Intracranial hypotension syndrome(IHS) is a rare disorder characterized by a decrease in cerebrospinal fluid pressure to less than 60 mm H2 O. The syndrome is associated with occipital headache radiating to the frontal and temporal zones. The current clinical case describes the manifestation of IHS in a twenty-five year old female with a history of suboccipital craniectomy due to Chiari I malformation nine years earlier. The patient was admitted to the hospital complaining about postural, mainly occipital, headache during the last three months, aggravated by being in an upright position. The magnetic resonance imaging(MRI) revealed engorgement of the dural venous sinuses, significant enlargement of the pituitary gland and download displacement or sagging of the brain with effacement of the perichiasmatic cisterns and the prepontine cistern, while the spinal T2 W MRI revealed a 7 mm × 2.5 mm dural defect with an extradural cerebrospinal fluid collection at the dorsal soft tissues of the cervical spine. The previous imaging did not reveal subdural effusions.

Central nervous system tuberculosis is the most severe form of extrapulmonary tuberculosis disease. We aim to review the diagnostic and radiological features, pathogenesis, and biomarkers of tuberculous meningitis. We also aim to look at the latest development of research of the disease. The diagnosis of tuberculous (TB) meningitis is difficult because the disease presents with unspecific clinical features. However, the disease has excellent clinical response to antituberculous therapy. Good prognosis depends on prompt diagnosis with treatment and radiological findings are very important. There is an increase in the levels of serum and cerebrospinal fluid (CSF) TNF-in TB meningitis patients. IL-6 level is also increased in patients with tuberculoma and exudates. There is an increase in the levels of serum and CSF TNF-α and IFN-γ in TB meningitis patients. There is also a rise in the levels of IL-8, IFN-alpha, IFN-gamma, IL-10, CSF matrix metalloproteinases, CSF tissue inhibitors of matrix Metalloproteinases, VEGF level, caspase-1 and IL-1β. Signal-regulatory protein alpha is overexpressed at mRNA level. High dose intravenous rifampicin (800 mg daily) is associated with reduced mortality in patients with advanced disease.

Objective:Blood oxygenation level-dependent functional magnetic resonance imaging(BOLD-fMRI)in combination with diffusion tensor imaging(DTI)can clearly show the specific relationship between brain tumors and motor pathways.The purpose of this study was to investigate the role of this combination in surgery for brain tumors involving motor pathways.Methods:From September 2018 to March 2019,preoperative BOLD-fM RI and DTI examinations were performed in patients with brain tumors involving motor pathways who were being treated in a neurosurgery department.The patients were divided into two groups according to the combined examination results.In one group,the motor pathways were intact;in the other,motor pathways were infiltrated by tumors.The surgical plan was total resection of the tumor as far as possible and with no damage to the motor pathway.The preoperative and postoperative motor dysfunction and the rate of total resection in the two groups were compared.Results:Of the 20 patients with intact motor pathways,15 had normal preoperative motor function,and 5 had preoperative motor dysfunction;of those 5 patients,3 exhibited postoperative aggravation of motor dysfunction.Total excision was performed in 16 patients with intact motor pathways and incomplete excision in 4.Of the 22 patients with motor pathway infiltration,8 had normal preoperative motor function and 14 had preoperative motor dysfunction;of those14,10 exhibited a postoperative increase in motor dysfunction.Total excision was performed in 8 patients with infiltrated motor pathways and incomplete excision in 14.Statistical analysis showed that there were significant differences between the two groups in preoperative motor function,postoperative changes in motor function,and total surgical resection rate(all p<0.05).Conclusions:First,whether tumors invade the motor pathway is an important factor affecting the degree of preoperative motor dysfunction.Second,the risk for postoperative motor dysfunction was lower in patients with intact motor pathways than

Objectives: To describe the versatility of acellular fetal bovine dermal matrix as an alternative to human cadaveric allograft for head and neck reconstructive procedures in children. Study Design: Case series with chart review. Methods: A database of pediatric operative procedures was queried for the use of acellular fetal bovine dermal matrix over a 16-month period. Indications for reconstruction were assessed and initial parental and surgeon satisfaction with the product were noted. Results: During the time period of 3/2012 and 7/2013 a total of 8 reconstructive procedures were performed on pediatric patients using acellular fetal bovine dermal matrix. Indications for use varied and included open and transnasal endoscopic repair of encephaloceles and soft tissue reconstructions including lateral pharyngeal wall repair, cleft palate repair, and facial recontouring operations. Acellular fetal bovine dermal matrix had a subjectively increased ease of use as compared to the surgeon’s prior experience with human cadaveric acellular dermis. Every parent vocalized a greater comfort level with the use of a bovine product over the alternative of human cadaveric tissue. The cost of acellular fetal bovine dermal matrix is slightly lower than the cost of human cadaveric acellular dermis. Conclusions: Acellular fetal bovine dermal matrix appears to be an acceptable alternative to human cadaveric acellular dermis for various forms of head and neck soft tissue reconstruction in children. Further prospective studies are warranted to assess for any differences in the long-term efficacy of this product as compared to other forms of allograft reconstruction.

Experimental high-velocity missile injuries were produced on both hind legs of 15 dogs.It was found that the concentration of vasoactive intestinal peptide-like immunoreactivity(VIP-LI)significantly elevated in both plasma and brain at the early stage after injuries.However,thechanges of VIP-LI concentration in the plasma and brain did not coincide with each other.The re-sults suggest that VIP-LI may participate in the post-traumatic general reaction.The significanceand machanism of the elevation of cerebral VIP-LI level after a severe non-cerebral trauma remainto be clarified.

Giant Thoracic Meningocele Causing Acute Respiratory Compromise

Objective: The authors report a rare case of giant thoracic meningocele causing acute respiratory compromise, treated with a ventriculoperitoneal shunt. Case Report: We report the case of a 36-year-old with severe scoliosis status post repair over a decade ago, neurofibromatosis type I, and a known large meningocele in the left thoracic cavity, presenting with new acute respiratory compromise. She was taken to the operating room for a lumboperitoneal shunt, but the operation was aborted due to her severe spinal deformity. Two days later, she successfully underwent a procedure for ventriculoperitoneal shunt placement. Upon discharge a week later, the patient was hemodynamically stable, able to move all extremities with good strength, and demonstrated improved oxygenation. In the following 7 months, the patient demonstrated continued minimal requirement on nasal cannula, and MRI showed a stable left thoracic giant meningocele. Conclusion: Ventriculoperitoneal shunting is a method of treating and stabilizing acutely symptomatic giant meningoceles.
