Since its approval by the Food and Drug Administration in 2011,transcatheter aortic valve implantation(TAVI)has rapidly evolved to become the preferred ultimate intervention for high-and intermediate-risk patients with severe symptomatic aortic stenosis.[1]This is due to its non-open-heart,minimally invasive and off-pump advantages.[1]Nevertheless,as a result of the frequent frailty and comorbidity profiles of patients undergoing TAVI,such as advanced cardiac dysfunction and extensive coronary artery disease,or technically difficult anatomy for the procedure itself,[2-4]it is common for these patients to experience critical circulatory collapse perioperatively.These factors are linked to elevated mortality rates,necessitating suitable mechanical circulatory support(MCS)to reverse the disastrous situations.[5]

Objective To investigate the correlation between serum renal injury molecule-1 (KIM-1) and neutrophil gelatinase-associated lipocalin (NGAL) in the remission of diabetic kidney disease (DKD) after gastric bypass surgery in obese DKD patients.Methods Seventy-nine obese patients with DKD who received gastric bypass surgery in our hospital from January 2021 to January 2022 were selected and divided into DKD remission group (n=47) and DKD non-remission group (n=32)according to whether UACR decreased to normal level after surgery.Preoperative and postoperative general data were collected and biochemical indices.

To the Editor:Acute kidney injury(AKI)is a frequent complication of both cardiac and major non-cardiac surgeries.Cardiopulmonary bypass(CPB)-assisted cardiac surgery was associated with a higher incidence of AKI than other surgeries without CPB,suggesting that CPB might be a risk factor for cardiac surgery-associated AKI(CSA-AKI).[1]However,previous studies have demonstrated conflicting results regarding the association of CPB and CSA-AKI,as the diagnosis of AKI has been made according to different criteria.[2-5]The most recent definition of AKI from an AKI Work Group,Kidney Disease:Improving Global Outcomes(KDIGO),allows for the determination of AKI based on a decline in the glomerular filtration rate(GFR)with subsequent increase in serum creatinine or a period of oliguria.[6]We performed a retrospective cohort study to evaluate the association of CPB and postoperative AKI,using the KDIGO definition among patients undergoing on-pump or off-pump coronary artery bypass graft(CABG).

Bariatric metabolic surgery has been confirmed as an effective,safe,and feasible method for achieving weight loss,treating obesity-related diseases,and improving the quality of life.Due to its simplicity,sleeve gastrectomy(SG)has been more commonly used in the past ten years than Roux-en-Y gastric bypass(RYGB).^([1])However,RYGB is superior to SG in terms of excess weight loss effect and type 2 diabetes mellitus(T2DM)remission over a 5-year follow-up period.

Coronary artery bypass grafting(CABG)tra-ditionally involves revascularization of the left anterior descending artery(LAD)as it is usually of greater prognostic value.[1]Isolated CABG for non-LAD lesions have also been descri-bed in literature.However,these were often in the setting of re-do CABG[2,3]or in patients with isol-ated right sided coronary lesion and concomitant valvular heart disease.

BACKGROUND Superficial temporal artery-middle cerebral artery(STA-MCA)bypass helps treat cerebral ischemia.However,the STA is not available for bypass in some conditions.Therefore,with some technical tips,the authors introduced a bypass technique using the occipital artery(OA).CASE SUMMARY Two female patients complained of hemiparesis.Brain magnetic resonance imaging(MRI)indicated contralateral infarction from the MCA steno-occlusion.On Diamox single photon emission computed tomography or perfusion MRI,the contralateral front parietotemporal reserve was diminished.On transfemoral cerebral angiography,the STA was thin with a weak flow;however,the OA was prominent.Direct OA-MCA end-to-side extracranial-intracranial bypass surgery was implemented instead of STA because the caliber was too narrow.The postoperative course was uneventful in both cases,with well-maintained bypass patency and neurological stability during follow-up.CONCLUSION OA might be an acceptable alternative for MCA cerebral ischemic cases with an unsuitable STA.

