of consciousness(MD 12 min,95%CI 7.2–17.4 min)and disappearance time of symptoms(MD 6 min,95%CI–13.8–25.8 min)for patients with AAI.Larger dose of XNJ Injection(21–40 m L)may speed up the time(almost 1 h earlier).Combination of XNJ and naloxone seemed superior to the naloxone alone for al the relevant outcomes.The average difference of time in consciousness recovery was 2 h and the number of AAI patients whose consciousness recovery within 1 h was above 50%the combination group than in the control group(RR 1.42,95%CI 1.29 to 1.56).No severe adverse events or adverse reactions of XNJ were reported in the included trials.Conclusions:Low quality of evidence showed XNJ may have equal eff

Evaluation of outcomes reported in randomized controlled trials for herbal remedies for adults with chronic hepatitis C

Background:Herbal remedies have been widely utilized in treating chronic hepatitis C (CHC) worldwide.Selecting appropriate outcomes to reflect both beneficial and harmful effects is a crucial step in designing randomized controlled trials (RCTs).This study evaluated the outcomes reported in RCTs on herbal remedies for CHC with comparison to the core outcomes recommended by the Cochrane Hepato-Biliary Group (CHBG),to check the consistency of the outcomes and to provide recommendation for future researches.Methods:A systematic literature search was conducted in Western and Chinese databases to identify RCTs on herbal remedies for adults with CHC.For each trial,all the outcomes reported in the results section were collected.Comparison between trial outcomes and CHBG core outcomes were evaluated and summarized with descriptive statistics.Results:A total of 116 RCTs involving 9154 participants were included;27 outcomes were identified.Commonly reported outcomes included alanine aminotransferase (64 trials,55.2%),adverse events (58 trials,50.0%),and end-of-treatment virological response (50 trials,43.1%).All trials indicated that the herbal remedies under investigation had a positive effect and was markedly more effective than the control.Nearly half of the trials reported that the combination of herbal medicine and antiviral drugs could ameliorate adverse events.Very few trials reported primary core outcomes relating survival and quality of life.The most frequently reported core outcomes are non-serious adverse events (54 trials,46.6%),viral response (27 trials,23.3%),and biochemical response (24 trials,20.7%).Conclusion:The variation and inconsistency in trial outcomes impedes research synthesis efforts,and indicate the need for comparable outcomes through the development of core outcome sets in CHC.The low concordance of outcome reporting could be improved by following CHBG core outcomes recommendation.

High resurgence of dengue vector populations after space spraying in an endemic urban area of Thailand:A cluster randomized controlled trial

Objective: To examine the resurgence rate, house density index(HDI) and parous rate of the Aedes aegypti vector after space spraying carried out by the routine spraying team,and compare with the rates after standard indoor ultra low volume(SID-ULV) spraying carried out by the trained research spraying team.Methods: Between March and September 2014, a cluster randomized controlled trial including 12 clusters(6 regular ULV, 6 SID-ULV) with totally 4 341 households was conducted, and around 20–31 houses in each cluster were selected for assessment. The parous rate and HDI of collected mosquitoes 2 days before and 1, 2 and 6 days after spraying were obtained and compared.Results: The HDI dropped significantly from the baseline 1 and 2 days after spraying to a non-zero value in the SID-ULV treated locations but not in the regular ULV group locations. However, by 6 days after spraying, the HDI of both groups had returned to the base value measured 2 days before spraying. There were no statistically significant differences in the parous rate between groups.Conclusions: SID-ULV is more effective in reducing Aedes aegypti populations.However, rapid resurgence of dengue vector after spraying in urban areas was observed in both groups.
