The prevalence of pediatric obesity continues to increase worldwide,bringing with it various metabolic,functional,social,and psychological complications.1 Both the prevention and treatment of childhood obesity must be based on multidisciplinary approaches combining nutrition,physical activity(PA),and psychological support.2 As an essential element of these multicomponent strategies,regular physical activity has been acknowledged as having beneficial effects on children's and adolescents,body composition,physical fitness,and metabolic profile,as well as on their health-related quality of life,social and psychological health,and academic achievement.3,4 Given that only a relatively small proportion of children and adolescents meet the public health recommendations for PA,5 and given that weight loss exercise-based interventions suffer from a high attrition rate,there is a clear need for appropriate PAs.

Trends of childhood diabetes in Southern Thailand: 20-year experience in a tertiary medical center

Background:The incidence and/or prevalence of both childhood diabetes and the percent of type 2 diabetes mellitus (T2DM) cases in children and young adolescents have been increasing worldwide.This study aimed to examine the 20-year trends of childhood diabetes in a single tertiary medical center in Southern Thailand.Methods:The medical records of pediatric diabetic patients,aged 0-15 years,diagnosed at Songklanagarind Hospital from 1995 to 2014 were retrospectively reviewed.Results:During the 20-year period,156 children were diagnosed with DM:99 (63.4%) with T1DM,43 (27.6%) with T2DM,2 (1.3%) with neonatal diabetes,and 12 (7.7%) with secondary diabetes.The estimated incidence of new patients with diabetes per 100 000 per year doubled from 0.12 in 1995-2000,to 0.24-0.28 in 2001-2004,and further increased to 0.39-0.48 in 2005-2012 and 0.59-0.64 in 2013-2014;and also the proportion of T2DM cases increased from 10%-15% during 1995-2003 to 25%-30% during 2004-2008,and 35%-40% during 2009-2014.At the time of diagnosis,the fasting plasma glucose and glycated hemoglobin levels were significantly greater in T1DM than T2DM patients while the lipid profiles were more abnormal in T2DM than T1DM patients.Conclusion:The estimated incidences of both T1DM and T2DM in pediatric patients have increased markedly over the 20-year period,and also the percentage of T2DM cases,from 10%-15% in 1995 to 35%-40% in 2014.

Suppressor of cytokine signaling 3 （SOCS3） proteins, a Pmember of SOCS family, are predominantly expressed in Th2 cells, and it has an important role in regulating the onset and maintenance of Th2-mediated allergic immune disease.1 SOCS can translocate from cytoplasm to nucleus when the level in cytoplasm increases, and localize to cell membrane where it binds to receptors. childhood malaria in the Niger delta area of Nigeria:mothers/care givers'perception,definition and treatment practices

Objective:The objective of the study was to evaluate mothers/care givers perception of malaria,their treatment practices and the effects on the outcome of malaria.Methods:Four hundred and sixty children were enrolled and their mothers/care givers interviewed.The children were screened for malaria parasitaemia and there after,blood specimens were obtained for biochemical and haematological evaluation from those children who met the criteria and tested positive to P.falciparum parasites.Packed cell volume,electrolytes,urea, creatinine,plasma glucose,and serum bilirubin were analyzed.Results:A total of 460 children were studied, 233(50.7%) males and 227(49.3%) females.Mild malaria cases were 112(24.3%) and severe malaria 348(75.7%).Those who presented early 106(23.0%) and those who presented late 354(77.0%).Perception and definition of malaria as well as the treatment seeking behaviors vary significantly with the level of education of the mothers and care givers.Those without formal education 68(51.9%) wrongly perceived that the etiology of malaria can only be diagnosed by native doctors compared to those with primary six education 61 (26.5%) and junior secondary education 10(10.1%).Only 43(9.3%) gave the correct dose of chloroquine syrup to their sick children,while 32(7.0%) gave at sub optimal doses.Conclusion:Wrong perception of malaria especially the complicated malaria and wrong treatment practices are major contributory factors to the high mortality and morbidity of malaria in Nigeria.There is therefore a need for health education to correct the wrong ideas about the cause and treatment practices of malaria as part of malaria control programme.

