Characteristics of elevated cardiac troponin i in patients with acute ischemic stroke

Objective To study prognostic characteristics of cardiac troponin i （cTni） elevation in acute ischemic stroke. Methods We retrospectively studied patients （n = 248） with acute ischemic stroke, acute ST-segment elevation myocardial infarction, and acute non-ST-elevation myocardial infarction who were treated between January 2013 and October 2015. Baseline demographic data and changes in cTni levels among these three groups were compared. Patients with acute ischemic stroke were assigned to either the cTni elevation group （cTni 〉 0.034 ng/mL） or the no cTni elevation group （cTni ≤ 0.034 ng/mL）. Logistic regression analysis was used to identify risk factors associated with elevated serum cTni in patients with acute ischemic stroke. Moreover, the duration of hospital stay and incidence of major cardiovascular outcomes were compared in patients with acute ischemic stroke, with or without elevated cTni. Results in this study population of patients with acute ischemic stroke （n = 178）, acute ST-segment elevation myocardial infarction （n = 35）, and acute non-ST-elevation myocardial infarction （n = 35）, patients with acute ischemic stroke with elevated cTni comprised 18.54% of subjects. Patients with elevated cTni were older and more likely to have a history of hypertension. in addition, these patients had higher levels of inflammatory markers, reduced renal functions, increased D-dimer levels, higher NiH stroke scores, and lower left ventricular ejection fractions. Logistic regression analysis showed that both percentage of neutrophil and NiH stroke scores were elevated; estimated glomerular filtration rate and left ventricular ejection fraction were decreased in patients with acute ischemic stroke who had elevated cTni, and they had more frequent major cardiovascular events during hospital stay. Conclusion Elevated cTni detected in patients with acute ischemic stroke, indicated a greater likelihood of poor short-term prognosis during hospital stay.

Value of transthoracic echocardiography combined with cardiac troponin i in risk stratification in acute pulmonary thromboembolism

Background Acute pulmonary thromboembolism （APE） causes right ventricular dysfunction （RVD） and cardiac troponin i （cTnl） elevation. Patients with RVD and cTnl elevation have a worse prognosis. Thus, early detection of RVD and cTnl elevation is beneficial for risk stratification. in this study, we assessed 14-day adverse clinical events and combined RVD on transthoracic echocardiography （TTE） with cTnl in risk stratification among a broad spectrum of APE patients. Methods The prospective multi-centre trial included 90 patients with confirmed APE from 12 collaborating hospitals. Acute RVD on TTE was diagnosed in the presence of at least 2 of the following： right ventricular dilatation （without hypertrophy）, loss of inspiratory collapse of inferior vena cava （iVC）, right ventricular （RV） hypokinesis, tricuspid regurgitant jet velocity 〉2.8 m/s. The study patients were divided into two groups according to clinical and echocardiographic findings at presentation： Group Ⅰ： 50 patients with RVD; Group Ⅱ：40 patients without RVD. Results More than half of the patients （50/90, 55.6%） had RVD. Nearly one third （26/90, 28.9%） of patients had elevated cTnl at presentation and only 4.2% on the fourth day after initial therapy. A multiple Logistic regression model implied RVD, right and left ventricular end-diastolic diameter ratio （RVED/LVED）, and cTnl independently predict an adverse 14-day clinical outcome （P〈0.01）. Receiver operating characteristics （ROC） curves revealed that the cut-off values of RVED/LVED and cTnl yielding the highest discriminating power were 0.65 and 0.11 ng/ml, respectively. Furthermore, the incidence of an adverse 14-day clinical event in patients with RVD and elevated cTnl was greater （40.7%） than in patients with elevated cTnl or positive RVD alone （0% and 8.3%, respectively）（P〈0.001）. Background: The potential impact of pre-existing coronary artery stenosis (CAS) on acute pulmonary embolism (PE) episodes remains underexplored. This study aimed to investigate the association between pre-existing CAS and the elevation of high-sensitivity cardiac troponin i (hs-cTni) levels in patients with ***: in this multicenter, prospective case-control study, 88 cases and 163 controls matched for age, sex, and study center were enrolled. Cases were patients with PE with elevated hs-cTni. Controls were patients with PE with normal hs-cTni. Coronary artery assessment utilized coronary computed tomographic angiography or invasive coronary angiography. CAS was defined as ≥50% stenosis of the lumen diameter in any coronary vessel >2.0 mm in diameter. Conditional logistic regression was used to evaluate the association between CAS and hs-cTni ***: The percentage of CAS was higher in the case group compared to the control group (44.3% [39/88]vs. 30.1% [49/163];P = 0.024). in multivariable conditional logistic regression model 1, CAS (adjusted odds ratio [OR], 2.680; 95% confidence interval [Ci], 1.243–5.779), heart rate >75 beats/min (OR, 2.306; 95% Ci, 1.056–5.036) and N-terminal pro-B type natriuretic peptide (NT-proBNP) >420 pg/mL (OR, 12.169; 95% Ci, 4.792–30.900) were independently associated with elevated hs-cTni. in model 2, right CAS (OR, 3.615; 95% Ci, 1.467–8.909) and NT-proBNP >420 pg/mL (OR, 13.890; 95% Ci, 5.288–36.484) were independently associated with elevated ***: CAS was independently associated with myocardial injury in patients with PE. Vigilance towards CAS is warranted in patients with PE with elevated cardiac troponin levels.

