Background Serum interleukin-6(IL-6)has a moderate diagnostic performance in pediatric acute appendicitis(PAA).The evidence regarding its capacity to discern between complicated and uncomplicated PAA is scarce.Methods We designed a prospective observational study to validate serum IL-6 as a marker for diagnostic classification between complicated and uncomplicated PAA.This study included 205 patients divided into three groups:(1)patients who underwent major outpatient surgery(n=57);(2)patients with non-surgical abdominal pain(NSAP)in whom the diagnosis of PAA was excluded(n=53),and(3)patients with a confirmed diagnosis of PAA(n=95).The PAA patients were further classified as uncomplicated or complicated PAA.IL-6 concentration was determined in all patients at diagnosis.Comparative statistical analysis was performed using the Mann-Whitney U test,the Fisher exact test and the Kruskall Wallis test.The area under the receiver operating characteristic curves(AUC)were calculated.Results Median(interquartile range,IQR)serum IL-6 values were 2 pg/mL(2.0–3.4)in group 1,3.9 pg/mL(2.4–11.9)in group 2,and 23.9 pg/mL(11.1–61.0)in group 3(Pcomplicated PAA had median(IQR)serum IL-6 values of 17.2 pg/mL(8.5–36.8),and those with complicated PAA had 60.25 pg/mL(27.1–169)serum IL-6(Pcomplicated and complicated PAA was 0.77(95%CI 0.68–0.86)and the cut-off point was 25.90 pg/mL,with a sensitivity and specificity of 84.6%and 65.6%,respectively.Conclusions Serum IL-6 has a good performance in discerning between complicated and uncomplicated PAA.A score including clinical and radiological variables may increase the diagnostic performance of this molecule.

Dear Editor, We are writing this letter to present a case of ontreatment isolated superior ophthalmic vein thrombosis(SOVT) that progressed to cavernous sinus thrombosis(CST) and further complicated with carotid cavernous fistula(CCF). SOVT is a rare but potentially lethal illness. It is normally presented with orbital congestion syndrome with proptosis, ophthalmoplegia, diplopia, and high intraocular pressure(IOP)[1]. Late detection of this disease can lead to devastating complications such as optic nerve compression。

Thoracic endovascular aortic repair(TEVAR)is the first choice of type B aortic dissection(T B A D).U However,when the dissection involves an aortic arch with close proximity to the supra-aortic branches,the situation becomes complicated and challenging for classical TEVAR.In recent years,the Hybrid-ENDO vascular-Open arch repair(HENDO)theory that consists of some treatment strategies specifically for aortic arch lesions,becomes popular.Based on the HENDO theory,the modified endovascular technique,simultaneously aiming at the reconstruction of the involved supra-aortic branches and coverage of TBAD,is the first choice with promising future development.According to the specific anatomic situation and the pathology of the disease,hybrid or open surgery can be performed selectively.

Objective:To review the problems encountered in surgical treatment of complicated pulmonary hydatid cysts and to evaluate the functional results in the surgery of complicated hydatid cysts.Methods:The medical records for 89 patients with complicated pulmonary hydatidosis were retrospectively investigated.The series consisted of 47 male and 42 female patients with a mean age of 32±8 years.Study performed during January 2000 to December 2007,all patients were treated surgically.Data related to surgical procedures performed,postoperative morbidity,hospitalization time,and cyst recurrence were collected from each individuals records,and the group findings were compared.Results:Among these cysts,58 were perforated,23 were infected,and 13 were cysts with pleural complications.Cystotomy plus capitonnage was the most frequently performed operative technique(n=43),followed by cystotomy plus closure of bronchial openings(n = 28),pericystectomy plus capitonnage(n=13),decortications(n=7),lobectomy and segmentectomy(n=3).The 11 cases with coexisting liver cysts were approached by right thoracophrenotomy.Postoperative complications developed in 12 patients(13.4%).Conclusion:Surgery is the primary mode of treatment for patients with pulmonary hydatid disease.complicated cases have higher rates of preoperative and postoperative complications and require longer hospitalization time and more extensive surgical procedures than uncomplicated cases.This underlines the need for immediate surgery in any patient who is diagnosed with pulmonary hydatidosis when it is indicated.

