Classification,prevalence and integrated care for neurodevelopmental and child mental health disorders:A brief overview for paediatricians

‘Neurodevelopmental disorders’comprise a group of congenital or acquired longterm conditions that are attributed to disturbance of the brain and or neuromuscular system and create functional limitations,including autism spectrum disorder,attention deficit/hyperactivity disorder,tic disorder/Tourette’s syndrome,developmental language disorders and intellectual *** palsy and epilepsy are often associated with these conditions within the broader framework of paediatric ***-occurrence with each other and with other mental health disorders including anxiety and mood disorders and behavioural disturbance is often the *** these are referred to as neurodevelopmental,emotional,behavioural,and intellectual disorders(NDEBIDs)in this *** prevalence rates for NDEBID have been reported in developed countries,up to 15%,based on varying methodologies and *** are commonly managed by either child health paediatricians or child/adolescent mental health(CAMH)professionals,working within multidisciplinary teams alongside social care,education,allied healthcare practitioners and voluntary *** services are common problems for children and young people with multi-morbidity,and often complicated by subthreshold *** repeated reviews,limited consensus among clinicians about classification of the various NDEBIDs may hamper service improvement based upon *** recently developed“Mental,Behavioural and Neurodevelopmental disorder”chapter of the International Classification of Diseases-11 offers a way *** this narrative review we search the extant literature and discussed a brief overview of the aetiology and prevalence of NDEBID,enumerate common problems associated with current classification systems and provide recommendations for a more integrated approach to the nosology and clinical care of these related conditions.

Does health insurance coverage lead to better child health?Evidence from China

Using data from the China health and Nutrition Survey,this study investigated the impact of health insurance coverage on children’s health in *** address the potential endogeneity between insurance and health,we adopted the instrumental variable probit *** found that insurance enrollment significantly improved the health of children,especially urban children,but there was no significant insurance effect across *** using the propensity score matching method,an alternative measure of child health,and a sub-sample of children aged above 10,our major finding of the insurance effect on child health is verified and *** further conducted the path analysis by identifying three potential paths through which medical insurance improved child *** found that child health behaviour and nutritional intake are the effective paths between insurance enrollment and health *** results provided implications for policy makers in constructing and developing the supportive system of child medical insurance in China.

The continuous assessment of child health services generates knowledge about the real needs of health services and actions in order to achieve better results and indicators. Thus, this study proposes to analyze child health indicators, based on the information systems of health services. This is a descriptive, ecological epidemiological study carried out using secondary data from the following information systems: Information System on Live Births and Mortality Information System, in a historical series of ten years (2005-2014) of the state of Rio Grande do Norte and its health micro regions. It was observed a decrease in infant, neonatal, preterm, late neonatal and postnatal mortality coefficients when compared to the first and last year of the observed period, as well as the concentration of these in micro-regions of health. There was also a positive association between the early neonatal component and infant death. In this perspective, it is important to evaluate the quality of prenatal care, childbirth, puerperium, the first week of integral health and follow-up of the child’s growth and development, as well as to relate the socio-demographic conditions that may be associated with infant morbimortality. Therefore, the importance of situational health assessment studies for the effectiveness of care provided is highlighted, identifying weaknesses and monitoring the effectiveness of strategies.

Stressful life events,psychosocial health and general health in preschool children before age 4

Background The impact of stressful life events(SLEs)in early childhood is often *** aimed to examine longitudinal associations between SLEs and psychosocial and general health in preschool *** Twelve SLEs occurring before the age of 24 months were assessed and categorized by frequency(no events,1-2 SLEs,and>2 SLEs)and overall tension(no events,low,and high)(n=1431).Psychosocial and general health were meas-ured three times at the age of 24,36 and 45 *** associations were examined by logistic regression models using generalized estimating equations to handle repeated *** Half(48.4%)of the families experienced SLEs,and 23.8%perceived high-tension SLEs before the children were aged 24 *** differences were observed in the association between SLEs and psychosocial *** to girls without SLEs,girls who experienced>2 SLEs[OR=3.31,95%confidence interval(CI)2.05-5.35]or high-tension SLEs(OR=3.01,95%CI 2.07-4.39)had higher odds of psychosocial problems from 24 to 45 *** odds ratios in boys were 2.10(95%CI 1.36-3.24)and 1.47(95%CI 1.06-2.03),***,only girls'risk of psychosocial problems increased after experiencing 1-2 SLEs(OR=2.15,95%CI 1.54-3.00)or low-tension SLEs(OR=1.90,95%CI 1.31-2.74).Regarding general health,children who experienced>2 SLEs(OR=1.96,95%CI 1.21-3.18)and high-tension SLEs(OR=1.60,95%CI 1.12-2.28)had higher odds of poor general health from 24 to 45 *** The findings emphasized that young children's psychosocial and general health can be impacted by experienc-ing SLEs in early *** and adequate support for families experiencing SLEs are needed to minimize the potential negative effect of SLEs on child health,particularly in girls.