Engineering trienzyme cascade-triggered fluorescent immunosensor platform by sequentially integrating alkaline phosphatase, tyrosinase and horseradish peroxidase

Mulit-enzyme cascades are a major type of chemical transformations and play a crucial role in biological signal transduction and metabolism. Herein, a trienzyme cascade-triggered fluorescent immunosensor platform was constructed by sequentially integrating alkaline phosphatase(ALP), tyrosinase(TYR)and horseradish peroxidase(HRP). The proposed platform was based on HRP-induced a rapid in situ fluorogenic reaction between dopamine(DA) and 1,5-dihydroxynaphthalene(DHA) to produce a strong yellow azamonardine fluorescent compound(AFC). The obtained AFC was clearly characterized by highresolution mass spectrum,1H NMR,^(13)C NMR and theoretical calculations. The integration of the twoenzyme system(TYR and HRP) or three-enzyme system(ALP, TYR and HRP) led to a maximum of 400.0-fold and 250.0-fold fluorescence enhancements, respectively. Using cardiac troponin i(c Tn i) as the model antigen, a trienzyme cascade-triggered fluorescent immunosensor platform was developed for quantitative detecting c Tn i in a wide linear range from 2 ng/m L to 150 ng/m L with a detection limit of 0.67ng/m L. in addition, the proposed platform was successfully applied in detection of c Tn i in serum of clinical patients. Overall, the developed fluorescent immunosensor performs powerful implications for researching enzyme cascade systems in the field of biomedicine.

Fluorescence immunoassay based on alkaline phosphatase-induced in situ generation of fluorescent non-conjugated polymer dots

Alkaline phosphatase(ALP) activity assay is not only significant to the clinical diagnosis of some related disease, but also momentous to the construction of ALP-based enzyme-linked immunosorbent assay(ELiSA). Herein, for the first time, we have discovered that ascorbic acid(AA) can specially react with N-methylethylenediamine(N-MEDA) to generate fluorescent non-conjugated polymer dots(NCPDs) under mild conditions. On the basis of the AA-responsive emission and ALP-catalyzed hydrolysis of ascorbic acid 2-phosphate(AA2P) to AA, we have exploited a fluorometric ALP activity assay with high sensitivity and selectivity. Furthermore, by means of conventional ALP-based ELiSA platform, a conceptual fluorescent ELiSA has been constructed and applied in the potential clinical diagnosis, during which cardiac troponin i(cTni), a well-established biomarker of acute myocardial infarction, has been chosen as the model target. We envision that such original fluorescent NCPDs generation-enabled ELiSA could become a versatile tool in biochemical sensing and medical diagnosis in the future.

Objective:This study aims to further explore the change of serum cardiac troponin i and plasma brain natriuretic petide combined with Tei index in neonatal asphyxia with myocardial injury,and analyzed its clinical value in neonatal asphyxia with myocardial ***:90 cases of term newborn were randomly selected,between January 2013 to December 2015,of which 60 cases of asphyxia as the observation group and 30 cases of normal newborns as control group,according to the degree of asphyxia observation group was divided into mild asphyxia group(30 cases) and severe asphyxia group(30 cases).Observation group were detected in the acute stage(within 24 h after birth) and convalescence(days 7 d)cTni,CK-MB,plasma brain natriuretic peptide level in *** observation group and the control group,and analyzed the relationship between BNP and cTni,CK-MB,Tei ***:The serum cTni,CK-MB,plasma BNP levels and Tei index of observation group in acute stage was significantly higher than the control group(Pi,CK-MB,plasma BNP levels and Tei index of severe asphyxia group in acute stage was significantly higher than mild asphyxia group(Pi,CK-MB,plasma BNP levels and Tei index of observation group was significantly decreased at comparison between myocardial injury in convalescent and acute stage(Pitively correlated with the serum cTni(r=0.762,Pi index(r=0.540,Pin mild asphyxia *** BNP level was positively correlated with the serum cTni(r=0.612,Pi index(r=0.680,Pin severe asphyxia ***:The combined detection of serum cTni,plasma BNP and cardiac Tei index may be an early and comprehensive assessment of neonatal asphyxia with myocardial injury and cardiac function,assessment of efficacy.