In this paper a direct coupling analysis method (DCAM) of hydroelastic responses of a very large floating structures (VLFS) in complicated geographical environment is presented. In this method the three-dimensional hydroelasticity theory of floating bodies is combined with the shallow water wave theory, to allow for proper description of the influence of uneven seabed and sheltering effect of islands on the hydroelastic responses of a VLFS deployed near island and reefs in shallow sea. This method and the numerical procedures were verified and validated by comparison-between the predictions and the model test results of a 3-module VLFS and an 8-module VLFS in two simulated shallow sea regions with different seabed topography.

complicated acute appendicitis(CAA)is a serious condition and carries significant morbidity in children.A strict diagnosis is challenging,as there are many lesions that mimic CAA.The management of CAA is still controversial.There are two options for treatment:Immediate operative management and nonoperative management with antibiotics and/or drainage of any abscess or phlegmon.Each method of treatment has advantages and disadvantages.Operative management may be difficult due to the presence of inflamed tissues and may lead to detrimental events.In many cases,non-operative management with or without drainage and interval appendectomy is advised.The reasons for this approach include new medications and policies for the use of antibiotic therapy.Furthermore,advances in radiological interventions may overcome difficulties such as diagnosing and managing the complications of CAA without any surgeries.However,questions have been raised about the risk of recurrence,prolonged use of antibiotics,lengthened hospital stay and delay in returning to daily activities.Moreover,the need for interval appendectomy is currently under debate because of the low risk of recurrence.Due to the paucity of high-quality studies,more randomized controlled trials to determine the precise management strategy are needed.This review aims to study the current data on operative vs non-operative management for CAA in children and to extract any useful information from the literature.

With the view of effectively fitting the complicated water level process of the lower Yellow River, polynomial regression, stepwise regression, parameters by ridge estimate and so on, are logically integrated. And the progressive transformation is introduced. Then a new method is put forward. The core difference of this new method from the same kind of methods lies in that in this method the strong coupling effect of weak influencing factors which is common in a complicated water level process is considered, that many effective methods are synthetically used to reduce the fitting model error, and that the necessary progressive transformation is introduced. The advantages of many theories and methods are logically integrated in this method, and the method can be easily used. The rationality and necessity of each step in this method are ensured by sufficient theories, so this method can be widely used to effectively simulate the inherent relations in the same kind of complicated data. Furthermore, many complicated water level processes of the lower Yellow River are fitted by this method, and all the fitting precisions are markedly higher than the precision by the other existing methods. Every component term in the fitting model has clear physical meaning.

AIM To evaluate the clinical and radiographic results of patients with complicated infectious spondylitis treated with single-stage anterior debridement and reconstruction using tantalum mesh cage(TaMC) followed by immediate instrumentation.METHODS Single-stage radical debridement and subsequent reconstruction with TaMC instead of autograft or allograft were performed to treat 20 patients with spinal deformity or instability due to complicated infectious spondylitis. Clinical outcomes were assessed by careful physical examination and regular serological tests to determine the infection control. In addition, the visual analog score(VAS), neurologic status, length of vertebral body reconstruction, and the correction of sagittal Cobb angle on radiography were recorded and compared before and after surgery. The conditions of the patients were evaluated based on the modified Brodsky's criteria.RESULTS The average VAS score significantly decreased after the surgery(from 7.4 ± 0.8 to 3.3 ± 0.8, P complicated infectious spondylitis.

Rational design/screening of more active,more selective,and less expensive catalysts has long been an important target in heterogeneous catalysis [1].In recent decades,the advances of density functional theory (DFT)calculations allow us to theoretically evaluate the activity of catalysts.As shown in Figure 1(a),kinetics serves as the link between the atomic-scale properties obtained by DFT and turnover frequency (TOF)of catalysts.