Longitudinal Care Evaluation in child healthcare

Objective: To evaluate the essential attribute of Primary health Care, longitudinal care, care directed at children from birth to two years old. Methods: This is a descriptive and exploratory study of evaluative character and quantitative approach, conducted with parents/caregivers of 186 children, younger than two years old, patients of primary health care services in the city of Santa Cruz/RN, Brazil. For data collection, the instrument Primary Care Assessment Tools (PCA Tools) was used, and the results of the questions dealing with longitudinal attribute were evaluated. The data were stored and processed in Statistical Package for Social Sciences (SPSS). The study was approved by the Research Ethics Committee of the health Sciences School of Trairí, under number 348896. Results: The mean age of children in months was 8.21;84.4% (n = 157) were assisted by the same doctor/nurse every time;in 81.7% (n = 152) of cases the doctor/nurse know the full medical history of the child;73.7% (n = 137) answered that the professional know their child more as a person than just as someone with a health problem;48.9% (n = 91) stated that the doctor/nurse do not know their family very well;86.6% (n = 161) reported finding the doctor/nurse understands what is saying or questioning;96.2 (n = 179) of respondents said the doctor/nurse answers the questions so that they understand;96.2% (n = 179) of the interviewed said they feel comfortable telling the concerns or problems of their child to the doctor/nurse;66.7% (n = 124) claimed that they would not change the service/doctor/nurse to another health service. Conclusion: It is concluded that the attribute was well rated by the mothers of children seen in primary health care services and that they can establish good communication and relationship with the health professionals who treat their children.

Association between Prenatal Environmental Factors and child Autism: A Case Control Study in Tianjin,China

Objective To investigate the association between autism and prenatal environmental risk *** A case-control study was conducted among 193 children with autism from the specialeducational schools and 733 typical development controls matched by age and gender by usingquestionnaire in Tianjin from 2007 to 2012. Statistical analysis included quick unbiased efficientstatistical tree （QUEST） and logistic regression in SPSS *** There were four predictors by QUEST and the logistic regression analysis, maternal airconditioner use during pregnancy （OR=0.316, 95% CI： 0.215-0.463） was the single first-level node（χ2=50.994, P=0.000）; newborn complications （OR=4.277, 95% CI： 2.314-7.908） and paternalconsumption of freshwater fish （OR=0.383, 95% CI： 0.256-0.573） were second-layer predictors（χ2=45.248, P=0.000; χ2=24.212, P=0.000）; and maternal depression （OR=4.822, 95% CI： 3.047-7.631）was the single third-level predictor （χ2=23.835, P=0.000）. The prediction accuracy of the tree was 89.2%.Conclusion The air conditioner use during pregnancy and paternal freshwater fish diet might bebeneficial for the prevention of autism, while newborn complications and maternal depression might bethe risk factors.

Parenthood in childbirth: Reports by Mothers and Fathers

Nowadays childbirth care in Brazil is mainly guided by the technocratic model, with a high rate of unnecessary interventions that may cause iatrogenesis. Emotional aspects inherent to birth tend to be disregarded in this scenario, which produces risks for the psychological health of the mother, father, and baby. The aim of this study was to look into the subjective experiences of fathers and mothers regarding childbirth in contemporary Brazil. For this purpose, a collective case study was carried out in which 30 birth reports published on personal blogs were analyzed, 15 written by women and 15 by men. The results pointed to the helplessness experienced by fathers and mothers in the face of technocratic care which may sometimes be violent, and to the idealization of health care professionals who work according to the humanized paradigm. The choice of the mode of delivery often appeared in the subjects’ discourse, denoting a scenario in which caesarean section is understood as a consumer good. Respect for the temporality of childbirth was considered essential to ensure that giving birth is not experienced in a traumatic way, and pain was reported as a central element to elaborate the symbolic death intrinsic to the process of parenthood appropriation. The lack of support from the environment seems to be a constitutive factor in the experience of obstetric violence and the writing of birth reports a resource to reinterpret this traumatic experience. We came to the conclusion that it is extremely important that the professional team be trained to understand the emotional aspects present in the childbirth experience, with interdisciplinary exchanges being a powerful resource to provide good quality of care for the parental couple.

Mothers’ health-related quality of life: Its relationship with children’s health-related quality of life and behavior in low-income families

Objective:To examine the association between mothers’health-related quality of life(HRQOL)and their children’s HRQOL and behavior in low-income ***:Mothers of 278 children aged 6-12 years from low-income families were invited to complete the child health Questionnaire Parent Form 50(CHQ-PF50)and the Strength and Dif-ficulties Questionnaire(SDQ)for their children as well as the 12-item Short-Form health Survey version 2(SF-12v2)and the Patient health Questionnaire-2(PHQ-2).Multiple linear regressions with mother-child pairs as the unit of analysis were performed to examine the associations between maternal and child variables with adjustment of mother-and child-level ***:Compared with the general population,low-income mothers had a lower mean SF-12v2 mental component summary score and their children also had lower mean CHQ-PF50 physi-cal and psychosocial summary scores and SDQ total difficulties *** of mothers with SF-12v2 scores below the population mean of 50 had significantly worse CHQ-PF50 scores and higher SDQ total difficulties *** mother’s PHQ-2 depression status had no association with the child’s CHQ-PF50 ***:Our findings suggest that more attention should be paid to reducing the negative impact of health problems on mothers’daily roles in childcare in low-income families.

利用中国健康与营养调查(CHNS)2009年度数据，运用OLS及分位数回归计量方法，对1291名18周岁以下儿童的健康状况(HAZ)进行理论分析与实证研究后发现，在剔除了生理遗传因素的影响后，家庭所在的地理位置、家庭长期福利水平、父亲受教育水平、父母的职业类别以及儿童的年龄都与儿童的健康有显著的相关关系。同时，孩子的性别及家庭中孩子的性别组成显著影响我国儿童的健康。具体而言，男孩的健康状况优于女孩，双子女的家庭规模最利于儿童的健康，且与家庭中只有同一性别的孩子相比，孩子性别比例更为均衡的家庭更利于儿童健康。