Screening of scFvs against cTni from Phage Display Antibody Library and Their Expression in *** Rosetta

The single chain variable fragments of antibodies(scFvs) against cTni were screened from the phage display antibody library by using cTni as the target antigen. After four rounds of panning, four clones(H2, G5, A9, B9) from the phage display antibody library were verified to show higher binding affinity for cTni by ELiSA and to contain the variable region genes of the light and heavy chains of scFvs by sequencing. The variable region genes of scFvs H2 and G5 were successfully amplified by polymerase chain reactions(PCR) and cloned into expression vector pPELB and expressed as a soluble protein in *** Rosetta, whose expression yield was about 2% of total proteins. The expressed proteins were purified by nickel(Ni) affinity chromatography and a single band is shown in the position of 28 kDa on SDS-PAGE. The western blot analysis result verifies that the expressed scFv proteins are capable of binding with monoclonal antibodies against hexa-histidine, indicating that they are hexa-histidin-tagged aim proteins. The immunoassay demonstrates that the expressed scFv proteins are able to specifically react with cTni molecules. The association constant(K_A) values range from 1.2×10 4 to 1.7 ×10 5 L/mol that are correspondent to the affinities of polyclonal antibodies against cTni from rabbits. These antibodies can be valuable reagents for the immunoassay of cTni.

A label-free electrochemiluminescence(ECL) immunosensor based on CdS-MoS composites as efficient luminescent material and sensing platform is fabricated for the detection of the acute myocardial infarction biomarker cardiac troponin i(cTni). The CdS-MoS composites was synthesized by hydrothermal method, it has excellent luminescent properties. Owing to layer by layer modification of immunosensor, electron transfer is blocked between luminescent materials and coreactants, and the ECL signal gradually decreased. The immunosensor showed a wide linear detection range of 5.0 pg m L-1 to 100.0 ng mL-1 with a detection limit of 1.0 pg mL-1. The method has been successfully applied to the determination of cTni in human serum samples. The proposed strategy can also be applied to the detection of other biomarkers.

Application of Nanogold Probe Coupled with Silver Enhancement in Rapid cTni Colorimetric immunoassay

A rapid, inexpensive, reliable, and flexible quantitative immunoassay for cardiac troponin i （cTni） based on the concepts of one-step dual monoclonal antibody “sandwich” principle. The low density protein array, the nanogold probe, and the silver enhancement on the gold particle were provided. The whole detection procedure of the assay could be fulfilled within 40 min with the pretreated colloidal gold-labeled detection antibody and supporting substrate. The assay showed good specific response to cTni with very low cross-reactivity ratio to the skeletal isoforms of troponin i （sTni）, cardiac troponin T （cTnT）, and myoglobin. 588 serum samples were assayed simultaneously by enzyme-linked immuno sorbent assay （ELiSA） and this colloidal gold method to test the validity of the method and the data were analyzed using the statistical package SPSS version 11.0 （SPSS inc.）. There was no significant difference between these two assays （P=0.66〉0.05）. The agreement between this method （〉 or 〈0.3 ng/mL） and ELiSA was 86%.

Perioperative changes of ventricular function and three indicators of myocardial injury during orthotopic liver transplantation

Patients undergoing orthotopic liver transplantation may develop significant haemodynamic instability, especially during anhepatic phase and immediately after reperfusion of the graft. The haemodynamic instability may be caused directly by myocardial depression due to pathogenic substances released from the liver, or by acute blood loss.^1 Creatine kinase（CK） and its MB fraction （CK-MB） are sensitive and specific indicators to reflect myocardial damage.^2 cardiac troponin i （cTnl） is a specific and sensitive marker of myocardial necrosis.^3 This study assessed perioperative cardiac function using three indicators （CK,CK-MB,and CTnl） to evaluate perioperative myocardial damage.
