Comparison of postoperative anterior segment changes associated with pars plana vitrectomy with and without vitreous base shaving

AIM:To compare changes in anterior segment topography and axial length(AL)evaluated with Pentacam and IOL Master after pars plana vitrectomy(PPV)performed with and without vitreous base ***:This prospective study included patients who underwent PPV or phacoemulsification+PPV(Phaco+PPV)for various *** who underwent total posterior hyaloid detachment and excessive vitreous base shaving with scleral indentation were referred to as complete PPV(c-PPV).The patients whom posterior hyaloid was separated as far as the posterior arcades and vitreous base shaving with scleral depression was not performed were classified as the partial PPV(p-PPV)*** patients underwent detailed ophthalmologic examinations preoperatively and lwk,I,and 3mo *** in the anterior chamber depth(ACD),anterior chamber volume(ACV),iridocorneal angle(ICA)f central corneal thickness(Cd),and keratometric measurements(K1 and K2)were evaluated with Pentacam *** in the AL measurements were analyzed with IOL ***:A significant increase in ACD was observed in c-PPV cases(P=0.02),but this increase was not significant in the p-PPV group(P=0.053).In contrast,ICA increased significantly in the c-PPV group(P=0.02)but decreased in the p-PPV group(P=0.09).BCVA was significantly improved in the c-PPV group from week 1(P0.05 for all).CONCLUSION:Incomplete posterior hyaloid excision and not removing the vitreous base in PPV surgeries may create a more stable anterior chamber,thus preventing the downward movement of the lens-iris diaphragm,and may cause ciliary body retraction,thereby reducing *** of these effects can

Assessment of intraocular pressure measured by Reichert Ocular Response Analyzer,Goldmann Applanation Tonometry,and Dynamic Contour Tonometry in healthy individuals

AIM: To investigate the accuracy of intraocular pressure (IOP) as measured by a Reichert Ocular Response Analyzer (ORA), as well as the relationship between central corneal thickness (CCT) and IOP as measured by ORA, Goldmann applanation tonometry (GAD, and dynamic contour tonometry (DCT). METHODS: A total of 158 healthy individuals (296 eyes) were chosen randomly for measurement of IOP. After CCT was measured using A-ultrasound (A-US), IOP was measured by ORA, GAT, and DCT devices in a randomized order. The IOP values acquired using each of the three tonometries were compared, and the relationship between CCT and IOP values were analyzed separately. Two IOP values, Goldmann-correlated IOP value (IOPg) and corneal-compensated intraocular pressure (IOPcc), were got using ORA. Three groups were defined according to CCT: 1) thin cornea (CCTthickness cornea (CCT: 520-580 mu m); and 3), thick cornea (CCT>580 mu m) groups. RESULTS: In normal subjects, IOP measurements were 14.95 2.99mmHg with ORA (IOPg), 15.21+/- 2.77mmHg with ORA(IOPcc),1522+/- 2.77mmHg with GAT,and 15.49+/- 2.56mmHg with DCT. Mean differences were 0.01+/- 2.29mmHg between IOPcc and GAT (P >0.05) and 0.28+/- 2.20mmHg between IOPcc and DC (P >0.05). There was a greater correlation between IOPcc and DCT (r =0.946, P =0.000) than that between IOPcc and GAT (r=0.845, P=0.000). DCT had a significant correlation with GAT (r=0.854, P=0.000). GAT was moderately correlated with CCT (r=0.296, P580 mu m) showed the least significant correlation between IOPcc and GAT (r=0.859, 0.000); while the thin cornea group (CCT< 520 mu m) had the most significant correlation between IOP

Repeatability and agreement of CCT measurement in myopia using entacam and ultrasound pachymetry

AIM: To evaluate the repeatability of central corneal thickness (CCT) measurement by entacam, and agreement of CCT measured by Pentacam and ultrasound pachymetry (USP) in Chinese myopia. Thereby investigate the possibility of Pentacam as a substitute for USP in CCT measurement before refractive surgery. The effects of corneal curvature measured by Pentacam on CCT were also evaluated. METHODS: One hundred and forty-eight right eyes of 148 individual with myopia were included in this study. Three successive Pentacam CCT measurements followed by 10 successive ultrasound pachymetry were carried out in the 148 eyes. Mean of CCT taken by each device was calculated for comparison. According to the CCT measured by USP, all the 148 eyes were divided into 3 groups: 560 mu m. For all eyes and each group the CCT obtained by Pentacam and USP were compared. Anterior corneal curvature of the 148 eyes was also adopted for correlation analysis with CCT obtained by ultrasound pachymetry. In addition, CCT measurement using 60 random selected Scheimpflug images was performed by 3 skilled investigators at different time, and this was repeated for 3 times by a forth investigator to assess repeatability of Pentacam CCT measurement using Scheimpflug images. RESULTS: Intraclass correlation coefficient (ICC) analysis revealed high intraobserver repeatability (ICC=0.994, F=158.60, P<0.001) for CCT measurement by Pentacam. The interobserver (ICC=0.998, F =494.73, P <0.001) and intraobserver (ICC=0.997, F=383.98, P<0.001) repeatability for Pentacam CCT measurements using Scheimpflug images were also excellent. There was high positive correlation between the CCT values measured by Pentacam and ultrasound pachymetry (r=0.963, P<0.001). Destructive effects on endothelial cells of grafts in cytomegalovirus DNA-positive patients after keratoplasty

