BACKGROUND Only a few cases of giant pseudomeningoceles have been reported in the ***,we report a giant pseudomeningocele of the cervical spine that was found after cervical laminectomy for an epidural hematoma following epidural *** SUMMARY A 47-year-old man presented with recurrent neck pain and posterior neck swelling after spinal *** resonance imaging of the cervical spine revealed fluid collection(5.6 cm×6.6 cm×11.2 cm)at the C3-6 level;this proved to be a *** related to the pseudomeningocele resolved following dural repair and fat graft *** Although rare,pseudomeningocele is a possibility in patients with recurrent back pain,radicular pain,or a persistent headache following spinal *** attention should be paid throughout the spinal procedure,whether conservative or non-conservative.

A series of368 cases of cervical spine injuries were collected from theSection of Orthopaedics and Rehabilitation,School of Medicine,SIU and studied to definethe so-called multiple level injuries and determine the clinical patterns for the benefitof diagnosis and treatment and evaluation of surgical *** the368 cases,3o casesbelonged to multiple level injuies of the cervical spine,which were divided into threegroups:(1)cervical spine fracture associated with a thoracic or lumbar spinal fracture(12 cases),(2)non-contiguous fractures in the cervical spine(10 cases)and(3)morethan two-leVeI contiguous injuries of the CerviCa1 spine(8 cases).The mechanism of In-juries and possible results of treatment were also discussed.

Objective： To provide clinicians with data supporting three different clearance techniques in the obtunded patients after severe polytrauma. Methods： This study gave an overview of the available and pertinent literature regarding cervical spine clearance in obtunded patients after severe polytrauma. Results： Currently, there were three accepted techniques for clearance of the cervical spine in obtunded patients after severe polytrauma. Each of these methods has advantages and disadvantages to both of the patients and the clinicians, Conclusions： There are continuous improvements in both computed tomography （CT） and magnetic resonance imaging （MRI） techniques that increase their sensitivities. The continued use of plain radiographs is called into question with respect to cost and time requirements. An algorithmic approach to the evaluation of the cervical spine in the obtunded patients will lead to fewer missed injuries.

Diffuse-type tenosynovial giant cell tumor （D-TGCT） belongs to a group of lesions believed to arise from tendon sheaths,bursae,or the synovium of diarthrodial *** etiology of D-TGCT is *** it shares histological characteristics with pigmented villonodular synovitis,however,D-TGCT is also termed extra-articular pigmented villonodular synovitis.D-TGCT,which occurs mainly in middle-aged women （20-50 years of age1）,usually involves large load-bearing joints （e.g.,knee,ankle,and hip）.Clinical case reports of D-TGCT involving the spine are rare.2 We retrospectively analyzed a series of three patients diagnosed with cervical D-TGCT in our department over a 10-year period.

Background: We describe a rare case of Candida albicans retropharyngeal infection with upper cervical spondylodiscitis associated with epidural abscess triggered by ingestion of a chicken bone. Case Description: A 63-year-old woman presented with posterior neck and bilateral shoulders pain three weeks after choking on a 2-centimeter chicken bone. Magnetic resonance imaging (MRI) revealed spondylodiscitis and epidural abscess with significant spinal cord compression at C2 and C3 levels. A Barium swallow showed a focal disruption of the posterior wall of the esophagus. A posterior upper cervical fixation (C2-C5) was initially performed. One week later an anterior decompressive procedure was carried out followed by iliac crest bony fusion, and repair of the esophageal defect. Intra-operative cultures showed heavy growth of Candida albicans. After a transitory post-operative neurologic worsening, and after six weeks of appropriate antifungal therapy, the patient achieved a full clinical and radiologic recovery. Conclusion: Prompt surgical debridement, fusion and stabilization combined with adequate antimicrobial agents are necessary to guarantee a good outcome and reverse the neurological deficits.

Background: Infants with congenital muscular torticollis are born with an asymmetric range of motion and a muscular imbalance in the cervical spine, as a result of a shortening or excessive contraction of the sternocleidomastoid muscle. Purpose: The study aimed to investigate passive range of motion (PROM) for rotation and lateral flexion, and muscle function of the cervical spine in children that had a history of CMT as infants. Study design: a prospective cohort study. Patient sample: 58 children at the age of 3.5 to 5 years that had been treated for CMT have infants participated in the study. Method: PROM was measured with protractors and muscle function was estimated with a modified Muscle Function Scale. Data from infancy were taken from earlier records. Result: PROM in rotation of the neck was mean 98.7° and PROM in lateral flexion of the neck was mean 69.1°. Symmetric PROM of the neck was found in 74% of the children for rotation and in 88% of the children for lateral flexion. Multiple regression showed that gender and PROM in rotation as infants had a significant impact on asymmetric PROM. Forty-five percent of the children had some degree of muscular imbalance in the lateral flexors of the neck. Conclusion: Possible risk factors for later asymmetric PROM are: gender, birth weight, gestation week and PROM in rotation as infants. These factors ought to be taken into consideration when developing guidelines for long-term follow-up.

Purpose:The purpose of this study was to compare the biomechanical stability provided by a novel PLA/nano-sizedβ-TCP bioabsorbable self-retaining cervical fusion cage(BCFC) device,and compare it with tricortical iliac crest bone graft and two types of PEEK cages according to their shape and anchoring design,and furtherly to compare with tricortical iliac crest bone graft plus anterior cervical plate(ACP). Methods:Quasistatic nonconstraining torques(maximum 1.5NM) induced flexion,extension,lateral bending(±1.5 NM),and axial rotation(±1.5 NM) on 32 sheep cervical spines(C2-C5).The motion segment C3-C4 was firstly tested intact,the following groups were tested after complete discectomy:autologous tricortical iliac crest bone graft, Wego cage,Solis cage,and ***,the group of autologous bone graft was evaluated with *** mean range of motion(ROM) was calculated from the loaddisplacement curves. Results:No significant difference in ROM between two types of PEEK cages and BCFC could be observed in flexion and extension,BCFC device significandy decreased ROM in lateral bending and axial rotation compared with all *** significant difference in ROM between BCFC and bone graft plus ACP could be determined in lateral bending and axial rotation. Conclusions:BCFC device showed comparable stability to autologous tricortical iliac crest bone graft and PEEK cages in single level ACDF models,it may be a viable alternative to current PEEK cages and anterior plate devices.
