Limiting blood loss following trauma is critical for survival and can reduce the risk of death from coagulopathy, infection, and multisystem organ failure. A hemostatic material with biocompatibility and effective hemostatic properties was developed to avoid blood loss. The thrombin spray was prepared by using the polypeptide RDAD16 as a stabilizer. The circular dichroism study showed that the chemical structure of the polypeptide did not change as a stabilizer. The atomic force microscope analysis showed that RDAD16 has formed a fibrous substance in the aqueous solution and could effectively encapsulate the thrombin. The simple coagulation experiment showed that the polypeptide RDAD16 could be the effective stability of the thrombin. The coagulant time of thrombin encapsulated by polypeptide RDAD16 was 14 s after 30 days which showed a significant difference compared with the control sample. Wound closure test in mice showed that thrombin embedded with Polypeptide RDAD16 does not affect wound closure.

A new dynamic equation of aerosol in air is derived, using a model-in-model, by equilibrium of buoyancy, gravity and pressure, together with conservation laws of mass, momentum and energy via Reynolds transport theorem and supplemented by corresponding scientific laws for related properties of air and aerosols. This new dynamic equation of aerosol in air is a set of non-linear partial differential equations involved six unknown functions of mass densities, pressure, air and aerosol speeds and temperature. It has features: 1, it belongs to certain type;2, it emphases the effect of buoyancy in equilibrium and potential energy, and the Archimedes principle of buoyancy is firstly extended to lateral directions based on logical deduction, the phenomenon of stirring a glass of oil-water mixture and the recorded of Hurricane Isabel (2003) from space station. The later shows the evidence of existence of lateral buoyancy;3, the mass densities of air and aerosol of a point in our model are varied in different directions due to traction and are treated as vectors, and they have been used in the calculation of lateral buoyancy.

Effectiveness of Using Natural Materials as a Coagulant for Reduction of Water Turbidity in Water Treatment

coagulation-Flocculation plays a significant role in drinking water treatment. Laboratory experiments were carried out in order to assess the effectiveness of using Conocarpus Leaves Solution (CLS) as a natural coagulant in conjunction with the synthetic chemical represented by Alum in the water purification. Biological test was carried out to confirm that these leaves are not toxic, followed by optimizing the dosage of alum and then Alum and CLS were applied to the turbid water whose turbidity level has two ranges, (20 - 35) NTU and (90 - 120) NTU, using the JAR Test. The parameters determined before and after coagulation were turbidity, pH and temperature. The experiments showed that the optimum dose of alum coagulant (individually) for high turbid water is about 18 mg/l with PH = 7 and 24 mg/l f with PH = 5 and 9. In addition, for the low turbidity water, the optimum dose of alum was lower than in the high turbid water. In terms of using Alum in conjunction with CLS, at high range of turbidity, the results show that at 33% ratio of leaves solution to alum coagulant, there are 50% and 75% turbidity reduction performed for the PH equal to 5 and 9 respectively. Although about 62% and 65% turbidity reduction were achieved at PH = 7 and PH = 9 in the low range level. However, low reduction in turbidity has occurred when the water PH = 5. The amount of leaves solution added to the water in the water treatment plant is highly important, hence it decreases the amount of using the synthetic chemicals by about 33% of the quantity that required for water treatment and that will help both, the water industry and the human health. More studies need to be achieved in particular different concentration of the Conocarpus leaves solution in order to improve the percentage of using the natural material as a coagulant.

Turbidity is a characteristic related to the concentration of suspended solids particles in water and has been adopted as an easy and reasonably accurate measure of overall water quality. The most widely applied water treatment processes, a combination of some or all of coagulation, flocculation, sedimentation and filtration to reduce or eliminate turbidity and improve water quality. In this research, proposed approach was adopted on the basis of applying two sequent treatments that used coagulation, flocculation and sedimentation processes under certain operating conditions of mixing speed, mixing time and settling time for each treatment. The environmentally friendly natural coagulants of date seeds (DS) or pollen sheath (PS) from local Iraqi palm was used in the first treatment and alum was used in the second treatment at their predetermined optimum doses to treat low ( NTU), medium ( NTU) and high ( NTU) ben- tonite synthetic turbid water. Experimental results clearly show that the proposed approach was superior in perform- ance in terms of residual turbidity compared with conventional approach using both of (DS) and (PS) natural coagulants in which it achieved a significant reduction in turbidity to less of 5 NTU that meeting WHO drinking water guidelines for all tested synthetic turbid water. Moreover, in some cases, it produced excellent water quality having residual tur- bidity less of 0.1 NTU. In addition to decrease the settling time to 30 minutes and minimize risks of alum dose required to 60%. These viable advantages are significant to current practices in advanced water treatment technologies such as reverse osmosis in cost, energy, effectiveness, safety and maintenance. So, it is recommended to consider proposed approach in this research work to be a novel pretreatment approach in advanced water treatment.

Background: Chronic liver disease is a disease process of the liver that involves a process of progressive destruction and regeneration of the liver parenchyma leading to fibrosis and cirrhosis chronic liver disease refers to disease of the liver which had lasted over a period of 6 months. This study was conducted to evaluate the D-dimer levels among Sudanese patients with Chronic Liver Diseases. Materials and Method: In IbnSina hospital, Khartoum state, Sudan, a case control study was conducted. For this study, 100 participants were selected, 70 of them were patients known diagnosed by chronic liver diseases as a test group. Other 30 participants were normal healthy individual as control group. The plasma D-dimer level was measured by using MINDRAY auto analyzer BS 380). Results: In this study, the plasma D-dimer level has statistically significantly higher in chronic liver diseases patients (mean ± SD 0.634 ± 0.215 ug/l) compared to normal healthy control group (mean ± SD 0.223 ± 0.077 ug/l) with P value 0.000. Conclusion: The present study revealed that the D-dimer levels were statistically significant higher in chronic liver diseases patients.

