Median neuropathy after multiple punctures of the forearm for catheterization:A case report

BACKGROUND Neuropathy may occur at some sites after catheterization for close examination of cardiac disease.Although the radial artery is considered a relatively uncomplicated site for catheterization,the radial artery and median nerve are in relatively close proximity,with the risk of median nerve injury depending on the angle of puncture.The purpose of this study was to report the outcomes of surgery performed for conservative therapy-resistant median neuropathy following forearm catheterization.CASE SUMMARY A 50-year-old woman experienced palsy from the right thumb to the radial side of the ring finger after catheterization from the radial artery of the right forearm.Paresthesia developed at the same site and a positive tinel-like sign was seen for the median nerve area at the high level of the puncture site.Nerve conduction study showed reduced compound muscle action potentials and loss of sensory nerve action potentials.Symptoms did not improve despite pharmacotherapy and the patient gradually developed flexion restrictions of the index and middle fingers.Median nerve injury and associated flexor tendon adhesion was diagnosed,and the patient was referred for surgery at 3 mo after injury.When the same area was opened,no injury to the median nerve epithelium was obvious,but the area of the positive tinel-like sign was highly adherent to surrounding tissue and to the flexor digitorum superficialis of the index and middle fingers.The surgery was terminated with adequate adhesion release.Rehabilitation was initiated postoperatively,improving neurological symptoms and range of motion of the fingers.Six months after surgery,the patient returned to daily activities without discomfort.CONCLUSION This case provides the appropriate diagnosis and treatment for a suspected peripheral nerve injury.

Establishing a long-term vascular access in patients exhibiting vascular access exhaustion is challenging. In this study, we reported a case of a direct catheterization in the superior vena cava of a hemodialysis patient with vascular access exhaustion and original dysfunctional catheter inserted via the left internal jugular vein. The direct catheterization was performed with cuffed tunnel catheter （CUFF） and guided by digital subtraction angiography （DSA） and multidetector computedtomography venography （MDCTV）. The DSA and MDCTV results revealed an occlusion in the right innominate vein and thromboses in the left innominate, right internal jugular, subclavian, and femoral veins. The distal end of the superior vena cava was localized clearly by the original CUFF under DSA. Directed at the distal end of the superior vena cava, a 0.5-cm secondary puncture was introduced below the lateral head of the sternocleidomastoid muscle via the right neck area. This study is one of the few reports regarding direct catheterization of CUFF via the superior vena cava of a patient with vascular access exhaustion and CUFF dysfunction on the left internal jugular vein. We believe that our study can provide a new alternative for inserting central venous catheter for such patient.

Background Natural disasters have been frequent in recent years. Effective treatment of patients with cardiovascular disease following natural disasters is an unsolved problem. We aimed to develop a novel miniature mobile cardiac catheterization laboratory （Mini Mobile Cath Lab） to provide emergency interventional services for patients with critical cardiovascular disease following natural disasters. A feasibility study was performed by testing the Mini Mobile Cath Lab on dogs with ST-elevation myocardial infarction （STEMI） model in a hypothetical natural-disaster-stricken area. Methods The Mini Mobile Cath Lab was transported to the hypothetical natural-disaster-stricken area by truck. Coronary angiography and primary percutaneous coronary intervention （PCI） were performed on six dogs with STEMI model. The transportation and transformation of the Mini Mobile Cath Lab were monitored and its functioning was evaluated through the results of animal experiments. Results The Mini Mobile Cath Lab could be transpdrted by truck at an average speed of 80 km/h on mountain roads during daytime in the winter, under conditions of light snow （-15℃ to -20℃/-68°F to -59°F）. The average time required to prepare the Mini Mobile Cath Lab after transportation, in a wetland area, was 30 minutes. Coronary angiography, and primary PCI were performed successfully. Conclusion This preliminary feasibility study of the use of the Mini Mobile Cath Lab for emergency interventional treatment of dogs with STEMI indicated that it may perform well in the rescue of critical cardiovascular disease following natural disasters.

BACKGROUND Spontaneous bladder rupture is relatively rare,and common causes of spontaneous bladder rupture include bladder diverticulum,neurogenic bladder dysfunction,gonorrhea infection,pelvic radiotherapy,etc.Urinary bladder perforation caused by urinary catheterization mostly occurs during the intubation process.CASE SUMMARY Here,we describe an 83-year-old male who was admitted with 26 h of middle and upper abdominal pain and a history of long-term catheterization.Physical examination and computed tomography of the abdomen supported the diagnosis of diffuse peritonitis,most likely from a perforated digestive tract organ.Laparoscopic exploration revealed a possible digestive tract perforation.Finally,a perforation of approximately 5 mm in diameter was found in the bladder wall during laparotomy.After reviewing the patient's previous medical records,we found that 1 year prior the patient underwent an ultrasound examination showing that the end of the catheter was embedded into the mucosal layer of the bladder.Therefore,the bladder perforation in this patient may have been caused by the chronic compression of the urinary catheter against the bladder wall.CONCLUSION For patients with long-term indwelling catheters,there is a possibility of bladder perforation,which needs to be dealt with quickly.

BACKGROUND Cardiac catheterization is among the most performed medical procedures in the modern era.There were sporadic reports indicating that cardiac arrhythmias are common during cardiac catheterization,and there are risks of developing serious and potentially life-threatening arrhythmias,such as sustained ventricular tachycardia(VT),ventricular fibrillation(VF)and high-grade conduction disturbances such as complete heart block(CHB),requiring immediate interventions.However,there is lack of systematic overview of these conditions.AIM To systematically review existing literature and gain better understanding of the incidence of cardiac arrhythmias during cardiac catheterization,and their impact on outcomes,as well as potential approaches to minimize this risk.METHODS We applied a combination of terms potentially used in reports describing various cardiac arrhythmias during common cardiac catheterization procedures to systematically search PubMed,EMBASE and Cochrane databases,as well as references of full-length articles.RESULTS During right heart catheterization(RHC),the incidence of atrial arrhythmias(premature atrial complexes,atrial fibrillation and flutter)was low(catheterizations(LHC),the incidence of ventricular arrhythmias has declined significantly over the last few decades,from 1.1%historically to 0.1%currently.The overall reported rate of VT/VF in diagnostic LHC and coronary angiography is 0.8%.The risk of VT/VF was higher during percutaneo

Pprimary percutaneous coronary intervention （PPCI） is the cornerstone of treatment to reduce infarct size andimprove outcomes in patients with ST-elevation myocardial infarction （STEMI）.1 The optimal site for initiation of reperfusion strategies is the patient＇s home or place where the infarction occurs.