Quantitative monitoring of minimal residual disease in childhood acute lymphoblastic leukemia using TEL-AML1 fusion transcript as a marker

Importance: By demonstrating with TEL-AML1, this study indicated that mRNAs transcribed from fusion genes are ideal targets for minimal residual disease (MRD) monitoring in childhood acute lymphoblastic leukemia, and that different thresholds are needed to apply them into the risk stratification. Objective: TEL-AML1 expression was measured at three time points to 1) determine cut-off values for predicting acute lymphoblastic leukemia (ALL) relapse;2) investigate the prognostic value of this method and how well the results at these time points correlated;3) determine the correlation between MRD levels assessed using this marker and that determined by immunoglobulin/T-cell receptor (Ig/TCR) rearrangement detection. Methods: TEL-AML1 expression in 62 children with ALL was quantitated by real-time quantitative PCR at day 15, day 33, and month 3. The relationship between patient outcome and TEL-AML1 level was analyzed at each time point. The correlation between the MRD levels determined by TEL-AML1 or Ig/TCR rearrangements was also analyzed. results: For day 33, 6.68 TEL-AML1 copies/104 ABL copies was determined to be the best cut-off value. Higher levels were correlated with relapse (P = 0.001). For day 15 and month 3, the best cut-off values were 336.5 and 0.85 copies/104 ABL copies respectively;patients with higher expression levels had lower RFSs (day 15: P = 0.027;month 3:P = 0.023). For days 15 and 33, MRD levels assessed using TEL-AML1 or Ig/TCR rearrangements were strongly correlated [Spearman rank correlation coefficient (ρ) = 0.729 (day 15), 0.719 (day 33);P < 0.001 (both)], and both methods were equally effective at predicting relapse. At month 3, there was moderate correlation between the results derived from the two markers (ρ = 0.418, P = 0.003);however, receiver operating characteristic curve analysis showed that TEL-AML1 was a better prognostic marker. Interpretation: TEL-AML1 is an effective marker for MRD assessment and relapse prediction in children with ALL.

Platelet-specific antibodies and differences in their expression in childhood Immune thrombocytopenic purpura predicts clinical progression

Importance: Immune thrombocytopenic purpura (ITP) is the most common bleeding disorder in children. Despite the highly spontaneously remission, still almost 20% of cases progress into chronic or refractory ITP, which seriously affects children's quality of life. Currently there is no method to predict the initial stage of childhood ITP. Objectives: To evaluate platelet-specific antibodies and compare differences in their expression in childhood ITP to predict clinical progression. Methods: This is a single-center prospective cohort study from April 2014 to October 2015. We enrolled children initially diagnosed as ITP. Anti-GPIIb/IIIa and GPIb/IX antibodies were assayed by enzyme-linked immunoadsorbent assay (ELISA) and patients were followed up for 1 year. We also analyzed the relationship between the expression of the platelet-specific antibodies GPIIb/IIIa and GPIb/IX and their respective clinical prognoses. results: Overall, 134 cases were enrolled including 77 boys and 57 girls with a median age of 19 months (range: 1 to 159). Positive rates of anti-platelet antibodies were 79.8%. After a 1-year observation period, 84.3%were diagnosed as newly diagnosed ITP and 13.4% were diagnosed as chronic ITP. Patients with anti-GPIIb/IIIa antibody had a higher risk for newly diagnosed ITP compared with patients who were anti-GPIb/IX antibody positive only (93% vs 25%, P = 0.005;87% vs 25%, P = 0.014, respectively). There were more anti-GPIb/IX antibody positive only cases, diagnosed as chronic ITP, compared with anti-GPIIb/IIIa antibody positive only cases and double GPIIb/IIIa and GPIb/IX antibody positive cases (75% vs 7%, P = 0.005;75% vs 13%, P = 0.014, respectively). Interpretation: Patients with anti-GPIIb/IIIa antibody (either single or double) were predicted to have a good prognosis, whereas anti-GPIb/IX antibody only predicted a poor prognosis. These results should be confirmed via a larger cohort multicenter study.
