The characteristic genetic abnormality of neuroendocrine neoplasms(NENs),a heterogeneous group of tumors found in various organs,remains to be identified.Here,based on the analysis of the splicing variants of an oncogene Focal Adhesion Kinase(FAK)in The Cancer Genome Atlas datasets that contain 9193 patients of 33 cancer subtypes,we found that Box 6/Box 7-containing FAK variants(FAK^(6/7))were observed in 7(87.5%)of 8 pancreatic neuroendocrine carcinomas and 20(11.76%)of 170 pancreatic ductal adenocarcinomas(PDACs).We tested FAK variants in 157 tumor samples collected from Chinese patients with pancreatic tumors,and found that FAK^(6/7)was positive in 34(75.6%)of 45 pancreatic NENs,19(47.5%)of 40 pancreatic solid pseudopapillary neoplasms,and 2(2.9%)of 69 PDACs.We further tested FAK splicing variants in breast neuroendocrine carcinoma(BrNECs),and found that FAK^(6/7)was positive in 14(93.3%)of 15 BrNECs but 0 in 23 non-NEC breast cancers.We explored the underlying mechanisms and found that a splicing factor serine/arginine repetitive matrix protein 4(SRRM4)was overexpressed in FAK^(6/7)-positive pancreatic tumors and breast tumors,which promoted the formation of FAK^(6/7)in cells.These results suggested that FAK^(6/7)could be a biomarker of NENs and represent a potential therapeutic target for these orphan diseases.

In April 2017,the first edition Chinese Society of Clinical Oncology breast Cancer(CSCO BC)guidelines were released.These guidelines were based on medical evidence,as well as the accessibility and cost effectiveness of various anticancer drugs in China.The CSCO BC guidelines include regimens with a high level of evidence,good product accessibility and high consistent consensus among Chinese experts as level I recommendations.Regimens with a high level of evidence but poor product accessibility or expert consensus are included as level II recommendations.

BACKGROUND breast cancer(BC)is the most common cancer among women globally.The WHO estimates 416,000 women are diagnosed to have BC annually in China,and the numbers of BC cases and deaths in China are predicted to rise by 16%and 48%,respectively,over the next 20 years(1).BC can be hereditary,and the most common cause of hereditary BC is an inherited pathogenic or likely pathogenic variant(henceforth called‘pathogenic variant’or‘PV’)in the BRCA1 or BRCA2 genes.BRCA1/BRCA2 PV carriers have a 17%–44%risk of developing ovarian cancer(OC)and a 69%–72%risk of developing BC up to the age of 80(2).PALB2 is a more recently established,highpenetrance BC gene:testing for which is now more widely advocated.PALB2 carriers have a 53%risk of BC up to the age of 80(3).In addition,PALB2 has recently been shown to be a moderate-risk OC gene with a 5%lifetime risk of OC(3).PV in these three genes accounts for around 4%of BC.Most of these cancers are preventable or can be better mitigated if they are detected earlier.

Triple-negative breast cancer(TNBC)is the most difficult type of breast cancer to treat.TNBC is defined by the lack of expression of three receptors:estrogen receptor(ER);progesterone receptor(PR);and human epidermal growth factor receptor 2(HER2).Chemotherapy is currently first-line treatment for TNBC;however,due to the high heterogeneity of TNBC,most patients eventually develop chemotherapy resistance,which is associated with a poor prognosisl-2.Emerging immune checkpoint blockade(ICB)therapies have been shown to have promising therapeutic efficacy in treating solid tumors.A phase III clinical trial reported that the combination of ICB and chemotherapy lengthened progression-free survival in patients with metastatic PD-L1+TNBC3;however,most patients had primary resistance or acquired resistance to ICB.Thus,the intrinsic mechanisms underlying ICB resistance are still under investigation4.

Triple-negative breast cancer(TNBC)has a poor prognosis because of its aggressive characteristics and lack of targeted therapies.1 Recently,it has been reported that TNBC is sensitiveto ferroptosis,an iron-dependent type of programmed cell death,making it a potential target for the treatment of TNBC.2 breast cancer stem cells(BCSCs),a small population of cancer cells that possess the infinite proliferative potential and tumor-initiating properties,play important roles in drug resistance.3 The responses of BCsCs to ferroptosis-inducing agents are controversial and remain elusive.