Coronary artery bypass graft(CABG)is associated with a high risk of mortality and morbidity;thus,assessment of surgery quality is necessary.In this perspective,we will focus on the structure,process,and outcomes measured as quality assessment.A set of 21 evidence-based structure,process,and outcome measures were selected as National Quality Forum.Of these,the Society of Thoracic Surgeons ultimately chose 11 individual quality measures grouped them into four domains used to assess the quality of CABGs.These four domains consisted of perioperative medical care,operative care,risk-adjusted operative mortality and postoperative risk-adjusted major morbidity.These measures have been useful as quality improvement tools in assessing the quality of CABG surgery.

Objective Open microsurgery,often with bypass techniques,is indispensable for complex aneurysms.To date,it remains unknown whether arterial anatomy or quantitative blood flow measurements can predict insufficient flow-related stroke(IRS).The present study aimed to evaluate the risk factors for IRS in patients treated with open microsurgery with bypass procedures for complex internal carotid artery aneurysms.Methods Patients with complex aneurysms undergoing bypass surgery were retrospectively reviewed.The recipient/donor flow index(RDFI)was preoperatively evaluated using colour-coding angiography.RDFI was defined as the ratio of the cerebral blood volume of the recipient and donor arteries.The sizes of the recipient and donor arteries were measured.The recipient/donor diameter index(RDDI)was then calculated.IRS was defined as the presence of new postoperative neurological deficits and infarction on postoperative CT scans.We assessed the association between RDFI and other variables and the IRS.Results Twenty patients(38±12 years)were analysed.IRS was observed in 12 patients(60%).Patients with postoperative IRS had a higher RDFI than those without postoperative IRS(p2.3 were more likely to develop IRS(p2.3,rather than RDDI,was significantly associated with postoperative IRS.This preoperative evaluation allows appropriate decisions regarding the treatment strategy for preventing postoperative IRS.

Improving the flexibility of combined heat and power(CHP)units is an important way to solve the problem of wind power accommodation in northern China.Firstly,this paper analyzes the principle of an extraction-type CHP unit,calculates its safe operation range,and analyzes its contradiction between heating and peaking.Secondly,the safe operation ranges of the CHP unit with several flexibility modifications are further calculated,which involve two-stage bypass,low-pressure cylinder(LPC)removal,heat storage tank,and electric boiler.Finally,based on the safe operation ranges,their effects on improving the capabilities of deep peak shaving and wind power accommodation are compared,and their adaptabilities to different wind scenarios are analyzed.The results show that:①all flexibility modifications can improve the deep peak shaving capability of the CHP unit,especially for the two-stage bypass and the electric boiler;②LPC removal modification can accommodate wind power to some extent,but most of wind power is still abandoned;③heat storage tank modification is unstable in different wind scenarios,which is determined by the surplus heating capability during the daytime.

BACKGROUND Postoperative atrial fibrillation(POAF)is a common yet understudied clinical issue after coronary artery bypass graft(CABG)leading to higher mortality rates and stroke.This systematic review and metaanalysis evaluated the rates of adverse outcomes between patients with and without POAF in patients treated with CABG or combined procedures.METHODS The search period was from the beginning of PubMed and Embase to May 18th,2020 with no language restrictions.The inclusion criteria were:(1)studies comparing new onset atrial fibrillation before or after revascularization vs.no new onset AF before or after revascularization.The outcomes assessed included allcause mortality,cardiac death,cerebral vascular accident(CVA),myocardial infarction(MI),repeated revascularization,major adverse cardiac event(MACE),and major adverse cardiac and cerebrovascular events(MACCEs).RESULTS Of the 7,279 entries screened,11 studies comprising of 57,384 patients were included.Compared to nonPOAF,POAF was significantly associated with higher risk of allcause mortality(Risk Ratio(RR)=1.58;95%Confidence Interval(CI):1.42−1.76,P<0.00001)with accompanying high level of heterogeneity(I^(2)=62%).Conclusions Patients with POAF after CABG or combined procedures are at an increased risk of allcause mortality or CVAs.Therefore,POAF after such procedures should be closely monitored and treated judiciously to minimize risk of further complications.While there are studies on POAF versus no POAF on outcomes,the heterogeneity suggests that further studies are needed.