Hemorrhage during trauma occurred in emergency situations is a significant challenge. It may be life threatening if it is not treated swiftly. A new device which can effectively stop bleeding to save life of injured person, especially in battlefield situations and accidents, is presented. A plasma generator is designed to generate a low temperature air plasma spray for treating wounds. The spectral spike at 777.4 nm in the emission spectrum of the plasma plume and the spatial distribution of this emission line’s spectral intensity indicate that abundant atomic oxygen is generated and sprays out of the generator by about 25 mm. Atomic oxygen carried by the plasma spray can quickly activate the cascading of coagulation processes and works as dry disinfectant to advance healing. Tests on blood droplets reveal the strong dependence of blood clotting on the amount of atomic oxygen applied in the plasma treatment, which is maneuvered by increasing the plasma treatment time or decreasing the exposure distance;in both approaches, the degree of blood clotting increases. Treated smeared blood samples show that an increase of the erythrocyte concentration and a drastic decrease of the platelet count are also correlated to the increase of atomic oxygen dose applied in the plasma treatment. The results reveal the mechanisms of air plasma blood coagulation and wound healing. As animal models, pigs were used in the tests of stopping wound bleeding from a cross cut in the ham area, from a hole in an ear’s saphenous vein, and from cuts to arteries in an ear and in a real leg, all stopped swiftly. Moreover, both artery cuts were secure to remove tourniquet;downgrade of tourniquet necessary wound in under 2 minutes was demonstrated. The healing progress of cross cut wounds was observed. The healing time was shortened to about half. This battery power plasma spray can be carried to or placed at anywhere available for first aid applications. Gastric antral vascular ectasia (GAVE) is an uncommon but often severe cause of upper gastrointestinal (GI) bleeding, responsible of about 4% of non-variceal upper GI haemorrhage. The diagnosis is mainly based on endoscopic pattern and, for uncertain cases, on histology. GAVE is characterized by a pathognomonic endoscopic pattern, mainly represented by red spots either organized in stripes radially departing from pylorus, defined as watermelon stomach, or arranged in a diffused-way, the so called honeycomb stomach. The histological pattern, although not pathognomonic, is characterized by four alterations:vascular ectasia of mucosal capillaries, focal thrombosis, spindle cell proliferation and fibrohyalinosis, which consist of homogeneous substance around the ectatic capillaries of the lamina propria. The main differential diagnosis is with Portal Hypertensive Gastropathy, that can frequently co-exists, since about 30% of patients with GAVE co-present a liver cirrhosis. Autoimmune disorders, mainly represented by Reynaud's phenomenon and sclerodactyly, are co-present in about 60% of patients with GAVE; other autoimmune and connective tissue disorders are occasionally reported such as Sjogren's syndrome, systemic lupus erythematosus, primary biliary cirrhosis and systemic sclerosis. In the remaining cases, GAVE syndrome has been described in patients with chronic renal failure, bone marrow transplantation and cardiac diseases. The pathogenesis of GAVE is still obscure and many hypotheses have been proposed such as mechanical stress, humoural and autoimmune factors and hemodynamic alterations. In the last two decades, many therapeutic options have been proposed including surgical, endoscopic and medical choices. Medical therapy has not clearly shown satisfactory results and surgery should only be considered for refractory severe cases, since this approach has significant mortality and morbidity risks, especially in the setting of portal hypertension and liver cirrhosis

Rokcall score versus forrest classification in endoscopic management of bleeding peptic ulcer

Acute upper gastrointestinal bleeding (UGIB) remains an important emergency situation. In the last two decades, major developments took place influencing incidence, etiology and outcome of patients with acute UGIB. Peptic ulcer bleeding is the most significant complication of ulcer disease being responsible for 50% of all cases mortality. Aim of the study: To compare between endoscopic clip application versus argon plasma coagulation in management of bleeding peptic ulcer (BPU). Patients and Methods: Sixty patients suffering from acute UGIB were randomly divided into two groups: group I included 30 patients treated with endoscopic clip application and Group II included 30 patients subjected to endoscopic APC. All patients were classified according to Forrest classification and the clinical Rockall score. Results: There were significant differences between the two groups as regard Forrest classification (P 0.05). Re bleeding was significant with higher Rockall score in group I (P 0.05). Conclusion: Endoscopic application of hemoclips have a less re bleeding rate than Argon plasma coagulation for treatment of bleeding peptic ulcer, although this was statistically insignificant, meanwhile APC is still less cost and easy. Clinical and endoscopic assessment (through Rokcall score and Forrest classification) could help in making best choice for endoscopic management.

Trauma-induced coagulopathy is classified into primary and secondary coagulopathy, with the former elicited by trauma and traumatic shock itself and the latter being acquired coagulopathy induced by anemia, hypothermia, acidosis, and dilution. Primary coagulopathy consists of disseminated intravascular coagulation and acute coagulopathy of trauma shock (ACOTS). The pathophysiology of ACOTS is the suppression of thrombin generation and neutralization of plasminogen activator inhibitor-1 mediated by activated protein C that leads to hypocoagulation and hyperfibrinolysis in the circulation. This review tried to clarify the validity of activated protein C hypothesis that constitutes the main pathophysiology of the ACOTS in experimental trauma models.
