Objective: To describe the clinical manifestation of patient with severe dengue, to identify the serotypes and genotypes of dengue viruses(DENV) which concurrently infecting the patient, and to explore the possible relationship of severe dengue with the concurrent infection of DENV. Methods: Dengue diagnosis was performed using NS1 antigen detection and Ig G/Ig M ELISA. Standard clinical and laboratory examinations were performed to obtain the clinical and hematological data. DENV concurrent infections were detected and confirmed using RT-PCR and DENV Envelope gene sequencing. Phylogenetic analyses were performed to determine the genotypes of the viruses. Results: The patient was classified as having severe dengue characterized by severe plasma leakage, hemorrhage, and organ damage involving lung, liver, and kidney. concurrent infection of DENV serotype 2 and 3 was observed. The infecting DENV-2 virus was grouped into Cosmopolitan genotype while DENV-3 virus was classified into Genotype Ⅰ. Both viruses were closely related to isolates that were endemic in Jakarta. Viremia measurement was conducted and revealed a significantly higher virus titer of DENV-3 compared to DENV-2. Conclusions: The occurrence of multi-serotype DENV infections was presented in a patient with severe clinical manifestation in Indonesia. The hyperendemicity of dengue in Indonesia may contribute to the DENV concurrent infections cases and may underlie the severity of the disease.

Objective:To establish the nature and extent of dual dengue and malaria infections in an endemic area through a longitudinal study.Methods:A prospective study was conducted from August 2005 to December 2010 to document the nature and extent of concurrent dengue and malaria infections in an area in central Kolkata,endemic both for dengue and malaria.Results: Of 2 971 suspected cases of dengue fever,in 605(20.36%) persons dengue infection was detected, of whom 46(7.60%,46/605) patients(40 and 6 suffered from secondary and primary dengue fever respectively) were simultaneously suffering from malaria(28 and 18 were infected with Plasmodium vivax(P.vivax) and Plasmodium falciparum(P.falciparum) respectively,such dual infections of dengue and malaria were detected in all the years of the study period,except 2007,indicating intense transmission of both dengue and malaria in the study area,and the phenomenon was not an isolated one,the rate of concomitant infections ranged from 25%in 2009 to 4.9%in 2005.Out of total population surveyed,1.54%(46/2 971) had concurrent dengue and malaria infection.Conclusions:These findings added a new dimension in diagnosis,treatment, epidemiology and control of dengue and malaria.The possible risk of concurrent dengue and malaria infectioas should always be kept in mind in endemic areas for early diagnosis employing modern technology and prompt and effective treatment to avoid serious complications.

There are estimated to be 220800 cases of prostate cancer diagnosed in 2015, making up 26% of all cancer diagnoses. Fortunately, adenocarcinoma of the prostate is often a highly treatable malignancy. Even though the majority of prostate cancer patients present with localized disease, prostate cancer still accounts for over 27000 deaths a year. There is a subset of patients thatare likely to recur after locoregional treatment that is thought of as a "high-risk" population. This more aggressive subset includes patients with clinical stage greater than T2 b, Gleason score greater than 7, and prostate specific antigen greater than 20 ng/d L. The rate of biochemical relapse in this high risk group is 32%-70% within five years of definitive focal therapy. Given these discouraging outcomes, attempts have been made to improve cure rates by radiation dose escalation, addition of androgen depravation therapy, and addition of chemotherapy either sequentially or concurrently with radiation. One method that has been shown to improve clinical outcomes is the addition of chemotherapy to radiotherapy for definitive treatment. concurrent chemoradiation with 5-fluorouracil, estramustine phosphate, vincristine, docetaxel, and paclitaxel has been studied in the phase I and/or II setting. These trials have identified the maximum tolerated dose of chemotherapy and radiation that can be safely delivered concurrently and established the safety and feasibility of this technique. This review will focus on the addition of concurrent chemotherapy to radiotherapy in the definitive management of high-risk prostate cancer.

In this paper,a parametric level-set-based topology optimization framework is proposed to concurrently optimize the structural topology at the macroscale and the effective infill properties at the micro/meso scale.The concurrent optimization is achieved by a computational framework combining a new parametric level set approach with mathematical programming.Within the proposed framework,both the structural boundary evolution and the effective infill property optimization can be driven by mathematical programming,which is more advantageous compared with the conventional partial differential equatiodriven level set approach.Moreover,the proposed approach will be more efficient in handling nonlinear problems with multiple constraints.Instead of using radial basis functions(RBF),in this paper,we propose to construct a new type of cardinal basis functions(CBF)for the level set function parameterization.The proposed CBF parameterization ensures an explicit impose of the lower and upper bounds of the design variables.This overcomes the intrinsic disadvantage of the conventional RBF-based parametric level set method,where the lower and upper bounds of the design variables oftentimes have to be set by trial and error;A variational distance regularization method is utilized in this research to regularize the level set function to be a desired distanceregularized shape.With the distance information embedded in the level set model,the wrapping boundary layer and the interior infill region can be naturally defined.The isotropic infill achieved via the mesoscale topology optimization is conformally fit into the wrapping boundary layer using the shape-preserving conformal mapping method,which leads to a hierarchical physical structure with optimized overall topology and effective infill properties.The proposed method is expected to provide a timely solution to the increasing demand for multiscale and multifunctional structure design.

