Ethical considerations of cellular immunotherapy for cancer

With the rapid development of immunology, molecular biology, and associated technologies such as next-generation sequencing, cellular immunotherapy has recently become the fourth major cancer treatment. Immunotherapies based on T cells, natural killer cells, and dendritic cells play key roles in cancer immunotherapy. However, their application in clinical practice raises several ethical issues. Thus, studies should focus on proper adherence to basic ethical principles that can effectively guide and solve related clinical problems in the course of treatment, improve treatment effects, and protect the rights and interests of patients. In this review, we discuss cellular immunotherapy-related ethical issues and highlight the ethical practices and current status of cellular immunotherapy in China. These considerations may supplement existing ethical standards in cancer immunotherapy.

Postoperative non-small cell lung cancer(NSCLC) patients require adjuvant therapy to improve their prognosis. In this study, we investigated the efficacy of a sequential combination of cellular immunotherapy(CIT) and chemotherapy for postoperative NSCLC. This retrospective study included 120 postoperative NSCLC patients: 60 cases received only chemotherapy;33 cases received chemotherapy and sequential CIT with cytokine-induced killer(CIK) cells;and 27 cases received chemotherapy and sequential CIT with alternate CIK cells and natural killer(NK) cells. Survival analysis showed significantly higher overall survival rates in the CIT group compared with the control group. Overall survival was higher in patients who received CIT with alternate CIK cells and NK cells than those who received treatment with only CIK cells. Multivariate analysis showed that adjuvant CIT was an independent prognostic factor for overall survival of patients with NSCLC. In subgroup analyses, adjuvant CIT significantly improved the overall survival of patients with less than 60 year old and positive lymph node. In conclusions, these data indicate that adjuvant CIT, especially with combined application of CIK cells and NK cells, is an effective therapeutic approach to prolong survival of patients with NSCLC, particularly for patients ≤60 years old with positive lymph nodes.

Granulocyte-macrophage colony-stimulating factor （GM-CSF） secreting cellular tumor vaccines contribute to the induction of potent antitumor immune responses in murine models and patients suffering from cancers. Hepatocellular carcinoma （HCC） is one of the most frequent and malignant cancers in China. We describe, for the first time, a GM-CSF releasing vaccine strategy that represents a step toward combating this type of cancer. In this study, a bystander cell-based GM-CSF secreting vaccine against murine HCC, Hepa 1-6/B78H 1-GM-CSF, was co-administered with a low dose of cyclophosphamide （CY）. After challenging with tumor and vaccination, immunological assays demonstrated that the cellular antitumor immune responses were efficiently activated and that tumor development was significantly retarded, which was dependent on synergy with CY. The promising outcome of the anti-HCC vaccine in the murine model demonstrates the feasibility of a future clinical application for this treatment in HCC patients.

HIV-specific chimeric antigen receptor(CAR)T-cells have been developed to target HIV-1 infected CD4t T-cells that express HIV Env ***,T cell exhaustion and the patient-specific autologous paradigm of CAR-T cell hurdled clinical ***,we created HIV-specific CAR-T cells using human peripheral blood mononuclear cells and a 3BNC117-E27(3BE)CAR construct that enabled the expression of programmed cell death protein(PD-1)-blocking scFv E27 and the single-chain variable fragment of the HIV-1-specific broadly neutralizing antibody 3BNC117 to target native HIV *** with T cells expressing 3BNC117-CAR alone,3BE CAR-T cells showed greater cytotoxic activity against HIV Envt cells with stronger proliferation capability,higher killing efficiency,and enhanced cytokine secretion in the presence of HIV Env-expressing ***,we manufactured TCR-deficient 3BE CAR-T cells through gene editing and demonstrated that these CAR-T cells could effectively kill HIV Env^(+) cells in vivo without the occurrence of severe graft-versus-host disease(GvHD)in NSG *** data suggest that we have provided a feasible approach to the generation of“off-theshelf”anti-HIV CAR-T cells in combination with PD-1 checkpoint blockade immunotherapy,which can be a powerful therapeutic candidate for the functional cure of HIV.

cellular therapies have revolutionized the treatment of hematological malignancies since their conception and rapid development. Chimeric antigen receptor (CAR)-T cell therapy is the most widely applied cellular therapy. Since the Food and Drug Administration approved two CD19-CAR-T products for clinical treatment of relapsed/refractory acute lymphoblastic leukemia and diffuse large B cell lymphoma in 2017, five more CAR-T cell products were subsequently approved for treating multiple myeloma or B cell malignancies. Moreover, clinical trials of CAR-T cell therapy for treating other hematological malignancies are ongoing. Both China and the United States have contributed significantly to the development of clinical trials. However, CAR-T cell therapy has many limitations such as a high relapse rate, adverse side effects, and restricted availability. Various methods are being implemented in clinical trials to address these issues, some of which have demonstrated promising breakthroughs. This review summarizes developments in CAR-T cell trials and advances in CAR-T cell therapy.

Objective This study aimed to compare the anti-tumor effects of cytokine-induced killer (CIK) cellsinduced by autologous cytokines in patients with breast cancer and those of allogeneic CIK cells fromhealthy *** We used conventional methods to induce CIK cells originating from two peripheral bloodmononuclear cell types (from patients with breast cancer and healthy adults). Killing activity was detectedusing an LDH assay, immunophenotypic changes were analyzed by flow cytometry, and the IFN-γ level ofculture supernatants was detected by *** The results showed that the proliferative capacity of the allogeneic CIK cells was significantlyhigher than that of the autologous CIK cells. Compared with autologous CIK cells, the allogeneic CIK cellshad significantly enhanced anti-tumor activity against SKBR-3 cells (P < 0.01) and IFN-γ secretion (P <0.05);moreover, they increased the ratio of CD3+ CD56+ cells and CD3+ CD8+ cells (P < 0.05).Conclusion Healthy adult-derived induced CIK cells exhibited a stronger anti-tumor effect than inducedCIK cells derived from patients with breast cancer. The results of this study could provide experimentalevidence for the clinical application of CIK cells.