AIM:To investigate corneal graft survival rate and endothelial cell density(ECD)loss after keratoplasty in cytomegalovirus(CMV)positive ***:This was a retrospective cohort *** analyzed the clinical data of patients who underwent viral DNA detection in aqueous humor/corneal tissue collected during keratoplasty from March 2015 to December 2018 at the Peking University Third Hospital,Beijing,*** further evaluate the effect of CMV on graft survival rate and ECD loss,patients were divided into three groups:1)CMV DNA positive(CMV+)group;2)viral DNA negative(virus-)group,comprising virus-group eyes pairwise matched to eyes in the CMV+group according to ocular comorbidities;3)control group,comprising virus-group eyes without ocular *** follow-up indicators including graft survival rate,ECD,ECD loss,and central corneal thickness(CCT),were analyzed by Tukey honestly significant difference(HSD)***:Each group included 29 *** graft survival rate in CMV+group were lowest among the three groups(P=0.000).No significant difference in donor graft ECD was found among three groups(P=0.54).ECD in the CMV+group was lower than the virus-group at 12(P=0.009),and 24mo(P=0.002)after ***,ECD loss was higher in the CMV+group than in the virus-group in the middle stage(6-12mo)postkeratoplasty(P=0.017),and significantly higher in the early stage(0-6mo)in the virus-group than in the control group(P=0.000).CONCLUSION:CMV reduces the graft survival rate and exerts persistent detrimental effects on the ECD after *** graft ECD loss associate with CMV infection mainly occurrs in the middle stage(6-12mo postoperatively),while ocular comorbidities mainly affects ECD in the early stage(0-6mo postoperatively).

Ocular findings in children with thalassemia major in Eastern Mediterranean

AIM: To investigate ophthalmologic findings in children with thalassemia major(TM) and compare the findings with healthy controls. ·METHODS: In a cross-sectional study,43 children with thalassemia major from pediatric hematology outpatient clinics from two university hospitals and age/sex matched 47 healthy children were included in the study. After a complete ophthalmic examination,tear function tests including the Schirmer test,fluorescein tear breakup time(BUT),ultrasound pachymetry,and axial length measurement were performed. Obtained data was recorded for statistical analysis and the values of right eyes were compared between groups. ·RESULTS: The mean best corrected visual acuity was 1.34 ±0.75 in TM and 1.08 ±0.28 in controls. It was found lower than 0.1 logMAR unit in 10(23.2%) children with TM and 2(4.2%) in controls,and the difference was statistically significant(P central corneal thickness was 540±26.95 in children with TM and 536.98± 20.45μm in controls(P >0.05). The mean axial length was 22.53±0.50 in TM and 22.57±0.43mm in the control group. The mean Schirmer test score was 19.94±6.91 in TM and 24.22±3.95mm in the control group(P 0.05). ·CONCLUSION: In TM,while corneal thickness,axial length,and BUT are close to controls,the Schirmer scores are less than normal. The study revealed that TM may be affected by the tear function and visual acuity.

Comparison of anterior segment measurements using rotating Scheimpflug imaging and partial coherence interferometry

AIM:To compare central corneal thickness(CCT) and anterior chamber depth(ACD) measurements using rotating Scheimpflug imaging and partial coherence interferometry.·METHODS:As part of the first phase of Shahroud Eye Cohort Study with 5 190 subjects of 40 to 64 years of age,CCT and ACD were measured using Scheimpflug imaging with the Pentacam(Oculus,Inc.,Lynnwood,WA,USA) and partial coherence interferometry with the Allegro BioGraph(Wavelight,Erlangen,Germany).·RESULTS:After applying exclusion criteria,we had data of 4 387 subjects with a mean age of 50.7±6.2 *** CCT with Pentacam and BioGraph were 528.6 ±33.2μm and 525.6 ±32μm respectively;the difference was statistically significant(P <0.001),but the correlation was high(R=0.920).Mean ACD measurements using Pentacam and BioGraph were 2.68 ±0.35mm and 2.62 ±0.33mm respectively;the inter-device difference was significant(P <0.001) with high correlation(R=0.944).The 95% limits of agreements between devices were-22.65μm to28.61μm and-0.16mm to 0.29mm for CCT and ACD measurements,respectively.·CONCLUSION:For both CCT and ACD,the BioGraph gave significantly lower values than the Pentacam(P <0.05).Despite the high inter-device correlation,the 95%limits of agreements were wide,and this may limit their interchangeability in measuring the CCT and ACD.

Optic disc area in different types of glaucoma

AIMTo evaluate the possible relationship of optic disc area with retina nerve fiber layer in different glaucoma *** eye each was chosen from 45 patients with ocular hypertension, 45 patients with primary open angle glaucoma, 45 patients with pseudoexfoliation glaucoma and 45 healthy controls followed in our hospital. The records of the patients were reviewed retrospectively. Optic disc area and circumpapillary retina nerve fiber layer measurements were obtained using optical coherence tomography. central corneal thickness was measured by ultrasound *** median disc area in the patients with primary open angle glaucoma was significantly higher than the patients with ocular hypertension (2.19 vs 1.90 mm2, P=0.030). The median retina nerve fiber layer was thinner in the patients with primary open angle glaucoma and pseudoexfoliation glaucoma than the patients with ocular hypertension for superior, inferior and temporal quadrants. After adjustment for age, no difference in central corneal thickness was found between the groups. Greater disc area was associated with thicker retinal nerve fiber layer for superior, inferior and nasal quadrants in the patients with primary open angle glaucoma. There was no correlation between disc area and central corneal thickness measurements of the *** size affects the retinal nerve fiber layer thickness in eyes with primary open angle glaucoma and is a possible risk factor for glaucomatous optic nerve damage.