Data streaming applications, usually composed of sequential/parallel data processing tasks organized as a workflow, bring new challenges to workflow scheduling and resource allocation in grid environments. Due to the high volumes of data and relatively limited storage capability, resource allocation and data streaming have to be storage aware. Also to improve system performance, the data streaming and processing have to be concurrent. This study used a genetic algorithm （GA） for workflow scheduling, using on-line measurements and predictions with gray model （GM）. On-demand data streaming is used to avoid data overflow through repertory strategies. Tests show that tasks with on-demand data streaming must be balanced to improve overall performance, to avoid system bottlenecks and backlogs of intermediate data, and to increase data throughput for the data processing workflows as a whole.

Bone loss from Wnt inhibition mitigated by concurrent alendronate therapy

Dysregulated Wnt signaling is associated with the pathogenesis of cancers, fibrosis, and vascular diseases. Inhibition of Wnt signaling has shown efficacy in various pre-clinical models of these disorders. One of the key challenges in developing targeted anti-cancer drugs is to balance efficacy with on-target toxicity. Given the crucial role Wnts play in the differentiation of osteoblasts and osteoclasts, acute inhibition of Wnt signaling is likely to affect bone homeostasis. In this study, we evaluated the skeletal effect of small molecule inhibitor of an o-acyl transferase porcupine（PORCN） that prevents Wnt signaling by blocking the secretion of all Wnts. Micro-computed tomography and histomorphometric evaluation revealed that the bones of mice treated with two structurally distinct PORCN inhibitors LGK974 and ETC-1922159（ETC-159） had loss-of-bone volume and density within 4 weeks of exposure. This decreased bone mass was associated with a significant increase in adipocytes within the bone marrow. Notably,simultaneous administration of a clinically approved anti-resorptive, alendronate, a member of the bisphosphonate family,mitigated loss-of-bone mass seen upon ETC-159 treatment by regulating activity of osteoclasts and blocking accumulation of bone marrow adipocytes. Our results support the addition of bone protective agents when treating patients with PORCN inhibitors.Mitigation of bone toxicity can extend the therapeutic utility of Wnt pathway inhibitors.

As a typical erasure coding choice, Reed-Solomon （RS） codes have such high repair cost that there is a penaltyfor high reliability and storage efficiency, thereby they are not suitable in geo-distributed storage systems. We present anovel family of concurrent regeneration codes with local reconstruction （CRL） in this paper. The CRL codes enjoy threebenefits. Firstly, they are able to minimize the network bandwidth for node repair. Secondly, they can reduce the numberof accessed nodes by calculating parities from a subset of data chunks and using an implied parity chunk. Thirdly, they arefaster than existing erasure codes for reconstruction in geo-distributed storage systems. In addition, we demonstrate howthe CRL codes overcome the limitations of the Reed-Solomon codes. We also illustrate analytically that they are excellent inthe trade-off between chunk locality and minimum distance. Furthermore, we present theoretical analysis including latencyanalysis and reliability analysis for the CRL codes. By using quantity comparisons, we prove that CRL（6, 2, 2） is only0.657x of Azure LRC（6, 2, 2）, where there are six data chunks, two global parities, and two local parities, and CRL（10,4, 2） is only 0.656x of HDFS-Xorbas（10, 4, 2）, where there are 10 data chunks, four local parities, and two global paritiesrespectively, in terms of data reconstruction times. Our experimental results show the performance of CRL by conductingperformance evaluations in both two kinds of environments： 1） it is at least 57.25% and 66.85% more than its competitorsin terms of encoding and decoding throughputs in memory, and 2） it has at least 1.46x and 1.21x higher encoding anddecoding throughputs than its competitors in JBOD （Just a Bunch Of Disks）. We also illustrate that CRL is 28.79% and30.19% more than LRC on encoding and decoding throughputs in a geo-distributed environment.

The functional-level test has been proposed as an alternative to reduce the complexity of test when VLSI gets larger and more complicated. It has been successful for circuits such as memories, PLAs and microprocessors. However, the functional-level test for general functional models has seldom been studied. This paper presents an object-oriented VLSI model and a functional-level fault simulation methodology for general functional model. Based on the proposed VLSI model, FFS (Functional-level Fault Simulator) with friendly visual interface has been implemented on Microsoft Windows platform by use of C++. It is an integral part of FMVS (Functional test Modeling and Verification System)-an extended subsystem of TeDS (Test Development System). The goal of FFS is to determine the fault coverage, generate fault dictionary and compact original test set at the function-level. In order to be efficient, FFS uses the concurrent and parallel mechanisms by taking advantage of the object-oriented VLSI model. The object-oriented VLSI model based fault simulation has been validated in the functional-level test by simulation results and the satisfying performance of FFS.
